Symptoms experienced were dizziness and blurred vision. EMS was summoned for precautionary assessment due to personal health condition. EMS decided to transport employee to hospital for evaluation. EE remains off work. The MSHA Investigator requested an MSHA 7000-1 be completed indicating the illness was non-work related.

On August 4, 2020 the employee struck a welding rod on the top side of EE's right hand in the thumb area causing an arching burn. The employee was returned to work without restrictions. On November 9, 2020, the employee was prescribed work restrictions following surgery to repair nerve damage.

On November 2, 2020, it was learned, through the litigation process, that an alleged loss of hearing was found to have industrial causation.

On 9/14/15 the employee was taken to the Company clinic, received stitches for a left eye orbit laceration and was released to regular work on 9/15/15. The employee was struck in the orbit of his left eye by a drag cable during an installation and sustained a laceration.

On 4/02/14, the employee requested medical treatment and was placed on modified duty which was accommodated. A conveyor belt flopped loose during a repair and made contact with the employee's hard hat. The Company physician diagnosed the employee with a head contusion and cervical spine strain.

The employee went to the company clinic on 8/1/11 and was placed on restricted duty. The employee reported that he lost his footing, fell and struck his R-knee cap on a protruding 5/8" bolt that secured the dust pan.

The employee went to the company clinic on 11/14/09 and was placed on restricted duty. The employee was using a putty knife to remove a decal, when the knife tore through the decal and struck L-palm, causing a laceration that required sutures.

The employee went to the company clinic on 8/28/09 and was taken off work. Employee was bolting pull plate on conveyor belt. As he finished, he informed his supervisor that he wasn't feeling well.

Employee was taken to the Company clinic on 4/2/2009 and was placed on restricted duty which is being accommodated. He alleges that he struck his head on the transmission filler tube on a loader which resulted in a lower back strain.

The employee went to the company clinic on 9/19/08 and was placed on restricted duty. While the employee walked to collect his tools, he tripped over caution tape, lost balance and fell forward against a PVC pipe, resulting in a back sprain.

Employee went to the company clinic on 12/3/08 and was placed on restricted duty. The employee received a Right Lateral Epicondylitis (tennis elbow) to R-arm while raking through material on the conveyor belt.

The employee went to see the company doctor on 11/9/2005 and was placed on restricted duty. While cutting a chute liner with a welding torch, the flame burned through the glove and burned the employee on the right hand.

Employee went to see company doctor on 4/4/05 and was placed on restricted work. Employee was struck in the face by a piece of metal that broke off from a damaged loader bucket tooth that was being hammered.

Employee went to see company doctor on 5/26/04. Stitches were applied and employee was returned to full duty. Employee was removing/installing teeth on loader when a small piece of steel broke and became imbedded in his R-leg.

Employee went to see company doctor on 5/10/2004 and was placed on restricted duty. Employee was performing service on dozer when he slipped due to grease/oil on sole of shoes and strained his back.

WHILE STEPPING DOWN THE LADDER OF A LOADER, EMPLOYEE STEPPED ONTO HIS FUEL HOSE WITH THE HEEL OF HIS FOOT WHICH CAUSED HIM TO SPRAIN HIS RIGHT ANKLE (ALLEGED). EMPLOYEE WENT TO SEE COMPANY DOC TOR ON 10/16/03 AND WAS PLACED ON RESTRICTED DUTY.

EE WENT TO SEE COMPANY DOCTOR ON SEPT. 3, 2003 AND WAS PLACED ON RESTRICTED DUTY. EE WAS ATTEMPTING TO RELOCATE DESK TO ITS PREVIOUS LOCATION AND WHILE PUSHING UNIT, HE FELT A SHARP PAIN IN HI S LOWER BACK, RESULTING IN A LOWER BACK STRAIN.

EMPLOYEE WENT TO HOSPITAL ON 7/26/03 AND STAYED OVERNIGHT FOR OBSERVATION. WHILE DRIVING A COMPANY CRANE IT ROLLED OVER, PARTIALLY PINNING EMPLOYEE AND EMPLOYEE RECEIVED MULTIPLE INJURIES. EMP LOYEE WAS TAKEN OFF WORK BY HOSPITAL PHYSICIAN.

EE WENT TO SEE CO. PHYSICIAN ON 12-12-02 AND WAS PLACED ON RESTRICTED DUTY. WHILE INSTALLING SOCKET FOR 7' CRUSHER, EE CUT R FINGER ON SOCKET LINER.

WHILE TIGHTENING A FITTING ON THE SHOVEL HOPPER DOOR (HYDRAULIC CYLINDER) HIS WRENCH SLIPPED AND HIS HAND STRUCK 2" PLATE EDGE OF THE DOOR WHICH RESULTED IN A LACERATION AND FRACTURE TO HAND.

WHILE CUTTING V-BELTS OFF OF MOTOR, EE WAS PULLING KNIFE TOWARDS HIMSELF & PUNCTURED HIS L-CHEEK. EE WENT TO SEE COMPANY DOCTOR ON 4/1/02, STITCHES WERE APPLIED & EE WAS RETURNED TO FULL DUTY.

