WHILE HE WAS STANDING AT THE OPENING IN THE HOPPER MORE MATERIAL FELL FROM ABOVE INSIDE THE SPRAY TOWER. WHEN IT HIT THE SCREW HOPPER SOME MATERIAL CAME OUT THE OPINING STRIKING THE MAN. HE LEANED OUT OF THE RIGHT SIDE OF THE CAB (RESON UNKNOWN) WITH THE HYDRALIC ARMS IN THE RAISED POSTION. THE A9M DROPPED AND PINNED THE EMPLOYEE AGAINST THE FRAME. HE LIFTED THE ARM OFF OF HIS BODY, SHUT OFF THE MACHINE AND WALKED APPROX. 20' BEFORE COLLAPPING.
MINER RECEIVED BLOW TO RIGHT SIDE OF FOREHEAD WHEN HYDRAULICALLY POWERED DRILL PROPELLED BOLT WRENCH OUT OF THE DRILL HEAD SOCKET. HE DIED FIVE DAYS LATER.
