Delphi Plant Metal/Non-Metal

A miner was conducting EE's morning pre-shift on the Cat Loader EE was operating, while walking around the equipment the operator stated EE stepped wrong twisting EE's ankle and then stepped again twisting it again. The miner was taken to the medical clinic and issued a walking boot to wear for 1 week or less if pain goes away. Miner returned to work next day.

A miner was walking out to the dumpster from the Shop and attempted to kick a rock out by the dumpster and the rock was froze to the ground causing miner to fall down. When miner fell the miner landed on face and knocked a crown off of a tooth. The crown had to be glued back on by a dentist.

EE was using a water hose washing off the concrete pad under the Plant. Employee stepped backwards off the concrete pad and felt knee pop. Employee continued to conduct regular activities while being monitored but pain has not went away. Physical Therapy was prescribed on 7/14/2022 making it reportable at this time.

EE was installing water pipe 18" to get water to the canal by the plant. While bolting the flanges together ee right middle finger got pinched between the two flanges that connects the two pipes together. Fracture of the right middle finger tip.

Drilling holes to bolt guards into place, steel piece got into right eye, employee said was wearing safety glasses, no witnesses.

Welding braces to a stand, metal flaked off, getting into left eye.

Walking to work area, Ee attempted to step over caution tape. The tape hooked on the heel of boot, causing EE to fall on leg.

Changing plates on 8x20 screen, tried to free a piece of metal, took gloves off to get a better grip, metal flew off, striking hand.

While replacing skirt board rubber, he lost his balance, reached out to grab hold with LT hand, sharp piece of metal penetrated his palm. Taken to ER due to time of day, follow up at Occ Health next day. 6 sutures.

Employee was filling the hydraulic system of the 992 C Cat Loader. Employee finished filling the loader and was coming down the ladder. When employee got to the 3rd step from the top his left foot slipped off and he began to fall. While falling, employee's right elbow struck the ladder rungs causing a contusion to his right elbow. Employee was off work and restricted for 4 days.

Employee was heating up catch-alls with propane tank & wood burner under Conveyor #25. As employee ducked to go under the conveyor his right foot slipped and he fell. As he fell his right leg twisted - felt pain at inside of right knee. Employee was carrying a propane tank & weed burner so he couldn't catch himself from falling. Went to clinic on 3-11. His right knee was sprained.

Employee was putting a drive belt on the conveyor. The belt broke and employee fell. When employee fell he got his left leg tangled on a hose and he fell on the ground and dislocated his right knee cap. Employee is off work.

This person is not an employee of Delphi Limestone Plant ID #12-00023. He is a customer. The driver was trimming off excess stone and caught his finger in the tailgate of his truck. Driver had the tip end of his right index finger cut off. He is off work. The driver is employed by Kokomo Gravel, Inc. Material & Trucking Service

Employee was going to change a screen and was looking for the correct size of screen at the Secondary Wash Plant. The screen was in a stack and 2 of the screens fell and smashed his left hand into an I-beam breaking left index finger & cutting left thumb. Was given a splint and is restricted to "no use" of left hand.

Employee states while driving a loader, the loader rocked causing him to hit his elbow on console causing pain in his right shoulder. Reported it to his supervisor. Didn't go to Doctor until 11-30-11.(No medical treatment)Thought he would be o.k. 3-28-12 still having problems with his shoulder. Sent to Co. Clinic on 4-6-12, received P.T. Claim became recordable.

Employee got out of his personal vehicle to go to work. Employee fell on ice as he was going to start his loader. Employee received pain in his back, neck, head and left shoulder. Employee treated at ER. Was to return to work immediately. Then he went to a chiropractor on his own. Chiropractor said he was to be off work from 1-25 to 1-31-11. Employee returned to work on 1-31-11.

Employee was in the pit building a water stand. Employee was flipping an I-beam over. When he picked up the beam, something popped in his left index finger. Received a sprain to his left index finger. Employee is restricted and is able to work while restricted.

Employee was kneeling down to look at the Grizzly shaker and his wrist scraped a sharp rock that was on the beam that bordered the walkway. Employee received a laceration to his left wrist. Stitches were required. Super glue was used to close the laceration. He was restricted with limited use of his left hand.

