Employee was backing away from dryer crusher and their elbow got caught on the clamshell door and when it did it put their wrist in a bind around the control lever guard of the walk behind skid steer they were operating.
Employee's finger was crushed while attempting to remove a piece of rubber from a conveyor
A small grass fire started at the Quarry due to unknown reasons. There where no employees working at the time of the fire and Local fire department extinguished the fire after being called by an individual passing by. Quarry employee was called in to establish a fire break in case of re ignition.
While attempting to cut a conveyer belt to replace it. the Cutting wheel a Maintenance Tech was using jumped forward causing it to strike there thumb.
On 9/22/2025 While attempting to open a door that was stuck employee felt a pain in their elbow. Employee chose not to be evaluated at the time and do first aid measures on their own. On 10/27/2025 employee went to the clinic was seen by a Doctor who diagnosed employee with Tennis elbow and prescribed a steroid pack.
Employee was using a poker bar during a gypsum starvation when material shifted pinching left hand between poker bar and bin chute. Employee went to the doctor on 9/15 was released to full work duty but was later prescribed an anti inflammatory on 9/22.
A fire developed under a alternative fuel (shredded tires) conveyor due to a piece of rebar accidently being added into hoppers and causing friction at the scavenger conveyor.
Employee was walking down incline and slipped but did not fall. Employee then felt back pain.
While commissioning a tire chip rotary feeder, the fire protection system detection wire burned up, causing the system to shut down on interlocks. Employee in the area reported hearing a loud pop, feeling heat, and then equipment on the tire feeding system shut down. No injuries or damage to equipment.
Maintenance employee was using a pipe wrench to loosen a drain plug when they felt a strain in their shoulder.
An employee lifted a bucket of dust to top of handrail and as they lowered the bucket of material, the employee felt a pull or strain in their right shoulder. First aid until 3/14/2025 when they received light duty and script meds.

Employee was cleaning in the plant and realized that they had stopped sweating and felt ill.
Maintenance employee was lifting a piece of shaft stock with the help of another technician.
EE suffered a bicep strain while descending down a fixed ladder
EE states they were using a poke bar in a silo to break up chunks of material. EE had shoulder pain and was treated as first aid until 07/01/2024 until they were diagnosed with a collar bone fracture.
Employee cut their left ear on a piece of metal while cleaning in the area, resulting in 7 stitches.
On 2/27/24 EE was taking 2" valve loose from water line pipe wrenches. In process of loosening the valve, the employee felt their left shoulder pop. This was a First aid until 3/12/23 when seen by Dr, given meds/physical therapy.
EE was using a bar to clear blockage, a large chunk of material fell from within the silo striking the bar. Bar struck EE in the face, causing laceration in mouth. Prescription antibiotic given at ER, no sutures.
Employee reported to work and began complaining of chest pains. Employee was diagnosed with esophageal spasms, not work related. Spoke to Norman, OK Field office supervisor who advised to complete 7000-1.

Employee was about to take first step down stairs, a puff of dust went across EE's face. As EE turned their head to cough, EE misstepped or missed it completely and landed on the second step with their left foot.
Quarry employee reports was putting rollers on, bent over, slipped, and twisted their back causing pain to their lower back. Injury was considered a first aid until 2/20/2023 when employee was given a Prescription for 10mg cyclobenzaprine and 20mg prednisone.

Material/hot gasses exited the feed hood and burned the supervisor in the face/hand and also projected dust into the supervisor's eyes. Supervisor was wearing standard safety glasses and cold weather hood and hard hat. Supervisor was taken for medical treatment.
Fatality occurred at Ada plant site from a fall from raw silos. Cause is undetermined at this time.
Employee was walking from the brick shed to the garage through the west entry door. While shutting door, the employee pinched their right index finger in the door jam and door.
Operator backed CAT D9T dozer over 38ft highwall. Operator suffered separated shoulder, bruises, etc.
Maint tech injured shoulder while lifting a bucket onto a bucket elevator. Injury was treated with first aid (ice, otc) until 2/23/2022 when physical therapy was prescribed.

