While cutting and welding on a manlift, an employee developed lower back pain. After notifying their foreman and resting, they sought medical attention. A safety representative escorted them to a clinic, where a physician diagnosed a lower back sprain. Employee was prescribed medication and stretching exercises

A process attendant clearing material under the East/West CCDC screw reached into a leak hole, where their finger contacted the moving screw, amputating its tip. They reported the incident immediately, received first aid, and was taken to the ER for a tetanus shot and further treatment.

EE was lubing up a "Pile Shackle" as preventative maintenance, the pin was in the "out" position and the pin was dislodged. EE held the pile shackle with EE's left hand to keep it stable and was greasing with EE's right hand, the pin pinched EE's nail off and also part of the tip of the pointer finger.

EE was giving signals by radio to the crane operator, gave command to "hold it" 3 different time and the crane continued to boom up. The brace that was rigged and to be connected had too much pressure, was being held with spud wrench, flung the brace around and struck EE inside the right forearm causing a 5"2" laceration.

After finishing grinding a gudgeon shaft, a maintenance mechanic accidently grazed the leg of another maintenance mechanic who they were working with. This resulted in 7 stitches to close wound on lower left leg. 4" portable hand grinder found to be in good condition and no defects.

Employee was repairing closing line cable on overhead bridge crane when fall protection harness lanyard became entangled in drive motor shaft while spotting the drum for repairs, pulling employee from their feet and against the coupling. The employee received broken ribs and bruising.

Employee was bench testing a radiator cooling fan. The fan became unstable and the employee went to catch the fan. The employees hand came in contact with the rotating blades and lacerated 4 fingers.

Employee approached compressor to unload and shutdown. As they approached, the connection between compressor and hose failed (safety pins, and whip check were in place) and the hose impacted left upper arm, resulting in a puncture wound from a bolt on the coupling clamp. Employee received 6 sutures to close wound. Returned to work on next shift.

Employee was holding a carriage bolt head to keep it in place while an impact was being used to remove the nut. The bolt pushed out and spun in EE's hand, lacerating the right index finger and requiring stitches.

The employee was rolling a piece of steel on a Webb plate bending role in the Maintenance Shop when EE's finger was caught between the plate and roll. The employee's left ring finger was crushed resulting in a laceration and fracture to the end of finger.

Employee was clearing a material blockage from Finish Mill #1 dust collector hopper and came in contact with the rotary feeder resulting in lacerations and crushing to the tips of the 2nd and 3rd digits on EE's right hand.

Mechanic was repairing primary feeder, guiding a bar with EE's left hand on top of the bar and running a hoist with EE's right hand. The bar moved unexpectedly and caught the mechanics left hand/fingers between the bar and the wall of the bin.

Employee was gouging in a mill. Wearing required PPE. After gouging employee felt an irritation in left eye. Employee flushed eye. The irritation went away. Employee returned to work with no concern. The irritation returned and employee talked to supervisor. Suprvisor took to DR. Dr prescribed ointment and eyedrops.

An employee stuck their left arm into inspection port on a hopper chute to unplug. An air knife gate above the inspection port shut on the hand when the dust collector cycled and when the cycle was complete the air knife gate opened and released the employee's hand.

Descaling in 3rd Stage with Jack Hammer when 2 pieces fell from roof. Hit headache deck busting through and hitting scaffolding before striking employee at bottom of cone. EE climbed out of unit on EE's own power, was transported by ambulance to Hospital ER. Examined, nine (9) stitches to parietal area at top of head, released with instructions for care of wound.

Mechanic was operating a 4 1/2" hand held grinder with a cutting wheel cutting a metal window frame. The cutting wheel stuck and kicked back coming into contact with the mechanics right hand/wrist resulting in a laceration.

A miner was operating a trackmobile that was struck by a rolling empty rail car that was found to have a faulty hand brake. After complaining of back soreness the next day the miner was taken to a medical facility and found to have likely strained his middle back area and was then placed on restrictive duty on 10/21/2011. The miner was given 3 prescriptions.

On 8/30/2011 at approximately 1:15 pm welder repairman was cutting a piece of metal on the shear, employee marked the metal plate were it needed to be cut, then pushed plate into place to make cut, moved hands slightly out of the way and pressed on foot pedal to cut the plate. As the plate cut employees left index finger was smashed between plate and shear deck.

Loading drill pipe into Atlas Copco L8 30 drill carousal with winch installed on drill mast. Sliding hook weight got caught on part of the mast about half way up. Man standing below at controls was struck in the hard hat by weight after it came loose and side down the cable. Hard hat webbing cut the mans scalp. 10 staples needed. No lost time.

Employee was attempting to remove a liner bolt from a raw mill. Employee reached down to remove the bolt, an employee on the outside of mill tried to lossen the nut that was holding the bolt with an impact wrench. EE let go but bolt had already turned. The spin of the bolt cut the employee on the inside of the right index finger and palm.

We were pulling out the slide gate out of #4 cooler fan on K-2. I was laying down on the catwalk so I could put the bolts in to hold the slide. The fan started while we were securing the gate. That's when a cloud of high pressure dust hit me in the face and eyes. I tried to flush my eyes 3 times before going to see the eye doctor. (Prescription medication was given.)

On 11/20/106 EE was working on A-Mill's dust collector changing bags. While he was in the dust collector cement dust got in EE's left eye. He tried to clean the cement out with company eye wash solution his eye continued to get more aggravated and he requested to see an optometrist. The optometrist cleaned his eye out and gave him a rx medication for his eye.

EE WAS REMOVING CHUNKS INSIDE OF A PREHEATER TOWER VESSEL USING AN AIR LANCE. EE PINCHED HIS RIGHT MIDDLE FINGER BETWEEN THE AIR LANCE AND THE EDGE OF TOWER PORT. FINGER WAS X-RAYED AT LOCAL H OSPITAL AND DETERMINED THAT EE HAD BROKEN A BONE CHIP FROM THE END OF HIS FINGER AND STITCHES WERE USED TO CLOSE A LACERATION.

EE HAD OPENED A PORT ON THE PREHEATER TOWERE TO CHECK FOR BUILD UP IN SIDE OF MACHINERY. HE WAS STARTING TO BECOME BLOCKED. AN AIR CONNER NEED THE OPENING WAS DISCHARGED IN AN ATEMPT TO REMOVE BUILT UP MATERIAL. THE HOT MATERIAL LOOSENED BYTHE AIR CONNEN WAS FLUSHED OUT OF THE HOLE ONTO THE EE S WRIST AND CAUSING SECOND DEGREE BURNS. EE WAS WEARING HIS PPE AT THE TIME OF THE ACCIDE

WHILE WORKING AT THE PLANT, EE REPORTED THAT HE WAS WEARING PERSONNEL PROTECTIVE DEVICES (CONFIRMED BY A WITNESS) & DID NOT RECALL THE SPECIFIC INSTANCE WHEN THE FOREIGN OBJECT ENTERED HIS EYE . HE INITIALLY THOUGHT INJURY WAS DUE TO A FLASHBURN & DIDN'T SEEK MEDICAL ATTENTION UNTIL FEB. 15. DIAGNOSIS WAS A SMALL PIECE OF METAL IN EYE.
