An unintentional roof fall occurred on the Unit 4, 1 North Main West Right Return, Entry 9 Crosscut 51. The roof fall measured approximately 20' in length, 18' in width, and 10' in thickness. A 103 (k) Order (8437817), was issued September 02, 2011, by MSHA Inspector.

Employee and coworker was lifting handling an I Beam when he strained/sprained his pelvis. Employee went to a doctor in Mt. Carmel, Illinois and was diagnosed with a hernia. Employee had surgery on September 08, 2011, in which at that time became a reportable accident and will be off on Workman' s Comp for an uncertain period of time.

Employee was loading supplies when a stack roof bolts rolled falling onto employees left leg. Injuries resulted in a fracture to his ankle which required surgery. He will be off work for approximately 6-8 weeks.

Employee was constructing on the beltline in a flooded area where water got into his boot. The water had a chemical substance in it that didn't react well with his skin resulting in 2nd degree burns. Employee worked throughout his shift and went to Good Samaritan on the 25th, employee will remain off work for an uncertain period of time.

Employee was lifting a timber when he felt his back go out straining/spraining his back. Employee was transported to hospital and now under doctors care remaining off work for an uncertain period of time.

Employee was installing guarding and while cutting thru the guarding the knife slipped. Injuries resulted in a laceration to his hand which required sutures.

An unintentional roof fall occurred on the Main West 2 Right Return between Entries 10 and 11, Crosscut 82. The roof fall measured approximately 55' in length, 18' in width, and 7' in thickness.

An unintentional roof fall occurred on the 1 North 2 Main West 1 Entry 8 Crosscut 93. The roof fall measured approximately in 45' length, 18' in width, and 12' in thickness.

Employee was constructing a stopping when he tripped and fell hitting his lower back on a half block. Employees SCSR was located on his lower back in which is where his pain originated. Employee finished his shift then went to Good Samaritan in Vincennes, Indiana where the doctor ordered him to stay off work for an uncertain period of time.

Employee was reclaiming curtains and picking up trash in the left side Entry 2 when a loose rock fell from the top striking the employee's arm. Injuries resulted as an abrasion and strain/sprain to his left arm. Employee will remain off work until released by the doctor on June 06, 2011.

Employee was operating the #3 Fletcher Roof Bolter (Double Boom); while attempting to dislodge steel from drill chuck he missed the steel and struck the drill chuck with his claw hammer causing a piece of steel to fly up striking his eye. Injuries resulted in a laceration to his eye which lead to surgery and will be off work on Workmans Comp for an uncertain period of time.

An unintentional roof fall occurred on the 2 South 1 Entry 6, Crosscut 10. The roof fall measured approximately 40' in length, 18' in width, and 7'-8' in thickness. A 103 (k) Order (8434030), was issued May 11, 2011, by MSHA Inspector, Phillip D. Herndon.

Employee was attempting to close the outby set of doors. The inby set and one of the outby doors were already closed and was closing the second door when his hand was smashed between the outby set of doors. Injuries resulted in lacerations and fractures to his hand.

Employee was lifting a solid concrete block out of the mud when he strained/sprained his lower back.

Employee was repairing the FSH2 Dozer using a sledge hammer to remove a pin on the track pads. Employee was striking down when his hand slipped which caused him to miss the pin and smash his finger resulting in a laceration.

EE was stepping over car cable with cable under his foot to pick up curtain when equipment was energized and cable reel came on. EE was thrown from equipment, resulting in a dislocation to his right shoulder. EE has since then been referred to an Orthopedic Surgeon for possible surgery and will be off work on Workmans Comp for an uncertain period of time.

An unintentional roof fall occurred on the Unit 4 Outby Feeder Right Return in the Entry 8, between Crosscuts 97 and 98. The roof fall measured approximately 80' in length, 18' in width, and 25' in thickness. A 103 (k) Order (8430282), was issued on April 28, 2011 by MSHA Inspector

Employee was handling a crib block when he smashed his finger between the crib block and the bolt resulting in a laceration to his left index finger. Employee will be off work for an uncertain period of time.

An unintentional roof fall occurred on the Main West 1, Crosscut 43, between Entries 4 and 5. The roof fall measured approximately 25' in length, 17' in width, and 5' of thickness. A 103 (k) Order was issued March 29, 2011, by inspector.