Crew was lowering material down to the employee with a bucket and a rope. The handle of the bucket broke and fell, bouncing off of the wall and striking the employee. When the bucket struck EE, it knocked EE off of a platform causing EE to fall on a pole.
Maint. employee strained right shoulder while pulling on a piece of belt conveyor while it was being changed. Given muscle relaxers and placed on light duty.
Warehouse tech was lifting a roll of belt skirting and as they set it down, it bounced, causing their left pinky to become smashed and dislocated.
Mechanic changing starter on vac truck. Starter fell and crushed left pinky finger between frame and starter. Required splint, antibiotics, and restricted duty.
Quarry technician was using air lance to clean out a grizzly. Employee felt minor discomfort in lower back until 5/23/21 when seen by a doctor and placed on restricted duty and given meds.
Employee was removing a dust collector bag, it became stuck, as EE tugged harder, had a pop in shoulder. Self-care measures did not help, seen by dr on 1/11/2021, injection and restricted duty.

Employee was changing a 2" ball valve and reported that they felt a slight pain on the right part of their back. Self care measures were not effective and on 3/24/2020 employee was seen by physician and prescribed light duty and anti inflammatory/muscle relaxers.
EE was cutting a hole in a piece of belt. Knife slipped and cut palm of right hand, requiring sutures.

Maintenance EE installing lagging on a head pulley. EE lying between belts on left side. EE stated that when they went to crawl out, they noticed pain in the right shoulder between the top of the shoulder and neck. As of Dec 9, 2019, the EE was still experiencing pain, went to dr, EE arm/shoulder was put in a sling, and was given prescription meds.
Employees were working to unplug the gas conditioning tower. Employee got dust in EE's eyes, diagnosed with scratched right cornea, given eye drops.
Employee was moving a 12' jackhammer bar during a downtime. As the employee was moving the bar, the tail end of the bar struck dryer crusher, which caused the employee's left wrist to be bent sideways sharply. Treated with OTC meds until not getting better. Brace and prescription meds given 8/27/2019.
Independent Contractor Driver accessed flatbed trailer while away from tarping station. Driver lost footing and fell to the ground from top of bags on flatbed cement trailer.
Maint employees were removing a large cracked bearing from the dryer crusher shaft. As the outer race was moving outward, it released suddenly, and rotated. Due to its damage, the ee's hand was caught between the race and shaft, which resulted in a fractured left ring/pinky finger.
Employee walking on silo roof fell, at the same level, to their knees fracturing left patella. While walking employee leaned down to pick up plastic debris. Did not notice it was wired to heavier object. As they grabbed it and continued walking the wire tightened, lost their balance, fell forward onto knees.
Lab EE slipped/fell in muddy area between the baghouse and MCC. The EE fell and landed on left side. At the time of the incident, EE said they had no injury from fall. Later the EE developed left shoulder pain and was given prescription meds and physical therapy on 1/18/19.
Employee was diagnosed with Carpal Tunnel on both wrists.

Employee was rolling a CO2 bottle using a cart dolly down an incline ramp by the preheater elevator when EE slipped and landed on EE's right knee and shoulder. Shoulder did not get better over time, and doctor placed EE in sling/light duty on 11/29/2018.
While attempting to remove a mill liner bolt, using a rivet buster, the rivet buster struck the employee's right wrist while maneuvering a manlift. X-rays showed no fracture, but prescription given on 10/12/18.
Employees were attempting to pick up a roll of 3/4" air hose and one employee suffered a lower back strain.
The employee was holding a bolt with a 1 5/8 inch wrench while another employee was torquing the nut. The wrench the employee was holding on the bolt slipped off as the nut was started to get torqued. When the wrench slipped off it pinched a finger between it and the beam they were bolting.
Employee was adjusting a saw horse that had a lever and could be moved up and down. EE reports that EE jerked on it because it was stuck and when EE did EE felt a sharp pain in EE's upper back between shoulder blades. EE reports that after about an hour the pain become constant and burning and that is when EE reported to EE's supervisor.
Employee was working inside a silo on an engineered scaffold platform. There was an opening between the outside of the silo wall and the scaffold platform where it was constructed around an obstruction. The employee was going to grab a tool and EE caught EE's left foot in the opening and twisted which resulted in a fracture.
Lab employee stepped backwards after grabbing the samples for raw meal and stepped into the pier of an I-beam. Employee suffered a fractured left ankle.
Electrician was using retractable box knife to remove tape from motor lead. Electrician cut palm of right hand, requiring 9 stitches.

