Amrize Cave In Rock Quarry Metal/Non-Metal

Employee using come alongs and chains to move curtain in Hazemag crusher. Chain broke and hit the employee in the hard hat. Causing the hard hat to give the employee a laceration on forehead and and safety glass causing a small cut on nose.

Employee reported on July 14 that he had sustained an injury at work on July 12, 2011 while using a come a long. He was sent for evaluation and the physician recommended further testing to confirm if his condition was directly related to his work activity. Received confirmation from physician on 07/20/2011 that is directly related to his work activity.

While performing a pre-shift inspection on a 992G Loader, the employee fell from the rear bumper when he attempted to cross to the other side via the rear bumper. He sustained a fracture of the right heel.

Employee was operating a Cat 988G loader when a hydraulic line failed causing hydraulic fluid to go through the front window and out the back window - Striking the Employee in the chest and ribs.

While working with a torch to cut a steel pipe that was apaproximately 12" from the highwall and anchored at the top. The pipe moved away from the highwall approximately 20" and dislodged rock. Several rocks fell, one striking injury on the head knocking his hard hat off. A second rock fell striking him on the head, causing a laceration.

The employee was getting out a prybar out of the service truck and hit his rt thunb on the side of the door causing a tiny fracture. He went to get an x-ray and was determined positive and drainage of blood under nail plus immobilization of the thumb.

The employee was walking the job site area, when he slipped on ice and twisted his ankle.

An employee was stripping a barge of the haul cables and the haul cable was caught on the outside of the timberhead causing tension. The employee went to release and the cable struck the employee in the nose/mouth causing a laceration and hair-line fracture to his nose. The employee was sent to the emergency room and conducted X-rays and an MRI.

THREE EMPLOYEES WERE REMOVING DUSTPIPE FROM THE PLANT. THE DUSTPIPE WAS SLID ALONG A HANDRAIL AND THEN DROPPED ONTO A WALKWAY. WHEN THE DUSTPIPE WAS DROPPED TO THE WALKWAY THE EMPLOYEE'S HAND WAS CAUGHT IN BETWEEN THE END OF THE DUSTPIPE AND A METAL AIR LINE.

Employee was using post hole driver to drive in a ground rod, "pinched" finger between driver and rod. Employee suffered severe laceration to left hand. Employee was taken to hospital where he received 12 internal stitches 11 external stitches.

Employee had placed barge reports in mailbox on #5 walkway. When he proceeded down steps his foot slipped and he fell down 6 steps to the ground. There was a heavy dew at the time which caused the steps to be slick.

Employee was holding onto a pulley of D screen with both hands. Another employee was using a 3 lb. hammer to tap the pulley off the shaft when he missed and struck Mike on the ring finger of the left hand.

Employee was lifting a section of chute about 5' long. The section of chute was on front of boat and being moved to captive barge. The employee lifted one corner of chute and felt sharp pain in his back. Delay in reporting due to waiting on medical records.

Employee broke his finger when it became pinched between the walk behind bobcat loader and the handrail in tunnel.

Employee raised up and his mouth struck a piece of angle iron. The employee did not have eyes on path. Employees top lip required sutures.

EMPLOYEE HAD JUST SERVICED A HAUL TRUCK. AS HE WAS COMING DOWN THE LADDER, BOTH FEET SLIPPED ON THE BOTTOM STEP. WHEN HIS FEET CAME IN CONTACT WITH THE GROUND IT JARRED HIS LOWER BACK.

THE EMPLOYEE WAS IN THE PROCESS OF REDIRECTING THE WATER. WHEN THE EMPLOYEE WAS TURNING THE WHEEL ON THE WATER VALVE IT BECAME STUCK. WHEN THE EMPLOYEE WAS ABLE TO FREE THE STUCK WHEEL HE PINC HED HIS RIGHT THUMB BETWEEN THE INNER STRUCTURE OF THE WHEEL AND THE CAP BOLT OF THE VALVE.

EE WAS SPLITTING A ROCK SAMPLE. HE REACHED DOWN TO PICK UP THE SPLITTER PAN WITH APPROX. 30 LBS.OF ROCK IN THE PAN. HE FELT A SHARP PAIN IN HIS LOWER BACK AS HE STARTED TO PICK PAN UP.

