Showing all 24Employee was operating the 345 excavator clearing in front of the face. The bucket dropped off a ledge causing the machine to jar. The shudder of the machine resulted in the operator hurting their lower back. Was seen by medical professional who restricted them to resting for one week.
Miner was drilling a hole in some metal when the drill bit caught the edge, causing the drill to twist resulting in a sprain of EE's wrist.
Operator was jack-hammering frozen material in L-7 hopper. When the Hilti tool slipped, EE's left hand contacted a scraper bolt which cut through the works gloves resulting in a laceration.
While running the stone splitter, one of the machine chisel's carbide teeth broke off sending a metal shard into the operator's neck.
An employee was assisting in re-welding a section of the mainframe liner, during the process, employee was exposed to arc-flash.

The equipment was inspected by a vendor mechanic, no repairs needed.
Employee was torching a steel plate on the Return Deister deck. This area has a 2 deck screen bed where screens are changed out weekly/bi weekly when worn. Torching and welding when needed. Conditions were windy that day and while torching a piece of slag went into employees ear. Plant was down for maintenance and all PPE was worn.
EE was disking an area to knock down weeds when a "splinter" got logged in EE's right eye. EE and another employee tried to flush the eye to remove the object without success. EE was taken to a local optometrist where the object was removed. EE was give a prescription for antibiotic eye drops.
Employee was loosening jaw die bolts using 1" impact, slipped and lost control of impact causing him to twist his body injuring his knee.
Employee was splitting a rock, the splitter hung up under pressure. Employee was reaching to turn the splitter off to release pressure when the rock split and jumped backwards and hit his left hand middle finger.