Employee walking in muddy area slipped and fell, hurting left shoulder. Given meds and restricted duty.
Employee was using a slide hammer while unplugging nozzles and felt a pop in employee's right wrist. Diagnosed with sprain, given meds/restricted duty.
While removing a hopper from under a silo the employee's finger became caught in between a scaffold pole and the hopper causing a dislocated finger.
While unloading tires from a 53 ft trailer to the shredder, the employee felt a growing pain in EE's abdominal area. When inspected area employee notice a bump inside EE's navel. Employee was seen by doctor and was diagnosed with a umbilical Hernia without Obstruction and without Gangrene. Return to work that evening with Limits of 15 pound
Employee was giving a truck driver directions while on the pack house dock. The employee stepped down from the 3ft, 3inch high dock, lost EE's balance when EE's feet hit the ground, and fell onto EE's elbows and rolled over, impacting EE's right shoulder. EE was treated with first aid until 7/26/17 when EE was placed on restricted and given meds.
Employee was lowering jackhammer to ground from L6 platform in shale bin area. The jackhammer slipped and the employee caught the tool in order to keep it from striking the ground. EE suffered a strained left elbow. EE was treated with first aid until ee was given cortisone shot and meds on 7/27/17.
Employee was lifting a truck tire/wheel to reinstall on pickup truck and felt a pop in EE's back.
JRAY truck driver was using SafeRack loading platform. EE raised the platform using the lever instead of the rope and pinched finger between the lever and a rock straining basket that was sitting on the platform. Two fingers on hand required stitches.
Employee descending stairs, and missed last stair. Employee was using 3 points of contact and caught self instead of falling. EE jarred right shoulder/bicep in the process. Torn right bicep diagnosed on 7/19/17.
Employee was carrying belt block up stairs in Feb. Employee was fine until May, when employee went to personal doctor, who said employee had a hernia stemming from carrying the blocks in Feb.

Employee was sitting while working on conduit. When EE went to get up, back seized and employee went down where EE was standing. Employee had a preexisting condition. At the time of the incident employee was not engaged in any activity that would aggravate pre-existing condition.
The employee stated that while the employee & co-worker where trying to lift a wire spool with a 2" in diameter piece of conduit, the conditions where wet and rainy. This resulted in the 2" conduit slipping from employee's hand, causing right index finger to be pinched in between the conduit and the jackstand.
Employee was grinding a weld using a 9 inch grinder when the grinder kicked back and struck employee across right leg on the shin requiring 15 staples to close.
Employee stumbled forward. As EE was falling, EE grabbed onto a mixer to catch self and strained right shoulder.
Employee was changing dust collector bags. Employee's goggles shifted and caused dust to get into left eye, scratching it. Employee was given antibiotics and pain drops.
Employee was using a Metabo grinder with a 6 inch cutoff wheel to cut a piece of metal on the kiln shell. The Metabo kicked back and cut the back of left hand above ring finger requiring 3 stitches.
Employee was climbing stairs to silence an alarm. The employee tripped/missed stair and fell. Employee was diagnosed with dislocated right hip.
Employee was in the process of aligning the bolt hole with a spud tool to install a bolt when the beam rolled and mashed right thumb between the spud tool and the flange of the beam.
Employee is employed as a welders helper. EE was wearing all PPE, including safety glasses, face shield, gloves, and fire resistant long sleeves to perform a grinding task. When they lifted up the face shield to see their progress, dust particles that had built up on the halo fell behind their safety glasses and into Rt. eye.

Employee was in equipment trailer and cut hand on opened box knife while reaching up onto shelf. The knife blade had been improperly installed causing the blade to remain exposed approx. 3/8" even when closed position.
Employee was helping another employee remove a 6 foot long piece of channel steel from a crate. When the other employee set his end on the ground, the channel on the injured employee's end got caught on the crate. As the employee attempted to dislodge the steel, it broke free and mashed the employee's left ring finger causing a laceration requiring nine stitches.
An employee was passing a can of spray adhesive to another employee and the can was dropped and pierced and adhesive sprayed out and caused a 1-2 second flash fire.

