Caught in, under or between a moving and a stationary object

Miners was installing drive belts on a crusher, when they got their little finger on left hand caught between shive and belt. This caused a fracture and a laceration requiring stitches.

Employee was doing maintenance in a primary crusher hopper. EE was installing new bolts in the structure under the hopper when EE stepped in a hole and twisted right knee.

While lifting a railroad tie to place in the rear of a maintenance truck, employee felt a shooting pain in right bicep with tingling in fingers.

Serious injury occurred to worker inspecting Dust Collector at the National Lime & Stone Plant. The worker was accessing the catwalk to inspect Dust Collector to see if there was any material in it before EE attempted to lift with the Crane. The worker fell about 20 feet to the level below through a catwalk.

Employee was dismounting the front end loader when EE slipped and fell injuring right shoulder and hip.

The injured employee and another employee were carrying a piece of wooden blocking when the injured employee tripped, causing him to fall. His fingers were pinched between the block and the floor.

Struck against stationary object

Employee caught toe on metal plate laying on the floor as he turned to walk and felt pain in his right knee. Case was not reportable until Dec.20 when employee had surgery.

A refuse bank failure occurred at this site causing a front-end loader and articulated haul truck to be forced over a high wall berm and to fall to benches below the level they were working at. No one was injured. National has yet to complete its ongoing investigation & evaluation of the conditions contributing to incident

On February 2, 2012, contractor R. D. Jones Excavating Inc. reported to National Lime and stone that material that was being excavated had slid out from the toe of the excavation and engulfed the tracks of the excavator.

Employee was placing a guard on the back of a flatbed truck when he pinched his finger between the guard and truck, amputating the tip of his left long finger.

EMPLOYEE WAS LIFTING A BEARING OFF THE SCREEN SHAFT USING A CRANE. THE BEARING SWUNG UNEXPECTEDLY, PINCHING HIS RIGHT PINKY FINGER BETWEEN THE BEARING AND THE ADJACENT SHAFT.
