EE cut the old cable and let it fall to the ground. EE then somehow got it wrapped around their leg and stepped on the other end. When EE started to walk, EE tripped self and fell to the ground/strain/sprain R shoulder. Incident happened 4/7/21 clinic didn't change regular job duties then, ee wanted second opinion & went to new doc on 4/22/21 & was placed on restrictions and MRI.

Rocks collected in a chute going to screen deck. EE was removing rock from the chute, using a spud bar. The bar slipped, pinching a finger between the bar and a beam supporting the screen deck. EE sustained a angulated & rotated fracture to the left 5th (pinky) finger, requiring a splint and possibly surgery.

Breaking oversized rock with excavator. While placing tool onto rock, the boom pressure remained, causing excavator to flip onto its side. EE had lap belt on, but upper body thrown into cab sides, fx clavicle. EE had to jump off about 8ft, spraining both ankles.

EMPLOYEE WAS HELPING REMOVE SCALE BEAMS FROM PIP FOR REPAIR. AT TIME OF ACCIDENT HE WAS GUIDING BEAM OUT OF PIT BEING LIFTED BY FRONT END LAODER. WHEN BEAM CLEARED POST IT WAS SITTING ON, IT S WUNG SIDEWAYS CATCHING EMPLOYEE'S FINGER BETWEEN BEAM AND CONCRETE WALL.

INJ. WA HELPING ASSEMBLE A CONVEYOR FRAME. ONE END OF CONVEYOR WAS HELD UP WITH FRONT END LOADER. INJ. WAS LAYING UNDER CONVEYOR SECTION, WELDING. WELD BROKE IN THE MIDDLE OF THE CONVEYOR AND THE CONVEYOR FEL 6" TO 12", FALLING ON INJURED.
A ROCK WAS STUC5 BETWEEN DUAL REAR WHEEL OF PARKED DUMP TRUCK. IN TRYING TO REMOVE THE ROCK, THE INJURED WRENCHED HIS BACK.
