RB #5 Coal

Foreman at RB#5 Mine reported a roof fall, he said it was in the #3 entry inside the mine from the back side. The fall measured approx. 20' X 20' X 4' thick. There was no one injured. He said it fell sometime between the morning and that afternoon.

A rock fall measuring approx 20' long X 20' wide X 3' thick was discovered at 1:30PM on 2/19/10. The exact time of the fall is undetermined. The fall was located in the 9th crosscut inby the north portals in the #5 entry. The #5 entry was dangered off on all accessible entrances and the #4 entry will become the designated travel way and primary escapeway.

A rock fall was discovered approx. 300' inby portal, in the intake roadway. The fall was one rock that measured 8' wide extending 6' down to a point. The rock was triangular in shape and was approx 3' high. The rock fell sometime between the morning preshift and when the foreman exited the mine, approx 3:00 pm.

A rock fall was discovered on 11/5/07. The fall was located in roadway going to bleeder. The rock fall was approx 90' long in all 3 entries. The fall was from 3' to 8' high.

The employee claimed he was picking up crib blocks on the outside of the mine and allegedly strained his upper back.

A roof fall was discovered approx 200' inby the portal in the return aircourse. The fall occurred sometime during the off shift between 7-31-07 and 8-1-07. The fall measured approx 12' wide by 40' long (extending into the crosscut) and 4' in height. The area was cleaned up and dresupported with cribs, collars and roof bolts.

During the examination of the no.1 conveyor belt it was discovered that the mine roof had set down & broke some cross collars that had been set along with cribs to support the area making the area unsafe for travel. The area was located at the 3rd crosscut inby the portal & measured approx 8'L x 8'W with the thickness undetermined due to the fall being caught by the cribs.

A roof fall was discovered at the beginning of the shift on the main line belt conveyor 11 crosscuts inby the portal. The fall measured approx. 20' long by 8' wide and 6' thick at its peak. The fall was removed and the area was supported with cribs and cross collars.

Injured stated that on 12-10-05 he was kneeling down to shovel some material when as he knelt down he struck his knee on a rock. He had no lost time until 12-14-05.

Injured stated he was shoveling loose coal from under the conveyor belt and when he mised a shovel full to empty on the conveyor belt, he felt a sharp pain in his lower back

INJURED WAS HOLDING A SECTION OF VENTILATION CURTAIN AS HE PULLED IT TO THE FACE WITH A MANTRIP BUGGY. THE CURTAIN HUNG ON THE MINE FLOOR & PULLED INJURED'S ARM BETWEEN THE STEERING ROD & THE MINE ROOF. POSSIBLY BROKE INJURED'S ARM.

INJURED STATED THAT AS HE WAS TRAMMING THE MOBILE BRIDGE CONVEYOR THRU A DIP INT HE MMINE FLOOR AND AS THE MBC MOVED OUT OF THE DIP IT ROCKED FORWARD RAISING THE REAR AND OPERATOR'S DECK OF TH E MBC, CAUSING INJURING TO BUMP HIS HEAD ON THE MINE ROOF. INJURED HAD NO LOST TIME UNTIL 11-10-03.

INJURED WAS SCOOPING THE ROADWAY WHEN AN OLD PIECE OF BLACK PLASTIC PIPE FLEW UP FROM UNDERNEATHHIS SCOOP AND STRUCK HIM ON THE CHIN, REQUIRING THREE STITCHES. INJURED HAD NO LOST TIME.

INJURED WAS APPLYING ROCKDUST BY HAND TO THE CONVEYOR BELT ENTRY. WHEN HE SLUNG A HANDFULL OF DUST HE STRUCK HIS HAND ON A BELT ROLLER, CUTTING HIS SMALLEST FINGER ON HIS RIGHT HAND REQUIRING STITCHES. INJURED HAD NO LOST TIME UNTIL 3-14-03

INJURED STATED HE WAS CRAWLING ACROSS SECTION AND FELT A SHARP PAIN IN HIS LEFT KNEE. INJURED'S FIRST DAY OFF WAS 4-5-02 AT WHICH TIME HE WENT TO THE DR.

A ROOF FALL WAS DISCOVERED IN THE 29TH CROSSCUT AT THE FIRST SET OF SEALS INBY THE PORTAL ON THE PUCKETTS CREEK WATERSHED. FALL APPROX. 55' LONG & 10' WIDE AT THE INBY END TAPERING TO A POINT ON THE OUTBY END. THE FALL WAS APPROX. 4' THICK. THE AREA WAS CRIBBED OFF TO DENY ACCESS TO THE UNSUPPORTED AREA.

INJURED WAS LIFTING CONVEYOR BELT STRUCTURE AND HAD A SHARP PAIN AND BURNING SENSATION IN HIS LOWER BACK. IT WAS DISCOVERED ON 8/13/01, HIS FIRST LOST WORK DAY AND DOCTOR VISIT, THAT HE HAD PU LLED A MUSCLE IN HIS LOWER BACK.

INJURED WAS LIFTING A ROCK FROM THE BRIDGE CONVEYER AND STRAINED HIS BACK.

INJURED STATED THAT ON 6-2-00 HE WAS HANGING A BLOCKCURTAIN AND TWISTED HIS KNEE. HE HAD NO LOST TIME UNTIL 8-10-00 AT WHICH TIME HE WENT TO THE DOCTOR.
