Linwood Lime Plant Metal/Non-Metal

As we were attempting to run coke through the system to empty the tank it was noticed that the coke was still smoldering and smoking, so the attempt to empty was abandoned, the fire suppression system was deployed, and the fire department called.

No actual flames were found, but a hot spot of smoldering coke was found on top of our coke tank. Water was used to put out the smoldering coke, it was later discovered that a steel beam underneath the smoldering coke and some material inside the tank had reached a temperature of 1200 degrees. The fire suppression system was deployed and the fire department called.

Employee was reaming a pin out of a shaft using a hammer and punch. When the pin let loose their hand slid down and got smashed between the punch and steel causing injury.

An EE was closing one of the kiln hood doors, as EE closed the door dust fell from the hood overhead. IE was in the area where dust fell and got it in IE's eyes.

The miner was working in the bag house. EE was standing on a pallet to hang an empty bag to fill with material. As EE was stepping off the pallet, EE's foot slipped causing injury.

The C92 conveyor belt had a slow speed sensor alarming. To remedy the issue employee was attempting to replace the drive belts by rolling them over the sheave. Employee did not follow proper lockout tagout procedure. Due to the conveyor not being locked out, as they were rolling the belts on the sheave the conveyor started and caught their middle and ring fingers causing injury.

A belt heater malfunctioned causing the C90 conveyor belt to catch on fire. A miner noticed the fire and tried to extinguish it. Once two extinguishers were used the miner could not put the fire out, so EE proceeded to call for someone on the radio to get the fire department on site. The Buffalo fire department came and extinguished the fire.

Miner was walking out of a doorway carrying a part the EE was going to do repairs on, when EE slipped on some ice. Slipping on the ice caused EE to fall to the ground and drop the part EE was carrying on EE's right ring finger.

Welding and fabricating new hand rail and catwalk at kiln. While working in aerial boom lift, worker moved lift basket crushing two fingers on right hand between boom lift basket and kiln shell.

Injured left shoulder while operating manual chain operated slide gate. Initially non reportable. Employee now being referred for further treatment and rehab by physician. **Attempted to report this on 7/15/20 electronically (have paper copy). Report does not look like it saved. Adding the report back in on 10/19/20.

Employee fell due to a loose cover plate on scale resulting in a leg injury.

Miner was wearing correct PPE but got dust in right eye due to high winds.

***At request of MSHA this report replaces initial report (2842715). Annual hearing test results determined employee may have significant threshold shift in right ear. Due to CORONAVIRUS pandemic unable to schedule and conduct retest. Per employee audiologist noted wax build-up in right ear attempted to remove... Report received via mail from audiologist 4/26/2020.

While tightening/adjusting bolts to level base ring, wrench slipped off of bolt crushing Rt pinky finger between wrench and ring plate.

Per annual audiology results significant threshold shift in left ear.

The brick rig was in use in the kiln to support brick being installed. Pogo sticks are set on the rig to the brick to keep pressure on them until the brick ring can be keyed. The brick rig shifted causing the pogo sticks to lose tension resulting in the brick above to fall, severing the last 3/4inch of EE's finger tip.

Grinding in back of Kiln, stopped grinding as grinder was slowing down it caught glove on left hand index finger.

Employee was walking down the stairs. Slipped, tried to catch self with right hand, hand hit upright form handrail - where upright meets frame work.

Coke belt to bins spilled coke due to skirt board problem near kiln hood and between buildings causing coke to smolder. Smoldering coke has now been extinguished.

High pressure hose used to clean rock pad. As employee was cleaning, end loader operator dumped rock in hopper, excess came out and hit employee in the wrist causing bruising and sprain of right wrist.

HAND RAIL GAVE WAY AND EMPLOYEE GOT forearm caught on broken area and ended up with laceration to right forearm.

Stepping off fork truck and rolled right ankle.

