OUR EMPLOYEE WORKING AT MOUNTAIN AGGREGATES QUARRY MINE SITE WAS FATALLY INJURED WHILE OPERATING A DOZER ON AN ACCESS RD IN WHICH THE EQUIPMENT LEFT THE ACCESS RD AND ROLLED DOWN AN EMBANKMENT.
EE left work early on 5/5/10 to attend a personal buisness. Prior to leaving work EE did not complain or report any injury. EE called in on 5/6/10, said he had hurt his back, didn't know if it was at work or home. He only saw dr 1 time, dr said he could RTW on 5/17/10 with no restrictions. Have not found any evidence of injury, still investigating EE's failure to report incident.
Employee was in the process of cutting tree when chainsaw exploded. Explosion resulted in first degree burns on hands and abdomen, third degree burns on upper leg.
A.C. was not working; mechanics fixed wire. Operator went back to watering road, turned air conditioner back on and small particles of rust blew out into his left eye under his safety glasses.
Employee was welding on primary hammers with automatic unit and looked inside crusher without properly wearing his shield.
EE was removing a drain plug from a bearing housing while changing oil, when the wrench slipped letting his hand strike a piece of metal, causing a laceration on his left hand.
EE was knocking pin loose on excavator with a slate bar. When the pin came out, arm fell down and slate bar hit EE in the lower part of his jaw.
Allegedly, the injury was caused when the employee was pulling the rear overhead door of a tractor trailer with his left hand and struck the right hand which was placed at the bottom of the door.
EE HAD FINISHED GREASING FUEL FILTERS OF DRILL.TURNED TO CLIMB DOWN STEPS, SLIPPED & FELL OFF THE DRILL PLATFORM. STRUCK GROUND WITH LEFT FOOT BREAKING LOWER LEFT LEG (BETWEEN KNEE & ANKLE)
