EE drove the cat skidloader to the shop to take bucket off and put the forks on. EE put front of bucket on the ground, could not get pins out to release the bucket. EE used a hammer to hit the pin. Once EE did that, the back of the bucket fell onto EE's foot.

Employee was cutting a skirt board rubber with a utility knife when the knife cut his right leg just above the knee. He was on his knees cutting the skirt board rubber pulling the knife towards his knee when the knife slipped off the rubber and cut his leg. He was taken to the emergency room for treatment where he received 1 stitch.

WHILE SHOVELING SAND TO MAKE SMALL TRENCH HE TWISTED AROUND TO DUMP SHOVEL WHEN BACK MADE POPPING SOUND LIKE BREAKING A STICK. LOWER LEFT SIDE OF BACK WAS VERY PAINFUL BUT COMPLETED WORK DAY & WENT HOME.DECIDED LATER THAT NITE TO GO TO E.R. WHERE DR. EXAMINED & PRESCRIBED MEDS WITH LIGHT DUTY. WAS TOLD IF PAIN PERSISTS TO SEEK FURTHER MEDICAL ATTENTION.

EE was cutting the outer jacket off of a piece of wire, holding the wire with his right hand and the utility knife in his left hand slicing back towards his right hand. The utility knife slipped and continued through his right thumb. Resulting in a cut requiring 11 stitches.

EE was using a 3"x3"x1/4" angle at 6" long as a pry bar to lift an 8' H beam at 30' long. His back popped causing him to seek medical attention right away. Doctor subscribed 3 medications for spasms and wants to see him again in two days.

EE was cleaning out (scraping) a lime chute behind the short head crusher. He heard a "pop" sound from his left bicep, and it started hurting. He called the plant supervisor to report it, but thought it would get better. It continued to hurt him so the supervisor took him to the med center.

Employee was lifting a 1.50" wash down hose up from side of wash bin to wash out a classifier tank & sand screw, the hose had been lowered over the side of the bin to drain so it wouldn't freeze, and evidently some water remained in the hose & froze. The hose was heavier than normal.

The men were attempting to replace a bearing on the yarder. EE was swinging a sledge hammer at a gear puller several times. On the last swing, the hammer missed its mark, causing his arm to travel further around. His left forearm struck a pipe hand rail that had been cut off with a pipe cutter prior to the beginning of the shift. This laceration required 8 staples.

While working on the bushhog mower gear box, EE was holding the shaft while another was driving it out with a hammer. When shaft moved injured's small finger was smashed between the shaft and the floor.

While working on a small pull-behind trailer, employee was attempting to raise the front of the trailer up. While he was cranking the jack handle around, the handle broke causing him to move forward and slash the underside of his right arm on the crank shaft that was left sticking up. He received sixteen stitches.

While trying to loosen a sheave, employee placed a block of wood against it. While still holding the block of wood a co-worker proceeded to swing the sledge hammer striking EE's finger & smashing it causing a fracture to his middle finger.

EE WAS USING A UTILITY KNIFE TO CUT THE RUBBER/WIRE STRAPS ON CONVEYOR BELTING AT THE OLD GRAVEL PLANT. HE WAS TRYING TO CUT IT IN ONE PASS, WHICH CAUSED THE KNIFE TO COME BACK TOO FAR AND TOO FAST, HITTING HIS RIGHT KNEE. THE KNIFE SLICED OPEN AND AREA APPROX. 3" LONG ON HIS RIGHT KNEE CAP.

EE WAS CUTTING CONVEYOR BELT WITH A UTILITY KNIFE. THE KNIFE SLIPPED, RUNNING OVER THE TOP OF HIS LEFT HAND. IT SLICED HIS INDEX FINGER, MIDDLE FINGER, AND RING FINGER OF THE LEFT HAND. HE WAS BLEEDING CONSIDERABLY, AND WAS TAKEN TO CLINIC IMMEDIATELY BY A FELLOW EE. THE CUT WAS CLEANED, HE RECEIVED 8 STITCHES, A METAL SPLINT WAS PLACED ON RING FINGER, AND ALL FINGERS WERE WRAPPED.

EE WAS REMOVING A BAD BEARING ON THE STONE PLANT WITH A HAMMER. HE WAS TRYING TO BREAK THE BEARING LOOSE FROM THE PULLEY SHAFT, AND A FRAGMENT OF METAL FLEW UP AND STRUCK HIS LEFT FOREARM. THE RE WAS A DROP OF BLOOD, AND HE THOUGHT IT JUST CUT THE SKIN. THAT NIGHT, THE ARM BEGAN TO ACHE AND HE NOTICE A KNOT AND SWELLING UNDER THE CUT. HE WENT TO THE MEDCENTER FOR XRAYS. EMBEDDED.

EMPLOYEE WAS WELDING ON A SCREEN USING SCALER TO CLEAN SLAG AND WELD. HE HAD SAFETY GLASSES ON AND WAS PREPARING WELD AREA. A PIECE OF MATERIAL WAS BLOWN IN BEHIND HIS GLASSES BY THE WIND. HE WENT TO EYE AREA, BRUSHED IT AWAY, THOUGHT IT WAS GONE. THE NEXT EVENING HIS EYE STARTED TO BECOME IRRITATED. WENT TO DOCTOR AND PRESCRIPTION WAS GIVEN.