4/22/19-In morning noticed that petroleum coke spill between 2 kiln bldgs smoldering. Location of spill made access initially difficult. Was evaluated how to access, extinguished ASAP 4/22/19. MSHA was not notified or an initial 7000-1 report done as issue was not viewed as fire or any risk of combustion. Failure to report resulted in paperwork citation #9382974 issued on 7/29/19.

Employee was driving endloader on uneven surface causing jerking motion after hitting a hole giving EE whiplash.

Employee was shoveling coal into a wheel barrow. Employee took wheel barrow outside the south door of #3 coal mill, as soon as they got outside they felt something fly in their eye.

EMPLOYEE WAS ASSISTING IN BRING UP A BELT IT CAME UNROLLED AND HIT EMPLOYEE IN THE HEAD.

Employee was on top of T331/332 bins at C336 Conveyor to get set-up to install new conv belt revive belt being flown-up by crane. Belt came unrolled and hit miner in head, causing a concussion.

Tightening bearings on base bolts on the trunion after tightening brought arm back touching kiln causing burn on arm.

Walking up stairs between 2nd and 3rd pier on K3. Reached the top of stairs and felt a pop in the knee.

EE raking out hood of kiln there was a back flash causing burn. **Miner was cleaning kiln grates with kiln stopped and fire out. Supervisor turned kiln to prevent warping. Kiln had rock in it, as the kiln turned, exposed hot rock. Process control system allowed gas back into kiln which ignited as soon as hot rock was exposed when kiln turned. EE not wearing PPE required**

Clear night walking from rock system step on a rock causing to roll/twist ankle.

Miner was performing maintenance work above coal mill kiln. Combination of hot environment and PAPR supplying hot air, they overheated. At ER was given on IV for fluid replenishment.

Miner was walking to car from the lime plant and lime dust got in eye. Miner went to doctor to have removed and received a prescription for after treatment.

Miner was spraying water to clean area. The water hit a large chunk of lime and it came back and hit the miner's face and got in eye. Prescription was given for post treatment.

Attempting to hitch a compressor to a fork lift, by hand. The hitch on the forklift was too high. When ee lifted the compressor to meet the hitch, the compressor fell and pinned hand.

Miner was walking outside, through plant, windy day. Lime dust got in left eye. Miner was wearing safety glasses. Was given a prescription for antibiotic.

Hearing loss determined by annual audiological testing. Found moderately severe loss. Determined that the average level in hearing was greater than 55db in each ear.

Employee was welding and grinding on date of incident. EE was wearing proper PPE, face shield and safety glasses. At some point, EE got a couple pieces of metal in right eye. Didn't bother EE until after the shift. Metal could not be flushed and had to be scraped out.

Lime Plant maintenance employee got lime dust in eye. The weather was windy. EE was wearing safety glasses with side shields. EE flushed eye several times with no relief. Supervisor told EE to see a doctor. The doctor confirmed a chemical burn from the lime dust and gave employee prescription eye drops for treatment of any infection.

Employee was carrying a piece of steel plate, with another person, up a cat walk. The steel plate slipped out from gloved hands and landed on finger.

Working on #3 kiln and wind was blowing and got dust in eye. Wearing appropriate safety glasses at time of incident.

While climbing ladder below another person, looked up to see if other person cleared ladder and material/dust fell in eye.

Employee was walking behind kiln #4 coal mill through the alley when wind blew dust particles in to his right eye.

Moving screens in warehouse when something got in his eye

Employee was turning off the air compressor for the terra-loc system and terra-loc blew into his eye.

Employee was installing drive belts onto a drive pulley by pulling the belts onto the shives; without loosening the tension nuts and had his finger pinched; caught between.

Walked past steel mesh laid on top of saw horses. Hand brushed against the cut edge of the steel mesh and cut his hand.