Employee was pulling a screen down at the secondary plant to position into place and felt his back tighten up. Employee didn't think any thing would come from it. The next morning he couldn't move his neck. He went to a chiropractor and the company clinic. Employee has a Thoracic back sprain. Was off work 1 day and restricted for 2 days.

Employee was using a sledge hammer and pry bar to remove a pin from the tooth of the Hitachi shovel when the sledge hammer slipped off the pry bar and hit employee's left index finger. Employee received a crush injury to his finger and later contacted cellulitis. Employee is restricted. No lost time.

Employee was climbing down the ladder on the 998 Cat Loader and lost his grip and fell approximately 5-6 feet to the ground and landed on right side of his body. Employee twisted his right knee and received a sprain to right knee. Employee was off work one day.

Employee was using a jack hammer to break rock out of the tail pulley at the #2 Conveyor. When the rock broke, the conveyor moved about 3 inches and pinned employee's hand and arm against the conveyor guard. Employee received a contusion/sprain to his right wrist. Employee was restricted.

Employee was getting out of the manlift after installing a hopper plate. As employee was exiting the manlift, he lost footing and twisted his right ankle. Employee received trauma and pain to his right ankle. Employee was restricted.

Emp. driving to shop to get parts. Getting out of truck, got dizzy, couldn't breathe, fell out of truck, couldn't get to his inhaler. Was found on ground, ambulance was called, taken to hospital. Having asthma attack, because he couldn't get to inhaler, had a panic attack. No injury from fall. Asthma attack not work related. Document to follow.

Employee was crawling out of the S-6 screen at secondary plant and his left knee popped out. Employee has a pre-existing left knee condition. Employee received a sprain and is off work.

Employee was cutting a belt from a catch-all with utility knife. Employee was pulling down on the knife when the knife ripped through the belt and continued on in motion and struck his right leg. He received a laceration/puncture to his right leg and received staples/stitches.

Employee was getting back into the fuel truck. As he was getting in the truck, his right knee hit the step that is on the fuel truck. He received a contusion to his right knee under the knee cap. He was off work for a few days and came back to restricted work.

Employee was putting a chain over conveyor to lift beam and dirt fell off conveyor into his face and eyes. Employee received a corneal foreign body to his left eye. The foreign body was embedded and had to be drilled out. There are no lost or restricted days.

Employee was using a chipping hammer to knock slag off the weld. A piece of hot slag flew up and went under employee's safety glasses and hit his left eyeball. Employee received a foreign body in the Cornea. Employee was restricted at first but later had to be off work.

EE was operating the Hitachi Shovel & digging in tight rocks all day. EE was jarred around while runing the shovel all day & felt pain in his right shoulder. Pain got worse when he sat down to rest at home that evening. He went to work on Tuesday & was sent to the clinic. Diagnosed as cervical strain; EE was put on restricted duties, but no work was available.

Employee was welding clamp and a piece of channel iron. Employee was clamping parts into place and the steel rolled and pinched employee's left ring finger between 2 pieces of steel causing a fracture to his left ring finger.

EE had been tightening & loosening bolts and nuts with an air impact tool almost all week. EE complains of swelling that began Monday 2/14/05 and the numbness in his right thumb began on 2/17/05. EE was sent to the company clinic on 2/17. He received a strain to his right hand/thumb. He is to wear a splint. EE HAD CARPAL TUNNEL SURGERY 4/4/05 (RIGHT HAND).

Employee was inside the shaker replacing main support beams. Employee was welding, grinding and cutting to replace the beams. Foreign debris got into his left eye. He had his welding shield on. Later that evening his eye started to hurt and the next morning he woke up with a lot of pain and matting in his eye. He received a severe abrasion.

Employee was bending over in the process of moving a ring (appox. 40 lbs.) on a pump. When employee rose back up, he felt a sharp pain in the middle of his lower back. Employee received a LS Strain. Employee was taken off work.

Employee was greasing the tail pulley of Conveyor #10 at primary plant. Pulling grease hose with both hands, lost his balance, tripped and fell against guard which was at waist height. Employee was close to guard and although he put his hands out, he hit the guard right at his mouth and broke 5 teeth and has to have extensive dental repair.

EE WAS ON HIS KNEES AND WAS HITTING THE PINCH BAR WITH A SLEDGE HAMMER AND ALSO PRYING WITH THE PINCH BAR TO ATTEMPT TO REMOVE ICE AND STRAINED HIS LOWER BACK. EE WAS RESTRICTED BUT NO WORK WA S AVAILABLE UNDER THE RESTRICTIONS. EE WAS OFF WORK.

