Roof fall information from D-2 in the plant mains found at 7:40am. It was reported to the call center at 7:45am. The roof fall is in #4 and #5 entry beginning in x-cut 5 thru 6. In #4 entry the roof fall is approx. 70'long x 20' wide x 4-6 high (thick).

Employee had previously twisted a knee while tightening bolts but continued to work, sought medical attention on 12/18/2018 where EE underwent a procedure and received stitches to rt. knee and returned to work the next day. No time lost.

NON-INJURY roof fall occurring at crosscut 45, on the #4 belt - in the intersection and to the right and left of entry.

NONINJURY Roof Fall-Approx. 20x20x5feet high in the #4 entry at crosscut 15. Fall discovered approx. 9:20AM, and reported to MSHA hotline at 9:28AM.

Employee was moving water lines when the employee felt pain in back and leg.

EE tripped on loose rock on Mine Floor & cut upper left leg. EE received stitches.

An unintentional Roof fall occurred off section Break 53 in Secondary escapeway. The cause is unknown and there are no injuries.

Employee was using a utility knife to cut tape & knife slipped and cut employee's hand. Employee received 4 stitches.

Employee stated that they were taking the scrubber motor ductwork off the miner when it mashed a finger. They received 10 stitches to their ring finger.

EE was taking a 1 1/2" water line loose at the pump when water line sprayed EE in the face and some debris got into eye under the safety glasses. EE went to ER initially, followed up with the eye doctor. EE had piece of metal in the right eye and it was removed. EE has no lost time. EE has returned to work.

EE squatted down when raised up & turned felt pain in lower back, went on to section started hanging curtain. EE said pain got worse, went to local ER. EE stated the dr. said it was muscle spasm. EE went to family dr. next day, EE is off work do not have a return to work date.

While working on cable reel on shuttle car, lid covering reel fell and hit EE's hand. EE went to ER and has a broken little finger and received 9 stitches. EE is off work until EE sees orthopedic doctor.

Roof fall break #8 plant mains #5 entry. 20' wide X 25' long X 5' thick. Roof/rib support in affected area: shale 4 or 5' resin bolts.

Employee moved shuttle car tire out of way with help of another employee. Bent over to pick up a pogo stick and felt pain in back.

Changing out belt control box, was pulling old box out of area, employee started to back out of way as old box started to turn over on EE's right lower leg and foot. EE had a cut half way up EE's leg on EE's shin requiring 9 sutures.

Roof fall was located in belt entry #3 of #4 belt line at break #60. Fall is approx 20'x30' and approx. 7-8' high. Cribs, Steel beams, jacks, and 10' cable bolts existed in the area prior to the fall. An action plan was submitted and includes plans for cleaning up and rehabilitation of the roof fall. The fall did not affect ventilation, miners, or equipment.

There was a roof fall located at break 12 in the #4 entry of the plant mains. The approximate size of the fall was 20' x 6' x 40'. It has been dangered and cribbed off and the #5 entry will be used as the travelway around the fall.

Miner cable was grounded and employee was using fault wizard to find bad spot. He thought he had it and took the fault wizard off and began using his meter to check if cable ground was cleared. This is when employee received a shock.

Employee smashed fingers with canopy

During the weekly examination of the plant mains a roof fall was discovered in the #2 entry at the #2 crosscut. The fall is apx. 20' x 30' x 5' high. It did not affect persons, equipment nor ventilation. The fall will not be cleaned up and the area will be supported as per the acknowledged action plan.

A roof fall was discovered in the track entry at crosscut 44 on #11 mains during a pre-shift examination. The roof fall is apx. 60 ft long X 7'high X 18' wide. The area was dangered off and an action plan submitted to MSHA. The fall did not affect persons, ventilation nor equipment. The fall will be cleaned up.

Employee had placed left hand on scoop bucket after hanging up 6" waterline when, water line fell striking fingers on left hand. Employee received 6 sutures to his 3rd digit finger and hairline fracture to 2nd digit finger on left hand. Employee has returned to work with no restrictions.

While doing a weekly examine a rock fall was found at Shell Branch Bleeder in the #2 entry at brk. 25-26. The size of the fall was approximately 4 to 5' thick 20' wide and 40' long. The ventilation has not been affected, cribs are being installed in the affected area.