The miner was closing a dust collector door, vacuum sucked the door shut; causing it to slam, which accumulated dust fell onto his face

Wind swirling around, dust particle in left eye

The miner was installing a thrust roller in place on #3 kiln. He was using a pry bar to move the thrust roller in place. The load shifted causing the pry bar to suddenly shift and caught in-between his hand and the thrust roller

picked up a side oh aluminum siding a movement of the siding cause a jerk reaction of the aluminum siding cutting his lower forearm

the miner was welding on a hot kiln felt overheated and felt light headed

Hot rock had fallen onto a stationary belt causing it to catch on fire

The employee was sawing a 1' thin walled conduit with a hacksaw. As the saw blade went through the conduit, his finger caught between the conduit end and the hacksaw handle

A windy day. The employee was walking to the parking lot at the end of the day to go home. He still had his PPE on. A gust of wind blew a speck of dirt into his eye. He went to the office and flushed his eye. He then drove home.

Employee was walking up a stairwell and felt his ankle pop. Employee did not report an injury until 10/04/2013

Not on the clock got out of my truck. Fell and broke my right foot. Company investigated with 2 PI then fired me. A truck driver helped me to the front office. Did not get his name.

No. 4 kiln open back door to kiln, still running - management said to open. Hot lime dust went into left eye. Went to doctor. Doctor said I had a stroke. Now I see black and white out of it. No color.

While closing the right side door on kiln 4 hood, he smashed his left middle finger between the latch and kiln hood.

On the above listed date, EE was in the process of dumping K-rock and the trailer tipped on its side. He hit his head on an unknown object inside the truck cab. The trailer, a 2001 Vantage, was towed away. The tractor, a 1996 Freightliner, has been inspected. It passed and is in accordance with 49 CFR part 396.

Employee hung himself

Employee was transferring oil into a pump and moved the oil barrel away from the wall in order to have a better position on the barrel. The barrel slipped from Employee's hands, crushing Employee's left little finger between the oil barrel and the wall.

Fx R index finger; new ee was at Hi Cal crusher with supervisor. Supervisor was checking rolls to see if they were free by grabbing the belt & moving back & forth. Ee did the same thing on the other side without being asked to and rolled his finger in the pulley.

Driver got his right hand caught in between tailgate of trailer.

Amputation of end of R little finger to first joint; caught by guard on equipment when attempting to close.

Second degree burns to forearm; holding a lit torch in his hand he reached with the other had to get material to cut and the flame touched his forearm. EE was not wearing a protective welders jacket or welders sleeves

Laceration right middle & ring finger. While moving a heavy piece of metal, identified as a cradle, along the top of an H-beam; his fingers were pinched when the metal was pushed off the end of the H-beam.

While performing maintenance work the ee walked through lime dust in the work area. The dust filtered through his boots and socks coming in contact with the skin of his feet causing second degree burns.

Left knee strain; twisted knee on ice in parking lot while getting out of truck.

foreign matter in eye; going up the ladder to work on the dust collector, dust blew behind his safety glasses into his right eye.

Foreign matter in eyes; putting slide back on in coal mill chute and dust flew in his right eye.

Heat exhaustion; employee was helping remove a motor in the Dolo Tower. Ee was in a JLG, tied off, beginning his descent, he collapsed due to the extreme heat and hit his head on the motor on the floor of the basket. He was out momentarily, when he came to he continued his descent.

Received lime burns to lower portion of both legs when stepped in pile of dust to shut off air line. Then walked across a drainage ditch with lime dust and water in it. Water was up to knees. Washed out boots, socks and pants with vinegar and water. Legs started to feel irritated and went to clinic.

Employee was shoveling from a pile of lime dust & it flushed down & over his boots. He went to clean out his boots & socks & treat his feet with vinegar to neutralize the lime dust. He then put back on his shoes & socks & returned to work. He later put on rubber boots & his feet sweated & the residual dust in his socks caused some burn area to both his feet.

Employee was attempting to dislodge a frozen chunk of rock that was stuck in the chute. Rock was dislodged and employee tried to catch it before it hit the shaker screen. Little finger was crushed against a "J" bolt. Tip of little finger to first knuckle was crushed and had to be amputated.

