EE was installing air header onto existing HDPE pipe valve. EE tightened the vic between the valve and header. EE realized the header needed to be rotated so EE used the impact and started to loosen the bolt. EE inadvertently loosened the bolt on the charged side of the valve causing the header to break free from the vic.

EE was swamping on the back of the track lowboy that was loaded with an electrical cabinet. The cabinet came in contact with the lockbox for the portal gate causing the lowboy to come to a sudden stop. The stop caused the employee to come in contact with the electrical cabinet.

The electric motor for the pump on the 1700 level lost one leg of the 3 phases. This caused the remaining two legs to heat up because they were drawing more amperage. The varnish on the coils and insulation started to smoke as a result of the motor heating up.

A hydraulic hose fitting failed on the Chippy hoist braking system. The hydraulic braking system lost pressure and the brakes set. The hoist was not operational.

Miner was operating a jack hammer to remove an unused concrete compressor pedestal. While operating the jack hammer the miner pinched EE's index finger between the jack hammer and a bolt protruding from the concrete pedestal.

Miner became light headed and passed out while standing on hoist cage. When passing out the miner collapsed on self twisting right leg causing a spiral fracture of lower leg.

Chippy hoist became inoperative while transporting miners. Miners were notified of the hoist problems and were transferred to the south conveyance of the double drum hoist. Miners were out of the mine within 20 minutes. Hoist problems occurred due to a burned-out coil on a contractor.

Sunshine Mine lost power due to damage caused by a tree contacting main electrical service line owned & maintained by Avista Utilities. Loss of power occurred while miners were working in Jewell shaft. They were temporarily stuck in shaft till power was restored & they were hoisted to surface. Power was restored by Avista at 10:04am & the miners were hoisted to surface at 10:10am.

The Jewell shaft double drum hoist engineer was dumping the north skip. After the skip was emptied, the skip was lowered before opening the wind gate at the shaft collar. The skip sat on the gate causing the rope to slack. When the slack was gathered, the hoist rope kinked. No personnel were down shaft at time of occurrence. Hoist rope will be repaired (cut).

A non employee committed suicide on land recently purchased from US silver/Galena mine. This is an isolated area near the cemetery in Osborne Idaho.

A geologist at the 6980 decline of the Sterling tunnel system, was in the process of connecting a trailer mounted porta-potty to a Kubota utility vehicle, for remove to the surface for sanitary cleaning. While in the process of connecting the trailer coupler to the Kubota rear ball mount, the geologist middle finger of his left hand was impacted between the two mechanisms.

Employee was tightening a # 7 Dywidag thread bar bolt with three cartridges of Ground lok/resin with a jackleg. The jackleg got away from the employee and the handle on the leg hit his right knee, dislocating it.

On 11/7/12, employee was trying to free the splitset bolt driver from splitset bolt that he previously installed with a jackleg drill. Employee pulled on the jackleg drill to free the driver, but the pressure had bled off the leg. The impact strained the employee's shoulder. On 11/16/12, employee went to the doctor. Physical therapy was prescribed. Physical therapy began 11/20/12.

A electrician and supervisor were trouble shooting un-energized equipment when the electrician suffered a medical related seizure.

A Geology Intern left foot became lodged between two branches resulting in a trip/fall into another branch in the ground, puncturing her left shin. Conditions that contributed to the accident were: steep ground; dense foliage; improper site-descent, and improper clothing (Boots). Damage was to the left shin from a puncture wound that required three stitches.

Employee complained of chest pains and was immediately taken to hospital. It was found to be a 100% blockage of one artery and he was immediately taken in for surgery to place a stint. Not a work related illness.

workers were putting a 5/8 inch cable over a cross beam, the first man was not prepared and it caught him off balance and he struck his ear on a upright of chamber causing a small laceration on his right ear.

Repair on 3100 was repairing rail with an Oxygen accetylene torch. Sparks got drawn behind bulkhead by short circuit to main ventilation exhaust. Result was large uncontrollable fire resulting in total mine shut down.

Material being dropped from the sheave wheel platform of the Jewell Shaft head frame struck a power pole guy wire causing power conductors to make contact and trip out the power to the Jewell Shaft hoists.

Employee was walking across mine yard at end of shift, slipped on ice, fell and hit his head on ground. At visit with physician on 3-14-11 the employee was given work restrictions by physician.