A roof fall was found at 67 Mains, on 6 track at break 8 between 4 & 5 entry. The fall was approx. 2' to 3' thick 18' wide and 30' long. 10 Heintzman Jacks with pizza pan heads will be set. This fall does not block any escape way or effects the ventilation.

Employee was in mine yard lifting a 3hp pump to bring into building when he felt something pull in his lower back. Employee became a lost time accident due to this injury.

Employee was bolting top in 3 heading when he tripped over a rock falling to ground. Employee complained of neck hurting, employee was placed on restricted duty on 8/30/2013, on 9/03/2013 company shut down due to coal market, on 9/16/2013 due to coal market when employee was placed on unpaid furlough.

Employee was using utility knife to cut holes in belt when he cut his left hand, 3 sutures required, returned to work with no restrictions.

A roof fall has occurred at Shell Branch Bleeder in #4 entry at crosscut #14, the size of the fall was 20' x 20' x 8' which involved a 14-9 remote control continuous miner. The area will be cleaned and additional roof support will be installed.

Employee Was at Laurel Fork Bleeders when he found a timber had fallen, while carrying timber to set back he slipped and fell hurting his back.

Employee was working at head drive when draw rock fell striking left leg. Employee received a fracture to his left fibula.

On 4/30/2013 employee riding in mantrip strained neck when mantrip struck parked mantrip. Employee became restricted duty on 7/23/2013 due to this injury.

Employee was tossing a bag of sand on top of a 20 ton locomotive when he felt a sharp pain in his lower back. Employee has been placed on restricted duty.

Employee was using a slate bar to move head motor, slate bar slipped striking employee in mouth breaking 3 top and 2 bottom teeth. Employee has returned to work with no restrictions.

Employee was at the end of Lo Lo structure when he placed his left hand on back of carrier dolly when rock came across dolly striking left hand fracturing his left ring finger. Employee has been placed on restricted duty.

Employee was standing on a pallet of block building a stopping when he fell off the pallet striking his left knee on the ground. Employee received a fractured to his left patella. Employee has returned to his regular job with no restrictions.

Employee was servicing a mantrip in the shop when he slipped off the mantrip falling and striking his left forearm against side of the mantrip. Employee received a fracture radius to his left forearm. Employee has been placed on light duty to his injury.

Employee was helping clean off top of continuous miner when rock flipped up striking right hand, sutures required.

Employee was bolting top when a piece of drawrock fell striking 3rd digit finger on left hand, sutures required.Employee has returned to regular duty.

Employee was at brk. 24 at 3 left on 002 section building a brattice. Brattice was not secured when it began to fall striking left thumb and back of head. Employee received a fracture to his left thumb from this accident.

Employee was helping a build a brattice while reaching a co-worker a brattice block his right foot got stuck in the mud causing right knee to twist tearing ACL ligament, employee became a lost time on 08/06/12 due to this injury.

A contract delivery truck driver at the Mines with temperatures around 100 degrees got out of his truck the air conditoner was not working at the time unstrapped a load of pipe when he staggered to the ground suffering from heat exhaustion, went to the hospital and doctors informed him that it was not life threatening but would like to keep him to check his heart.

Employee was in 2 heading on 002 section bolting top when a piece of draw rock fell striking employee on right forearm. Employee received 8 sutures due to this injury. Employee has returned to regular duty with no restrictions.

Metal ban on a bundle of timbers broke loose striking top of employee left forearm cutting arm, 6 sutures required, employee has returned to reg. duty with no restrictions.

Employee was getting lo lo ready to be scoop, moving miner and bridge cable out of way when drawrock fell striking him on head and right shoulder employee received cut to head. Employee has returned to reg. duty with no restrictions.

Employee was tramming roof bolter when a piece of draw rock fell striking left little finger, employee has been placed on light duty.

Employee was making a belt setup when a piece of drawrock fell striking head, employee was placed on restricted duty until 8/10/11.

On 7/28/11 employee carrier deck got on lo lo structure then dropped off striking ground with a sudden jerk. Employee became a lost time on 9/9/11 due to a strained lower back.

A roof fall was found in the Old Works at 6 right panel off from the Holly Bush Submains, in the # 1 entry at break 9 thru break 10. The size of the fall was approximately 20'W x 70'L x 7'H. No ventilation of equipment effected, the area has been dangered off.

A roof fall was found in the Old Works at 6 right panel off the Holly Bush Submains in the # 2 entry. The size of the fall was approximately 30'W x 30'L x 7' thick. No ventilation or equipment affected, the area will be dangered off.