Jacking up the tail pulley framework and the jack slipped and hit the employee in the knee. The knee was cut, however, it did not bleed. The injury was reported and treated with first aid. Two weeks later the area became infected and the employee requested medical attention.

Moving a portable conveyor using a fork truck. Employee was hooking the strap of the conveyor to the fork on the fork truck as the operator moved the forks forward. Little finger was pinched between the fork and the railcar. As soon as the employee felt the pinch he pulled his finger away resulting in the laceration.

Washing out the lime dust pit and water mixed with lime dust splashed into his boots. He received 2nd degree burns to the right foot.

While greasing the tail pulley on the secondary crusher the employee's foot got stuck in deep mud. When trying to pull his foot out of the mud he twisted and strained his right hip.

Employee was changing belts on elevator and hand was pinched between belt and drive sheave causing an open fracture of the left ring finger and contusions to the hand.

Coming down stairs after checking HiCal system and slipped on ice on the stairs from freezing rain. Employee caught himself with his arm and strained the arm and rotator cuff.

Employee states that he was pulling himself up the ladder to get into the loader and he strained his uppper back. This was an aggravation of a preexisting condition.

Employee was filling the water truck and received a bee sting on his right hand. The next morning there was swelling of the hand and arm.

After cleaning oil from bearing housing using brake cleaner, employee began welding repair. An oily rag under the housing caught fire, fell to the floor near the bucket of oil and brake cleaner. A flash occurred and fire went up the employee's pants. Employee received third degree burns.

Employee was working overhead using a cutting torch to remove sheet metal screws and the metal splattered and ricocheted off of the sheet metal onto his face and metal got into his eye behind his safety glasses.

Employee was opening the gate for the HiCal elevator when dust came off the belt and went into his eye. Employee was wearing safety glasses at the time of the incident.

Employees were replacing the motor and gear reducer at the HiCal system. The manlift was positioned on the railtracks underneath the HiCal system. A rail car hit the manlift pushing it into a steel beam that wrenched the boom and knocked the basket off. Injured worker sustained a right leg fracture and left leg soft tissue injuries.

Employee states while assembling trunion with a co-worker, the bearing housing fell on his left hand. Employee was checking clearance with bearing housing, while at the same time, co-worker was tightening another bearing. Tightening caused the trunion to move and the housing spun and fell on his hand.

Employee was working at removing a 150pd piece of metal from the dust chamber reject chute on kiln#1. Metal was too large to pass through the lower opening so a rope was tied to the metal & EE began pulling the metal up to the top opening when it became lodged. EE worked to dislodge the metal by manipulating the rope & strained his back.

EE stated that he was cleaning off the back door to the tail pulley C-433. As he was lifting it to put it back, his lower back popped.

Employee was cleaning out railcar and developed breathing trouble and eye irritation.(220062630028-narrative: Employee was cleaning railroad cars of corn and soybeans had allergic reaction.

Employee was walking to supply shed between two kilns when dust blew into his right eye.

Employee was in basket of a J&G lift raising it up so as to repair an overhead light fixture. The "stop" control on the foot control pedal did not respond causing employee's left index finger to get pinched between basket and overhead pipe.

Employee was performing maintenance work on the back of #4 kiln and felt something in his right eye.

EMPLOYEE WAS USING A WRENCH TO TORQUE A BOLT ON A CYCLONE WHILE WORKING OUT OF AN AERIAL PLATFORM.

EMPLOYEE WAS WALKING DOWN STEPS AND TWISTED ANKLE AND FELL.

EMPLOYEE WAS LIGHTING ROSEBUD WITH THE NOZZLE IN A 50 GALLON BARREL. PROPANE FLASHED AND BURNED BOTH HANDS.

A 35 TO 40 MPH WIND BLEW POWDER OFF OF THE ROOF OF THE SURROUNDING BUILDINGS.

