Straight Creek #1 Mine Coal

Helping two men remove a set of air lock doors. When he removed a jack a piece of draw rock fell hitting and cutting his upper lip.

This mine is in inactive/non-producing status. Barometric Pressure change with water blown from trap in SC #2 seal. Curtain torn down by rock. Safety Dept Personnel and MSHA Inspector found 5% methane. Men were immediately pulled and power turned off. Action plan put in place under the direct supervision of two MSHA Inspectors who were on-site.

Fell crossing belt, landing on tailbone and back.

Picked up a mini master hoseduster and turning it around. Felt pain in lower back shooting thru left leg.

Greasing Dolly Wheels.Started to get up and twisted right knee.

Lifting belt stand to place a crib under to line belt. The stand was stuck in mud. Hurt back.

Unloading bolting supply using bar and pulled lower back causing sharp pain.

Lost Footing & Slipped on slope belt. Knee popped as if dislocated.

Pulling Rock out of #7 Head Drive. When lifting rock, felt sharp burning in back.

While helping make a belt splice, went to move knocker over, when EE got hit by a 4 lb hammer by fellow employee.

Bolting roof when piece of rock fell and hit hand.

Walking in Roadway, slipped in rut and broke right ankle.

Cutting a piece of rope with utility knife when knife slipped and cut into the left leg

Twisted ankle when crossing belt.

Was cutting a piece of belt when knife slipped out of belt and cut leg below knee.

Went to grab strap while bolting and cut thumb on right hand.

Lifting bucket up on low trac when one panel of the K-stopping slid off top striking left wrist.

Rock fall in crosscut 65 of north main 1 in 5 heading (20X20)

Got off Shuttle Car to check cable. Tripped on cable and fell down, fracturing/breaking left wrist.

Loading rock dust. Something flew under safety glasses and went in left eye causing pain and swelling.

Miners were attempting to tram a continuous miner by installing power wires directly into a receptacle in a power center without using a proper disconnecting device. When they attempted to set and reset the breaker, the breaker arced causing serious burn injuries.

Helping load rock duster in Mac 8 mantrip. Pulled something in upper leg.

Fell thru side of slope where belt is inclosed.

Employee was lifting tail roller off forks of backhoe when he got finger mashed by weight of roller.

Miner crossing the belt on cat walk. Got a piece of metal in his hand.

Employee fell from belt while working on structure. Landed on back. Complaining of pain in left shoulder.

Employee had mud on feet. Stepped on rail to cross #5 Belt. Fell on left arm. Hurt elbow. Swelling and bruising.

Mechanical Failure, elevator shut down, men sent home, reported for repair.

EE was outside loading supplies to take U.G. EE was cutting steel straps on a bundle of roof bolts and cut his right index finger.

EE was struck by a rib roll on the left ankle while tramming the miner on 008 section. No broken bones, pain and swelling only.

EE was putting together water line for the continuous miner with a banding tool. His right hand slipped & his right pinky finger was caught between the water line & the banding tool while the hammer was coming down. He mashed his pinky resulting in an open wound, loss of nail, & bone was broken at the tip of the finger. No lost work days. Returned to normal work activity.

While bolting, a piece of draw rock fell and cut right arm, above elbow. Resulted in 14 stitches.

EE was working on #5 belt scraper. He was pulling out a belt pin, when he slipped and fell across the belt. He scraped and bruised his right rib and his right leg.

EE was struck by a piece of draw rock while bolting. It hit his left arm and shoulder. He suffered from cuts on his arm and soreness in his shoulder. No broken bones.

Employee was carrying an old bull gear that was removed from a head drive. He dropped the gear on his right hand resulting in swelling and bruising.

Ee was standing on a ladder hanging a curtain. He fell off of the ladder while climbing down. He injured his right knee. Pain and swelling.

EE was replacing a tire on std car. He was pressing down on the wrench when it slipped off the bolt and hit his right leg just above his ankle. Resulting in swelling and bruising.

Employee was walking down the Rim Seam entrance and slipped in mud. He twisted his back and cut his right knee. The fall resulted in a minor back injury and an abrasion on the right knee.

EE was driving a pin in with a crib block on a piece of equipment. The repetitive motion caused his right wrist and arm to become sore. This resulted in a mild sprain. He visited Pineville ER and was released the same day. No lost work days.

Injure was repairing a Kennedy Stoppings on the belt line. A rib rolled off and struck him on the left shoulder, resulting in bruising and swelling.

Elevator down. No accidents or injuries. Went down at 0330, repaired at 1730.

EE was mucking the #5 belt at the overcast. A piece of draw rock fell and struck him on the right shoulder and the arm. This resulted in pain, bruising and swelling of the right arm.

EE was replacing a tire on the std car on section 008. The tire slipped, resulting in EE straining his back (upper) and his neck. Pain in back and neck.

