Employee was installing chains on his truck at the start of shift. He removed the chains from the rear of the trailer and started to carry them to the truck rear driver location. The ground in the area was icy and he slipped and fell on his back. He proceeded to put the chains on, walked away from the truck and then felt a sharp pain in his left rib.
Employee was placing lunch bucket in personal pickup and reached across seat to place it on passenger seat side. Rear muscle in lower right leg was extended in process resulting in slight muscle tear.

Employee was changing bits on cutter drum of Wirtgen 2200SM. He was wearing safety glasses but when he tapped a retainer clip to remove it, a small piece of metal (apparently) flew over his glasses causing a cornial abrasion. There was no metal found in his eye.

EE WAS SERVICING 988B LOADER. DOOR HATCH OVER TRANSMISSION COMPARTMENT HAD FROZEN MUD AND ICE AROUND EDGES. EE TRIED TO FORCE DOOR OPEN INSTEAD OF USING NORMAL PROCEDURE OF CHISELING AWAY MUD AND ICE FIRST. FORCING ACTION CAUSED STRAIN IN LOWER BACK WITH PINCHED NERVE.
