Troubleshooting a hydraulic clamping system on a Symons 5-1/2 Cone Crusher. The pump had been run out of oil due to a leak. The pump was off, at this time they removed three lines inside of the unit to release air. The lines were not properly installed, resulting in bypassing of the relief valves. This resulted in lacerations to EE's right hand.

The employee was setting the grease on the jaw. Employee left the working surface and slipped and fell 2-4 feet. which results in the injury to their foot.

Was shoveling in the tunnel and sprained their back when twisted to put material onto conveyor.

Employee was picking rock off of the rip-rap pile when a rock higher up rolled down and hit their pinky finger.