EE was roof bolting when a bolt strap fell and cut his right forearm. Resulted in 7 stitches.

Employee was rockdusting 008 section at the intake. He accidently threw some rock dust in his eye while using the rockduster. His left eye was swollen and irritated. Seeked medical attention.

Employee was opening a can of oil. His hand slipped with trying to open the lid. He cut his middle finger on his right hand on the lid. Required 6 stitches. No lost time.

Employee was driving roll pins out of a tail roller. A piece of metal chipped off the punch and struck his left wrist above his gloves. Cut on wrist, open wound.

Employee was walking through a vent curtain. He had his right hand on a crib and a piece of rock dislodged and struck his hand. This resulted in a broken bone in his hand. He was not operating any machinery.

Employee was shoveling belts. A blunt object flew up in the air and hit his right elbow, resulting in a bump. Some swelling and pain.

EE popped his left knee while lifting his lunch box, EE complained of pain and swelling.

Employee was performing a belt splice. He bent over to grab the belt with his right hand and had the belt pin in his right hand. He leaned down and stuck the pin into his left eye. The pin penetrated the muscle area in the corner of his left eye and scratched the first layer of his eye. It did not affect his vision. No lost work Days.

Employee was hanging a cable while standing on top of a scoop. When he stepped off the scoop, he jammed up his right leg & hip. Complained of pain in his right leg, hip & back. Pinched nerve in right hip.

EE was removing the canopy on the off side of the roof bolter. He lowered the canopy too far down and it fell. It hit him in the shoulder and the back. He had a small cut on his head and bruises on his upper and lower back. He had a back injury and was transported by helicopter to the University of Tennessee Medical Center.

Rock fall reported to mine management at 5:05 am. The location of the fall is the right corner intake side of the #2 seal. The fall was 10' long, 7' wide and 4' thick. No accidents or injuries to report. The MSHA hotline was called and fall was reported. Three roof bolts where pulled out as of result of fall. This area is currently being cleaned up and bolted.

EE was loading a car tire into a scoop bucket. He sprain a muscle in his lower back. Left side of lower back and pain down left leg.

Employee was pulling broken unit back together when the handle came out of the rope hoist, striking his right thumb resulting in a broken bone and 4 sutures.

While watching his cable, employee put his hand between the boom and the frame of the shuttle car. When he let the boom down, his left hand was in a pinch point. He injured his pointer, index and ring fingers. Open wounds and avulsion of the fingers.

Employee hit his head on the right side directly above his temple. He walked into an open window at the mine office. He cut his head on the corner of the window frame resulting in an open wound about 2" long. No lost time or restricted duty.

Employee was struck in the right elbow by a bolt wrench while bolting. No open wound or broken bones. Swelling and bruising occurred.

While installing a shaft on the Joy continuous miner on section 009, his right hand got caught in the machine, cutting the tip of his pinky finger. Required surgery.

Employee was working in the #2 heading when he was struck in the back by a piece of rock. Injury on lower back near kidney area. Swelling & bruising on lower back. Sent to U.T. Medical Center for examination & x-rays.

Employee's left hand got caught in the belt clamps while working on teh slope belt. Resulting in 3 fractured fingers.

Employee was going through the mandoors when an air current blew the doors together and mashed his middle finger on his right hand - open wound - no lost time.

EE was struck in the right eye by a cable hanger while roof bolting. Some bruising & swelling of right eye. No lost time.

EE was bolting #2R. A piece of draw rock fell, striking him on the right leg, just above the ankle. The impact fractured the bone located in that area. This occurred on MMU Section 009. (Fractured fibia)

Employee fell of a 6' ladder while hanging a curtain the landed on his back, bruising his right shoulder. first aid applied at mine site. no lost time.

Employee was struck by a piee of rock on the left side of his nose, resulting in a small cut & bruise while roof bolting.

Elevator down on 4-26-07 at 1:00 pm. It was temporarily repaired. Severe rain and weather worsened the condition. Elevator down on 4-27-07. National Elevator was called to repair. Elevator repaired on 4-27-07.

Employee was roof bolting on section 9, He was struck by a piece of draw rock on his left arm hand and thumb resulting in a bruised left arm. No missed work days.

Roof fall in the intersection of #4 entry 008 section at the last open break along anchorage. Ventilation not affected. No injuries.

There has been a roof fall outby spade #5539. 10-12" thick and apprx 50'long. The area has been dangered off. It is not abstucting air flow and poses no danger at this time. A clean up plan has been posted at the site off the fall and a copy has been submitted to both state and federal inspectors.

While using a grinder, a foreign object flew into his left eye. He was wearing safety glasses. A minor injury. No lost time.

EE strained his elbow while putting up a bent roof bolt. Soreness & pain.

Elevator down. The Govenor Rope has some rusty places where it rests on the shive wheel at the bottom. National Elevator found broken strands. Rope is ordered. National Elevator will replace ASAP.

