Employee was at primary crusher for plant 17 checking for leaks on a skid steer when a rock came out of the crusher causing a small laceration above their left eye.

Miner was inside Stedman crusher preparing to weld on reel. As they stood up, they placed a foot on the reel, dislodging wedge and rotating reel upward catching their leg and pushing it up against the apron.

Miner was torching on old storage bin and heard a pop, fell to ground and injured right knee.

Employee fell down steps trying to get feeder shut off to stop overloading conveyor belt.

While remotely operating auto welder in primary crusher, miner experienced arc flash burn to left eye due to reflection.

Cutting bearing off shaft, grinder kicked back and hit left hand cutting it open. Required stitches to close.

Employee was drilling a hole in a steel pipe when the drill bit caught and caused ee to lose control of the drill. The drill rotated out of ee hand and struck ee on the face, lacerating lip. Laceration needed stitches to close.

A malfunction in a pneumatic grease gun caused grease to spray onto his face and into his eyes, causing irritation to both eyes. This incident is reportable because the employee received prescription eye drops to use in his eyes.

Employee was drilling a 1/2 inch hole in a conveyor frame. The drill "caught" as it exited the steel and the torque from the drill twisted his right hand. Injury is fractures of F-3 & F-4.

Was trimming weeds when something flew into his eye. Foreign object was imbedded.

Employee struck a chipping hammer with another hammer, causing a chip to break off, striking him in the knee. Resulting laceration required stitches to close.

Two employees were using an excavator to lift a dump truck bed from the truck body. One employee was in the bed, hooking up the lifting chains when the operator accidently moved the bucket striking the first employee.

Employee was cutting a piece of HDPE with a circular saw. He reached under the HDPE to see how close to the sawhorse he was with the saw and his finger came in contact with the rotating saw blade.

The employee was welding inside the primary crusher when he got a piece of welding slag in his eye. Employee did not seek medical treatment until 10/17. He was off work 10/18 & 19.

CUTTING STEEL WITH TORCH. BLOW BACK SHOWERED FACE WITH SLAG AND A PIECE GOT INTO HIS EYE.

Felt pain in low back while operating an electric jackhammer.

EMPLOYEE WAS LIFTING A BEARING OFF THE SCREEN SHAFT USING A CRANE. THE BEARING SWUNG UNEXPECTEDLY, PINCHING HIS RIGHT PINKY FINGER BETWEEN THE BEARING AND THE ADJACENT SHAFT.

GOT PIECE OF DIRT IN EYE WHILE RUNNING ROCKBREAKER.

RECEIVED "FLASH BURN" OF BOTH EYES WHILE WELDING WITH ANOTHER EMPLOYEE.

ROCK CAME OUT OF CRUSHER AND HIT MINER IN MOUTH. MINER WAS USING ROCK BREAKER TO DRAG STONE INTO CRUSHER WHEN ROCK CAME OUT. CHIPPED THREE (3) TEETH.

BURNED LEFT INDEX FINGER WHILE USING OXYGEN/ACTYLENE CUTTING TORCH TO CUT OFF BOLTS. TORCH REPORTEDLY BACKFIRED AND CAUGHT GLOVE ON FIRE.

WAS CUTTING OVERHEAD BOLT, SLAG FELL INTO RIGHT EYE. EE RECEIVED MORE THAN ONE DOSE OF PRESCRIPTION EYE DROPS.

HE WAS CLEANING UP WITH CACUM TRUCK. THE HOSE BECAME PLUGGED. WHEN IT OPENED, IT RECOILED & STRUCK HIS NOSE.

CUTTING PIEVE OF STEEL WHEN SLAG FLEW IN EYE. PRESCRIPTION EYEGLASSES WITHOUT SIDE SHIELDS AND CUTTING GOGGLES WERE BEING WORN.
