Employee was cutting 3/4 inch rubber hose with pocket knife when knife slipped cutting thumb on right hand requiring 4 stitches.

A mechanic was removing a bearing from the ball mill discharge auger shaft with a hammer and a shard of metal stuck another mechanics right arm.

Employee was grinding on a pipe when some metal filings struck his eye. The employee was wearing safety glasses and a full face shield.

Employee was installing screens on screening tower and felt a pain in arm while pushing screen into position.

A Maintenance employee was walking down a stairway carrying a grease gun in his left hand and had his right hand on the handrail and mis-stepped placing his right foot in an awkward position and twisting his right knee.

The employee was removing angle iron framing and wood from a MCC building. The employee used methods of drilling and cutting during the task, and all work was above the employee's head. The employee was wearing a pair of safety glasses at the time of the incident. A piece of debris flew in-between the employee's safety glasses and forehead and lodged in his (R) eye.

Employee was moving moving railcars, when the bracket on the railcar that the cable is attached to broke and struck the employee in the face, causing a laceration on his right cheek.

EE was removing an air exhaust muffler from a compressed air dryer system.After releasing the residual air from the air tank and line the EE unscrewed the valve not realizing there was still some stored pressure inside the line which blew small desiccant beads out striking the back of both hands and the back of both legs.

EE CUT FOREARM WITH RAZOR OPENING A BOX OF PAPERBAGS.

WASS RETRIEVING SAMPLE BUCKET FROM WORK PLATFORM WHEN THE PLATFORM BROKE CAUSING THE EMPLOYEE TO FALL. EMPLOYEE SPRAINED BOTH ANKLES.

EE WAS CLEANING A DUST COLLECTION PIPE (BY HITTING PIPE WITH A HAMMER) WHEN FOR UNKNOWN REASONS THE EE REACHED INTO THE DOOR AND UP APPROX 8 INCHES WHERE HE CAUGHT HIS 3 FINGERS. (INDEX-ABOVE 1ST JOINT, MIDDLE-ABOVE 2ND JOINT, RING-ABOVE 1ST JOINT) EE DOES NOT REMEMBER MUCH OF THE EVENTS.
