End dump rolled down embankment flipping and landing on its roof. Employee was pronounced dead at the hospital.

A delivery driver employed by a water supply vendor was in route to the facility when the tanker truck overturned and slid off the road. Investigation ongoing
Victim was removing sand from the sand wall with a Volvo L120E loader and the wall sloughed off covering the loader and victim with sand.
Employee was found entangled in conveyor 12 tail pulley. Responding authorities pronounced the employee deceased. Incident is still under investigation.
A miner received fatal injuries. After a bridge collapsed that the miner was operating a skid steer loader on. The miner was clearing refractory brick form a rotary kiln. The bridge that was installed by the property owner collapsed. The skid steer fell approximately 10 feet into a hopper, causing the payload of brick to engulf the operator causing fatal wounds.

Employee was shoveling coal off a conveyor belt, belt moved forward causing miner to fall down transfer chute onto belt conveyor that transferred EE to stacker tube where EE fell onto coal pile.
Employee received head injuries while attempting to re-rail a Longwall electrical power car using air lifting bags in the 10 Headgate-16 block spur.
While cleaning raw coal material from on top of the locked out Raw Coal Stacking Conveyor, the Miner was standing on the conveyor ahead of the conveyor feed chute. The belt and material moved backwards, sweeping the Miner into the chute and covered Miner with material.
Employee was out of their truck and on the pit floor while the 993 (LD270) Loader was being trouble shot trying to find a leaking hose. While walking to their 785 truck the employee traveled under the bucket of the 993 loader and was struck causing severe crushing injuries from their waist down.
May 9, 2024 at 0800 hours when a manlift operator ran EE's manlift, an 8008 JLG, serial number 0300310942, into the canopy covering the C35 conveyor belt coming out of C35 tunnel. The operator was pinned between the roof edge and the man basket hand rail.
Employee was fatally injured while working on a conveyor system.
Truck driver was found unresponsive in the cab of EE's overturned truck at the bottom of the feed ramp drop off. There were no witnesses to the accident. Investigation is still pending.
An over the road truck driver was crushed while trying to unload gravel. Driver was parked in a jackknife position while trying to unload some of the gravel in EE's bed. While attempting to dump, EE's lift cylinder snapped and the trucks trailer fell on top of the driver's side killing the driver. 911 was called immediately and MSHA was called after.
While doing general work in the section employee was struck by a shuttle car and received crushing injuries to their upper torso.

Employee disregarded lock out tag safety training and climbed into a hopper without de-energizing equipment and without notifying other workers in the area. Loader operator had no way of knowing that anyone would be inside the hopper with the feeder belt running. Employee in the hopper was buried and crushed when loader operator dumped 18 tons of stone into hopper
Employee was fatally injured when a belt component malfunctioned (the bridal bar had a mechanical failure and broke) on the C1 Longwall take-up and struck employee in the chest causing blunt force trauma.

At time of filing it is believed a front-end loader struck the back of the tractor trailer. The position of the victim at the time of the accident is still under investigation. The victim was found under the rear driver's side tires of the tractor.
EE was involved in a traffic accident on the road that leads to the plant. EE was extricated from the vehicle and transported to the hospital where EE later died.

Our mixer driver was hauling for Freeport McMoRan at their Morenci mine location, the load was rejected and sent back, driver followed the escort down a hill with a full load, lost brakes and turned the mixer on its driver's side.
EE was operating a loader at our pit. A co-worker was in a dump truck & was there to get loaded with but was having trouble with the air in the truck. The co-worker radioed to EE to go ahead and load another truck. EE returned from loading another truck and crawled underneath the co-worker's truck without telling the co-worker. Customer truck driver pulled into tarping area behind another truck and failed to set parking brake before exiting the truck. The driver did not use wheel chocks as required. The customer truck driver was fatally injured when customer's truck moved forward and pinned customer truck driver against the other drivers truck.

A customer's truck became caught in overhead wires when the driver deployed the automatic tarp in driveway while exiting the mine. The driver exited vehicle safely and was electrocuted when the driver attempted to climb back in to the truck while the tarp arms were still in contact with overhead power lines. The accident resulted in a fatality.
A fatal incident occurred while traveling the external slope on a rubber tired four wheeler.
Employee was working on removing a pin from a leveling cylinder on the back of the rock truck. While the employee was performing this work, the haul truck bed fell on top of the employee.
For unknown reason an independent driver lost control of belly truck which caused it to jack knife, throwning driver from the cab. Driver was then run over by same truck.
Driver was at parked truck (#5 lot) to start pre-shift inspection. Truck started to move. Driver fell under the truck and truck ran over driver's legs from about the knee area down.
An employee was fatally injured when EE was struck by a third-party truck driver who failed to stop at a posted, lighted, STOP sign and made a sudden turn off the primary travel way. Local law enforcement arrested the third-party truck driver and charged the driver in connection with the accident.