Timber movement in Silver Summit Shaft. Damage or impairment is unknown at this time. Informational update for 7000-1 filed on 1/29/10. Upon further examination a major timber failure was found in the Silver Summit Shaft. At this time ventilation has not been compromised, but major shaft repair will be required to re-establish safe travel. Per 2/5 doc 220100340005.

A 2-yard LHD bucket was slung under the Chippy Hoist skip being hoisted up the Jewell Shaft. A guide shoe came loose causing the bucket to rotate into and tearing out 2 shaft guides causing the bucket to lodge into the timber. 4 shaft crew members riding inside the skip were uninjured but briefly stranded before being transfered to the adjacent Double Drum Hoist skip.

Employee was exiting 2 yd loader, slipped on mud on step, lost balance & banged right elbow on loader. Medical treatment included therapy. Employee reported injury on 8/5/08 but didn't seek medical attention until 8/19/08.

The employee was installing rock bolts in the back with a jackleg drill. After collaring a hole the EE briefly looked away. When he looked back, a rock had come loose and it bounced off the machine hitting the employee in the upper lip. The result was cut lip that required sutures and a chipped tooth.

Employee was working to repair a plug on an underground locomotive. He inadvertently let the positive and negative battery leads make contact. The contact created an arc which caused burns to the left hand. The employee received medical attention that included prescription medication.

The hoist operator of the Jewell Shaft double drum hoist was lowering three employees down the Jewell Shaft when he noticed a spike in the reading on the hoist amp guage. He immeditely stopped the hoist and the hoist was not moved again until personnel went down on the chippy hoist to do a visual inspection of the shaft & the skip the employee's were riding on.

EE was bolting the footwall area of his heading, when the slab fell into the jackleg and pushed EE against the opposite rib, causing injury to his right shoulder.

Employee was pushing a metal shipping crate loaded with supplies onto the skip in the shaft. As he pushed the crate onto the skip, the loaded supplies shifted pinching his finger (left pinky finger) between the supplies and the shipping crate. Resulting injuries were torn fingernail and broken bone in finger tip.

The employee opened a man-door leading into the shop area. After stepping through the man-door the employee had his hand resting on the door frame while he attempted to unlatch the large shop door. The man-door swung shut and pinched his hand between the door and frame. This caused a laceration on the ring finger of the right hand requiring sutures.

The employee was showering at the end of his shift and was washing his face when he felt a foreign object in his left eye. This injury did not involve any aspect of compliance with rules & regs, nor did it involve mine equipment or the mining system. It did not involve the skills, profiency, training or attitude of the miner. PPE does not apply due to location of injury.

The employee was in the mine dry changing into his work clothes to start the shift. While trying to pull his work pants on he lifted one of his legs and pulled something in his back. After seeking medical attention the employee was put on lifting and work restrictions.

A valve on a slurry discharge line was inadvertently left closed, causing a pump line to build enough pressure to leak out of one of the fittings. The EE was not wearing safety glasses as required. Some slurry containing lime got into the EE's left eye, causing discomfort. The EE received medical attention, including prescription medications.

The employee was operating a 22B track mucker in a track drift heading. While mucking along the left side of the rail the mucking machine became unstable and tipped, briefly pinning the employee between the machine and rib.

The employee was using a nail bar to pull a nail out of a timber, the nail bar slipped off of the head of the nail and struck the employee above the left eye.

The Chippy hoist operator was bringing an empty skip up the shaft at normal operating speed. As he started to set the brake to slow down, the brake system fully engaged, causing the hoist to immediately stop. This caused the wire rope to whip and jump off of a sheave wheel.

The employee was lifting a tarp off of some cable spools and apparently disturbed a nest of wasps. The employee got stung 3 times and began to show signs of an allergic reation so he was transported by ambulance to a hospital where he received medical treatment.

The employee was taking a broken clamp off a length of pipe. His arm came in contact with the pipe as he tried to remove the clamp and a metal splinter became imbedded in his left arm.

Miner had completed installing a 6' resin bolt. Miner was lowering the jackleg drill. The driver hung on the installed bolt and dropped as the drill was lowered. The bolt dropped and hit the miner's little finger on his right hand which was on the drill. It struck the little finger between the knuckle and the first joint causing a fracture of the bone.

