Demopolis Plant Cemex Inc Metal/Non-Metal

Employee was walking through the clinker storage shed and while doing so walked across a spot of unlevel ground and slipped on some loose clinker, folding EE's right ankle underneath EE and landing on EE's right shoulder causing pain to both the ankle and shoulder. EE's right fibula was fractured.

Employee was troubleshooting issues with material flow at a cement finishing product mill. During this process the employee opened an inspection door to an air slide and when EE opened the door a flush of material came out and got in EE's eye.

While working on the Preheat Tower, some material/dust particles fell from above and got in the employee's left eye.

Employee was working on troubleshooting and replacing damaged bags on a dust collector. As EE was walking around the dust collector, EE struck an air receiver with left shin causing a gash that required stitches to close.

Employee was repairing rotary feeders that were out of service and while inspecting the system EE stepped backwards and stepped off a small elevated work platform. When EE stepped backwards and felt self lose EE's balance EE reached EE's right arm out to brace self on a handrail and this is when EE felt discomfort in EE's right shoulder.

Employee stepped on rock while checking tailgate of dump truck before leaving site.

Employee was assisting with the removal of an air cannon nozzle and pipe it was installed to from the raw mill triple gate. After the pipe had been removed from the triple gate and was being lowered to the ground with a come-a-long, the pipe shifted and struck the employee's hand in between the pipe and a hand rail.

On 02/14/2023 EE was assisting staging mill liners at #1FM when they felt a strain in right shoulder. Initially EE reported that shoulder was not bothering them badly and choose self treatment. On 2/27/2023 EE reported shoulder was not better and requested medical attention. The doctor placed them on work restrictions and physical therapy to aid recovery.

Employee was assisting in repairs to a conveyor that had torn in two. While working to bring the two pieces together by using a winch cable and belt clamps the employee manually maneuvered the belt and it was during this time they felt momentary pain in left shoulder. The pain went away and the employee continued working. They felt extreme pain the next morning when they woke up.

Employee was using a bar to remove an obstruction in the tire chute when the bar slipped as EE was pulling upwards, striking EE in the mouth. This resulted in a laceration to the lower lip that required four stitches.

Coworker struck employee's hand while working together to install refractory

Maintenance mechanic lacerated right pinky finger while attempting to manually maneuver a damper at the kiln main baghouse with a pipe wrench. The pipe wrench slipped and pinched EE's finger. The laceration required two stitches to close the wound.

On 1/5/22 employee missed step while travelling down flight of stairs and landed awkwardly on left knee. The incident was treated as a first aid at that time. After several days the employee said that the knee was continuing to hurt so the local clinic ordered an MRI to be taken. On 1/27/22 it was determined that the employee had a tear in the meniscus thus making this an MTI.

Employee was preforming physical cement testing inside the lab. During this time the employee was removing a stud from an expansion bar. The stud became stuck so the employee decided to use a screwdriver to help remove it. It was at this time that the screwdriver slipped and punctured employee's right hand. The wound was thoroughly cleaned and a steri strip was applied.

While exchanging oxygen and acetylene bottles the employee had an oxygen bottle tip over and fall on employee's right foot.

Employee was jogging a material conveyor screw and while performing this task material blew in the employees direction and got into EE's right eye.

Employee was working in the area around the plant level box. While working EE went to cross over an air slide. While in the process of crossing over the air slide the employee fell landing on their right shoulder.

Employee was using a pry bar to remove a gearbox from a shaft. While using the pry bar it slipped and EE's left ring finger was pinched between the bar and the frame of the equipment being repaired. Employee received a laceration to the finger that required seven stitches to close.

Employee was manually inspecting a rotary feeder that had just returned from the repair shop. The rotary feeder was not hooked up to any power source but was being turned by employee by hand. While turning rotary feeder EE's right middle finger got caught between a vane and the housing. Employee was taken to ER where EE's fingernail was removed and EE received three stitches.

A Transport owner/operator truck driver was on site to be loaded with cement when the driver fell while entering the cab of truck. Driver sustained a fractured wrist.

Employee had a reaction resulting in a rash to the arms, legs, feet, stomach, and torso areas of the body after handling ceramic fiber blanket material.

