Amrize Fredonia Quarry Metal/Non-Metal

Mechanic was repairing dust control air cylinder on rig 56, left hand slipped against filter box causing minor contusion/abrasion.

Employee was not using proper lifting techniques or body alignment while shoveling spillage out from under S12 take up.

Employee had been lifting and working in an awkward posture leading up to the incident. EE began to squat down and suddenly developed severe lower back pain. Employee states that it took quite some time to stand up due to the pain.

A Caterpillar D7 was used to push stripping material over the edge of the spoil pile during rainy, wet conditions and at night. The dozer operator stated that ee got too close to the edge and come to rest sideways, approximately 100' below. On April 7 it became a medical incident due to the employee receiving physical therapy.

Employee was working at the Secondary Plant on Station #2. They were sliding screens out. He was trying to lift himself up on to the end of the 2nd deck so he could help get the screens out. When putting all his weight on his arms, his right shoulder gave away and popped out of place.

WHILE LIFTING THE PUMP/FILTER UNIT FROM ONE PORTABLE FUEL TANK TO ANOTHER THE EMPLOYEE FELT HE PULLED SOMETHING IN HIS BACK. THE EMPLOYEE CONTINUED TO HIS WORK DRIVING A TRUCK AROUND 0830 THAT DAY THE EMPLOYEE REPORTED THAT HIS LEG WAS GETTING NUMB AND HE WAS TRANSPORTED TO THE LOCAL CLINIC/HOSPITAL COR CARE.

(Note: the shift starts 6:00 pm through 6:00 am the following day). While removing a rock jammed between the skirt boards, the rock came loose and the bar moved unexpectedly pinching the employee's finger between the bar and the side skirt board.

Employee was cleaning around the plant with a walk-behind bobcat. He had made several passes around a concrete pillar. The employee back into the pillar pinching the palm of his hand between the steering controls and the pillar.

Employee was welding on grizzly bar in feeder, when another employee engaged in horseplay by spraying water on him with a garden hose from above. The employee stopped welding and began walking around. When the employee was asked what was wrong he stated he had been shocked.

Employee was putting side rubber on P-3 belt line. He was using a pry bar and the bar slipped hitting him in the lip. He was taken to a clinic where he received three stitches and returned to full duty.

Employee was trying to dislodge tramp metal from the Big Ball Mill feeder by rotating the drive sheave on the gearbox. When he rotated it forward it finally stopped and would not go further. When he let go of the sheave, the outer edge of the sheave cut the palm of his right hand. There were no v-belts attached as they were burned off when the feeder choked down.

EMPLOYEE WAS ATTEMPTING TO CLEAN THE MUD OFF THE CONVEYOR TAKE UP PULLEY BY USING A 4 FOOT STEEL BAR. THE BAR WAS CAUGHT BY THE PULLEY PINCHING EMPLOYEE'S LEFT HAND BETWEEN THE BAR AND EDGE OF THE CATWALK.

FORK CAME OFF OF THE FORK LIFT AND EMPLOYEE WAS LIFTING THE FORK TO PUT BACK ON THE FORKLIFT.

EMPLOYEE WAS CLIMBING UP A 6' ALUMINUM LADDER AND WAS NOT TIED OFF, LADDER SLID DOWN TO GROUND. DID NOT RECEIVE MEDICAL ATTENTION UNTIL 7-13-02.

EE WAS CLIMBING LADDER OF THE HAUL TRUCK. HIS FOOT SLIPPED OFF THE STEP AND HE FELL BACKWARDS TOTHE GROUND. IT HAD BEEN RAINING.

EE WAS WALKING WHEN HIS RIGHT FOOT GOT CAUGHT IN A NYLON SLING THAT WAS LYING ON THE GROUND. WHEN HE STARTED TO FALL, HE TRIED CATCHING HIMSELF WITH HIS HANDS, AND FRACTURING HIS RIGHT WRIST.

EE STATED HE JAMMED HIS NECK WHEN DESCENDING FIXED LADDER LID SHUT HITTING HIS HARD HAT

THE EE WAS WALKING UP AN ELEVATED CATWALK WHEN HE SLIPPED & FELL. HE HIT HIS NOSE ON THE WALKWAYCUTTING HIS NOSE. STITCHES WERE REQUIRED TO CLOSE THE WOUND

EMPLOYEE WAS MOVING A CONTAINER OF WASTE AND INJURED HIS BACK.

THE INJUREDPERSON WAS TALKING TO THE CRANE OPERATOR, WHILE STANDING ON THE RIGHT REAR OUTRIGGER. THE CRANE OPERATOR WAS SIGNALED TO MAKE A PICK AND SWUNG INTO POSITION. WHEN HE SWUNG ON AROU ND THE EMPLOYEE HAD NOT MADE IT DOWN. THE COUNTER WEIGHT CAUGHT HIM SMASHING HIM INTO THE CRANE'S ENGINE COMPARTMENT.