EE was cutting a piece of belt at the #2 head drive. The knife slipped off the belt and cut his leg. He cut his left leg on the inner calf-3in. cut- received medical attention. no lost time.

Employee was trimming belt when the knife slipped cutting the inside of his left pointer finger resulting in a cut 1/2" requiring stitches. No lost time.

Employee flipped over an office desk and fell on left side - dislocating left shoulder.

Employee twisted right knee while pulling a piece of curtain out of a muck pile. Right knee twisted and sprained - no lost time.

While installing a kennedy stopping, a rib rolled off hitting employee on his left foot. This resulted in a hair line fracture on top of his left foot.

Employee was struck by a draw rock, it hit left side of head, scratching a small cut approx 1/2" to 1" long. Not very deep, no stitching was required. An abrasion, capillary blood only.

Employee was working on #4 belt structure. He was tossins structure when he cut and mashed his middle finger. The resulted in a broken bone top of middle finger on left hand.

Employee was working on a slope. He slipped while holding a metal beam. The bean hit his foot (left) Some swelling and bruising. 1st Aid Applied. Went to hospital.

EE was shoveling #2 belt when a rock from crib rolled off and struck him in the mid-section of his back. Small scratches, no bruising at the time, no bleeding. Complaining of pain in that area. He was asked if he had pain anywhere else and he said no.

Unsafe act by employee; he was working around a moving mantrip, the car struck his right shin (right leg) causing swelling and bruising.

Employee was walking around "break" and stepped in a hole, twisting left ankle. He is on light duty for 1 week. Swelling, pain, bruising.

Unplanned rock fall 23 breaks from the face. No injuries, no air blockage. Occurred in the old works.

Employee stepped off supply cart and fell onto the scoop bucket. Resulted in breaking bone in left hand. Light duty from 6-9-06 thru 6-15-06.

Employee walking on belt line, he slipped and hit elbow on belt structure. Injured right elbow, some bruising. No lost time, restricted light duty.

Elevator not working properly. No injuries or accidents. National Elevator was called. They repaired switch. Elevator is now functional.

EE was roof bolting and when the drill head dropped down, the steel fell out of the hole, striking him in the head. EE was wearing hard hat. There were no lacerations or cuts. Not a serious injury.

EE STEPPED OFF TAILPIECE AND LANDED ON A BLOCK OF COAL AND TWISTED HIS ANKLE. NOT A SERIOUS INJURY.

Employee slipped and fell on track while prying the switch mechanism, resulting in pain in lower back. Not a serious injury.

Employee was roof bolting on operator side and saftey rope broke. Employee fell backwards about 6 feet on back. Some bruising and swelling. Injury was not serious.

Contractors were moving a large piece of metal when the witness lost his grip and dropped the metal on the injured's hand. all four fingers of right hand impacted by heavy piece of metal-swelling in finger-accident not serious.

SCOOP ROLLED OVER FOOT. SWELLING & BRUISING ON LEFT FOOT.

HIT BY DRILL STEEL IN THE PROCESS OF DRILLING A BOLT HOLE.THE STEEL BENT & BROKE, HITTING EMPLOYEE IN THE LEFT ARM AS HE WAS THROWING HIS ARM UP TO PROTECT HIS FACE.

ROOF FALL ABOVE BOLTS IN AN INTERSECTION APPROXIMATELY 8 FEET HIGH 15 X 18' LONG & WIDE. WATER SEEPING THROUGH TOP, MINING UNDER A CREEK.

He was stretching out pump cable around corner through break when rib rolled off striking worker in head and shoulder, knocking him to the ground.

While dislodging rock in miner conveyer the rock bar glanced off rock and punctured top of foot through boot.

Blind side of car pulling miner cable; caught left foot between car and rib. No break in skin or broken bones; swelling of ankle.

HE WAS SCOOPING ON RETURN ROADWAY ON OLD 2+4 SECTION WHEN SOME ROCK FELL OUT OF THE TOP. NO LOST TIME ACCIDENT. RETURNED TO WORK THE SAME DAY.

THERE WAS NO ACCIDENT, THE ELEVATOR HAS A PROBLEM AND IS TEMPORARILY TAKEN OUT OF SERVICE, ELEVATOR COMPANY WILL BE HERE TO TAKE CARE OF THE PROBLEM.

TOP HAD DETERIATED OVER A LONG PERIOD OF TIME, SOME DRAW ROCK HAD FELL FINALLY SOME HAD CAME LOOSE FROM THE ANCHORAGE. FALL WAS CLEANED UP AND SUPPORTED BACK. THIS MINE IS BEING REBOLTED IN AL L AREA'S. THERE IS NO PRODUCTION AT THIS MINE.

EMPLOYEE WAS REMOVING TIMBERS ON 2 SECTION BELT LINE WHEN A ROCK FELL FROM THE ROOF ON HIS RIGHT LEG.