Employee was stepping onto a crib when he twisted his right knee resulting in a torn meniscus. Employee will be off work for surgery and therapy for an uncertain period of time.

An unplanned roof fall occurred on Crosscut 69 Entry 6 Main West 1 Beltline. The roof fall measured approximately 30' in length, 20' in width, and 7' of thickness. A 103 (k) Order (8433152), was issued on March 16, 2011 by MSHA Inspector.

An unplanned roof fall occurred on Crosscut 4 Entry 10 3 North 2 Beltline Walkway. The roof fall measured approximately 15' in length, 15' in width, and 8' of thickness. A 103 (k) Order (8433153), was issued on March 16, 2011 by MSHA Inspector.

Employee was taking a rock off of the miner when the rock smashed his finger in between the rock and the rib resulting in a fracture.

An unplanned roof fall occurred on Crosscut 93 Entry 2 towards Entry 3 35' Main West 2 Return. The roof fall measured approximately 55' in length, 17' in width, and 5' of thickness.

Employee was struck by a high line on his lower back, making him fall to the ground. Employee didn't complain of any severe back pain and was just treating it at home. The accident occurred on January 31, 2011, although he didn't go to the doctor the day of and wasn't taken off work until February 23, 2011 by doctors orders.

Employee was complaining of pain and popping in his shoulder. Continued to work until it kept getting worse and went to the doctor a week in half later. Is currently off of work under doctors orders.

An unplanned roof fall occurred on Cross-Cut 36 Entry 6 Main West 1 Secondary Escape way. The roof fall measured approximately 60' in length, 18' in width, and 8' of thickness.

Employee was a passenger in a Genco mantrip, when traveling up the mine slope out of the mine, when they collided with another Genco mantrip. Employee was struck against an unknown object, resulting in a strain/sprain to his back. Employee went the next day to Dr. where he received treatment.

Employee was striking down on a 1" bolt trying to dislodge another bolt using a 8lb. sledge hammer. The bolt he was trying to remove was a tank mounting bolt on the Pod Duster #2. When he swung down, his hand hit the side of the tank resulting in a fracture and laceration.

An Unplanned Roof Fall occurred on Cross-Cut 100 between Entry 7-8 Main West 2 Primary Escape way. The Roof Fall measured approximately 35' in length, 17' in width, and 5'-5 1/2' of thickness.

Employee was picking up trash from the beltline when a metal band came in contact with his forearm resulting in a laceration that required sutures to close.

Employee was unloading roof bolter plates and pins from the pallet when the metal banding came in contact with his forearm resulting in a laceration that required sutures to close. Employee completed his shift then went to doctor in Princeton, Indiana to receive sutures.

A roof fall was discovered in the Cross-Cut 12 between Entries 3 and 4 on the 1A Submain. The roof fall measures approximately 56' Long, 18' Wide, and 6' in Thickness.

Employee was unhooking a chain from a slinger duster when the chain came loose and struck the right index finger. The right index finger sustained a simple fracture.

Employee had just washed off dozer and was performing the required pre - operational check, and about to check the fire extinguisher when he slipped off of the equipment landing on his left wrist. Employee sustained an incomplete non - displaced fracture of the left radius.

Employee had soreness/aching in his shoulder from performing repetitive work. Had carpal tunnel surgery October 2010 hoping this would take care of the pain. Pain persisted and worsened by performing the same work, he reported back to doctor complaining of same pain. Doctor scheduled MRI January 2011, results show a tear in shoulder, scheduled surgery March 2011.

Employee was closing manually operated equipment doors by hand. Upon closing the first door, the second door closed on the left hand. The employee sustained an incomplete fracture of a bone in the left hand.

Employee was installing tie wire onto a roof bolt plate. Above the roof bolt plate was a damaged/sharp edged steel pie pan. As the employee tied the wire, his right index finger came into contact with the sharp steel resulting in a laceration that required four sutures to close.

The rock measured approximately 7' long, 2.5' wide and 6" to 8" thick. Employee sustained a fractured left forearm, and a torn right patella tendon.

Employee was roof bolting when he drilled a crooked hole into very hard roof strata. When the drill pot was lowered, the steel snapped back and struck the employees right forearm, resulting in a simple fracture to the right radius.