Employee was pressing pins at clinker drag when ee strained back while handling the press. While the incident occurred on 03/09/2019 it was treated as a first aid because the employee did not want to pursue further treatment at the time. On 05/14/2019 following a visit to a back specialist, employee was prescribed physical therapy to aid in recovery.

Employee was cutting with a torch on the preheater tower when some hot material fell from where ee was cutting and got caught in shirt sleeve, causing second degree burns to left forearm area.

Worker was spot welding expanded metal to the walls of raw mill duct without the use of a welding hood, resulting in a flash burn to the left eye.

Employee was removing support steel and had attempted to push the demolished steel over. The steel caught the employee, pulling them to the ground.

Employee received a contusion to the right hand while working to remove a chute from off of a platform.

EE was passing material inside a vessel when material dislodged and struck EE in the head. EE was wearing a hardhat and using all proper PPE for the designated work. The material caused a small laceration and received medical treatment that resulted in 5 staples to close the injury. EE was released and returned to work the next shift.

Injured Employee was using a vacuum hose to vacuum up waste cement dust. As they were vacuuming, debris became lodged in the vacuum hose causing the hose to "kick back" causing the Employee to lose control of the wooden handle used to control the vacuum hose. The wooden handle struck the Employee in the left side partially rupturing the spleen.

When employee went to take a step they felt a pain in their back.

EE was putting holes in a trash can for drainage with a pickaxe. EE finger got caught in between the handle of the pickaxe and the metal trash can. Due to the force of swing it fractured ee left index finger right below the fingernail.

Supervisor was helping another employee when hot material from the coal mill take off shot out at a distance of 12 ft striking the supervisor on the back of the neck and ear. This caused 2nd degree burns. EE was treated at UAB and released back to full duty with no restrictions.

Installing metal floor grating on the raw mill, witness was sliding grate into position and didn't realize employee that was helping guide had hand in a pinch or mashed position. Causing the grate to mash left hand between the grate and floor frame.

While the employee was using a grinder on some metal, EE felt something go in left eye under the proper PPE. They carried EE and flushed eye, but EE had a corneal abrasion and they gave EE some antibiotics for it.

Employee was grinding on a metal hopper. Wearing the proper PPE (safety glass and full face shield) employee removed the face shield and a piece of metal shaving fell under safety glasses in right eye.

Employee was assisting replacing burner piping in Kiln. While sliding burner sleeve into position, the lever hoist (come-a-long) chain broke and struck employee across the head.

EE was hammering on a steel sprocket when EE stated a shard had struck EE in the arm. The cut was very small. EE was taken to the work comp doctors office where they advised the shard was to tiny to remove. They then sent EE back to work. EE did receive one stitch.

On the above date, Mechanical Maintenance laborer was blowing dust from the 58 Dust collector. The door was open on the dust collector and when the dampener opened the draft closed the door on the employee's hand. The employee received a slight fracture to hand and was released by doctor to return to work same day.

While walking down a slight incline in the clinker shed, a Production employee slipped and fell backwards. As he fell, he bruised his wrist when he landed on it while attempting to catch himself. Wrist is bruised. Doctor prescribed ibuprofen and work restriction to not lift heavy objects for 1 wk.

Emp lost grip on ladder between 2nd/3rd rung, fell approx 2 1/2 to 3 ft onto floor. Right knee hit floor. No medical treatment needed, no prescription, X-ray negative.

During wear plate replacement; old plate slipped scraping employee's lower legs.

EE was injured when trolley style manual chain hoist fell during coal mill roll replacement and chains struck him in the lower back. EE was x-ray'd with no impairments found. EE was seen by doctor and pain RX's prescribed and removed him from work for one week due to soreness and inflammation. EE in PT.

EE was working in hot area sweeping down tubes with hot materials inside the tubes. Before lunch break, EE began to experience cramps in arms and legs. He told supervisor who contacted cleaning manager/safety manager. Wet cloths were applied to back of neck and Gatorade given. He was taken in ambulance to local hospital treated for dehydration and released.

While walking from barge office to the barge bins, employee slipped in a muddy spot and his left arm extended up behind his head as he attempted to maintain his balance. (He did not fall). Employee experienced dull pain in his shoulder which was diagnosed in late August 2013 as a rotator cuff tear. Employee will undergo surgery for repair.