A roof fall has occurred at Old Holley Bush Submains on 001 section. The fall was located between brk. 32 and 33, the size of the fall was approx. 6 thick, 60' long and 20' wide. the area has been crib and dangered off.

A roof fall was found at 6 right in the Holly Bush Submains at break 17 - 18 in the #5 entry. The size of the fall was approx. 20'W x 50'L x 6'Thick. No ventilation or equipment effected. The area has been dangered off.

Employee was using a belt knife to cut rope when knife slipped cutting right thumb sutures required. Employee has returned to regular duty with no restriction.

A roof fall has occurred in the Old Works located at Double Sub-mains in the 3 entry, 2 left, break 31. No ventilation or equipment effected, the area has been dangered off. The size of the fall is approx. 40'L x 30'W x 5'Thick.

A bolt and plate was laying on the ground. When employee let down head on roof bolter it caught the roof bolt plate causing the roof bolt to fly up striking head just above right eye. Sutures required, employee has returned to regular duty with no restrictions.

A roof fall was found in the Double Submains in the #1 entry at break 11, the size of the fall was approx. 20'x20'x6'. No ventilation or equipment effected. Additional roof support of timbers and cribs have been placed in affected area.

A roof fall was found in the old Double Submains at break 11 in the #2 entry. The size of the fall was approx. 50'L x 20'W x 4'. The area has been dangered off and additional roof support of cribs and timbers will be added.

Employee was walking when he tripped grapping a roof bolt strap to keep from falling. Employee received a cut to his 2nd index finger on right hand, sutures required. Employee has returned to work with no restrictions.

A roof fall has occurred at Old 16 submains, the fall is in the #4 entry on old 16 belt at brk.14-15, the size of the fall is approx. 20'W x 6'T x 50'L. The area has been timbered and dangered off.

A roof fall has occurred in the #4 entry between brk. 10 and 11 in the Hogg Mains primary escapeway. The size of the fall was approx. 20' x 7'x 70'. The area will be cribbed, timber and dangered off and the primary escapeway will be brought around the fall into the #5 main entry.

Employee was breaking loose a rock that was on the continuous miner tail when rock broke loose catching right index finger, sutures. Employee has returned to regular duty.

Employee was using slate bar to dig under headdrive, employee used hammer to hit slate bar when a piece of slate bar broke off striking his lower lip, employee received 2 sutures and returned to regular duty.

On 12/01/10 we were notified by the Department of Labor that EE has pneumoconiosis.

A roof fall was found in 1st left at break 2 and 3 in the #4 entry at Double Submains, the size of the fall was approx. 20'W x 80'L x 6"thick. area of the fall has been cribbed off.

A roof fall was found in the #4 entry of Plant Mains, at Brk.10 from the outside, the fall was approx. 5'thick, 20'wide, 20'long. Ventilation or escapeway was not effected. The area will be timbered and dangered off.

A rockfall occurred in the #3 entry on 16 beltline at break 10, the size of the fall was approx. 35'L x 40'W x 6'Thick. No ventilation or machinery affected. Area is being cleaned and additional roof support is being added.

A rockfall was found during a weekly examination at Holly Bush Submains, the fall was located at brk. 40 in the #2 entry extending thru 2 left entry. No ventilation or machinery has been affected. The area has been crib and dangered off, and additional roof support has been added to #1 entry.

A rockfall was found during a weekly examination at 67 Mains, toward 001 section in the #1 entry, between brk. 11 & 12. The size of the fall was approx. 20'w X 30'L X 3' thick. No ventilation or machinery affected. The area has been dangered off.

Employee was in locker room trying to open pad lock with 2 hammers, hitting hammer against hammer when a piece of hammer broke off lodging into right wrist. Employee has returned back to work with no restrictions after object being removed.

Employee cut left hand while using utility knife cutting rubber packing, employee received 2 sutures and returned to work with no restrictions.

Employee was using lifting jack on headdrive. Lifting jack bar slipped out while jacking headdrive striking employee upper lip, 4 sutures required. Employee has returned to reg. duty with no restrictions.

Employee was in 3 entry belting up, Employee was pulling on a top roller to place on belt rail. Employee says lower middle back was hurting due to pulling on roller. Employee became a lost time on 7/20/2010.

