DRILLING TOP IN CONCRETE IN SLOPE PORTAL. CHUNK OF CONCRETE FELL HITTING EE ON LEFT ARM, RIGHT HAND & RIGHT WRIST.
THE CREW WAS TRYING TO REMOVE AND EXISTING TAIL SECTION FROM THE #3 CONVEYOR. THE WELDMENTS HAD BEEN BURNT, BUT THE SECTION COULD NOT BE REMOVED. THE SECTION WAS RERIGGED AND LIFTED AGAIN, THE SECTION LIFTED ON ONE SIDE. EMP WALKED TO THE REAR OF SECTION AND ANOTHER EMP SHOOK THE TAIL SECTION CAUSING IT TO COME LOOSE. WHEN SECTION BROKE LOOSE IT SWUNG, PINNING EMPS RIGHT LEG.
EE WAS WORKING IN A SMALL AREA NEAR A COLUMN ON THE SIDE OPPOSITE RIGGING. THE REMAINING 2 EE'S WERE CUTTING THE WELDS ON THE RIGGED SIDE OF THE UNIT. THE LOADS APPLIED BY THE CRANE VIA THE RI GGING ( NYLON STRAPS) AND THE LIFT OF WEDGES EXCEEDED THE HOLDING POWER OF THE REMAINING WELDS, CAUSING THE WELDS TO FRACTURE, ALLOWING THE ASSEMBLY TO HINGE UP AND PIVOT COUNTER-CLOCKWISE PI
THE ELEVATOR OLD PORTAL WAS INOPERABLE FOR A PERIOD LONGER THAN 30 MINS DUE TO FAILURE OF THE MOTOR GENERATOR SET.

EE & CO-WORKER WAS LOADING TRACK RAILS ONTO MAN BUS WHEN CO-WORKER LOST HIS GRIP DROPPING ONE END OF RAIL CATCHING EE'S HAND [FINGERS] IN A PINCH POINT.
I WAS USING LIFTING JACK TO RERAIL TRACK JEEP WHEN I FELT SHARP PAIN IN MY GROIN AREA.
EE WAS WALKING DOWN STAIRS AT STACKER CONVEYOR WHEN HIS FEET SLIPPED & HE GRASPED HAND RAIL TO PREVENT FALL CAUSING INJURY TO HIS BACK.
EE AND CO-WORKER WERE WALKING TOWARD TOOL BOXES IN THE BELT ENTRY OF THE P-5 LONGWALL PANEL INBYTHE LAST SLANT BEFORE THE HEADGATE WHEN HIS EAR STRUCK THE JAGGED EDGE OF A ROOF PLATE CAUSING A LACERATION.
EE WAS WALKING ALONG TRACKWAY BETWEEN P-4 AND EAST 2 WHEN HIS LEFT KNEE TWISTED.
EE WAS TRANSPORTING SHEAR TO FACE TO BE PLACED ON PAN LINE WHEN SHEAR FELL OFF THE FORKS OF SHIELD HAULER CAUSING HAULER TO BOUNCE AND EE'S HEAD STRUCK UNDERSIDE OF CANOPY.
A FALL OF ROOF OCCURRED ON P-5 LONGWALL SET-UP FACE IN THE #1 ENTRY. ROOF FALL WAS LOCATED BETWEEN SPAD #4+40 AND 5+50. ROOF FALL WAS APPROXIMATELY 70 FEET LONG, 7-9 FEET HIGH AND 24 FEET WIDE .
A FALL OF ROOF OCCURRED ON P-5 SECT IN THE 2 LEFT ENTRY BETWEEN SPAD 1 + 10 AND 2 +20 IN THE PILLAR LINE OUTBY THE LAST OPOEN CROSSCUT. ROOF FALL MEASURED 321/2 FT L, 21 FT W, AND 6-8 FT IN H ROOF FALL WILL NOT BE CLEANED UP.
EMPLOYEE WAS WALKING THROUGH DOORS GOING TO TRACK LOADING AREA WHEN DOORS CLOSED SUDDENLY, STRIKING EMPLOYEE IN THE BACK AND CHEST.
