A shuttle car driver was tramming through a crosscut from Entry # 5 to Entry # 6. The car driver stopped spoke to the victim. The victim proceeded past the car, traveled through a curtain into the intersection of Entry # 6. The victim told the car driver he was clear. The car proceeded through the curtain and struck the victim between the shuttle car and the left rib.

Injured was operating a 777 end dump. EE was being loaded with a 992 loader. As the 992 loader operator dumped a load into the bed of the end dump the operator of the end dump was jarred. EE went to the Dr. on 8/4/2023 and was placed on restricted duty for a back strain.

Employee was installing tie wire onto a roof bolt plate. Above the roof bolt plate was a damaged/sharp edged steel pie pan. As the employee tied the wire, his right index finger came into contact with the sharp steel resulting in a laceration that required four sutures to close.

Employee was roof bolting when he drilled a crooked hole into very hard roof strata. When the drill pot was lowered, the steel snapped back and struck the employees right forearm, resulting in a simple fracture to the right radius.