EE BRUISED LEFT FOOT WHILE LOWERING MOTOR OFF BASE; MOTOR SWUNG AND HIT FOOT.

EE WAS CHECKING ON SYSTEM CHUTES; WENT TO STEP BACK DOWN ONTO CATWALK AND ROLLED HIS RIGHT FOOT.

WHILE WORKING ON LIMIT SWITCH, HAND GOT CAUGHT BETWEEN BINDERATOR TAB AND LIMIT SWITCH, CUTTING RIGHT HAND.

EMPLOYEE WAS OVERSEEING WORK BEING PERFORMED ON CRUSHER, TRIPPED AND FELL INTO SHOVEL LAYING IN CORNER.

EMPLOYEE WAS WORKING ON FEEDERS, BENT OVER TO PICK UP BOLT AND BUMPED FEEDER WHICH CAUSED LIME TO FALL INTO HIS LEFT EYE.

EE WAS LIFTING PUMP HOUSING INTO BACK OF TRUCK TO TAKE TO MAINTENANCE SHOP FOR REPAIRS WHEN HE FELT A POP, AND PAIN TO HIS RT. ELBOW.

EE RELATES THAT ABOUT A MONTH AGO HE FELL OFF A CHEST-HIGH PLATFORM AND CAUGHT HIMSELF ON AN I- BEAM, HOWEVER HE HAD AN EXACERBATION OF INJURY WHEN HE WAS DOING REPETITIVE PULLING, PUSHING AND WRENCHING OF EQUIP. WHILE REPLACING CYLINDER

EMPLOYEE REPORTED THAT HIS LEFT HAND WAS INJURED WHEN HE WAS HOLDING A SHAFT WITH BOTH HANDS AND A CO-EMPLOYEE MISSED THE SHAFT TO BE HIT WITH A SLEDGE HAMMER AND STRUCK HIS HAND. EMPLOYEE GIV EN MINOR CARE FOR ABRASION AND PLACED NO RESTRICTED DUTY WITH RECHECK AND MMI RELEASE EXPECTED ON 9/10/02.

EMPLOYEE REPORTED TO SUPERVISOR ON 7/30/02 THAT ON 7/18/02 HE WAS HANDLING A BAGGING PALLET WHEN IT STRUCK HIS GROIN AREA AND THAT HE IS CONTINUING TO HAVE PAIN AND SWELLING AS A RESULT. EE WAS TREATED FOR A CONTUSION AND TAKEN OFF WORK.

EE WAS CHANGING SCREENS WHEN LIME DUST GOT INTO HIS EYE. EE DID NOT REPORT INCIDENT IMMEDIATELY & SOUGHT TREATMENT THE FOLLOWING MORNING. EE WASREFERRED ADDITIONAL OPTHOMOLOGY AS A RESULT OF SEVERITY. EE WAS INSTRUCTED TO WEAR GOGGLES AND TAKE OFF WORK UNTIL FOLLOWING SCHEDULED DAY.

EE REPORTED ON 5/8/02 THAT ON 5/7/02 HE WAS REPAIRING A SECTION OF ELEVATOR & HAD THE TOP OFF TO PUT THE CHAIN BACK ON. THE CONDITIONS WERE WINDY & HE WAS WEARING PRESCRIPTION GLASSES WITH SID E SHIELDS BUT NO GOGGLES. EE REPORTED THAT HE GOT LIME DUST IN HIS EYE. EYE DOCTOR TOOK OFF WORK ON 5/9/02-5/10/02 DUE TO SIZE OF ABRASION.

EE NOTICE WHITE LESION ON LATERAL ASPECT OF EYE, WITH NO FOREIGN OBJECT SENSATION. EE TRIED TO TAKE OUT WITH TWEEZERS AND PULLED OUT SOME UNKNOWN TISSUE. EE WENT TO ER FOR TREATMENT ON HIS OWN . EE DID NOT NOTIFY SUPERVIOSR UNTIL RECEIVED INVOICE FOR SERVICES. EE GIVEN PRESCRIPTION.

