Employee states that he was experiencing pain in his back from a shift earlier and the pain became more extreme after a sudden jolt while riding in the man bus.
Employee was operating a shuttle car when a piece of draw slate measuring 18 inches by 36 inches by 3 inches fell striking him on his head and face. This resulted in a slight concussion and laceration over his left eye. He is expected to miss two weeks of work.
During the pre-shift examination of the # 3 conveyor belt a roof fall was found in an intersection 10 crosscuts inby the the belt head transfer point. The roof fall measured 18 feet by 20 feet by 5 feet.
During an inspection of the intake air course a roof fall was found in an intersction approximately 900 feet from the mine portal. The fall measured approximately 18 feet by 20 feet by 5 feet in thickness.

Employee, was operating a continuous miner in the #3 entry crosscut when a piece of draw rock measuring 2 feet by 4 feet by 5 inches fell from an area of the mine roof near the coal rib striking him on the left shoulder and arm. This resulted in the employee's wrist being fractured.
An unintentional roof fall was found during a routine inspection of the return air course. The roof fall is located approximately 180 feet inby survey point 10971 in the number 7 entry. The fall measures 16 feet by 25 feet by 6 feet.
Employee was operating a continuous miner when a piece of draw rock measuring 2 feet by 3 feet by 6 inches fell from an area next to the coal rib striking him on the left shoulder and leg. He was taken to a local hospital where he was diagnosed with a small fracture in his left ankle.
Employee was operating a Log Airdox model 482 scoop to clean loose coal in the #7 entry when a piece of draw slate measuring 3 feet by 3 feet by 4 inches fell striking him on the head and shoulder. EE suffered an abrasion on the left side of his head and shoulder and a slight concussion.
A roof fall measuring 8 feet by 19 feet by 40 feet has fallen in the number 7 entry approximately 3000 feet outby the working section. The area has been dangered off and additional support has been added. The fall does not change ventilation.
ROOF FALL OCCURED 60 FT IN BY CONTROL #10775 IN THE #5 ENTRY. LATER, ANOTHER ROOF FALL 60 FT INBY CRONTROL #10786 IN THE #7 ENTRY.
A ROOF FALL WAS FOUND AT THE BEGINNING OF THE SHIFT. THE FALL MEASURING 5' X 18' X 25' WAS LOCATED APPROX. 250 FEET INBY THE CONVEYOR BELT PORTAL OF THE CONVEYOR BELT USED TO TRANSPORT COAL TO THE LOADING FACILITY. NO INJURIES.
EE WAS OPERATING A BATTERY POWERED MINE BUGGY WHEN KNEE CAME IN CONTACT WITH THE MINE ROOF FRACTURING HIS RIGHT ANKLE.
EE WAS LIFTING A BELT ROLLER WHEN HE FELT A PAIN IN HIS LOWER BACK AND LEFT LEG. HE HAS CONTINUED TOWORK UP UNTIL MAY 11, 2004 WHEN HE STATED THAT THE PAIN IN HIS LEG HAD GOTTEN WORSE.

EE ROOF BOLTER OPERATOR WAS HANGIN GROOF BOLTER CABLE, HIT HSI LEFT INDEX FINGE RON A ROOF BOLT PLATE, CAUSING A SLIGHT INCISION.
MINER OPR. STARTED PUT HIS KNEE DOWN ON MINE FLOOR, AND BUMPED HIS KNEE ON A ROCK.
ROOF BOLTER OPR. WAS BETWEEN ROOF BOLTER AND RIB LINE WHEN A PIECE OF DRAW ROCK FELL FROM MINE ROOF HITTING EE ON HIS LEFT SHOULDER.