Employee was using a prybar to strip concrete forms following a concrete pour and strained his right shoulder by over exertion from pushing and pulling on the prybar.
EE was in process of removing a follow plate from a slurry pump when the plate slipped smashing his right index finger between the plate and the pump housing resulting in a cut. The cut required five (5) sutures to close.

EE was exiting company vehicle when wind caught door slamming it on EE's middle finger of left hand causing cut which required medical treatment (e.g. 4 sutures).

EE was attempting to drive out a stuck 11/2" bolt on the East bearing of the West Secondary Crusher with a sledge hammer. During the process, a metal shard came off imbedding into his right calf. Following removal, the incision required two sutures to close.
EE was removing a follow plate from #3 West Slurry. Inside opening on plate was washed out and had sharp edge. Upon lifting, sharp edge cut through work glove and into little finger on right hand. Cut required five (5) sutures to close.

Employee was descending down Whole Tire System incline. He slipped and fell backwards. In an effort to get up, ee reached for handrails and conveyor framework. He placed his (left hand) fingers into a pinch point between the troughing idler and support bracket of framework, resulting in lacerations to his left hand ring and middle fingers.

Employee strained lower back removing tail pulley from bed of pickup. Incident originally reported as first aid. During follow-up examination with company doctor, employee referred to physical therapy on 12/8/11 due to continued discomfort.
Employee cutting a hole in liner for kiln feed pipe with lock blade knife. Lock mechanism on knife was accidentally disengaged and blade closed on top of right index finger causing laceration. Laceration required five sutures to close.
While performing preventative maintenance on #2 Kiln Induction Fan, employee's left leg struck a puddy knife that had been left between a bolt & column by another employee. Sutures required to close laceration.
Unintended rupture of 6" natural gas line. Damage to equipment and electrical components. Incident still under investigation. Cost of damage not yet determined.
EE switching rail car on track #7. Due to snow & ice build up on track, switch did not properly latch in place. As rail car passed over switch, switch kicked back striking EE. Initial medical treatment (including x-ray with negative result) provided 2/14/11. Subsequent medical evaluation performed 3/7/11 including x-ray that revealed fracture to 9th rib.

EE was working on #2 spillback door. He had completed his work and went to remove drop light when spillback door fell, catching his finger between door and edge of hopper smashing tip of right index finger. Injury required eight sutures to close.
EE was changing dust silos and was in the process of removing a blank from existing dust line. EE was removing an over tightened nut on bolt with a breaker bar. EE was shoveling clinker dust on the burner floor, when he turned to unload shovel his right knee popped. Initial diagnosis - strain. MRI on 9/14/2009 confirmed cartilage tear behind right knee.
On 7/29/09 EE reported he was experiencing back pain resulting from shoveling clinker dust on 7/27/09 within spill back area. Initial exam performed 7/29/09. Doctor prescribed physical therapy treatment during follow up visit on 8/3/09. Initial PT treatment provided 8/4/09.
EE began performing blade change on Separator #1 by removing grating from separator door. EE stepped down into separator, lost balance and grabbed grating w/left hand, causing grating to shift and fall on right hand. Resulted in laceration to right middle finger that required 4 sutures.

#1 raw mll liner replacement project. EE was using "T" handle jack hammer to remove #1 raw mill liner bolts. EE's hand slipped causing his right finger to be smashed between bit latch and jack hammer barrel. X-rays confirmed fracture to right finger between hand and first knuckle. Placed in splint and returned to work.
Employee was using a ladder to conduct inspection on #3 bucket loader. EE fell from ladder onto left shoulder, arm & wrist.
Ee was walking on concrete pad located at the base of whole tire incline to begin tire pick-up. He tripped and fell on concrete form w/stake located on west side of pad causing laceration to outside of left knee cap requiring 4 sutures.
EE was moving displaced used tires from underneath kiln when dust fell behind safety glasses and irritated his left eye. EE flushed eye with water at repair shack. Eye was irritated but no pain. Next morning EE awoke to find his left eye swollen and painful. EE was taken to ER. Eye was flushed and RX given. EE followed yo with regular eye doctor, given another RX & released.
EE was using air lance to unplug cement in feeder spout of packing machine. EE placed lance in spout and hit feeder causing lance to kick back and cut ee's right hand between thumb and index finger. Injury required 5 stitches.
EE was pulling drag chain back together to install pin when he strained his lower back. Requested medical treatment 1/21/08. Placed on light duty w/6 physical therapy treatments.
Employee was laying brick in a kiln. When employee stepped backwards to pick up another brick he turned his right ankle.

