While installing structural steel doors over coal bin EE went around safety rail without using fall protection on the door which was still attached to the crane to try to move the door & close a gap between the doors. The door shifted causing a support I-beam to move shifting the doors which caused him to fall. MSHA was notified 12/27/05 that he had expired from his injuries.

The Euclid R-Haul truck Comp. No. 671 was hauling a load of stemming material to the drill bench in the Wadesville Pit. The truck went over an embankment, upset, coming to rest on the driver's side causing fatal blunt force trauma to the driver located inside the cab. The cause of this accident is yet to be determined pending further investigation.

Disassembling a crushing plant. two employees were attempting to remove the motor that drove the hazmac crusher by using a boom truck. The motor became stuck on the motor mount bolts that had been cut off using a torch. The employees then tried to pry it from its stuck position when it freed and came forward.

AT APPROX 12:10 A.M. NOV. 8, 2005 THE OPERATOR OF A MACK TK TANDEM HAUL TRUCK WAS FATALLY INJURED WHEN THE TRUCK HE WAS OPERATING EXITED THE HAUL ROAD, RAN THROUGH A GUARD RAIL AND OVERTURNED. THE HAUL ROAD IS LOCATED BETWEEN WV STATE ROUTES 90 & 93. MSHA DID NOT INVESTIGATE THE ACCIDENT AT THE TIME IT OCCURRED. IT WAS BELIEVED MSHA DID NOT HAVE JURISDICTION OVER THE ROAD.

EE and Another EE responded to a stuck pallet alarm on Unit IV cooler. He became trapped when the cooler pallet released on its own causing the dump arm and wheel to swing up and strike him in the head, pinning him between the dump arm wheel and the rail.

Two employees were repositioning a conveyor. One was on the loader which was pulling the conveyor. The victim was directing and somehow got caught under the conveyor tire, crushing him.

Employee was looking at sump pump with another employee. He fell backwards through an open hole, falling about 12 ft. into the sump, hitting the back of his head on the way down. He was transported by company ambulance to NNRH ER where he was pronounced dead on arrival.

The worker fell from the bag plant roof where he had been cleaning to the truck dock approximately 17 feet below. The specific explanations for being near the edge or for falling are unknown. Weather conditions and working surface believed to be non-contributory. There were no direct eyewitnesses to the fall. The fall resulted in the death of the worker. Investigation ongoing.

(Under investigation) (Per Supervisor) Employee was assigned task of pushing overburden wall down with bulldozer. This was being done to install a water jack (floating pump) to pump out old mine cut. A short period of time later bulldozer and employee were discovered under water in mine cut by another employee.

Employee fell to concrete floor at base of preheater - apparently from the second level where he had been performing work. Death was apparently immediate. There were no witnesses to the accident and it is not known how the employee fell or whether he tried to access his work from the safety handrail along the catwalk.

Employee was operating a 50 ton Cat end dump truck & was at dump site with his 3rd load of stone from pit. While attempting to dump the load from truck at dump site, which is elevated some 20' above the plant, his truck went over the dump face backwards & flipped end for end 180' landing on its top, wheels to sky, at bottom of dump site at plant level. He was still in cab.

Worker was fatally injured while driving a CAT 769C Water Truck along a standard mine haul road, returning to the water stand to re-fill the water tank. The water truck drifted left, onto the berm, travelling approximately 160' before "cartwheeling" and ejecting the worker. The worker does not appear to have been wearing the supplied seat belt.

At this time he chose to travel over a pile of dirt. as he was traveling over the pile of dirt he got too close to the edge of the pile and the equipment overturned and landed on him.

The victim was operating a track dozer to clean off a drill bench in preparation for drilling. The bench was 45 feet above the quarry floor. Apparently the victim was attmpting to turn the dozer around on the bench, backed off this area, and fell to the quarry floor.

Employee was standing on a temporary platform just below a jaw crusher attempting to remove two toggle seat wedges from the pitman. Shortly after removing the two bolts holding the wedges in place, the pitman unexpectedly moved fatally crushing the employee.

Employee was making dredge repairs using hand tools: 3/4" ratchet & 4'-long cheater pipe. He took dredge pump apart and left the ratchet and pipe on the end of the shaft. He started the engine and then engaged the clutch. The pipe came around and hit him on the left side of his skull. Employee was hospitalized and died of his injuries on 7/3/05.

(MSHA Report of Investigation: Loading a steel plate (5' wide x 10' long x 3/4" thick; approx. 1,500 lbs.) onto flatbed truck using chain attached to front-end loader. The victim was on bed of the truck steadying & guiding plate onto the truck when the hook (no safety latch) became detached from the plate, and the victim & plate fell to the ground. Plate crushed victim.)

The victim was walking in front of the #11 scoop that was traveling in #1(intake) entry of the 14A longwall to deliver posting material to the worksite. The operator of the scoop stated that he saw the victim walking on the side opposite of the operator. He then looked back & did not see him. He exited the cab & walked to the front of scoop & found the vicim injured on the ground.

Technician was attempting to repair a steering problem on a wheel loader. He replaced steering pilot valve, started engine, raised bucket and attempted to steer machine. When it would not steer to the right, he dismounted machine without shutting off engine. While attempting to remove steering shaft, the steering activated and steered to the right crushing the victim.