EMPLOYEE WENT TO SEE COMPANY DOCTOR ON 2-8-02 AND WAS RETURNED TO FULL DUTY; PRESCRIPTION MEDICATION WAS PRESCRIBED. EMPLOYEE HAD AN ALLERGIC REACTION TO LUBRICANT SPRAYED ON LOADER CAB WHICH CAUSED A RASH AND SWELLING TO HIS FACE.

WHILE HE WAS OPERATING THE JAW CRUSHER, A TOOTH FROM THE DRAGPLATE SHOT OUT OF THE JAW CRUSHER, GLANCED OFF ANOTHER OBJECT AND WENT THROUGH A CRUSHER WINDOW STATION AND STRUCK HIS SHOULDER.

EE WENT TO CO. CLINIC ON 10-23-01 AND WAS PLACED ON RESTRICTED DUTY. WHILE ATTEMPTING TO ATTACH A MAGNET STROKE GAUGE TO A RUNNING SCRREN HEAD, EE'S LEFT MIDDLE FINGER WAS PINCHED BEWTWEEN MOV ING EQUIPMENT, WHICH CAUSED A CONTUSION TO HIS LEFT FINGER.

EMPLOYEE WENT TO THE COMPANY CLINIC ON 8/30/01 AND WAS PLACED ON RESTRICTED DUTY. WHILE REMOVING SCREEN CLOTH FROM SHAKER, WHILE PULLING ON SCREEN, EMPLOYEE STRAINED RIGHT SHOULDER.

A FELLOW EMPLOYEE WAS DISCONNECTING SOCKET FROM IMPACT WRENCH; THE AIRLINE CAME LOOSE & THE AIR HOSE HIT THE OTHER EMPLOYEE ON THE MOUTH. HE SAWCOMPANY DOCTOR ON 8/7/01 & STITCHES WERE APPLIED

EMPLOYEE WENT TO SEE COMPANY DR. ON 5-29-01, RECEIVED STICHES IN HIS L-MIDDLE FINGER AND WAS RETURNED TO MODIFIED DUTY. RIGGING SLIPPED WHEN REMOVING PIN FROM GANTRY; 3 L-MIDDLE FINGERS WERE P INCHED BETWEEN COUNTERWEIGHT OF DRAGLINE WHEN CHAIN OF COMEALONG CAME LOOSE.

EE WENT TO SEE THE COMPANY DR ON 5-9-01 AND WAS PLACED ON RESTRICTED DUTY. WHILE INSTALLING HANGER, HANGER HIT FEEDER WITH EE'S FINGER IN THE MIDDLE WHICH CAUSED A LACERATION TO R-RING FINGER.

EE WENT TO SEE COMPANY DR ON 4/23/01 AND WAS PLACED ON RESTRICTED DUTY. EE SLIPPED IN MUD AND BUMPED HIS KNEE AGAINST CONVEYOR WHEN HE ATTEMPTEDTO SPLICE A BELT. THE INJURY RESULTED IN A CONTU SION TO HIS RIGHT KNEE.

EE WENT TO SEE A DR ON 3/30/01 AND WAS PLACED ON RESTRICTED DUTY. WHEN PUTTING IN A CONVEYOR SECTION, EE WAS LINING UP CONVEYORS WITH A BAR. CONVEYOR SLIPPED JERKING THE BAR DOWNWARD STRAINING ROTATOR CUFF OF L SHOULDER.

EMPLOYEE WENT TO SEE A DOCTOR ON 3-15-01 AND WAS PLACED ON RESTRICTED DUTY. EMPLOYEE STRAINED BACK WHILE CLEANING UP LIFTING ROCKS ONTO WHEELBARROW.

EMPLOYEE WAS DRIVING A WATER TRUCK AND WHILE ATTEMPTING TO MAKE A RIGHT TURN, DUE TO EXCESSIVE SPEED THE TRUCK ROLLED OVER. EMPLOYEE SAW DOCTOR8/4/00 AND WAS RETURNED TO FULL DUTY; STITCHES WERE APPLIED.

EE WENT TO SEE DR. ON 7-24 AND WAS PLACED ON RESTRICTED DUTY. EE WAS CLIMBING UP THE LADDER OF SKIP LOADER AND SLIPPED OFF OF TOP STEP, STRAINING LOWER BACK.

WENT TO SEE DR. ON 2/8 AND WAS PLACED ON RESTRICTED DUTY. WALKING TO LOADOUT, EE FELL IN SCALE ACCESS WAY AND INJURED HIS SHIN AND LEFT SHOULDER.

EE WENT TO SEE DR ON 1/26/00 AND WAS PLACED ON RESTRICTED DUTY. WHILE SHOVELING, REPLACEING A GUARD, HAND SLIPPED OFF GUARD AND EE SMASHED PINKY FINGER OF RIGHT HAND.