The EE was working in the surface carpenter shop cutting wood on a table saw. The cut off piece of material began to hang up in the saw blade & the EE reached for the piece of material, resulting in one of his fingers making contact with saw blade.

The EE was walking between the rails in a track drift and stepped in a hole, between two rail ties, twisted his left knee.

Driller was moving drill to new azimuth. Had hooked cable end hook to drill, sticking hook in top of pipe on drill. When pressure was applied to cable to pull drill, the hook slipped out of pipe and snapped back, striking the driller on the right cheek of his face.

The employee was working on transporting some old, partially filled 8"steel pipe. In the process of offloading the pipe from a timber truck by hand he tried lifting one end by himself & strained muscles in his left hip.

Removing two poles which were hooked to a crane to lift poles out of the way. The first pole was sawed off. As the second pole was being sawed, the load shifted and employee was not able to get out of the way fast enough. The pole bumped him and he fell head first into the hole dug for the replacement pole resulting in two stitches in the forehead.

STEPPED OFF STAIRS TO BLACKTOP AND FELT A SHARP PAIN INTO HIP AND DOWN LEG.

LIFTING MATS THAT WERE LAYING ON BUNDLES OF SPLITSET BOLTS AND PULLED MUSCLE IN LEFT SHOULDER. FIRST TREATED AND RELEASED ON 4-23-01.

DRIVING A BOLT INTO THE BACK AND A ROCK FELL AND CUT HIS ARM.

WALKING IN PARKING AREA AND SLIPPED AND FELL ON ICE AND LANDED ON BACK. FIRST REPORTED 2-12-01.

DRILLING HOLE IN BACK FOR A ROCKBOLT WHEN A ROCK FELL AND STRUCK HIM ON THE HEAD.

WHILE LOWERING THE CAGERS IN SINGLE DRUM ON THE NORTH SIDE THE HOIST WAS BEING SLOWED TO STOP ON THE 3700 LEVEL WHEN THE HOIST SUDDENLY GAINED SPEED AND WENT APPROXIMATELY 80' BEFORE THE HOIST MAN COULD STOP IT. ON WEDNESDAY AT 1000 WE FOUND A BAD RELAY COIL AND REPLACED IT.

12/30/00 WE SENT THE SOUTH END OF THE ROTATROL IN TO HAVE IT CLEANED & CAM TURNED. ON 1/2/01 WEINSTALLED THE ROTATROL & HAD THE HOIST RUNNING AT APPROXIMATELY 11:00 AM.

EMPLOYEE WAS LOADING HIS ROUND WHEN A ROCK FELL AND HIT HIM ON THE BACK OF HIS HEAD.

ON 12-13-00 AT 6:00 AM WHILE ATTEMPTING TO LOWER THE CREW, THE JEWELL CHIPPY HOIST ROLLED BACK. THE HOIST WAS TAKEN OUT OF SERVICE. EXTENSIVE TESTS WERE PERFORMED WITHOUT FINDING THE CAUSE AND A FIELD ENGINEER FROM GE INDUSTRIAL SYSTEMS WASCALLED AND ARRIVED ON 12-14-00. A VRG RELAY WAS REPLACED AND EXTENSIVE CHECKS WERE MADE BEFORE PLACING HOIST BACK IN OPERATION ON 12-15-00.

ON 12-12-00 AT APPROX 3:00 AM THE PRIMARY 2300 VOLT CONDUCTORS FEEDING THE CRUSHING PLANT SHORT CIRCUITED IN THE CONDUIT. POWER WAS RESTORED ANDTHE GRAVEYARD CREW WAS HOISTED AT APPROX 5:30 AM. DAY SHIFT STARTED AT 6 AM. SINCE POWER NEEDED TO BE TURNED OFF TO FACILITATE REPAIR TO THEFAILED CONDUCTORS, ORDERS WERE GIVEN TO EVACUATE THE MINE AT 8:55AM, POWER RESTORED 1:15 PM.