As the affected employee was climbing onto the 990 Loader he felt his right knee pop when he transitioned from the first step up to the second step. Employee felt pain and wanted to go to the doctor for evaluation. Doctor did an X-Ray which revealed fluid on the employee's knee.

While employee was reaching through a handrail to tighten a u bolt, the ratchet slipped from the nut causing his right wrist to strike against a sharp edge on uni strut causing a laceration. Employee received four sutures.

Employee accessed the manlift, high heat and humidity, his left hand slipped from top rail of manlift, when his hand caught on the second rail, he had pain in his left thumb. Employee reported to company however did not want medical attention. On 10/23/12 he was seen by Dr whom after an Xray determined his left thumb was dislocated.

Employee was drilling for power shot placement in the chalk tank and felt his right shoulder pop and experienced pain. He went to the company doctor and had follow up visits. Employee communicated that his arm has continued to have irritation and pain. Employee has torn rotator cuff and had surgery on 03/04/2014.

Employee was assisting in offloading an I beam from a forklift when his left hand glove was caught between the fork of the forklift and the I beam causing a laceration to his left pointer finger. Employee received sutures. No restricted duty.

On 6/29/11 while closing caps on the Kiln Trunion Rolls employee felt back pain. On 7-19-11 working on the four hole wear plate at the Roller Mill, the pain in his lower back became worse. ON 7/25/11, physician states that EE has damage to his SI joint, recommended restricted duty, physical therapy and medication.

EE was pushing a wheelbarrow full of cement dust. Was wearing all PPE including safety glasses. Turned wheelbarrow over to dump dust into bobcat, dust came up and entered his left eye. Taken to first aid office where washed eye and placed eye drops into eye, same procedure at 2:30pm on 6/9/11. EE came into off with his eye swelled, red and almost shut, doc appt was made.

Tumblers wedged between wear plates of finishing mill. As employee hit wear plate with sledge hammer, small piece of metal tumbler was ejected striking him in the shin area. Metal was imbedded in leg. Required removal and stitches. Released to full duty.

Employee was preparing to splice a conveyor belt, while kneeling he turned to the side and felt pain in his lower back. Was sent for medical attention on 2/22/11 place on restricted duty with physical therapy.

Employee was working with his crew at the bottom of the pre-heater tower north end area. He was cleaning build-up material. Cemex employee was driving a Bobcat towards him & his crew to fill bucket with material. Once bucket was full, the operator leveled the bucket flush with the ground. Employee's foot caught under the bucket. He screamed to get bucket lifted. Taken to doctor.

Employee sitting long time bent over causing pain in back. On 2/7/11 employee attended physician, placed on restricted duty ordered physical therapy.

Employee lifting dust collector bags, felt pain in shoulder. Physician placed on restricted duty - no overhead lifting left shoulder.

Employee turned from exiting preheater tower elevator when debris flew into his left eye. He was wearing safety glasses however wind was blowing at the time. Doctor removed debris and gave a prescription.

EE was cleaning the rocks and dust off the band. The work he was doing was above his head. He began to experience a hard pain in his chest. After break he worked on the 7th floor when he had same sharp pain, air shortage and felt his legs go numb. At this point he sat down. Employee asked him if he was OK & took him to 1st Aid. Ambulance called & he was taken to hospital.

Employee touched his left knee to the side of the finish mill causing a burn. Visited Dr. with restricted duty and prescription. On 5/18/10 employee was admitted into hospital for infection.

Injury suffered a lifting injury to his left arm while attempting to service the sweeping machine he was operating. He attempted to raise and latch the rear door of the sweeper containment compartment in order to dislodge stuck swept material in the compartment.

Employee lifting metal out of chute when felt pain in right shoulder. Shoulder began hurting again in July sent for MRI placed on restricted duty on 5/11/10.

EE using an air lance to blow material off of kiln Dizr, felt pain in lower back. Place on restricted duty no lifting over 20 pounds.

EE was lifting a 5 to 10 trash can over into dumpster when he felt pain in his right shoulder radiating down to elbow. EE reported, however after continued pain went to the physician on 1/26/10 at which time he was placed on restricted duty for three weeks.