EE was replacing a rub plate in the primary feeder. He was exiting hopper/feeder area when he climbed down to the cat walk and stepped on a rock turning his left ankle. He requested medical attention 12/10/07.
EE placed equipment guard on table in order to use ballpin hammer to staighten guard. While swinging hammer with both hands, EE missed and struck his right shin with hammer, causing a contusion. EE was prescribed anit-inflamatory with further evaluation. Employer was notified on 10/2/07 that physical therapy began on 9/28/07.
EE was wiring kiln chain back with welding rods when chain broke loose striking him in the mouth, chipping front left tooth and loosening right front tooth.
EE was jack hammering mud ring from inside #1 Kiln. While working on one section another section broke loose from kiln shell, falling and striking EE on left side of body.
EE was dismounting tire trailer where he & coworker were hand throwing tires onto conveyor. EE caught right leg between tire trailer & conveyor causing EE to fall and lodge leg. EE was on ground, leg x-rayed, leg immobilized & follow up scheduled with orthopedic dr. for MRI. Placed on light duty until further notice.
EE was letting old switchmaster (trackmoblie) down on rail when machine slammed down on rail jarring EE's lower back. EE did not request medical treatment until 5/14/07. (Chiropractic adjustment)
Bank gave way 13 ft high by 14 ft wide, trench 40 ft in length. Trench box was in place 16 ft tall by 10 ft wide. Laying single 30 in pipe. Box was set for the next joint man to step in. Employee stepped out of the trench box and bank gave way resulting in an injury consisting of a broken leg.

Ee was using screwdriver to break cap off of 55 gal drum of oil. He removed his gloves and began to pry cap. Screw driver slipped and cut fingers on right and left hands. He was treated in er and released without restrictions.
On 10/3 EE was walking on walkway between finish mills when he slipped on oil causing a strain to lower back (unreported). On 10/9 he was setting mill basket on top of #2 Raw Mill. Basket shifted when he began walking on it causing him to re-strain lower back. He requested medical treatment 10/9 and is continuing follow up treatments with chiropractor.
Ee states he was walking down a flight of stairs carrying his tool box. Ee turned the corner left to access another flight of stairs when his right knee popped causing ee to fall down. Pain is to inner side of right knee. Knee was immobilized with knee brace until follow up visit.
Employee was moving welding unit and twisted back causing strain.
Employee states while performing maintenance he sustained cement dust in his left eye causing irriation. EE received prescription from examining physician with follow-up visit to ophthalmologist.
EE strained his upper back while lowering twin compressor air filters to a co-worker.

EE was underneath 50 ton haul truck changing transmission filters. Upon exiting he complained of pain in right knee. EE did not requested medical attention until 12/9/05 and return to work with no lost time. Coninued with dr follow-ups. Dr has no requested physical therapy treatments beginning 1/4/06.
EE WAS REPLACING BUCKETS ON THE CLINKER ELEVATOR WHEN HE KNOCKED BUCKET LOOSE FROM CHAIN, LODGING BUCKET BETWEEN WALL AND ANOTHER BUCKET. WHEN EE TRIED TO DISLODGE & REMOVE IT, BUCKET DROPPED AND SMASHED HIS FINGERS BETWEEN BUCKETS REQUIRING SUTURES.
EE was assisting in the recovery of used conveyor belting. While inspecting belting as it was being wenched across the ground the tangled belting untwisted and impacted EE knocking him to the ground. Injury resulted in fracture to left wrist and left leg below knee.
EE was pulling with a 48" pipe wrench trying to turn #2 clinker cooler breaker that had locked up with material when he strained his lower back. EE did not request medical attention until 9/27/05. Given prescription and placed on restricted duty. Re-check in one week.
EMPLOYEE WAS GETTING OFF OF THE #3 PIER AT THE #2 KILN. WHILE PUSHING WITH HIS ARMS, HE STRAINED HIS RT. SHOULDER & HIS RT. WRIST. EE DID NOT REQUEST MEDICAL ATTN. UNTIL 09/26/05. HE WAS GIVEN A PRESCRIPTION & PLACED ON RESTRICTED DUTY. RE-CHECK IN ONE WEEK.
EE WAS PULLING JAMMED TIRE FROM WHOLE TIRE SYSTEM BELT WHEN HE FELT A SHARP PAIN IN HIS RIGHT FOREARM AND ELBOW. HE HAS BEEN PLACED ON RESTRICTED DUTY.
Employee was pulling a clinker sample from the manufacturing process when his dipper hit the conduit overhead causing cement dust to fall into both eyes. Employee was given prescription and released.