BENT OVER TO UNTIE LINE, EE FELT SHARP PAIN IN LOWER BACK. EARLIER EE WAS JERKING ON A WIRE.

EE HAD WORK CLOTHES IN LOCKER. HE CHANGED INTO WORK CLOTHES, WORKED THE WHOLE DAY, & NOTICED A SPIDER BIT ON HIS LEG AT THE END OF THE SHIFT WHILE CHANGING HIS CLOTHES. BROWN RECLUSE SPIDER BI TE.

EMPLOYEE AND A CO-WORKER WERE REMOVING A TROUGH ROLLER FROM THE BACK OF A PICKUP TRUCK WHEN EE'S LEFT ARM POPPED.

EMPLOYEE WAS OPERATING A CRANE ASSISTING OTHER EMPLOYEES IN MOVING A WATER PUMP. EE HAD TO GET IN AND OUT OF CRANE SEVERAL TIMES, AND HIS LOWER BACK STARTED HURTING. EE DID REPORT INJURY TILL 4/26/02.

EE WAS ATTEMPTING TO CUT RUBBER 8" WATER LINE. HOSE WAS HANGING FROM A P & H CRANE WITH THE COUPLING SITTING ON GROUND EE WAS USING A BOX KNIFE, PULLING KNIFE TOWARD HISMELF, WHEN KNIFE SLIPPE D CUTTING HIS LEG.

EE JUST DISMOUNTED FROM THE WATER TRUCK AND STARTED TO WALK AWAY. THE GROUND WAS UNLEVEL AND HE TWISTED HIS ANKLE.

EE JUMPED FROM THE SCREEN DISCHARGE HOPPER OVER TO A PLATFORM WHICH WAS AN ELEVATION DROP. WHEN EE LANDED ON THE PLATFORM, HE JARRED HIS LOWER BACK.

EE WAS CHANGING A ROLLER ON A CONVEYOR BELT AND STRAINED HIS RIGHT SHOULDER.

EMPLOYEE WAS USING A BACKHOE AND A 1/4" CHAIN TO LIFT A 10" PIECE OF PIPE THAT IS 24' LONG. THE CHAIN BROKE AND ONE OF THE CHAIN LINKS STRUCK EMPLOYEE ON THE RIGHT SIDE OF THE JAW.

THE EMPLOYEE WAS WORKING ON A DOZER TRACK USING AN OVERHEAD HOIST. HIS HAND GOT CAUGHT BETWEEN THE DOZER TRACK AND THE HOIST HOOK. THIS CAUSED HIS THUMB TO BE SMASHED.

THE EE WAS SHOVELING FROM UNDER A CONVEYOR BELT AND WHILE PULLING THE SHOVEL OUT FROM UNDER THE BELT, HE STRAINED HIS BACK. EE WAS TOLD NOT TO RETURN TO WORK BY DR UNTIL HE RETURNED FOR A FOLL OW UP VISIT ON 5/9/00.

EE WAS HELPING REMOVE SCRAP STEEL. WHEN THE STEEL WAS BEING MOVED IT FELL ON HIS FOOT. HIS FOOT WAS FRACTURED BEHIND THE STEEL TOE OF HIS BOOT.

WHILE WELDING AN EE RECEIVED FLASH BURN TO BOTH EYES. THE REFLECTION OFF THE WALL BEHIND THE EE IS BELIEVED TO BE THE CAUSE OF THE BURNS.

STRUCK BY 10 LB WEDGES USED ON R90 TON EUCLID TRUCK TO HOLD ON WHEELS TO HUB. LUG NOTS WERE REMOVED BEFORE WEDGES WERE LOOSEN.

EMPLOYEE WAS CHANGING SCREENS WHEN HE STARTED HAVING PAIN IN HIS LOWER BACK. HE THOUGHT IT WOULD GET BETTER SO HE DID NOT GO TO THE DOCTOR. THREE DAYS LATER IT GOT WORSE AND THE EMPLOYEE WENT TO THE DOCTOR.
