While operating the Portable Powerscreen Jaw crusher, the employee had a rock fall from above that struck the operators control panel. The control panel door stuck the employee in the back. The extent of the injury to the employee was unknown so the emergency response team called an ambulance immediately. Employee was taken to the emergency room.

EE WAS ATTEMPTING TO PLACE A JACK UNDER A SERVICE TRUCK. OTHER EE HAD ALREADY PLACED A JACK UNDER THE LEFT SIDE AND WAS ALREADY TAKING OFF LEFT REAR TIRES. THE JACK SLIPPED AND THE TRUCK CAME DOWN THE EE LEFT ARM WAS UP AGAINST THE FUEL TANK WHEN THE TRUCK CAME DOWN AND IT BROKE HIS ARM