EE WAS ASSISTING CO WORKER IN LOADING CRIB TIES IN BUCKET OF SCOOP WHEN HIS RIGHT RING FINGER WAS CAUGHT BETWEEN TWO CRIB TIES.
EE WAS ATTEMPTING TO RUN AS A SURFACE TRANSFORMER FAILED WHEN HIS FEET TRIPPED IN THE LOOSE GRAVEL AND HE FELL ON HIS LEFT HAND.
EE WAS STANDING AT HEADGATE, #2 SHIELD WITH FOOT ON TOP OF BOLT EYE ON RELAY BAR. SHEARER HAD LEFT HEADGATE TRAVELING TOWARD TAILGATE. PAN WAS PUT ON BANK PUSH FROM APPROX SHIELD #10. AS RELAY BAR CAME ON PUSH, IT RAISED UP ONTO PANLINE CLEVIS SLOT, CATCHING HIS FOOT BETWEEN BAR & CLEVIS.
EE REACHED DOWN TO MOVE TRAILING CABLE FOR ROOF DRILL OUT OF ROADWAY WHEN SLACK WAS TAKEN OUT OF CABLE CAUSING AN INJURY TO HIS BACK AS HE GRASPED THE CABLE.

EMP AND ANOTHER EMP WERE PLACEING ROCK DUST ON MINE FLOOR AND LIFTING AND HANDLING CRIB TIES WHEN BACK WAS INJURED. FIRST SCHEDULED WORKDAY LOSTBY EMP WAS 1-7-97. MINE SCHEDULE TO CLOSE AT END OF MONTH.
HE WAS UNLOADING BELT STRUCTURE FROM SHIELD HAULER WHEN I LOST MY BALANCE. THE TOP ROLLER HE WAS CARRYING TWISTED CAUSING AN INJURY TO MY LEFT ELBOW.
EMP WAS CARRYING A COMPRESSOR WHEN HE TRIPPED OVER AN I BEAM ON FOOTWALL, LANDING ON THE BEAM, CAUSING INJURY TO HIS BACK AND LEG.
EE TRIPPED AND FELL AGAINST TRACK ROOF DRILL THAT WAS PARKED IN THE BOTTOM AT ELEVATOR LANDING AS HE ATTEMPTED TO GET OUT OF OPERATOR'S COMPARTMENT OF MANBUS.
EE WAS STANDING AT REAR OF CONTINUOUS MINER (BOOM END) REMOVING A CONNECTING LINK FROM CONVEYOR CHAIN WHEN CONVEYOR CHAIN FELL OUT OF TROUGH AND STRUCK LEFT KNEE.
EE WAS OPERATING LOCOMOTIVE TRAVELING INBY ON SOUTHMAIN TRACK WHEN HE WAS STRUCK ON THE FOREHEAD BY AN UNKNOWN OBJECT. IT IS POSSIBLE THAT EE WAS STRUCK BY A LOW HANGING ROOF TRUSS.
EE AND SEVERAL CO-WORKERS WERE USING THE SHIELD HAULER TO REMOVE SCOOP THAT WAS DOWN ON TOP OF RAILS. AFTER SCOOP WAS REMOVED SHIELD HAULER WAS MAKING A RETURN TRIP WHEN IT STRUCK A TRACK RAIL AND PUSHED IT ON TOP OF EE'S LEFT FOOT.
I WAS JUMPING FROM BELT CROSSOVER TO FOOTWALL TO AVOID BEING STRUCK BY A BROKEN SLOPE BELT. I STRUCK TRACK RAIL WHEN I LANDED.
EE WAS WALKING TOWARD FACE AREA FROM MAN BUS WHEN HE STEPPED ON A CONCRETE BLOCK PARTIALLY SUBMERGED IN MUD CAUSING LEFT KNEE TO TWIST
EE WAS INSTALLING ROOF BOLTS AT A ROOF FALL LOCATION WITH HYDRAULIC STOPER WHEN HE LOST HIS BALANCE AND FELL HOLDING DRILL
EE WAS SHOVELING ON THE RAW COAL STOCKPILE - CLEANING COAL FROM AROUND THE STRUCTURE WITH RIGHT FOOT LOWER THAN LEFT FOOT WHEN BACK WAS INJURED WHILE THROWING COAL TO HIS RIGHT.
