Was cleaning out a hopper and the gate valve closed on right hand, injuring fingers

Repetitive motion mixing grout causing strained elbow.

EE was cleaning up a pile under storage bins removing a heavy ice chunk from under a conveyor belt resulting in shoulder strain.

EE was turning off the bleeder valve on propane tank on the fork lift.

Was repairing big sifter leaks and when I picked up the door and turned I felt an instant pain in my groin and abdomen area. It felt like a strain or pulled muscle. Was diagnosed as a hernia 12/8/10. See Attachment.

Checking for spark on furnace and it ignited and caused a flash burn on right eye.

Was using hand drill, drill bit got stuck in the hole. Drill turned, pinching hand between it and a bolt, cutting finger.

Was removing small piece of square tubing - pinched finger, causing a small cut which became infected.

Was using an acetelyne torch, the torch sparked back after being shut off burning left hand.

EE was pulling out a 52" super sack of perlite and strained back causing a pinched nerve.

Stepped in a deep hole, causing knee to get cut.

Was cleaning a mold. Moved latex to fit into a mold. Hand was on edge of mold and got caught between molds which cut finger tip off.

Was unloading a trailer load of perlite ore. Was on east side of trailer, opening it to unload. wind came from the west and blew perlite into eyes.

Was pulling rocks apart on pullline & placing them in boxes & pallets.

Right foot slipped off of short platform he was standing on and caused back strain.

Was talking to another EE while using staple gun and accidently shot fingers with gun.

EE was cleaning molds and leaned to the left to scrape the mold. There was a metal pole behind EE, two molds caught & lifted up which caught EE between the pole and the mold under my left ribs. EE twisted & pulled herself out.

Was pulling rock out of mold - rock slipped and smashed left ring finger.

I pulled a stone out of a mold and it came back back and hyper-extended my wrist.

Pulled pallet from stack & received a splinter in his left hand.

STAPLE GUN MISFIRED WHILE HAND WAS IN THE WAY.

Pulled chiseled taupe stone 24X24 from mold and hurt arm while bracing weight of mold.

Mold was being turned, when left thumb was caught between the table and mold.

While stapling the air hose got pulled which made the stapler jerk and stapled thumb.

Was unhooking trailer when hitch slipped forward and smashed foot between trailer and bumper.

Toe was under edge of low boy trailer while unhooking chains. Trailer came down on foot, crushing toes.

Lifting a wooden pallet from one stack to another.

Kneeling on the concrete while setting door, when he stood up his knee popped.

Wrench slipped and hit left cheek on face, spitting skin open.

LIFTING BRAKE DRUM (STRAINED GROIN).

While standing on a ladder, he lost footing and fell off, hitting his head.

He dropped a gear box on fingertip.

Was using a cut torch to cut metal when his coverall sleeve caught fire, burning left arm just above elbow to wrist.

While unloading trailer, the door closed on his hand.

Was getting iron braces stacked so we could hook them up to the crane and hand was between two of them and a little finger got broke.

HOLDING TIES WHILE WELDING, FLASHES BURNED BOTH EYES.

Slid pallet with side of foot.

The rope for the train car puller was jerked out of hands. Broke 4th and 5th fingers on right hand.

WAS INSPECTING BACK-FILLING OPERATION. ONE MAN WENT TO MOVE THE COMPACTOR WACKER AND EE REACHED TO HELP HIM MOVE IT & WHEN HE DID EE FELT A SNAP & SHOOTING PAIN IN UPPER ARM.

WAS VACUUMING AND BACK POPPED OUT.

WORKING ON A TWO-LEVEL CATWALK HE STEPPED IN A HOLE, LOST HIS BALANCE AND FELL THROUGH. HE HIT HIS ARM AND RIBS ON CATWALK; FELL APPROXIMATELY 6 TO 7 FEET. BROKEN RIBS, BRUISES, SCRAPES.

EMPLOYEE WAS CUTTING THE BANDING STRAPS FROM A BUNDLE OF BAGS AND THE KNIFE SLIPPED AND CUT HIS LEFT HAND.

NO ACCIDENT; ROUTINE WORK HAS CAUSED CARPAL TUNNEL SYNDROME.

LIFTING BAGS OF PUMICE, RESULTING IJN A HERNIA.

WAS LIFTING A BAG OF PUMICE AND PUTTING IT DOWN AND FELT PAIN IN GROIN, RESULTING IN A HERNIA.

WAS GETTING A TEST AND ACCIDENTALLY STUCK HAND IN ROTER IN TOP OF MICRO SIZER WHICH CUT TIP OF MIDDLE FINGER ON RIGHT HAND.

GUST OF WIND BLEW IN THROUGH DOOR AND BLEW PUMICE IN EYE.

NO ACCIDENT--ROUTINE WORK HAS CAUSED CARPAL TUNNEL SYNDROME.

HE WAS WALKING PAST THE GRAVITY TABLE, HIS HAND BRUSHED UP AGAINST SHARP METAL LEAVING A 1 INCH CUT IN TOP OF LEFT HAND.

EMPLOYEE WAS LOADING CRANE ON LOW BOY, WHEN LANDING GEAR CAME DOWN & LOW BOY CAME DOWN ON HIS FOOT, SMASHING RIGHT FOOT, BREAKING BIG TOE & CAUSING TENDON AND LIGAMENT DAMAGE.

HE WAS DOING MAINTENANCE ON BAGGING ROBOT, AIR RAM OPENED EXTENDING FINGERS OUT AND STRUCK HANDCAUSING A CUT IN LEFT INDEX FINGER.