HE WAS KNEELED IN A 3 1/2' TALL CLINKER COOLER AND LIFTED A 30 LB GRATE WHEN HE HURT HIS BACK. HE WENT TO THE DOCTOR AND WAS TESTED FOR A LUMBAR STRAIN. IT NEVER HEALED. 3-6-2001 HE WENT TO TH E DOCTOR AND WAS PUT ON TEMPORARY DISABILITY. AN MRI SHOWED A BULGED DISK.
EE PULLED HIS MEASURING TAPE UP AFTER MEASURING A SILO & TURNED TO WALK AWAY WHEN HE FELT PAIN IN HIS LEFT KNEE. A PREVIOUS INJURY SET UP ARTHRITIS & DEGENERATIVE BONE. ON 2/15/01 EE WENT TO OUTPATIENT SURGERY TO HAVE THE KNEE CLEAND AND SCRAPED.

EE WAS OPERATING A FORKLIFT AND BAGGING CEMENT PRODUCT, THE NEXT MORNING, HE COULDN'T LIFT HIS LEFT ARM. THE DR. GAVE HIM THE NEXT 9 WORKDAYS OFF. INVESETIGATION CONCLUDED IT WAS CAUSED BY REP ETITIVE MOTIONS.
EMPLOYEE MOVED A PORTABLE CEMENT MIXER INTO THE BURNER FLOOR ELEVATOR. AS HE MOVED IT OUT, THE UPRIGHT LID FELL ON HIS LEFT HAND AND CUT IT. HE RECEIVED ONE STITCH.

EE WAS RATCHETING HEAD BOLTS THAT WERE ABOUT 5-6' HIGH. THAT EVENING HE WAS SORE BUT THOUGHT IT WOULD GET BETTER. ON 8-12-99 HE WENT TO DR. AND WAS TREATED. ON 12-22-99, HE WENT FOR THOROUGH E XAM, DIAGNOSE WITH IMPINGED RIGHT SHOUDLER. SCHEDULED FOR SURGERY 1-27-2000.

EE WAS UNHOOKING A WINCH TRUCK CABLE HOOK, HE SAW SLACK IN THE LINE AND PLACED HIS HAND ON THE CABLE TO PULL IT OUT AND THE CABLE RETRACTED PINCHING/SEVERING THE END JOINTS OF MIDDLE 3 FINGERS LEFT HAND.

EE TWISTED AS HE GOT OUT OF THE PICK UP AND STRAINED HIS LOWER BACK. THIS IS A REPEAT OF A 7-21-98 BACK INJURY WITH THE ORIGNIAL DATE OF INJURY BEING 5-6-89.

ON 5/12/97 EE WAS DRIVING A HAUL TRUCK & AS A RESULT OF THE BUMPY [THIS INJURY WAS INITIALLY TREATED AS A NON-OCCUPATIONAL HEALTH INJURY] ROAD/RIDE, HE DEVELOPED PAIN IN HIS NECK, BACK & SHOUL DER. ON 1/22/98 THE WORKERS' COMP COURT JUDGE ISSUED THIS A TEMPORARY DISABILITY.

TWO MONTHS AFTER HE REPORTE D INJURY TO THE SHOULDER AND DIDN'T KNOW WHEN ORWHERE IT OCCURRED! ON AUGUST 7, EMPLOYEE HAD SURGERY ON HIS RIGHT SHOULDER AND REMAINS OFF WORK.