Employee is a roof bolt operator, while setting canopy a piece of drawrock fell cutting left index finger, employee received 2 sutures and has returned to regular duty with no restrictions.

A rock fall has been found during a weekly examination at East-West Mains break #58 in the #1 entry, The fall area is adjacent to #25 seal, the size of the fall was approx. 30' x 25' x 5'. Additional roof support of cribs beams and steel arches will be installed in the fall area.

Employee was working on stopping in the return when a piece of rock fell off of rib hitting employees left foot, employee became a lost time on 6/1/10 - 6/7/10 with bruised left foot. Employee has been released to reg. duty with no restrictions.

Employee was cutting on belt when knife slipped cutting left thumb, 4 sutures required.

A rock fall had occurred on 14 beltline break 39 in Holley Submains, 3 entry, the size of the fall was approx. 6'H x 40'L x 60'W. Additional roof supports of cribs, beams, and 10' rope bolts were added to the effected area, also additional rope bolts were added to surrounding area's inby the fall.

At 13 belt drive at a ( 500 KVA power center) there was an electrical fire in the transformer of the power center located in 67 mains, the power center was de-energized, miner's were evacuated, fire was extinguish, all local agencies were contacted, no injuries, and power center has been removed from service.

Employee twisted lower back while avoiding a piece of drawrock that fell, employee has been placed on restricted duty.

A roof fall was found at Marion Branch in the #2 entry 50ft. behind the fan, the fall was approx. 20' wide, 10' long, 3' thick, this area will be cleaned and supported as required in the roof control plan.

A roof fall was found at Marion Branch at approx. 12:15 pm, in the #1 entry at the drift mouth behind a stopping, the fall was approx. 5' thick, 20' wide and 10' long, this area has been dangered off.

Employee was sawing timbers, when he cut his left index finger with saw, sutures required.

Employee was bolting top in #3 heading when a piece of drawrock fell hitting him in the mouth, 5 sutures required. Employee has returned to regular duty with no restriction.

Employee was bolting top when a piece of draw rock fell hitting right forearm, 5 sutures required, employee was placed on light duty.

Employee was setting a stiff jack at 6 headdrive, when a piece of drawrock fell, hitting left hand ring finger. 5 sutures required, employee has returned to regular duty with no restrictions.

A roof fall was found at Right Fork Submains in the #5 entry between brk. 38 & 39, the fall was approx. 7' high, 20' wide, and 60' long, this area is located in a worked out area and in the process of being sealed off. The area has been cribbed and dangered off.

Employee was placing guard on tailpiece when glove caught between bearing and proxmity switch causing injury to 2nd digit finger on right hand, sutures were required. Talked with employee on safe work habits. Employee was placed on light duty.

A roof fall was found at 1 Seam Mains Brk. 26 between 3&4 entry, the fall was approx.5'x20'x100' the area has been dangered and sealed off. The information was called to the safety director at 2:05 pm.

Employee had used a utility knife to cut a plastic tie, when it slipped cutting his left forearm, employee had received 6 sutures. Employee returned back to reg. duty with no restrictions on 1-16-09.

A roof fall was found in the old works at the Short Block Mains in the #4 entry, the fall was approx. 20'x30'x5' wide, the area has been dangered off.

Employee was breaking up a rib roll with a hammer when something hit the top of his right hand on 12-17-08 employee continued to work until 2-18-09 when he became a lost time due to having surgery to remove object on top of right hand. employee returned to work on 2-23-09 with no restrictions.

Employee was taking up belt slack to change tailroller out and belt clamp broke, hitting top of left hand, fracturing 3 bones on top of hand.

Roof bolt operator drill steel got hung as he reached to get wrench drill, steel fell out hitting top of left hand, employee has fractured his 4th metacarpal shaft. Employee has return to regular duty.

A roof fall was found at 6 left, off right fork submains, #3 entry brk.34, the fall is approx. 20' wide, 30'long, 10' wide, the effected area was dangered off.

At approx. 1:08 a roof fall was discovered at the 2nd panel on the left of Rightfork Submains. The fall was 5' high x 20' wide 50' long. Ventilation was not affected; no equipment involved. Top was laminated. Additional roof support of cribs and beams has been placed in affected area.

Employee was tramming roof bolter to left break in the belt entry, when a rib rolled off hitting him on the left side of his face. Employee received sutures to left side of his forehead, eye, and nose. Employee was placed on restricted duty until 6-24-08, employee has returned to regular duty with no restriction. Safety talk was held with employee on safe work habits.