While ee was using air lance to clean out feed end/seal housing, finger was pinched between air lance and door frame. Left pinkie finger had to have 3 stitches.

While using a metal pry bar to attempt to loosen build up of material ee injured his left shoulder. Doctor diagnosed inflammation of tendons and placed on restricted duty.

Employee was lifting a come-along over his head when he felt pain in his right shoulder. Dr diagnosed as impingement syndrome and requested light duty for three weeks.

EE felt pain and numbness in both wrists and hands. Dr diagnosed with carpal tunnel and placed on restricted duty.

EE climbed inside 49 Fan housing lowered his body holding onto blade and fell pain in left shoulder.

While installing a light fixture ee set a transformer on the handrail of the manlift and caught tip of finger between transformer and the rail. EE went to Dr. on 08/10/2009 discovered fractured finger.

EE was in a manlift attempting to tighten bolts on the roller mill. He felt a strain to his back.

Employee was removing bolts from floor grating in cooler with a cutting torch. The grating and bolts were in an overhead position and one of the "hot bolts" fell into the welding glove. The burn was about two inches below the left thumb on the wrist. The bolt was in a corner and access was difficult to make the cut. Movement was restricted and therefore reaction time was slower.

Wind blew particle into employee's eye. He was wearing safety glasses and face shield. Doctor prescribed antibiotic eye rinse after removing particle.

Employee caught glove between sandpaper and shaft while sanding a shaft with a lathe, The sudden motion severed the tip of his right thumb.

Employee was pulling a cable over his right shoulder when he felt a pull in shoulder. The employee visited the doctor with no limitations. After continuous pain the employee was refered to a specialist who put him on light duty on 9-1-08.

Employees were sweeping cement dust, EE bumped a wall that had a lot of dust on it. The dust fell on him and he inhaled some of the dust.

Employee waslking down preheater steps & missed a step causing pain in knee. Tried physical therapy. Became a lost time when employee had surgery April 1, 2008. First day off April 1, 2008.

Employee mashed left hand ring finger while opening a door on screw conveyor. Fractured tip of finger, is on light duty.

Employee bent down to lift an idler and felt a pull in his back. He has a pulled muscle and placed on light duty.

EE was attempting to move a forklift from the vacinity where a duckwork was being moved by a crane. EE's left forearm was caught in a pinch point between the forklift roll cage and piece of duckwork, resulting in a compound fracture to the EE.

EE opened door to 58 dust collector and dust flew into eyes.

While cleaning plugged chute, pry bar hit employee in right shoulder causing pain. Diagnosed with mild shoulder separation.

Employee was chipping with a hammer during a welding process and a piece of metal lodged in the right eye. He was taken for medical treatment where the metal was removed and he returned to work.

Due to continuous motion injury employee underwent surgery to his right hand.

Employee was moving an electric motor when his finger was caught between the motor and the push cart the motor was sitting on. The employee was wearing gloves at the time. This resulted in a laceration requiring stitches on the right hand ring finger.

EE was shoveling cement dust to clean under a belt. He states he started having back pain from the bending and twisting.

While jogging the blower on the 08-04 FK pump, dust flew into the employee left eye.

Injured employee was assisting a maintenance employee at the tire system when the singulator moved suddenly sliding him down an angle iron frame trapping his right leg between two pieces of angle iron.

Employee reached to tie a wire on a thermal couple when he felt a catch in his left shoulder. He reported this to the employer on 5/1/06. He went to the Dr. on 5/9/06 at which time he was place on light duty. The Dr. released him to work with with no use of left shoulder.

Dump trailer raised to discharge load. Load partially adhered to dump bed causing a weight imbalance. This shifted the center of gravity to the right causing the vehicle to roll over onto its side. Driver removed seat belt & attempted to exit the vehicle at the beginning of the roll. Attempt failed causing him to fall back into the vehicle striking the door fracturing his clavicle.

The employee was cleaning a swivel chute on the chalk reclaimer when his foot slipped, causing his knee to strike against a metal beam. This caused pain and swelling in the knee area.

Employee attempting to open a door on triple flap gate when suction from air flow pulled door closed onto tip of finger (middle) on left hand. Employee put on restricted duty as 12/7/05 by doctor.