EE REPORTED HE WAS PULLING A LOT OF WIRE REPETITIVELY. HE IS AN ELECTRICIAN AND NORMALLY HAS TO DO THIS, BUT WAS HAVING TO DO A LOT MORE ON DAY OF INJURY. HE WAS HAVING PAIN IN HIS WRIST, ALON G WITH NUMBNESS, INTO HIS FINGERS AND TINGLING AT TIMES. OTHERWISE, NO COMPLAINTS. HE DID NOT FALL OR INJURE THE WRIST IN ANY SPECIFIC WAY, BUT WAS JUST DOING REPETITIVE MOTION.

WHILE PERFORMING MAINTENANCE ON A KILN, EMPLOYEE REPORTED LIME GOT IN HIS EYE WHEN THE WIND BLEW THE PARTICULATE.

EE HIT FINGERTIP WITH HAMMER WHILE INSTALLING NOSEBRICK IN KILN

EE WAS WALKING INTO TUNNELWHEN HIS EAR PROTECTIN SNAGGED ON A CORRUGATION WHIC SUBSEQUENTLY JERKED HIS HEAD AND NECK.

WHILE DRIVING FORKLIFT ACROSS RAILROAD TRACKS, MATERIAL SHOOK LOOSE & DROPPED OFF TOP OF FORKLIFT. IT GOT INTO HIS LEFT EYE CAUSING IRRITATION.

EE WAS PERFORMING CLEAN UP OPERATIONS AND WHILE SWEEPING A GUST OF WIND BLEW LIME DUST AFTER A DOOR WAS OPENED. THE LIME DUST WENT UNDER EE'S PROTECTIVE EYE WEAR AND RIGHT EYE BECAME IRRITATED .

EMPLOYEE WAS PUTTING ON HIS PROTECTIVE EYEWEAR WHEN LIMEDUST FELL IN HIS EYE! THE LIMEDUST HAD SETTLED ON THE EYEWEAR.

EE EXITING JL6 BASKET AND HIT KNEE ON EDGE OF BASKET. EE DID NOT REPORT INCIDENT UNTIL 11-20-00.

EMPLOYEE WAS MOVING A HOSE TO A NEW LOCATION WHEN LOOSENING FITTING AND BREAKING SEAL. THE FORCE WHEN HOSE BROKE FREE FLIPPED THE END UP AND ITO EMPLOYEE'S MOUTH, SPLITTING EMPLOYEE'S LIP ON THE INSIDE AND BREAKING A FRONT TOOTH.

EMPLOYEE PULLING STEEL PLATE UP TO A PREHEATER IN ORDER TO WELD IT. EMPLOYEE FELT STRAIN IN TESTICLE AREA.

EE PULLED BUCKET OF TOOLS UP TO AN ELEVATED CATWALK, TURNED AND WALKED APPROX. 10' WHEN HIS BACK BEGAN TO HURT.

EMPTYING HOPPER, HAND WAS CAUGHT BY STEEL PIPE AND BRACING.

WHILE GETTING WELDING CABLES IN KILN SO THAT ANCHORS COULD BE WELDED, LOOSE LIME DUST GOT BEHIND SAFETY EYE WEAR IN EMPLOYEE'S EYE.

COMPRESSED AIR WAS BEING USED TO DISLODGE ICE FROM A PIPELINE. AS THE ICE BECAME DISLODGED THE COMPRESSED AIR AND ICE WERE FORCED OUT OF THE END OF THE PIPE. THE COMPRESSED AIR EXPANDED AND TH E FORCE OF THE EXPANDING AIR KNOCKED THE INJURED OVER. INJURIES INCLUDED DISLOCATED SHOULDER, BROKEN CLAVICLE AND MINOR CUT.