EE WAS PREPARING TRAILING CABLE ON CONTINUOUS MINER FOR SPLICING WHEN KNIFE SLIPPED CAUUSING A LACERATION TO HIS LEFT INDEX FINGER.
WHILE INSTALLING 48" ROOF BOLT IN MUD SLIP OF 1 LEFT FAULT AREA; A ROCK FELL FROM A DISTANCE OF 108" HIGH STRIKING ROOF DRILL CANOPY AND MY LOWER BACK. FIRST SCHEDULE WORKDAY LOST BY EE WAS 1- 9-97. MINE SCHEDULE TO CLOSE AT END OF MONTH.
A FALL OF ROOF HAD OCCURRED BETWEEN THE #7 AND #8 SEAL ADN BETWEEN THE #9 AND #10 SEALS. THE ROOF FALL BETWEEN #7 AND #8 SEALS MEASURED 21'WX6'HX LENGTH UNKNOWN.. ROOF FALL BETWEEN #9 AND #1 0 SEALS MEASSURED 16'W AND APPRXO 6'H LENGTH UNKNOWN.
EE WAS INSTALLING COMPONENTS FOR DEWATERING SYSTEM. IN AN ATTEMPT TO GET ON TOP OF SUM AT FOOT SOF SLOPE TO INSTALL A PIPE EE REMOVED BELT AND HAT FOR CLEARANCE. AS EE LUNGED FORWARD TO GET ON TOP OF SUMP HIS HEAD STRUCK BRACKET THAT WAS BOLTED TO ROOF TO LIFT SUMP.
THE MOTOR CREW HAD TWO SHIELDS THAT WERE READY TO BE UNLOADED AT MATERIAL HOLE FOR WEST 1 (LONGWALL SET-UP). AREA WHERE SHIELDS WERE TO BE UNLOADED HAD 8 SHEAR JACKS LYING ON MINE FLOOR. EE WAS ASSITING CO-WORKER TO POSITION THEM ON FORKS OF SHIELD HAULER FOR TRANSPORTATION.. WHEN ACK SLIPPED AND HIS FINGER WAS CAUGHT IN PINCHED POINT BETWEEN TWO JACKS.
EE WAS ASSISTING SEVERAL CO-WORKERS IN THE REMOVAL OF AN OLD DISCHARGE CHUTE AT P-4 HEADER. DISCHARGE CHUTE HAD BEEN DIS-ASSEMBLED IN SEVERA SECTIONS AND AS EMPLLOYEE ATTEMPTED TO MOVE CHUTE OUT OF TRAVELWAY WHEN IT SLIPPED OT OF HIS GRASP AND CAUGHT FINGER AGAINST MINE FLOOR.
EE WAS JACKIGN WRECKED MATERIAL CAR BACK ONTO THE TRACK WHEN THE JACK FELL PINCHING HER THUMB BETWEEN JACK AND FRAME OF MATERIAL CAR.
EE WAS USING A BELT KNIFE TO CUT CONVEYOR BELT IN PREPARATION TO INSTALL A SPLICE WHEN KNIFE SLIPPED AND STRUCK LEFT LEG APPROX. 2 1/2INCH ABOVE KNEE CAUSING A LACERATION. INJURY OCCURRED WHIL E SPLICING P-4 SECTION CONVEYOR.
EE WAS LIFTING A TOP CAT TRACK ROLLER ONTO DOZIER AT ROCK DUMP WHEN HIS BACK WAS INJURED.
