Employee climbed on C20 counterweight guard to remove ice from bend pulley when EE slipped and fell fracturing right wrist

A contractor without a contractor ID installing arc flash labels at the mine walked down a slight incline off a small cement pad, lost EE's balance and broke EE's right leg.

Employee was working on drill, removing radiator hose when employee stepped backward into a hole for the service hatch. Employee tried to catch self with left arm and felt pain in the left shoulder.

EE was standing on the adjacent drive belt guard at the time. EE slipped and fell, landing on a crate of tools on the catwalk below. Contusion to the right knee / Laceration to right arm resulting in 13 stitches.

Employee was at the wash sand screw and went to step over a pipe but a block was there. EE stepped on the block and fell, catching self with left hand. EE did not report the incident to management until 3/8/24.

Employee was doing maintenance in a primary crusher hopper. EE was installing new bolts in the structure under the hopper when EE stepped in a hole and twisted right knee.

Lost footing and fell backwards striking a metal corner.

Miner was at the garage using a pressure washer to clean off the top the skid steer. EE was trying to stand up on bucket, came back down and pivoted on right foot, slipped and fell, hurting right ankle.

EE was cleaning stone off a catwalk. While walking backwards, EE lost EE's balance and grabbed the handrail with EE's left hand for support. EE felt something pull/pop in EE's left elbow.

Serious injury occurred to worker inspecting Dust Collector at the National Lime & Stone Plant. The worker was accessing the catwalk to inspect Dust Collector to see if there was any material in it before EE attempted to lift with the Crane. The worker fell about 20 feet to the level below through a catwalk.

Employee was dismounting the front end loader when EE slipped and fell injuring right shoulder and hip.

The operator was removing a piece of steel that was plugging a chute. After retrieving it right foot slipped causing left knee to buckle.

Employee tripped on a rock and twisted employee's body trying to catch self resulting in back pain.

It was reported to the company immediately. Safety staff picked him up to be seen by a doctor. It was diagnosed as a fractured patella. He will see a specialist on Thursday at 9:15 a.m.

Employee was climbing an extension ladder to tie off the top when his foot slipped and he fell about 5 feet, breaking three ribs and collapsing a lung.

The employee was walking along the catwalk along C-7 conveyor when the last section of Gripstrut catwalk slipped out from under his feet causing him to fall about 9 feet to the ground. The section of catwalk was not bolted down.

Employee was leaning against a handrail pulling up a ladder when the handrail broke, causing him to fall a total of about ten feet.

WAS WALKING TO HIS CAR AT THE END OF HIS SHIFT AND SLIPPED ON ICE AND FELL.

WORKING ON STEDMAN CRUSHER, TRIPPED AND FELL.

STANDING ON A LADDER PULLING A CONVEYOR BELT. FEET SLIPPED OFF LADDER AND HE CAUGHT HIMSELF WITH RIGHT ARM.

WAS ADJUSTING BELT SCRAPPER, LOST HIS BALANCE AND FELL 5 FEET TO THE GROUND.

TWISTED HIS ANKLE GETTING OUT OF BOBCAT SKID STEER.
