Clark No 1 Mine Coal

The employee was utilizing a utility knife to trim a piece of belt when his hand inadvertently slipped and cut his left hand. The laceration was 2.5 inches long and better than 1/4 inch deep. He received 5 sutures in order to close the wound. He was treated and released to full duty.

A roof fall occurred above the anchorage zone in the left mine return 8 breaks underground from the fan duct work .

Employee was bolting the roof in the face of the #2 entry. As he was pulling the drill steel from the drilled hole a small piece of roof rock struck him. This caused a laceration to his forearm. The wound was cleaned and he received 8 stitches.

While using a cutting torch to repair the section feeder upon completion he inadvertently put his right hand down on a piece of hot metal burning the palm of his hand.

While bolting a piece of rock fell cutting his left thumb

While bolting a rock fell and hit boom and broke and a piece hit employees forearm cutting it.

While installing the last roof bolt in the last row a piece of roof rock was dislodged that struck the employee on the head, neck and left shoulder. The resulting impact caused an incision behind the left ear requiring 4 sutures and a contusion to his head, neck and left shoulder.

Employee stated he had passed out along the #1 belt conveyor. There were no witnesses. He was transported to the hospital and released with negative findings. The company treated this as an illness since there was no mechanism of injury.

While riding on a diesel powered man trip enroute to the active mining section the conveyance ran over a ledge in the roadway thus causing the employee hand which his face was resting on to strike his jaw. The impact of which fractured his tooth.

Reaching above head using a come-a-long and pulled a muscle in his back.

The injured employee was riding in a diesel man trip conveyance traveling to section when the man trip slid off of a bridge causing the vehicle to rise up abruptly fall back striking the mine floor. The resulting force caused him to feel immediate pain in his lower back. He was diagnosed with ruptured spinal disc. The injured had a previous history of back problems.

A roof fall was discovered during the on-shift examination by the section foreman . The fall was above anchorage and did not impeded ventilation. The location of the fall was at survey spad number 660 in number 4 entry.

This report is being filed as part of a msha citation that is currently being contested. So I have to file this report so msha will not shut my coal mine down . There was an weather anonomaly that deposited over 5.5 inches of rain fall that compromised or return airway after we had withdrawn the miners 2.5 hours earlier and overwhelmed our dewatering pumps.

The employee was installing a rub rail back on the continuous miner when he inadvertently smashed his right thumb resulting in a fracture to the thumb tip.

The employees was walking the belt entry into the mine at 17 break on the number 4 conveyor belt when he slipped and fell to the mine floor striking his knee and right shoulder.

The roof bolter operator was drilling a hole when his forearm was struck with a wrench. The resulting impact caused a simple fracture of his left forearm.

Employee was loading bolts on the Fletcher RR II bolter when he bent over to pick up two 48 inch roof bolts and suffered a muscle strain to his lower back.

A rubber tired personnel carrier rolled out of the adjoining break through and struck the employee's left leg between the scoop door and the frame of the machine resulting in a fracture above and below the knee. The steepness of the grade was a contributing factor.

Walking down the haul road and fell and bruised eye. Employee stated he had an episode due to his medication.

While operating the Fletcher Roof Ranger II roof bolter a piece of rock fell and struck the employee's right hand fracturing his metacarpal bone. Employee was swinging the drill pot in when a rock struck his hand.

A Section Supervisor hanging up sight rods for center lines and a piece of draw rock fell from between the bolts and struck the individual in the back. He was diagnosed with severe degenerative spine disease. Note: This accident was originally reported on 8-30-2013 to MSHA data base and for whatever reason was not retained.

The employee was trouble shooting the battery bell from the scoop jumper cables while they were energized resulting in an arc flash that resulted in a second degree burn on the employee's left hand.

Carrying crib block and felt pain in his lower back.

Employee was drilling the roof and a piece of rock fell and hit the canopy and broke into two pieces. On very sharp edged piece struck the employee on the right side of his right foot. The employee was take to Shelby Baptist Hospital where he received three stitches and was released with no restrictions.

The employee was installing roof bolts in the number 9 entry face with a Fletcher Roof bolter when a piece of roof rock fell and struck his upper arm causing a laceration. He waived the ambulance care and was accompanied to the Shelby Baptist Hospital where he was treated and released. The wound took 10 sutures to close.