AT 1005 THE JEWELL CHIPPY HOISTMAN CALLED AND STATED THE HOIST WAS DOWN. ELECTRICIANS AND UPPER MGMT. WERE INFORMED. WE THEN CALLED THE HOISTMAN TO CONFIRM THE HOIST WAS NOT OPERATING. THE HOI ST WAS OPERATING AND THEY COULD NOT FIND ANY PROBLEM. WE DECIDED THAT UNTIL WE COULD DETERMINE THERE WAS NOT A PROBLEM, WE WOULD CONSIDE THE HOIST DOWN AND START IMMEDIATE EVACUATION AT 1020.

RECEIVED WORD FROM THE CONSIL THAT THE HOIST WAS DRAWING TOO MANY AMPS. DETERMIONE IT TO BE UNRELIABLE AND BEGAN AN IMMEDIATE EVACUATION FROM ALL LEVELS UNDERGROUND. ELECTRICIANS WORKED ON THE HOIST AND FOUND A SMALL MERCURY SWITCVH THAT HAD FAILED AND REPLACED IT. RECEIVED WORK THE HOIST WAS RUNNING GOOD AT 1005 AND PUT THE CREWS BACK TO WORK.

EMPLOYEE WAS USING A DIE GRINDER TO ENLARGE A HOLE IN A BRAKE PLATE WHEN HIS HAND SLIPPED AND CONTACTED THE TOOL BIT.

REMOVING A SINKING HAMMER FROM THE TOP SHELF OF THE PARTS CABINET WITH A 1 TON CRAWL. WHEN THE SINKING HAMMER WAS LIFTED, IT CAME TOWARD THE EE CAUSING HIM TO BACK INTO A 83 MACHINE. THE EE ST RUCK THE 83 MACHINE IN THE LOW BACK AREA. FIRST TREATED AND RELEASED 11-16-00.

EMPLOYEE STEPPED OFF LHD LOADER AND TWISTED HIS ANKLE.

PULLING ON HUNG STEEL. FIRST TREATED 10-26-00.

EE SLID HAND DOWN EDGE OF MAT AND CUT HAND ON SHARD OF METAL.

JUMPED UP ON THE LEG OF A JACKLEG MACHINE TO SHOVE THE CHUCKTENDER DOWN, THE LEG TWISTED AND THE EE FELL ON HIS LEFT LEG. FIRST TREATED AND RELEASED ON 10-31-00.

EE BUMPED HIS RIGHT ELBOW WHILE SWINGING A PICKEROON. THE EE WAS CHECKING GUIDES AND WHEN HE SWUNG THE PICKEROON HE STRUCK HIS RIGHT ELBOW ON A WALLPLATE.

ALLEGED SILICOSIS- THIS IS BEING INVESTIGATED.

ON 9/13/00 WHILE DOING THE SAFETY CHECK ON THE CONSIL HOIST, WE WERE CHECKING THE DOGS. TO DO SO, YOU MUST RELAX THE CABLE. THIS SET OFF THE SLACK-ROPE INDICATOR. THE LIGHTBULB FOR THE SLACK-R OPE INDICATOR WAS BURNED OUT. WHEN THE SLACK ROPE SHUT OFF THE HOIST, THE HOISTMAN DID NOT THINK IT WAS FROM THE SLACK-ROPE BECAUSE THE LIGHT DIDN'T COME ON. HAPPENED AT 6:45 AM. ELECTRICIAN

EE WAS HELPING CHANGE OUT A CHLORINE BOTTLE WHENTHE VALVE WAS TURNED BACK THE SYSTEM BEGAN TO LEAK & THE EE INHALED CHLORINE GAS. THIS WAS REPORTED 9/14/00. FIRST TREATMENT WAS 9/14/00

WHILE DRILLING TO BOLT THE BAKC, A ROCK DISLODGED AND HIT EE ON THE SHOULDER AND BACK.

ATTEMPTING REPAIR ON 400 AMP CIRCUIT BREAKER. A SHORT CIRCUIT OF THE PHASE TO PHASE CAUSED ELECTRICAL ARC FLASH RESULTING IN FALSH BURNS AND QUESTIONABLE ELECTRIC SHOCK TO 40% OF THE BODY.

EMPLOYEE WAS PUTTING OUT FIRE ON PARTNER'S CLOTHES AND BURNED HIS HANDS.