EMPLOYEE WAS REMOVING A RETURN ROLLER FROM THE CONVEYOR. EMPLOYEE HAD HIS RIGHT HAND ON THE END OF THE PINCH BAR AND HIT ROLLER. EMPLOYEE DIDN'T FEEL ANY DISCOMFORT FOR A DAY OR SO. HE THOUGHT THE PAIN WOUDL GO AWAY. HE HAD PAIN IN HIS WRIST WHEN HE WOULD GRASP OR PICK UP AN ITEM. HE REPORTED IT TO HIS SUPERVISOR 11-20-03 AND WENT TO THE CLINIC. EE RECEIVED FRACTURE TO RIGHT WRIST.

EE WAS CRAWLING INSIDE A SHAKER TO CHECK THE SCREEN. AS EE WAS CLIMBING OUT OF THE SHAKER HE TWISTED HIS LEG. EE STRAINED HIS RIGHT KNEE.

EMPLOYEE WAS USING 2 PRY BARS TO RE-POSITION A SCREEN & SUDDENLY HAD A SHARP PAIN IN HIS SHOULDER BLADE. EMPLOYEE RECEIVED AN ACUTE THORACIC SPRAIN.

EMPLOYEE WAS CUTTING A HOSE WITH A UTILITY KNIFE AND CUT HIS HAND. STITCHES WERE REQUIRED.

EE WAS CLEANING THE ICE OFF A CONVEYOR WITH A SPUD BAR. THE SPUD BAR SLIPPED AND SMASHED EE RIGHT PINKY FINGER. EE RECEIVED A LACERATION & A FRACTURE & NAIL AVULSION.

EMPLOYEE WAS INSIDE LOADER PULLING RAIL CARS. HE TURNED THE FAN ON AND WHEN HE DID DUST AND DIRT BLEW INTO HIS LEFT EYE. HE RECEIVED A PRESCRIPTION AND SURGERY TO REMOVE ALL THE FOREIGN BODY.

EMPLOYEE WAS SHOVELING UNDER A CONVEYOR AND DUG TOO DEEP. THE SHOVEL TWISTED CAUSING HIS WRIST TO TWIST. EMPLOYEE RECEIVED A STRAIN TO HIS LEFT WRIST. HE WAS RESTRICTED FROM 10/30/01 TO 11/06/ 01.

EMPLOYEE WAS OPERATING THE BOBCAT FOR 4-5 HOURS AND WAS JARRED & JOLTED AROUND. EMPLOYEE WAS STRIFF WHEN HE GOT OFF THE BOBCAT. EMPLOYEE STATED HE HAD TO HAVE HELP TO GET OUT OF BED THE NEXT M ORNING. EMPLOYEE IS RESTRICTED TO LIMITED LIFTING.

EE WAS PICKING UP A PIECE OF BELTING AND THROWING THE BELTING ONTO THE PONTOON BOAT. EE FELT SOMETHING PULL IN HIS LOWER BACK. EE RECEIVED A STRAIN TO HIS LOWER BACK HE RETURNED TO WORK ON 9-1 0-01 WITHOUT RESTRICTIONS.

EMPLOYEE WAS CHANGING SCREENS AT THE SECONDARY PLANT. EMPLOYEE TRIED TO TAP ON BOTTOM OF SCREEN WHEN HE CUT HIS RIGHT HAND ON A SHARP WIRE. EMPLOYEE RECEIVED STICHES.

EE WAS REPAIRING CONVEYOR BELT & CAUGHT HIS LEFTRING FINGER BETWEEN TROUGH IDLER AND BELT. HE RECEIVED A CONTUSION & WAS RESTRICTED TO WEARING ASPLINT.

HYDRAULIC PUMP IN THE HITACHI EX1800 EXPLODED & FILLED THE ENGINE COMPARTMENT WITH HYDRAULIC FLUID. WHEN FLUID HIT THE TURBO ON THE ENGINES, FLUID WENT UP IN A BALL OF FIRE. FIRE DESTROYED THE SHOVEL. OPERATOR EXITED THE CAB & SLIPPED ON THE TRACKS SCRAPING HIS HAND. NO MEDICAL TREATMENT WAS GIVEN.