Employee was twisting fan into position on the kiln shell when he felt sharp pain in his right groin. Employee put on light duty by company doctor on 11/15/05.

Pinched finger between pry bar and hand rail. Tip of finger (pinkie- right hand) cut and needed stitches.

Employee was testing equipment in MCC085 room and equipment arced. Starter was smutted and meter was burnt. Employee received burn to back of right hand.

(EE needing surgery.) EE bent down to clean chunk of material from weigh feeder - foot slipped and pulled muscle behind left knee. EE put on light duty eff. 2/28/05.

ee was walking down sloped ramp at barge loading when he slipped, twisted his left knee. on 10/12/05 EE had knee surgery. returned to work on 10/17/2005 on light duty and will start physical therapy.

EMPLOYEE WAS LOADING FILING CABINET INTO BACK OF PICK-UP. AS HE SHOVED THE CABINET INTO THE PICK-UP HIS LEFT INDEX FINGER WAS CAUGHT IN BOTTOM DRAWER OF FILING CABINET WHICH SEVERED END OF FINGER.

Drilling hole and from above hot preheater dust blew out on upper neck and lower head burning occipital scalp light duty began 8-27-04.

Strained right wrist raising bed on gator in shop. EE put on light duty 7-13-04.

EMPLOYEE WAS CLIMBING LADDER ON THE PRE-HEATER, REACHED UP AND FELT SOMETHING PULL IN HIS LEFT SHOULDER. DOCTOR'S FINDINGS: IMPINGEMENT SYNDROME AND PARESTHESIA. EMPLOYEE ORDERED PHYSICAL THERAPY ON 8/5/04.

EE HAD BEEN SQUATTING DOWN USING OX-ACETYLENE TORCH. WHEN HE STOOD UP HE FELT SHARP PAIN IN RIGHT KNEE. KNEE BEGAN TO SWELL AND EE FELT A LOT OF PAIN. EE WAS PUT ON LIGHT DUTY BY DOCTOR.

EE WAS BENDING OVER (DOWN) TO CHECK STARTER ON MCC. FELT PAIN IN LEFT KNEE. EE HAVING KNEE SURGERY 4/19/04.

FLAMMABLE VAPORS FROM A HYDRAULIC RESERVIOR ACCUMULATED IN THE TRUNNION GUARD. IGNITION WAS CAUSED BY CUTTING/WELDING IN THE AREA. HYDRAULIC OIL WAS CONTAINED BY A FLAMMABLE SOLVENT.

PICKING UP GRINDER AT WORK - FELT PULL IN LEFT ELBOW. EE PUT ON LIGHT DUTY 10/17/03 AND PHYSICAL THERAPY.

WHILE THE EE WAS WORKING ON A GRATE, HE DROPPED A BAR, THAT BOUNCED UP AND STRUCK HIM IN THE CHIN. THIS IS A LACERATION TO HIS CHIN.

EE WAS OBSERVING ANOTHER EE WORK, HE BENT DOWN TO CHECK THE PROGRESS OF THE EE AND WHEN HE TRIED TO STAND UP, HE COULDN'T. WAS PUT ON LIGHT DUTY UNTIL 4/21/03 THEN WAS TAKEN OFF WORK. THIS WAS FIRST DAY OF LOST TIME.

EE WAS USING A SHOVEL TO CLEAN SPILLED CLINKER AND SUSTAINED A ROTATOR CLUFF INJURY IN HIS LEFT SHOULDER.

WHILE EE WAS STEPPING FROM A BARGE TO THE RAMP, HE SUSTAINED A PULLED MUSCLE IN HIS LOWER BACK AND WAS PLACED ON LIGHT DUTY FOR 3 DAYS.

EE WAS USING THE 980 FEL TO CLEAN THE CHALK RECLAIMER. THE EE WAS ON THE GROUND AT THE ACCESS LADDER ON THE 980. THE EE STRAINED A MUSCLE IN HIS BACK AS HE PULLED HIMSELF FROM THE GROSTEP ON T HE LOADER.

WHILE PAINTING IN THE FINISH MILL, EMPLOYEE DEVELOPED A RASH ON THE BACK OF HIS NECK AND UPPER BACK.