AS A SAFETY PRECAUTION, THE POWER WAS CUT TO THENORTH SUBSTATION WHILE INVESTIGATING AN INJURY TO AN ELECTRICIAN ATTEMPTING TO REPAIR A 400 AMPCIRCUIT BREAKER. THE JEWELL DOUBLE DRUM HOIST WAS AFFECTED FROM 3:30 PM TO 4:35 PM. THERE WERENO CREWS UNDERGROUND AND THE AFTERNOON SHIFT WASCANCELLED. POWER WAS RESTORED TO THE HOIST AT 4:35 PM.

FIXING A LIGHT AND SPIDER BIT HIM.

SUPERVISOR WENT TO THE #4 SHAFT HOIST ROOM ON 3100 LEVEL IN AN ATTEMPT TO RESET THE HOIST AND FOUND THE HOIST ROMM FULL OF SMOKE. THERE WERE NO SIGNS OF FIRE.

EE WAS USING PRY BAR TO STRETCH THE AIR HOSE ON THE JUMBO. THE EE SLIPPED & FELL WHEN THE BAR SLIPPED CAUSING INJURY TO LEFT KNEE. THIS INCIDENT REPORTED AT TIME BUT EE DID NOT SEEK TREATMEN T UNTIL LATER.

WORKING IN THE ROCK HOUSE SHOVELING WET, HEAVY MUCK AND THROWING THE MUCK ON THE BELT AT SHOULDER HEIGHT, EE PULLED A MUSCLE IN HIS RIGHT SHOULDER AND SOUGHT TREATMENT ON 7/20/00.

WORKING IN ANTIMONY PLANT, DUMPING BARRELS INTO TANK, PINCHED END OF FINGER BETWEEN AN ANGLE IRON AND BOTTOM OF BARREL.

A JUMBO WITH A SMALL LEAK IN THE HYDRAULIC HOSE BURST INTO FLAMES IN THE 31SR1D AREA AT APPROX. 5:40 P.M. THE CREWS WERE EVACUATED FROM THE MINE. SUNSHINE MINE RESCUE TEAM ENTERED MINE & DET ERMINED THAT THE FIRE WAS OUT; THEY SECURED THE AREA AND SWEPT THE EXHAUST SIDE OF THE MINE TO BE CERTAIN IT WAS CLEAR.

BOLTING IN 37ER1-H WHEN FOOTWALL AND FACE AIR BLASTED AND EE WAS STRUCK WITH ROCK. CUT LEFT RING FINGER REQUIRED STITCHES.

ROCK BURST OCCURRED IN 37ER1-H EAST. MINIMAL DAMAGE ON EAST SIDE WITH 2 TONS DISPLACED.

WORKING UNDERGROUND BARRING DOWN BAD GROUND, ROCK FELL FROM BACK AND HIT HIM CAUSING MULTIPLE BRUISING, HAND, NOSE, SHOULDER, HEAD.

SLIPPED AND FELL ON POINTED END OF GRIZZLY PICK AND PUNCTURED RIGHT EYE.

WORKING IN ANTIMONY PLANT CHANGING A BAG FILTER IN THE TAILS TREATMENT. PULLING OUT FULL BAG THAT WAS STUCK AND STRAINED LEFT SHOULDER.

ON JUNE 16, 2000 AT 3:08 P.M. A TREE FELL ONT HE POWER LINE CAUSING A TRANSFORMER TO BLOW AT THE MINE. THIS TOOK OUT THE POWER TO THE CHIPPY HOIST, HOWEVER WE COULD STILL RUN THE DOUBLE-DRUM H OIST BY ISOLATING THE POWER. THE POWER WAS FULLY RESTORED AT 4:28 P.M. AND CREWS WENT TO WORK. AT 6:00 P.M. THE POWER WENT OFF AGAIN AND WAS RESTORED AT 8:50 P.M. ALL CREWS SAT ON STATIONS DU

RIDING UP IN CAGE WHEN HOIST KICKED OUT. EMPLOYEE WAS BOUNCED AROUND AND STRAINED HIS BACK. EMPLOYEE DID NOT SEEK TREATMENT UNTIL AUGUST 3, 2000.

WORKING UNDERGROUND IN 37 S78 PACKING EQUIPMENT,LOST BALANCE AND FELL AGAINST FOOTWALL, BRUISINGLOWER BACK (TAIL BONE). REPORTED AS A MINOR IN-JURY 5/22/00. EMPLOYEE SOUGHT MEDICAL TREATMENT ON 6/8/00.