EE WAS HELPING INSTALL A GEAR BOX. HE HELD CRIBBING WHILE A CO-WORKER WAS HITTING IT WITH A SLEDGE HAMMER. EE'S HAND WAS BETWEEN CRIBBING AND ANGLE IRON IN A PINCH POINT AND WAS MASHED.

WHILE HOOKING UP A BUCKET TO #121 SKID LOADER, EMPLOYEE WAS HOLDING UP BUCKET PINS WITH HIS FOOTAND WHILE ADJUSTING BUCKET HE WENT THE WRONG WAYWITH THE LEVER AND BUCKET PINCHED HIS FOOT AGAIN ST FRAME OF LOADER. HE WAS GIVEN A CAST TO WEAR AT WORK BUT HAD NO RESTRICTIONS.

EMPLOYEE WAS GREASING A TRUCK. ALLEGES HE WAS CRAWLING OUT FROM UNDERNEATH THE TRUCK AND TRIED TO GET UP TOO SOON AND HIT THE TOP OF HIS SHOULDER ON THE TAILGATE BAR. HE DIDN'T GO TO THE DOCTO R UNTIL 4-21-00. HE DID NOT KNOW IT WAS WORK RELATED UNTIL 4-27-00.

EMPLOYEE WAS WAITING FOR THE PLANT TO RE-START. HE CLIMBED UP THE BIN SUPPORT TO SIT ON AN I-BEAM TO GET A BREEZE. AS HE WAS COMING BACK DOWN, HIS FOOT SLIPPED AND HE FELL APPROX. 11 1/2 FEET TO THE GROUND. HIS LEFT SHOULDER STRUCK ACROSS BAR CAUSING A FRACTURE. EMPLOYEE LOST HIS HARD HAT DURING THE FALL AND STRUCK HIS HEAD ON THE GROUND CAUSING A LACERATION AND CONCUSSION.

EE WAS BUILDING UP CRUSHING SURFACE ON ROLL CRUSHER WHEEL. WHILE WELDING EE WAS EXPOSED TO WELD ARC & RECEIVED FLASH BURNS TO HIS EYES. EE'S WELDING HOOD HAD BEEN RAISED AT THE TIME.

EE WAS TEARING DOWN SCREENS AND FELL THROUGH HOLE ON WALKWAY WHERE DRIVE BELTS ARE POSITIONED. DRIVE BELTS HAD BEEN REMOVED FOR REPAIR OF SCREENS.

EMP WAS HELPING TO CLEAN BEHIND OFFICE. EMP WAS THROWING SCRAP STEEL IN LOADER BUCKET AND WHILE PULLING A PIECE OF SCRAP FROM UNDER OTHER PIECES, A PIECE OF SCRAP FELL OUT HITTING HIS FOOT ABO VE THE STEEL TOE OF SHOE. EMPS FOOT WAS BROKEN IN 3 PLACES. IT WAS METAL SCRAP.

EE WAS SAMPLING BEDROCK FROM A HIGHWALL FACE. WHEN HE DISLODGED THE SAMPLE, IT CAUSED A ROCK TO ROLL 5-6' STRIKING EE IN THE LEFT LEG. EE WAS GIVEN 6 STITCHES. IT SHOULD BE NOTED EE REFUSED SE VERAL TIMES TO USE A MANLIFT THAT WAS PROVIDED &CLIMBEB HIGHWALL TO OBTAIN SAMPLE.

EE ARRIVED AT WORK PRIOR TO 700 AM HE ALLEGES HE WAS EXITING HIS PERSONAL VECHILE TO START HIS SHIFT WHEN SOMETHING POPPED IN HIS BACK. HE WENT TO THE CLINIC THE NEXT MORINING AND WAS GIVEN BE DREST FOR TWO DAYS TO BE OFF WORK UNTIL 11-7-94.

EMPLOYEE WAS USING A GRINDER. THE GRINDER KICKED BACK AND EMPLOYEES FINGER WAS CUT. EMPLOYEE HAD TO HAVE STITCHES.

EMPLOYEE WAS USING A CUTTING TORCH. HOT METAL FELL INTO EMPLOYEE'S BOOT & BURNED HIS FOOT. IT WAS DETERMINED LATER TO BE 3RD DEGREE BURN.