EE WAS ATTEMPTING TO LOAD A LARGE ACETYLENE CYLINDER ON A JEEP WHEN HIS BACK WAS INJURED
EE WAS OPERATING RAM CAR AND HAD HIS HAND AROUND THE POST OF THE CANOPY. A SMALLER PIPE FITS INTO A LARGER PIPE AND THE BOLTS ALLOWED MOVEMENT OF PIPE. WHEN RAM CAR TRAVELED OVER ROUGH BOTTO M FINGER WAS CAUGHT IN PINCH POINT BETWEEN TWO PIPES.
EE WAS SHOVELING LOOSSE COAL, MUD AT 40 NO. 1 DISCHARGE HEADER WHEN HER FEET SLIPPED AND HER RIGHT ANKLE WAS INJURED.
EE AND OTHER WAS USING LIFTING JACK TO LIFT BELT CAR ABOVE TOP OF RAIL WHERE TRACK FROM ELEVATOR LANDING AND SLOPE LANDING INTERSECTS SO THAT THE UNDERSIDE OF BELT CAR WOULD CLEAR. WHEN BELT C AR BECAME FREE THE SLACK IN THE COUPLING CAUSED CAUSE JACK AN DBAR TO KICK OUT CAUSING DAMAGE TO EE RIGHT ANKLE AND FOOT.
EE WAS ATTEMPTING TO THROW TRACK SWITCH FOR MATERIAL CARS WHEN HIS FEET BECAME STUCK IN MUD AND WHEN HIS BODY MOVED HIS LEGS WERE RESTRICTED CAUSING A INJURY TO HIS LEFT KNEE.FIRST WORK DAY LO ST BECAUSE OF INJURY WAS 8-29-96.

WHILE TRAMMING RAM CAR, EE STRUCK A HOLE IN MINE FLOOR, BOUNCING HARD ON CAR SEAT AND HURTING HIS BACK.
WHILE SHOVELING ON 40 #1 BELT CONVEYOR WHE HER RIGHT LEG GAVE AWAY CAUSING HER TO FALL DOWN INJURING HER LEFT LEG.
EE WAS WALKING ACROSS THE FACES OF WEST 1 SECTION WHEN HE STEPPED IN A HOLE AND TWISTED HIS BACK.
EE WAS UNLOADING CRIB TIMBERS WHEN HE FELT A SHARP PAIN AS HE PITCHED A TIMBER OUT OF RAM CAR BED. INJURY OCCURRED ON SECTION P-4.
EE WAS WALKING OFF OF SECTION AT END OF SHIFT WITH OTHER CREW MEMBERS WHEN HE STEPPED IN A HOLE. ON ROADWAY CAUSING HIM TO FALL INJURING HIS BACK.
WHILE SERVICING CONTINUOUS MINER EE PICKED UP A GREASE BUCKET. WHEN HE STARTED TO TURN WITH IT HE FELT PAIN IN HIS GROINT. INJURY OCCURRED ON WEST 1 SECTION.

EE WAS BOLTING MONO RAIL TO SOUTHMAIN 1 CONVEYOR FOR VULCANIZING STATION. I BEAM FELL AND HIT HIM ACROSS LEFT SHOULDER AND PINNED HIS LEG AGAINST HIS CHEST.

MESH LINING WAS BEING PUT UP ON LONGWAL RECOVERYWHEN THE SHIELDS WERE PULLED, THE MESH WAS PINCHED, AND STRETCHED THE CABLES THAT THE SLING AND COME A LONG WERE ATTACHED TO. THE SLING BROKE AN D THE CABLE HIT EMP IN THE FACE.
EE AND COWORKER WERE TRAVELING ON LOCOMOTIVE TRANSPORTING A ROLL OF CONVEYOR BELT TO P-4 SECTION SHEN A SMALL ROCK FELL FROM ROOF AND STRUCK EE'S LEFT ARM AS THEY PASSED THE FIRST KICKBACK ON SOUTH MAIN NO. 2 CONVEYOR.