WORKING UNDERGROUND AT 3100 STATION CLEARING GRIZZLY, HIT LARGE ROCK AND DOUBLE JACK BOUNCED BACK AND SPRAINED WRIST.

EMPLOYEE, WORKING UNDERGROUND ON 3100 OPERATING LOCOMOTIVE, FAILED TO OPEN AIR DOORS. HE WAS GOING TOO FAST AND PLOWED THROUGH AIR DOORS.

REMOVING AIRLINE IN SHAFT. EMPLOYEE HAD RIGHT FOOT ON LADDER. LACING BROKE AND THREW ARM OVER A WALL PLATE TO CATCH HIMSELF FROM FALLING. WALL PLATE HAD RUSTED CABLE WRAPPED AROUND IT AND E MPLOYEE CUT HIMSELF ON RUSTY CABLE.

WORKING UNDERGROUND IN ER1-H EE SHANKED 6' STEEL WHILE DRILLING CLOSE TO FACE, CAUSING METAL SLIVER TO GO INTO RIGHT EYE.

DIAGNOSED WITH CARPAL TUNNEL SYNDROME. RIGHT HAND SURGERY 11/27/00.

EE WORKING UNDERGROUND AS A MINER CLAIMS HE HURT HIS LOWER BACK, BUT DOES NOT KNOW WHAT HAPPENED TO HURT HIS BACK.

3-28-00 DIAGNOSED WITH BILATERAL CARPAL TUNNEL, LEFT HAND SURGERY 5-19-00.

EE RIDING UP THE JEWELL SHAFT WHEN DUST BLEW INTO SKIP AND LANDED IN EMPLOYEES LEFT EYE. EE WAS WEARING PRESCRIPTION SAFETY GLASSES.

WHILE WORKING UNDERGROUND, EE BLOW PIPING BULK HEAD, ROCK FELL FROM HANGING WALL HIT AND CUT ARM.

WORKING UNDERGROUND ON 2700 LEVEL, OULLING JACKLEG DRILL STEEL OUT OF RATTY HOLES AND PULLED MUSCLE IN BACK. SOUGHT TREATMENT ON 03-27-2000.

BARRING DRIFT BACK THROUGH THE BACK LAGGING, A THREE LB ROCK CAME OUT OF THE BULKHEAD OF THE SET OF TIMBER HEWAS STANDING UNDER AND BOUNCED OFF THE RIB AND STRUCK HIM IN THE LEFT LEG(CALF MUSC LE). SOUGHT TREATMENT ON 03-27-2000.

HOISTMAN MISSED STATION ADN EMPLOYEE TRIED TO GRAB THE BELL LINE OT STOP THE SKIP. HIS HAND GOT TWISTED BETWEEN THE SKIP AND THE CEMENT AND SPRAINED LEFT WRIST.

WORKING IN ANTIMONY PLANT WHILE CLEANING PERRIANPRESS AN OBJECT FLEW UNDER SAFETY GLASSES AND INTO EYE.

WORKING IN THE ROCKHOUSE, EMPLOYEE INSTALLING A NEW GEAR BOX ON #3 BELT. WHILE PUTTING ON GEAR BOX, HAND SLIPPED OFF OF BOX CAUSING HIM TO HIT ELBOW ON LOADING STATION AND BRUISED RIGH T ELBOW. INJURY OCCURRED 2-26-00.

WORKING ON THE SURFACE, WALKING ACROSS THE MINE YEARD, EE SLIPPED ON ICE AND TWISTED LEFT KNEE. EE SOUGHT TREATMENT 2-24-00.

WORKING UNDERGROUND IN 37ER1H, EMPLOYEE DRIVING AN LHD LOADER WHEN DUST FLEW OUT OF BUCKET AND INTO LEFT EYE.

WORKING UNDERGROUND IN 41ER6, TAKING MINI-BATS OUT OF BARREL, TURNED SIDEWAYS AND STRAINED LOWR BACK.

EE WORKING IN THE PLANT PULLING PLATES FROM ELECTROWINNING CELLS. ADJUSTING CELL PLATES WHEN CELL ARCED AND CAUSED A FLASH BURN TO THE EYES.
