EE was taking a break in a job trailer. While removing EE's safety glasses a small dust particle entered EE's left eye. EE notified us on 4/27/26 that EE sought med. treatment on 4/23/26. Dx w/Corneal Abrasion & Given Rx. Reporting within 10 days.

Employee was performing maintenance on conveyor belt. While removing a bearing with a mallet, a piece of metal broke off, became embedded into the EE's right pec. On 5/13/25 we were notified the employee had a piece of metal removed and was able to continue working. Reporting within 10 days.

Employee was repairing blown hydraulic hose on a loader when the wrench slipped off the hose, which caused the employee to strike their right index finger against the hose clamp, causing a laceration. Employee did not seek medical treatment until 12/16/2023. Employee was ref. to a hand specialist and was given a finger splint on 01/11/2024.

Employee was removing bolts on the counterweight on the excavator. When EE removed the last bolt, the counterweight fell (hydraulic ram bracket failed). The counterweight struck the ladder causing the employee to fall & hit their head. EE was taken by ambulance & was diagnosed with a fractured skull.
Employee removing a pin from a tilt cylinder on the loader and the pin fell causing left hand to be caught between the pin and the loader. The employee sought medical treatment and received stitches.

On August 27th, employee advised EE had been hospitalized due to flu like symptoms. After multiple tests employee state EE was diagnosed with a water type pneumonia. Medical records have been requested to verify work relatedness, but they have not been received to date.
EE was performing maintenance while standing on the counterweight of a loader. EE misstepped and fell. EE broke collarbone.
Employee was trying to pry free a clamping bar on a screen deck using a pry bar. The pry bar slipped causing the employee's right index finger to get caught between the pry bar and the structure of the screen box. The employee sought medical treatment and received stitches.

EE was pushing a wheelbarrow of mud out of the surge pile tunnel to go dump it. In the process the EE stepped on a piece of dry mud which caused them to lose their footing and strain their back because they continued to hang onto the wheelbarrow. They sought treatment with a chiropractor.
Employee was removing nut from bolt using cordless impact wrench on bolt & holding nut with box wrench. Bolt, nut & box wrench twisted causing EE's finger to get caught between box wrench & frame of equipment. EE sought treatment & pin was placed in fingertip for fracture.
EE was tightening bolt on screen deck clamp board & felt pain in right shoulder. EE did not seek medical treatment until April 26, 2018. EE was diagnosed with a labral tear on May 4, 2018 & reported back to work with no restrictions. EE will be having surgery in the near future, in the interim EE continues to work normal job.
On 7/18/17, EE was setting up a crane with the crew. While pulling the cable thru the block of the crane EE injured EE's shoulder. EE sought medical treatment & was diagnosed with a strain. EE recently went back to the MD & was diagnosed with a rotator cuff tear. EE has continued to work regular job. EE will be having surgery in Nov.
Employee was climbing off loader using the attached ladder & while on the last rung employee's foot got caught & employee fell on their side. Employee sought medical treatment & is currently off work.
Employee was lining up a wear plate on a conveyor chute. As ee went to set it down, ee pinched right middle finger between the wear plate & inner lip of the chute. The employee sought treatment & was diagnosed with a fracture as well as a laceration in which ee received stitches.