THE INJURED EE STATED HE HAD COMPLETED THE INSTALLATION OF ROOF BOLTS IN THE BOLT AND TRACK ENTRY OF THE SOUTHMAIN SECTION. THE FOOT JACK WAS RAISED AND ATRS WAS LOWERED. AS THE CANOPY WAS LOW ERED IT STRUCK EE ON THE HEAD AND SHOULDER. EE FIRST WORKDAY LOST BECAUSE OF INJURY WAS 9-19-96.

AT APPROX 9 AM THE MAN ELEVATOR AT THE FRANKLIN BEND PORTAL BECAME INOPERATIVE. ONE OF THE DOOR SAFETY SWITCHES MALFUNCTIONED. THE ELEVATOR WAS DOWN FOR SEVERAL HOURS, DUE TO AVAILABILITY OF P ARTS.
THE ELEVATOR AT FRANKLIN BEND HAD DAMAGE WHICH INTERFEBED WITH USE OF THE ELEVATOR FOR MORE THAN THIRTY MINUTES.
EE WAS ATTEMPTING TO REMOVE A BUNDLE OF ROOF BOLTS PLATES FROM MIDDLE OF ROOF DRILL. WHE ROOF PLATES WRE REMOVED A BUNDLE OF ROOF BOLTS FLIPPED UPWARD AND STRUCK EE ACROSS BRIDGE OF NOSE. THE ACCIDENT OCCURRED ON SECTION P-4.
EE AND COWORKERS HAD TAKEN HOSES AND HIGH VOLTAGE CABLE OFF OF "DOLLY'S" ON MONORAIL AND LOADED THEM INTO BUCKET OF SCOOP. THEY WERE ALLOWING "DOLLY'S" TO FALL INTO BUCKET OF SCOOP FROM 100' C UT BACK WHEN HIS RING FINGER LEFT HAND WAS CAUGHT IN PINCH POINT AS "DOLLY'S" WERE REMOVED.
EE WAS DRILLING TOP (FROM OPERATOR'S SIDE) WHEN A PIECE OF ROCK/DUST STRUCK HIM IN THE LEFT EYE.
EE WAS STANDING UPON A RAISED PIPE TO ALLOW HERSELF TO REACH COAL DISCHARGE CHUTE THAT WAS STOPPED UP WITH ROCK DUST SACKS. AS SHE STEPPED DOWN HER FOOT ANKLE TURNED OVER CAUSING HER TO FALL. WITH ARM STRIKING FOOT WALL AS SHE ATTEMPTED TO BREAK HER FALL. ACCIDENT ACCURRED AT 40 NO. 1 HEADER.
EE STATED THAT HE WAS CLIMBING INTO OPERATOR'S COMPARTMENT OF LOCOMOTIVE WHEN BACK WAS INJURED. PRIOR TO INJURY TO BACK THE MOTOR CREW HAD UNLOADERED BATTERIES, CRIB TIES AND OIL.
EE WAS CHANGING BATTERIES ON THE NO.2 RAM CAR WHEN JUMPER GROUNDED CAUSING A FLASH FIRE THAT BURNT HIS LEFT HAND. ACCIDENT OCCURRED ON SECTION WEST 1.
EMP WAS CARRYING PARTS TO THE TRACK JEEP PARKED AT THE END OF TRACK ON EAST 1 SECTION WHEN HE TRIPPED OVER A BAG OF ROCK DUST AND FELL STRIKING HIS HEAD ON THE EDGE OF THE JEEP.

EE AND OTHER EE HAD PLACED ABOUT 2 PALLETS OF ROCK DUST ON TOP OF AN OVERCAST. EE STATED THAT THERE WAS TWO REMAINING SACKS OF ROCK DUST TO BE MOVED. HE WAS ON HIS KNEES AND ATTEMPTED TO MOVE SACK OF ROCK DUST ABOUT 5 FT WHEN BACK WAS INJURED.
THEY WERE RECOVERING CABLES ON THE ABANDONED 4010 LONGWALL PANEL WHEN HIS BACK WAS INJURED.

EMPLOYEE LIFTED A WOODEN CRIB TIE TO BE USED TO JACK TRACK ROOF DRILL UP THAT HAD WRECKED WHILE COMING OUT OF SIDE TRACK WHEN BACK WAS INJURED. EMPLOYEE'S FIRST WORKDAY LOST BECAUSE OF INJURY WAS 12-18-95.
