Miner was negotiating stairs to perform maintenance on a screen. When they turned to face screen, their foot slipped off of the stair tread causing them to trip down the remaining two steps and sit down on top of their right foot/ankle. Miner was evaluated for right ankle strain.

Employee was loading conveyor onto the transport truck grabbing the railing when EE's hand slipped falling straight backwards onto EE's left side.
EE was pulling a 1 1/2 inch fire hose to clean off piece of equipment, as EE was walking backwards, tripped in rut and fell to ground.
On 04/04/2022 EE climbed down the ladder on the loader that EE was operating to close the gates to the haul road. As EE stepped down on the lower rung of the ladder, it broke away from this support. Team member was walking on unstable ground. Sunk into mud up to knee, went to turn to grab a stable ladder and felt a pop in the right knee.

Employee was walking by the mud pump and slipped in the mud, injuring lower back.
Prior to shift, team member fell through doorway into break room landing on left side and breaking hip. Walkway and break room are finished concrete slab which were clean and clear of identifiable obstructions. No other environmental factors were identified.

Employee was replacing a 10" diameter pipe that went through the catwalk (expanded metal) that he was working on. After he removed the pipe and co-workers were raising the new pipe, the employee turned to retrieve an air hose and his left leg fell into the 10" hole in the catwalk where the old pipe used to be.
EE had just finished fueling the EQ. when he laid the hose/nozzle down to climb down from the machine. As he put the nozzle down he stepped on it and it slipped out from under his foot causing him to fall. EE fell to the ground and landed on his stomach.

EE came down the ladder on a front end loader fowards to step on an Abutment. He missed the abutment and fell on his left shoulder, injuring it.

Splicing a conveyor belt on the radial stacker. As employee was cutting the belt the belt ripped and knocked the employee off the belt. Employee fell approx. 10 feet to the ground.
