Driver was rolling tarp. EE released the handle, it spun back hitting ee on the cheek.

Employee was adjusting the kick plate on a bagging station when they placed left hand between the raised kick plate and the support structure underneath. They then cycled the machine by depressing the pneumatic control button causing the kick plate to lower pinching the left thumb between the plate and cross structure causing a laceration just below the thumbnail.

Employee was assisting with a Ball Mill Screen installation when employee's left thumb struck the backside of a structural bar on the frame of the screening system causing a hairline fracture.

Employee reached inside the engine compartment of a Kubota ATV to adjust the throttle cable and his left index finger contacted a linkage pin causing a small laceration just below the fingernail.

Employee was assisting another employee remove a screening deck from the SWECO screening system when he felt a pain in his lower back.

Employee contacted the end of a bagging spout with his left little finger causing a laceration.

Employee picked up a sheave (pulley) that weighed 35 pounds and he turned the part over using his right hand to locate a part number on the sheave when he felt a pain in his right shoulder.

Employee was removing a bag of material from the reject line when the back of his right hand contacted the conveyor belt causing a friction burn.

Employee was carrying a small fan up a flight of stairs when he noticed a pain in his lower back.

Employee states that he was getting down off a forklift and when he put his left foot down on the floor he heard a pop. Later that night his foot began to swell so he went to the emergency roo m where an x-ray revealed two fractured bones near the joint where his small toe joins the foot.

Employee was lifting a bag of material when he felt a pain in his lower back.

Employee was stepping down from one catwalk onto another when he stepped on a piece of clay with his right foot which twisted his right ankle causing a sprain.

Employee was lifting a 100 pound bag of material off of a pallet, when he felt a pain in his left shoulder, which subsided and he finished his shift. Three weeks after the initial injury, the employee did not have any pain, but his shoulder was popping. An MRI revealed a tear of the rotator cuff in his left shoulder.

Employee was repairing a light fixture on the top level of Plant 1 Tyler System. While repairing the fixture, employee fell down a flight of stairs. The fall dislocated his left shoulder, ring and pinky finger of his left hand.

Gas leaking from pipe ignited and caused burns on head and arms.

Employee was at the control panel for the pug mill at Plant 1 when he heard a pulley belt making a loud noise. The employee walked down a short flight of steps to go investigate what was making the noise. When he reached the ground floor he felt a pain in his right leg behind the knee. Employee was diagnosed with a muscle strain in the right leg.

Employee was picking up a pallet that had fallen off a conveyor onto the ground. The employee reached down and grabbed the pallet with his right hand and began lifting it, when he felt a pain in his right shoulder.

Employee bent over and picked up a bag of material without bending his legs and felt a pain in his lower back. Diagnosis - acute lumbar strain. Prognosis - Good.

Employee was walking down a stairway and missed the third to last step causing him to lose his balance, fall forward and strike the concrete floor with his chest. Diagnosis - Chest Contusion.

The management employee slipped on wet pavement while traveling to the office and twisted his knee. He returned to work and continued working at his regular job until continued pain and problems with his knee lead him to have surgery on 06/24/05. Due to the surgery he was out of work until his return on restricted duty on 07/18/05.

A bag of material fell off the pallet and wedged between the pallet and elevator frame. Employee tried to pick it up with one hand and felt a pain on right side.

Installing side plates on JC Steele feeder. Went to help pull side plates to re-insall and twisted or overextended and felt something pull in lower back.

Employee was cleaning under east clam shell with bobcat removing material when clam shell opened releasing material. Material went inside his boot causing burns on both feet.

EMPLYOEE WAS WALKING OUT THE DOOR FROM THE FOREMAN'S ROOM IN THE SHIPPING DEPT WHEN HE TRIPPED ON A PIECE OF BAR GRATE. EE LANDED ON HIS HANDS. THIS CAUSED AN IMPACT ON HIS ELBOW WHICH TOR THE TENDON FROM THE BONE.

EE WAS CUTITNG A PIECE OF GUM RUBBER WITH UTILITY KNIFE, WHEN EE CAME TOT HE END OF THE RUBBER THE KNIFE SLIPPED AND CUT HIM ON THE LOWER RIGHT THIGH AREA. THE LACERATION REQUIRED 3 STITCHES.

EE WAS REMOVING WIRE CABLE FROM FRONT END LOADER. CABLE HAD A FRAY WHICH PUNCTURED EE'S RIGHT HAND PINKY FINGER. EE RECEIVED 3 STITCHES.

EE WAS FOLLOWING KILN SHUT-DOWN PROCEDURES. WHEN THE KILN IS SHUT DOWN, THE KILN ASST MUST KEEP THE ZURN FAN SPINNING FOR APPROX 1/2 HR. HE BUMPS THE FAN WITH A 2X4 BOARD. EE CAUGHT THE BOARD IN THE FAN CAUSING IT TO SLAM THE BOARD DOWN (ON THE END HE WAS HOLDING) ON THE FAN HOUSING, MASHING HIS RT INDEX FINGER. FINGER REQUIRED 7 STITCHES.

EE WAS REALIGNING BAGGED MATERIAL ON THE SOUTH PALLETIZER WHEN HE WALKED OVER THE SAFETY LINE, CROSSED AN ELECTRIC EYE WHICH ACTIVATED THE RAKE ARM, WHICH PINNED HIM BETWEEN THE ARM AND ACROSS BAR. EE WAS BRUISED IN THE PELVIS AND LOWER BACK AREAS.

EMPLOYEE ALLEDGES THAT HE SLIPPED, HYPER-EXTENDING HIS GROIN AREA WHILE WALKING OFF A PLATFORM OF A SUPER SACK MACHINE. EMPLOYEE WAS OBTAINING A SAMPLE OF PRODUCT. EMPLOYEE WAS DIAGNOISED WI TH A HERNIA ON 3-28-00.

INSTALLING COUNTERWEIGHT BOX IN MAST GUIDE WITH ASSEMBLY LAYING FLAT. BOX ROLLERS NET ALIGNED AND WEDGED IN GUIDES. EE WAS ATTEMPTING TO ALIGN BY HAND. BOX SLID FORWARD AND MASHED FINGER TIP.