THE OUTSIDE CONCRETE FLOOR HAD A THIN LAYER OF RAW DUST ON IT. A RECENT RAIN CAUSED THE FLOOR TO BECOME SLICK. AS THE EE WALKED ACROSS THE FLOOR HE SLIPPED ON A WET SPOT AND FELL, DISLOCATING HIS RIGHT SHOULDER.

EE WAS SCREWING AN ATTACHMENT INTO A SPRING. THE SPRING BROKE, FLYING BACK CUTTING THE EE LEFT MIDDLE KNUCKLE. HE HAD 3 STITCHES.

EE IS THE ELECTRICAL SUPERVISOR FOR THE DEMOPOLIS PLANT. THERE ARE SEVERAL AREAS IN THE PLANT WITH NOISE LEVELS ABOVE 85 DBA. SOMETIMES THE ELECTRICIANS DO WORK IN THESE AREAS.

EMPLOYEE IS A REPAIRMAN AT THE PLANT. THERE ARE SEVERAL AREAS IN THE PLANT WITH NOISE LEVELS ABOVE 85 DBA. SOMETIMES THE REPAIRMEN DO WORK IN THESE AREAS.

THE EE WAS HITTING ON A PIECE OF PIPE & HE FELT PAIN IN BOTH OF HIS HANDS & ARMS. LOST TIME STARTED ON 08/11/02 WITH SURGERY ON HIS RIGHT WRIST. (2 DAYS LOST.) LOST TIME STARTED ON 8/30/02 WIT H SURGERY ON HIS LEFT HAND. (1 DAY LOST.)

EE WAS USING A LONG BAR TO PUSH BUILD-UP OFF THE BURNER PIPE, PIPE SLIPPED INJURING HIS LOWER BACK.

THE EE STEPPED OVER A PIECE OF METAL AND TWISTED HIS RIGHT KNEE.

EE WAS CLEANING THE XXXX MATERIAL BUILDUP ON THECHALK STACKER. THE EE WAS ASCENDING A CAGED LADDER FROM THE TURNTABLE TO THE BOOM. THE TOP SUPPORT ON THE LADDER BROKE CAUSING THE LADDER TO LEAN OUT. THE EE HIT HIS BACK ON THE INSIDE OF THE CAGE WHILE HOLDING ON TO THE LADDER. STARTED PHYSICAL THERAPY ON 11/20/01, PUT ON LIGHT DUTY 12/18/01.

EE IS AN ELECTRICIAN AT THE PLANT. THERE ARE SEVERAL AREAS IN THE PLANT WITH NOISE LEVELS ABOVE 85 DBA. SOMETIMES THE ELECTRICIANS DO WORKIN THESE AREAS.

THE WELDER WAS FLIPPING A FILLER BAR END OVER END WHEN HE FELT SOMETHING IN HIS BACK (A FILLER BAR IS A STEEL PLATE THAT A TIRE ON A CEMENT KILN RIDES ON).

EE WAS STANDING ON A BARGE ANOTHER EE HANDED HERDUST COLLECTOR HOSE: SHE DROPPED IT AND SAID HER BACK WAS HURT. THE DR SAID LOW BACK PAIN ALLOTHER TESTS NEGATIVE. SENT TO PT 11/10/00 & PUT ON LIGHT DUTY UNTIL SEEN AGAIN ON 11/16/00

AFTER EMPLOYEE DUMPED METAL OUT OF A CONTAINED, THE CONTAINER FELL ON HIS FINGER WHEN HE TRIED TO LATCH IT. IT WAS TOO CLOSE TO THE FORKLIFT. HIS LEFT RING FINGER WAS MASHED AND HAD SUTURES.

THE EE WAS TRYING TO OPEN A DOOR BY LIFTING ON IT. THE DOOR WAS ON THE ROLLER MILL AIR DUCT. HE STRAINED HIS LOWER BACK WHILE LIFTING.

EMPLOYEE WAS INT EH RPCESS OF ISNTALLING THE LIFTING BLOCK ONTO THE CABLE OF THE MOBIL CRANE. THE BLOCK TIPPED OVER STRIKING THE EMPLOYEE ON THE RIGHT FOOT, CAUSING A FRACTURE OF THE 3RD META CARPAL BONE.