EE BENET OVER TO PICK UP A PIECE OF CABLE LAYING ON THE GROUND. WHEN HE DID HE FELT A SHARP PAIN IN HIS LOWER BACK. HE THOUGHT HE WALKED IT OFF BUT COULD NOT STRAIGHTEN UP. ON 3-25 HE CAME T O WORK AND WENT TO THE DR. LUNBAR STRAIN.
HE WAS JACKHAMMERING CLEAN A SEVERED SCREW CONVEYOR. HE HAD THE UNIT LOCKED OUT AND WAS WEARING PROPER SAFETY SHOES, HAT AND EYEGLASSES. AS HE GUG OUT THE CONVEYOR, IT SHIFTED DUE TO CLEARANCE AND SLID DOWN THE SIDE OF THE MAN'S LEFT FOOT. HE SUFFERED A CONTUSION TO THE FOOT AND REMAINS OFF WORK.
HE WAS GRINDING USING AN AIR POWERED PORTABLE GRINDER. HE APPARENTLY APPLIED TOO MUCH PRESSURE AND THE HEAD SHREDDED AND PIECES FLEW APART AND ONE STRUCK HIM ON THE TRIGGER FINGER. HE WAS WEAR ING A PAIR OF COTTON GLOVES. THE LACERATION ON THE KNUCKLE REQUIRED STITCHES.

WHILE CLOSING A VALVE ABOVE AN AIR COMPRESSOR, EE FELL AND HI HIS RIGHT EAR ON THE CORNER OF A DOLLY AND LACERATED HIS EAR.
EE WAS REPLACING A 6 INCH SHAFT WHICH WAS SUSPENDED FROM A STEEL FRAME WITH A COMALONG. THE STEEL FRAME HAD A WELDON IT THAT BROKE WHILE LOWERING THE SHAFT IN POSITION THIS METHOD HAD BEEN USE D BEFORE. THIS ACCIDENT CAUSE A FRACTURE TO THE HAND WHEN THE SHAFT FELL ONTO HIS HAND WHEN THE COMEALONG PULLED THROUGH THE STEEL STRUCTURE.
EMPLOYEE WAS APPLYING PRESSURE WITH A POCKET KNIFE DOWN A PIECE OF NYLON ROPE WHICH WAS HELD IN HIS LEFT HAND. WHEN THE KNIFE SLICED THROUGH, HE CUT THE TENDON ON HIS LEFT THUMB WHICH WAS DIRE CTLY BENEATH THE KNIFE BLADE.
EMPLOYEE WAS SHOOTING A RING (BUILD-UP) OUT OF A KILN PORT WINDOW. THE KILN HOOD PULLED POSITIVE AND BLEW DUST INTO HIS EYE WHILE HE WAS WEARING SAFETY GLASSES.
EMPLOYEES WERE PASSING BRICK ANOTHER EMPLOYEE DROPPED BRICK ON HIS FINGER.
EMPLOYEE & CO-WORKER WERE TAKING DOWN A SCAFFOLD. EMPLOYEE PULLED THE PIN ON ONE SECTION & THE ADJACENT SECTION FELL INWARD PINCHING THE TIP OF HIS RIGHT THIRD FINGER - STITCHES WERE REQUIRED & THE FINGER NAIL REMOVED.
EMPLOYEE WAS WEARING A NEW PAIR OF BOOTS WHEN HE TRIPPED AND FELL OVER THE SIDEWALK CURB. HE SUFFERED FROM A LOWER BACK PAIN THREE DAYS LATER AND WAS HOSPITALIZED ON 1-9-95. EMPLOYEE HAD SURGE RY FOR A BULGED DISK ON 1-20-95. HE WILL LIKELY BE OFF WORK FOR 3 MONTHS. EMPLOYEE HAD A TWO YEAR MEDICAL HISTORY FOR TREATMENT OF A DEGENERATIVE DISK.

EE STATED THE PUMP & WHILE ATTEMPTING TO GET THE CEMENT TO FLOW FROM THE BIN TO THE PUMP HE TAPPED ON THE 16IN METAL CHUTE WHEN HE DID CEMENT FELL DOWN THE CHUTE TO THE PUMP CAUSING A 11/4IN C AP TO COME OFF THE INSPECTION SPOUT HE CUT HIS HAND OVER THE HOLE BUT THE FORCE WAS TOO GREAT THE CEMENT FLOWED INTO THE RT SIDE OF HIS FACE & INTO HIS RT EYE
