Employee lifted a small table that was being used as a weight to hold the cardboard down. When he swung with the table in his hands to his right to set it down, he smashed the back of his right hand and upper ring finger into a metal tray shelf that was attached to the wall.

Employee bagged 50 bags at 50# each on a pallet. He said on his last bag he felt a sharp pain from his groin to his stomach. He went to the emergency room. He is excused from work at least until his next doctors visit on 1/21/2011.

Employee went to unplug an extension cord that was plugged into a drill. He had one hand on a metal beam and touched the prong on the extension cord at the same time causing him electrical shock.

EE was cutting a bolt from a steel beam with a cutting torch. A hot coal fell on the bottom of his pants burning a hole on his pants then falling into th etop of his slip on steel toe boots. He received medical treatment on the burn on his foot. He was told to take two days off from work.

EE was shoveling a powder type ore with a shovel and by pulling it with a solid rake. He did this for 30-45 mins. The shift ended, then he called in the next day. He went to the emergency room, then made a doctors appointment. Has a pain in his lower back, hip and left leg.

EE was packaging 50# bags. He felt a tightness in his back. We had a Save a Back class on 8/11/10. EE decided to do some exercises that he had learned in the class. At that time he got a sharp pain in his right side/lower back. He was unable to finish the day. He has a doctor's appointment in one week.

Employee drove a small endloader up to a spill at the east side of Granusol. It had rained hard all day. Employee stood on a wet surface as he turned to put a shovel full of ore in the loader bucket. As he turned, he twisted his body, then slipped. He did not fall down but pulled a muscle in his lower back.

EE was packaging 50# bags on a turn table. He pulled a muscle in his lower middle back. EE reported the incident on 8/17/09

Employee was sweeping and shoveling at the screen plant. The mill was down. He left his area to go over to the next mill. He sat down to talk to two other employees. he said he had a sharp pain go down the middle of his back. He then reported this and went to the Dr. and Has a doctor's excuse until further notice.

Using a jackhammer to break up ore build up. Pulled muscle in back.

EE was holding up the inlet spout of a super sack to the discharge chute of a portable hopper used to do transfers. The hopper had approx 2000# of material inside of it that would not flow out. The operator hit the top of the hopper with the loader bucket, causing the hopper to fall back and pin EE's hand in between the forks of the forklift and hopper discharge chute.

Employee was lifting 50 # bags from packaging machine from a North position turning and stacking at waist level to a South position or 180 degree turn.

EE went into the Granusol Mill. The operator was emptying out a baghouse. He had the side door open for visual aid. EE saw a clump of ore in- side the door of the baghouse. He stuck his hand inside to remove the clump. A airlock was on and turning. EE stuck his hand in the airlock.

Employee was poking out a chute going from the dryer to a bucket elevator. The bucket elevator was running at the time. His bar that he was using to poke out the ore struck a bucket, causing it to kick back and strike him in side of the face.

Slipped on a step to a platform scale. Fell and hit right upper ribs on scale read out.

EE WAS WALKING BETWEEN THE DRYER AND BURNER ON PLATFORM WALKWAY. HIS LEFT FOOT SLID OFF THE SIDE OF PLATFORM, DROPPING ABOUT 3", CAUSING HIS LEFT ANKLE TO TURN AND CAUSING HIM TO FALL TO THE GROUND. AT THE TIME HE WAS FALLING, A COWORKER WAS COMING DOWN THE STAIRS AND HELPED HIM UP.

Rained the night before the shift started. Puddles of water on the floor. The floor is concrete with dust. EE was carrying a bucket of oil. he slipped on puddle of water and fell on his back.

EE IS WORKING ON LIGHT DUTY W/RESTRICTIONS. HE WAS INSTRUCTED TO MOVE SACKS WITH FORKLIFT (NOT TO HAUL). HE STATED WHEN HE WAS HAULING HE GOT A JOLT FROM HITTING A POTHOLE, CAUSING PAINS IN HIS ARMS/LEGS.

PACKING 100# BAG FROM BAGGING MACHINE TO PALLET. TWISTED MUSCLE IN BACK.

EE SLIPPED ON LOOSE ORE IN PACKAGING AREA. HE HAD A NEAR FALL, CAUSING HIM TO JAM MUSLCES IN BACK AND NECK.

HELPING GRANUSOL OPERATOR OTHER EE CHANGE SCREENS. WHILE WALKING AROUND THE ELEVATOR EE HIS HEAD UNDER THE CATWALK. HE THEN RAISED HIS HEAD TO SOON, KNOCKING HIM TO THE FLOOR.

AFTER PERFORMING HIS REGULAR JOB DUTIES THE NEXT MORNING HE WOKE UP WITH A STIFF NECK AND BACK. HIS REGULAR JOB DUTIES CONSISTED OF PACKING 50# BAGS AND OPERATING A FORKLIFT.

THERE WAS A POWEROUTAGE. EE WAS IN THE MILL STARTING UP THE MILL. HE TRIPPED OVER A AIR HOSE LAYING ON THE FLOOR.

A BLOW LINE BUSTED (HOLE) FILLED AREA FULL OF DUST. THE OPERATOR COULD NOT SEE THE BOTTOM STEP OF THE STAIRS. HE MISSED THE LAST STEP CAUSING HIM TO FALL INTO THE WALL OF A ELEVATOR. INJURY TO RIB (LEFT SIDE).

EMPLOYEE WAS MOVING FLOOR PLATFORM SCALE BY HAND TO CLEAN. WHILE MOVING THE SCALE HE SPRAINED HIS BACK.

WHILE EMPTYING OUT A FINISH BIN, EE DECIDED TO USE A SLEDGE HAMMER TO GET THE MATERIAL TO FALL FASTEP. HE TWISTED HIS BACK.

AFTER DISMOUNTING A FORKLIFT THE EE SLIPPED ON LOOSE ORE ON THE FLOOR.

EE TRIPPED ON ADJUSTMENT HANDLE ON FLOOR & FELL AGAINST STAIRS

EE WAS BREAKING UP LUMPS OF ORE WITH A HAMMER AND SHOVEL. HE HAD A MUSCLE STRAIN IN THE GROIN AREA. HE WENT TO THE DOCTOR 7 DAYS LATER AND WAS PRESCRIBED SOME MEDICINE TO PREVENT INFECTION.

EE WENT TO THE DOCTOR, HE HAD AN ELEVATED MANGANESE LEVEL DURING A ROUTINE PHYSICAL.

WHILE DESCENDING STAIRWAY EE SLIPPED AND FELL CAUSING RIBS TO BE BRUISED AND LOWER BACK.

WHILE DESCENDING THE CONVEYER TUNNEL AT THE CALCINER HE TRIPPED OVER HIS FOOT JAMMING HIS WRIST, KNUCKLE OF SMALL FINGER ON RIGHT HAND (BRUISED).