The employee was using a utility knife to cut a cleaning rag to size when he was hit by a piece of roof. This either startled the employee or struck his arm resulting in him cutting his left forearm. The injury resulted in three sutures and a two week course of antibiotics. The employee returned to work on his next regularly scheduled shift.

While waiting on the continuous miner to pass through a cross-cut, a piece of draw rock fell from the roof, striking the employee on the chest, hand, upper leg, ankle, and foot.After Sending EE to ortho Dr. the Dr. determined that the EE had a torn ligament in his foot, requiring a hard cast & 2 week leave of duty.

Cutting H-beam for tail piece, raised up to adjust oxygen and caught ear on belt structure

Changing a motor on a scoop - it dislodged and fell on employee's hand, pinning it to the ground, smashing left ring finger.

He was roof bolting in #2 Left and a rock fell from the roof, striking him in the head

Heel of left boot was cut off by employee while carrying 2" air pump. He slipped on grounding mat at DC power center - mat was damp and slippery. He twisted his left knee and was slightly swollen.

Driving down roadway #3 entry & hit bump in road driving his head into canopy. Employee said he was ok and did not need accident report. 9/10/12 employee reported to Mine Superintendent with a neck brace on and was sent to clinic to get his neck checked out.

Mine site received approx 6" of rain in less than 3hr period. Mine lost power twice allowing surface sump to be overrun with rain water. Rain water then ran into the main return, interrupting mine ventilation. No injuries.

Stress from displacement from 22' fault, not enough cribbing in the intersection. Lost 3 joins of 4" water line.

Unloading cribs - pulling crib off top of stack, it slipped out of hand, he tried to catch it and crib smashed right thumb.

Tramming the Fairchild scoop outside, as scoop passed 6 break, the pump motor kicked off and scoop shut off. Injured tried to stop scoop from rolling backward by using foot brake and then panic bar, but it kept rolling until it struck the rib at 6 break. Injured was slammed into control panel, injuring left shoulder. Injured states he cannot raise arm past mid-torso.

Putting tools up in box when lid fell shut smashing his right hand.

Rock fell, glanced off hard hat, struck employee on back near shoulder

Operating S&S scoop. Tramming into cross vent, as he trammed forward, the scoop slid onto rib, pinching his skin on left shoulder. Avulsion of skin on shoulder approx. 5" x 5".

Working on Shuttle Car - rock fell out of Top between bolts, striking him on left hand

Shoveled up some rocks and twisted to throw on tailpiece - felt sharp pain in lower back.

Bolting top - piece of draw rock fell from top striking injured in right upper arm

The continuous mining machine encountered an unidentified and ungrounded exploration drill hole causing an immediate inundation of water into the face area of the mine's working section. The water inflow ended abruptly once the drill hole had drained, and the face area was pumped clear of the water. There were no injuries to the miners on the working section.

Employee was welding inside a belt head rock box. Subjected to reflective flash off metal plate behind him. Did not report incident until emergency room called mine for insurance details.

Cutting rubber hose with knife - hands were wet and mucky - slipped and cut across finger

A fall occurred in the #9 crosscut, along #1 belt line. The roof fell above the anchor of the 4ft.resin bolts and comprised 5ft of gray, slickensided shale. It measured 40ft x 25ft and it crushed 4 sections of conveyor structure. The area was posted and cribbed, and intersections from #6 to #9 break were bolted with 10ft cable bolts.

There was an unintentional fall of roof in the #8 crosscut, off #3 entry at the Clark No.1 mine. Debris from the fall knocked 2-3 blocks out of the return side stopping, this however did not interrupt section ventilation as we had two additional return airways. The area was cribbed off to prevent further roof issues.

An unintentional fall of roof occurred at Spad No.1032, in the #3 entry, at the Clark No.1 Mine. The roof fall blocked the flow of air from the right split off section 001. The roof at the location of the fall comprised mostly slickensided shale. Area cribbed off and ventilation was restored. MSHA informed at 4:10 am.

Roof Fall in Room #3 intersection S pad 2 and 30. Area was timbered off and cribs built.

Walking down belt line and slipped resulting in hyper extended knee

Roof Fall - occurred at intersection of cross cut #15 in the belt line entry. Involved in the fall was #1 Belt line (structure, water line and air line) It was cover with approx. 8-9 ft of gray shale rock. Cause of accident was geological conditions.

Water line hit employee on left side of face while moving 2" air pump

Employee walking around miner while the miner was trimming from #4 to #6 and was crushed between the tail boom and the rig. His injuries were mid-section and left leg bruising.
