On 6/13/25 employee received medical treatment in ER for a reopening of a surgical incision. While employee was inspecting a trench, the tripped on leveling rake and fell on right knee, reopening a surgical incision from a personal knee surgery. Employee required surgery, was released from the hospital on 6/15/25, placed on restricted duty on 6/16/25 which is being accommodated

While standing on the second to last step on a staircase, plant operator was washing down material from underneath a conveyor belt when EE fell backwards onto the ground. Emergency sustained a head laceration requiring sutures.
While performing repairs on screens, maintenance employee cut themselves with a utility knife sustaining a thigh laceration. Emergency personnel dispatched to facility where the employee was transported via ambulance for further medical treatment. Employee received sutures and was discharged later that day.

On 10/16/2023 employee was assisting in a pit move relocating a 2.5" electrical cord. Employee reports EE began to experience discomfort in EE's right shoulder the following day 10/17/23 but did not notify management until 10/18/23. On 10/23/23 employee requested medical evaluation where EE was diagnosed with a strain to EE's right shoulder and placed on modified work duty.

While performing daily clean up of the sweeper truck, employee experienced slight discomfort in right shoulder while removing the suction hose.
Employee was operating the water truck when EE began experiencing gradually increasing back pain. On 4/12/21 EE requested medical treatment, and was prescribed physical therapy.

EE diagnosed with a sinus infection. Symptoms experienced were sinus pressure, heartburn and light headedness. EMS was summoned for precautionary assessment due to underlying health condition. EMS decided to transport EE to hospital for evaluation. EE was off work on 8/6 and 8/7/20. This was a personal medical, non-occupational event.

On 1/15/19 employee was taken to the company clinic and received stitches for a bursting laceration. Employee's left hand, while performing clean-up duties, was caught between a metal spray bar and the side wear plate of the loader bucket, resulting in a bursting laceration to the left middle finger.

On 11/28/18, Vulcan was informed by its third-party insurance administrator through the litigation process that an alleged 9/13/16 continuous trauma claim to the back and neck due to physical stress and strain of job duties and activities are occupational.
EE was sitting in the haul truck waiting for the excavator operator to finish EE's truck. While loading the haul truck the excavator bucket clipped the tail end of the haul truck. EE stated that the haul truck shook, several witnesses stated that truck did not move at all.

The employee was treated at the company clinic on 5/22/12 and was placed on restricted duty. While exiting the water truck, the employee failed to maintain 3 points of contact. This resulted in a fall and a fracture to her left wrist.

The employee attempted to install a V-belt on a feeder sheave when the counter weight of the feeder rotated resulting in a R-index finger laceration (required 4 sutures) and contusions to R-index, middle and ring fingers.

The employee went to the company clinic on 5/29/2007 and was placed on restricted duty. While attempting to remove the inner race of a double inter lock bearing with a hammer, a piece of steel became embedded in the employee's L-forearm.
The employee went to the company clinic on 4/6/07 and was placed on restricted duty. The employee missed footing platform while stepping from one VMC loader to another, causing him to slip down several steps while holding onto the handrails, which resulted in a laceration to L-knee.

The employee went to see the company doctor on 10/23/2006 and was taken off work by the doctor. While performing a pre-trip inspection, the employees R-leg fell between the left side track and push bar, resulting in a dislocated R-knee.
The employee went to see the company doctor on 3/7/2006 and was placed on restricted duty. The employee slipped and fell while transferring a roller bearing from the service truck to the workboat, causing the roller bearing to crush the employee's L-hand.

The employee went to see the company doctor on 10/3/2005 and was placed on restricted duty. When loosening cable clamps holding a cable loop in place, the cable sprung loose and struck the employee on the top of L-hand, causing a contusion and puncture wound
The employee went to see the company doctor on 9/23/05 and was placed on restricted work. While descending stairs, the employee lost his footing and twisted his R-ankle, resulting in a sprain.
Employee went to see company doctor on 2/16/2005 and was placed on restricted work. Employee had an alleged strain to L-shoulder and L-wrist while cleaning decks on plant with a shovel.

Employee was taken to Kaiser Permanente on 7/17/04 and was taken off work. High-pressure water hose was tangled in the head pulley and gravity take-up snubber pulley, causing it to pull employee's right arm between idler and the moving conveyor belt resulting in two compound fracturers.
Employee went to see company doctor on 4/3/04 and prescription medication was issued for right eye. While cleaning eccentric tube of shaker, particle of sand, from unknown source, entered right eye which caused an abrasion.
EE WENT TO SEE COMPANY DOCTOR ON 1-24-04 AND WAS PLACED ON RESTRICTED DUTY. EE SLIPPED OFF STEP WHILE ASCENDING CRANE WHICH CAUSED AN ABRASION AND CELLULITIS TO HIS LEFT LEG.

EE WAS TAKEN TO THE COMPANY DOCTOR ON 7-12-02, STITCHES WERE APPLIED, AND WAS PLACED ON RESTRICTED WORK. WHILE INMSPECTING SCREEN FOR PROBLEMS, EE NOTICED A LEAK AND STUCK HIS FINGER IN SCREEN TO LOOK FOR MISSING BOLT. THE HEADBOX AND SIDE OF SCREEN LACERATED AND FRACTURE HIS L INDEX FINGER.
EMPLOYEE WENT TO SEE COMPANY DOCTOR ON 2/7/02 AND WAS PLACED ON RESTRICTED DUTY. EMPLOYEE WAS WORKING ON A WATER TANK TO RAISE IT UP 4' AND WHEN HE LIFTED A STEEL PIPE, HE STRAINED HIS LOWER B ACK.

EMPLOYEE WENT TO SEE DOCTOR ON 9-27-00 WHO PLACED HIM ON RESTRICTED DUTY. WHEN UNPLUGGING CONVEYER JUNCTION BOX, A PIECE OF ASPHALT FELL STRIKING EMPLOYEE'S RIGHT TWO FINGERS.
EE WAS ON THE TIRE OF A 988B SKIP LOADER WHEN HE SLIPPED OFF AND SCRAPED HIS LEG AGAINST THE HOIST RAM CAUSING A LACERATION TO HIS RIGHT LEG. EE WAS GIVEN SUTURES AND ANTIBIOTICS. AS WELL AS A FOLLOW-UP APPOINTMENT WITH DOCTOR.