EE WAS PULLING A PIECE OF 3" ANGLE IRON OUT OF A SCRAP PILE, WHEN A PIECE OF 12" I-BEAM FELL ON EE'S LEFT FOOT.

EMPLOYEE WAS PRYING WITH A CROWBAR ON A TOWER FOR A STACKING CONVEYOR. THE BAR SLIPPED & EMPLOYEE FELL APPROX 3 FT HITTING HIS BACK & HEAD ON THE GROUND.

EMPLOYEE WAS CLEANIGN UP ROCKS AROUND NEW #9 CONVEYOR. WHEN ONE OF THE ROCKS HE HAD THROWN ONTO THE CONVEYOR FELL BELOW THE BOTTOM OF CONVEYOR BELTS. EMPLOYEE PROCEEDED TOR EMOVE THE GUARD A ND REACHE DINTO TRY AND RETRIEVE THE ROCK. HIS RIGHT ARM BECAME CAUGHT IN TEH PINCH POINT OF HT ETAIL PULLEY, RESULTIN GIN HIS RIGHT ARM BEING TORN FROM HIS SHOULDER.

EMPLOYEE WAS KNEELING ON THE GROUND WIT HIS HAND ON A ROCK TO PUT WATER ON HIS FACE (IT WAS A VERY HOT DAY) THE ROCK SLIPPED AND EMPLOYEE FELL AGAINST A SHARP EDGE OF ROCK CUTTING HIS ARM.

EMPLOYEE JUMPED OUT OF A TRUCK AND PULLED A MUSCLE IN HIS BACK.

EMPLOYEE WAS TIGHTENING BOLTS ON A TRACK OF A DOZER AND SLIPPED OFF THE DOZER.

EMPLOYEE STEPPED OFF A CEMENT WALKWAY AT THE WASHPLANCT AND SLIPPED AND SPRAINED HIS ANKLE.

EMPLOYEE SLIPPED ON WET CEMENT AND FELL OFF THE STEP AND TWISTED HIS BACK.

EE WAS GOING DOWN THE STEPS OF THE RIPLE DECK AND STEPPED ON A ROCK AND TWISTED HIS ANKLE.

INJURED WAS HELPING CO-WORKERS PULL #6 PUME. THEY HAD A FORTY FOOT SECTION OF PIPT OUT AND WERE TRYING TO LAY IT DOWN. CO-WORKER WAS WALKING THE PIPE OUT AND INJURED STOOPED DOWN TO GET A WREN CH TO TAKE OFF THE FITTING ON TOP. THE PIPE BROKE IN HALF AT A COUPLING, FELL, AND STRUCK INJURED ACROSS LEFT SIDE OF HIS BACK AND LEFT KNEE. IMPACT OF PIPE FRACTURED HIS LEFT KNEECAP. THERE

IN PROCESS OF UNLOADING ROLL CRUSHER EMPLS WENT TO GET SOME WOODEN CRIBBING MATERIAL TO SET CRUSHER ON. EMPLS WERE UNLOADING WOODEN PLANKS WHEN INJ DROPPED HIS END. HIS FOOT WAS AGAINST OUTRIG GER OF CRANE SO HE COULD NOT MVOE HIS FOOT OUT OF WAY. WOOD PLANK STRUCK HIS FOOT.

EE WAS STANDING ON A BELT LOOKING TO SEE THAT THE STONE ON ANOTHER BELT WAS ADJUSTED PROPERLY. HE THEN TOLD THE PLANT OPERATOR TO TURN IN THE OTHER BELT, THE OPERATOR TURNED ON THE WRONG BELT. THE BELT THAT WAS TURNED ON WAS THE BELT EE WAS ON, THIS CONTUSIONS AND ABRASIONS TO THE RIGHT LEG.

INJURED WAS USING A PUNCH AND HAMMER TO DRIVE A TAIL PULLED OFF THE SHAFT. A METAL CHIP FLEW FROM THE SHAFT AND LODGED IN HIS EYE.

INJURED WAS CHANGING THE HYDRAULIC FILTER ON AN O&K RH30 SHOVEL. HE DROPPED THE FILTER AND IT CRUSHED HIS FINGER AGAINST THE FRAME.

INSIDE BIN, 10 FT LADDER PLACED AGAINST UNECURED 2 X 10 BOARD CLIMBED LADDER, PLANK SLID FELL 6 TO 8 FT FROM LADDER TO FLOOR OF BIN.
