Employee was opening door to the office trailer. A snake went between EE and the front door. EE reacted by turning away from the door quickly. When EE did this EE twisted knee and strained knee.

Employee was washing spilled material beside a rock crushed. He was 8 feet away from the crusher. A 5 inch rock was ejected from the crusher, fell 12 feet to the ground and landed on his right foot. The rock landed close to his ankle and broke a bone in his right foot.

STONE FROM CRUSHING CAME FROM HAMMERMILL AND GOT IN MINERS EYE. WHEN HE TOOK HIS SAFETY GLASSES OFF TO INSPECT A CONVEYOR BELT BECAUSE WHERE HE HAD TO LOOK WAS IN A DAR AREA AND HIS SAFETY GLA SSES WERE TO DARK TO SEE.
THE MOTOR ON DRAGLINE DL4 RAN HOT. THE OPERATOR ATTEMPTED TO RELIEVE PRESSURE BY LOOSENING THE CAP ONE NOTCH, BUT THE CAP BLEW OFF, CAUSING 2ND DEGREE BURNS TO HIS RIGHT ARM, CHEST AND FACE.

WAS CHECKING HIS HYDRAULIC LEFT ON HIS TRUCK. HE WENT UNDER THE LIFTED BED OF TRUCK AND THE BED FELL ON HIM. IT TRAPPED HIM AND RESULTED IN HIS DEATH.
CLIMBING OFF OF 235 BACKHOE AND SLIPPED OFF THE STEP.