ON 12-1-95 A ROOF FALL OCCURRED ON SOUTH MAIN SECTION AT SPAD 53+60. ROOF FALL MEASURED 10 FT. LONG, 15 FT. WIDE AND 5 FT. THICK. ROOF FALL WAS AT SAME LOCATION AS ROOF FALL #8-95.
HE WAS CHECKING AN AIR PRESSURE HOSE ON SHIELD #118 WHEN THE PAN CONVEYOR MOVED OVER AND DROPPED DOWN ON HIS FOOT.
EE WAS WALKING DOWN STAIRS TO CRUSEHR FLOOR WHEN FEET SLIPPED AND EMPLOYEE FELL AGAINST STAIRS INJURING HIS BACK AND NECK.
ANOTHER EMPLOYEE WAS USING A HAMMER WHEN IT SLIPPED AND HIT EMPLOYEE ON LEFT HAND AND WRIST.
AT APPROX. 8 AM ON 11-18-95 AN ROCK OCCURRED IN THE NEW SO MAIN HEADING 53+60. THIS FALL IS APPROX. 30' LONG 20' WIDE AND 5' THICK. THE ROOF STRATA IN THE AREA OF THE FALL IS OF LAMINATED SAND STONE AND WAS BOLTED WITH 48" POINT ANCHOR ROOFBOLTS. THIS FALL WILL BE CLEANED UP AND RE-SUPPORTED. CALLED INTO MSHA B'HAM DISTRICT 7:15 AM 11-20-95. JERRY VANN TOOK THE CALL.
EMPLOYEE WAS REPLACING A LEAD IN A BATTERY RECEPTACLE ON THE SHIELD HAULER WHEN CONDUCTORS BECAME GROUNDED. EMPLOYEE RECEIVED ELECTRICAL SHOCK CAUSING 1ST AND 2ND DEGREE BURNS TO HIS RIGHT AND LEFT HAND.
A FALL OF ROOF OCCURRED ON THE 4012 TRACK ENTRY. APPROC 89' OUTBY SPAD 27 & 29. ROOF FALL MEASURED, 30'L,21'3"W, 6'6" THICK.
EMPLOYEE WAS WALKING ON INCLINED MINE BOTTOM AT 40 NO. 1 TAILPIECE WHEN HER FEET SLIPPED AND SHE FELL CAUSING INJURY TO HER BACK.

EE STATED THAT HE WAS PLACING ROCK BY HAND ONTO PAN LINE WHEN HE FELT A PAIN IN HIS BACK. OVER A PERIOD OF SEVERAL DAYS PAIN PROGRESSIVELY GOT WORSE.
EE WAS PLACING MATERIAL BETWEEN ARCHES WHEN A PIECE OF TREATED WOOD [4"X6"X32"] FELL & STRUCK HIM ON THE HEAD.
BELT ROLLER FLIPPED UP FROM UNDER SCOOP AND HIT LEFT ARM.
EE WAS HANDLING MATERIAL [ROOF BOLTS & "T"-BOARDS] WHEN HE FELT A KNOT IN GROIN AREA.
INJURED WAS USING A LIFTING JACK TO LIFT FLAT CAR WHEN JACK LEVER STRUCK RIB CAGE.
EE STEPPED IN HOLE ON SIDE OF TRACK AND TWISTED HIS LEFT ANKLE.
EE WAS OPERATING RAM CAR WHEN RAM CAR TRAVELED THROUGH HOLE IN MNE FLOOR INJURING HIS BACK.
INJURED & 2 CO-WORKERS WERE TRAVELING INBY ON SOUTHMAIN TRACK. THEY HAD PASSED THE DERAILER WHENMAN BUS BEGAN TRAVELING AT A HIGH RATE OF SPEED [UNCONTROLLABLE] CAUSING MAN BUS TO WRECK ABOUT 18 CROSSCUTS INBY DERAILER & COLLIDED WITH HEADER FRAME THAT HAD BEEN TAKEN OUT OF SERVICE.
EE AND OTHER AND ANOTHER WERE TRAVELING INBY ON SOUTHMAIN TRACK. THEY HAD PASSED THE DERAILER WHEN MANBUS BEGAN TRAVELING AT A HIGH RATE OF SPEED UNCONTROLLABLE CAUSING MANBUS TO WRECK ABOUT 1 8 CROSSCUTS INBY DERAILER AND COLLIDE WITH HEADER FRAME THAT HAD BEEN TAKEN OUT OF SERVICE.
INJURED WAS HELPING CLEAN UP ROOF FALL ON 40 NO.1 TRACK/BELT ENTRY WHEN HE MASHED HIS FINGER WHILE UNLOADING ACCETYLENE TANK OUT OF RAISED BUCKET OF LOW-TRAC FRONT END LOADER.
EMP WAS GETTING OUT OF OPERATORS COMPARTMENT OF SCOOP WHEN HIS RIGHT ELBOW STRUCK FRAME OF SCOOP
EE WEEKLY WAS TIGHTENING FITTING ON AN EMULSION PUMP WHEN A SLIVER OF METAL ENTERED HIS LEFT INDEX FINGER.
A FALL OF ROOF OCCURRED ON THE 40 HEADING BELT/TRACK ENTRY BETWEEN SPAD NO. 19+45 AND 21+30. ROOF FALL MEASURED 63' IN LENGTH, 24' IN WIDTH, THICKNESS UNKNOWN.
EE WAS ROCK DUSTING BOLT LINE WHEN SHE TRIPPED OVER ROCK IN ROADWAY CAUSING HER TO FALL AND TWISTED HER LEFT LEG.
EMP WAS PLACING A BOLT IN THE RELAY BAR THAT CONNECTS THE SHIELDS TO THE PAN. SHIELD WENT ON PUSH AND CAUGHT HIS THUMB BETWEEN THE BOLT AND PAN.
EE WAS CHECKING THE TOP SURFACE ON TOP OF THE 40 FAULT HILL ARCH WHEN A PIECE OF THE WOOD LAGGING THAT HE WAS STANDING ON BROKE AND HE FELL STRIKING HIS ARM ON A SHARP EDGED ROCK CAUSING A LAC ERATION.
EE WAS CARRYING A CONTAINER OF SEALANT WHEN HE STEPPED IN MUD AND TWISTED HIS LEFT KNEE.
EE STATED THAT HE WAS UNLOADING A BOTTLE OF ARGON IN THE POCKET WHEN THE BOTTLE ROLLED OFF THE EDGE OF THE JEEP ON HIS LEFT FOOT.

EMPLOYEE WAS SHOVELING LOOSE COAL AND ROCK ONTO SLOPE CONVEYOR. EMPLOYEE STATED HE TOOK A CAP PIECE (WEDGE BOARD) AND WAS RAKING MATERIAL FROM ON TOP OF PAN LINK WHEN CAP PIECE WAS GRABBED BY CONVEYOR AND HAND PASSED OVER TOP OF ROLLER CAUSING INJURY TO RIGHT THUMB.

A ROCK FALL WAS DISCOVERED ALONG THE MATERIAL ROADWAY ON SECTION--SOUTH MAIN (0-26). THIS ROCK IS APPROX. 100' LONG, 20' WIDE, AND 4' THICK-- THE AREA OF THE FALL WAS BOLTED WITH 36" X 3/4" CO NVENTIONAL ROOF BOLTS. THIS FALL WILL NOT BE CLEANED UP.
EE STATED THAT HE HAD PLACED A PIECE OF WOODEN LAGGING MATERIAL UNDER REAR BOOM STAB JACK. WHEN JAK WAS LOWERED, WOOD KICKE DOUT AND STRUCK EE ACROSS THE NOSE CAUSING AN INJUR. FIRST WORKDAY LOST BECAUSE OF INJURY WAS 7-14-95.