Employee was bolting top when something got into his right eye, employee was wearing safety glasses at the time, employee has been treated and returned to regular duty with no restrictions.

A roof fall was found in the out by area of South Mains, Brk.93 entry #4, the fall was approx. 7ft high, 30 ft. long, 30ft. wide, the area has been cribbed and dangered off.

A roof fall was found during the weekly examination in the old works of the short balck mains. Located in break 10 in the #3 entry. The fall is approx. 7ft thick by 40ft wide by 60ft long.

Employee was checking a blowed hose on bridge carrier when he told operator to turn pump on to see where leak was in hose, he had finger placed over where leak was causing oil to pump into finger causing right finger to swell. Employee has return to regular duty

A roof fall was found in the South Mains Break # 86 the fall extended from the cross cut in #1 entending into the crosscut in #3 entry. the fall is approx. 6ft. thick by 20ft wide by 140 long.

Employee was helping move 002 section when he strained his back while pulling cable. Employee has been placed on light duty until 4-21-08

A piece of rock fell on the miner cable behind the standoff. It was determined that a fall did not cause the miner to be stuck but the cable was grounded. This is not a reportable roof fall.

A roof fall was found during a weekly examination of the mine. The fall was found at 10:00 am. and was called out when the person making the examination got to a mine phone. The fall was located in the south mains break 81, entry #4. The fall is approx. 7 ft thick by 20 ft. wide by 40 ft. long.

EMPLOYEE WAS HANGING BELT WHEN HE SLIPPED AND HIT RIGHT SIDE AND RIGHT LEG ON SCOOP BUCKET, EMPLOYEE WAS PLACED ON LIGHT DUTY.

A roof fall was found in the outby area of the South Mains, break # 72 entry 5 was approx. 50 ft. long by 20 ft. wide by 7 ft thick and break # 65 #4 was approx. 20 ft. by 20 ft. by 7 ft. thick. Additional support has been added to the area. The information was called out to the Supt. at 11:51 am.

Employee was replacing tail jack on miner when jack fell hitting 4th digit finger on his left hand. 3 sutures were required, employee has returned to regular duty on 12-14-07.

Employee was operating a roof bolter when his glove got caught between drill steel and roof bolt twisting his right thumb and fracturing it, employee has returned to his regular duty.

While making a weekly examination of the 1 seam mains a roof fall was found. The fall was approx. 60ft long by 18ft wide by 7ft thick. The area will be dangered off and additional support placed in the area.

Employee was drilling hole in top when a piece of coal with rock fell hitting employee on nose and right hand, 4 sutures were required on nose, employee has returned to regular duty with no restrictions.

Employee was bolting top when drill steel got stuck in top. Employee turned to get wrench when drill steel fell out and hit top of right hand on the 2nd and 3rd knuckle. He received a fracture and has been placed on light duty.

Employee tripped and fell on right knee. Employee has been placed on restricted duty, on 10-31-07 employee became lost time.

Employee said he was picking up a bucket of bits when he strained his lower back, employee was placed on light duty.

A rock fall in old works area was found, it's location is Hog Mains, #5 entry, break 22. Ventilation was not effected and no persons were involved. This area was cribbed and dangered off.

Employee alleged that when he squatted down his right knee popped, employee was placed on light duty.

A rock fall was found at the Small Block Bleeder, #3 entry, between Brk. 42-43, at 8:25 am. The fall was approx. 5' high x 20' wide x 60' long. Additional roof support has been added to the area.

A rock fall was found at the South Mains, #5 entry, between brk. 83-85; the fall was approx. 100' long x 20' wide x 12' thick. Additional roof support has been added to the affected area.

Employee was shoveling lo lo on 002 section when he strained his lower back. Employee has been placed on light duty.

A roof fall was found at 6:50 pm in #3 belt entry, at break 9-10 on 11 beltline. The fall was approx. 70' to 80' long, 18' to 19' wide, and 6' to 7' thick. Additional support was added.

A roof fall was found at 9:45 am in the small block submains bleeder, at brk 14, in entry 3 & 4. The fall was approx. 7' x 20' x 80'. The opening was 20' w & 8.5' h. Additional roof support was added to the area.

A roof fall was found at 8:00 pm while making the weekly exam in the small block submains bleeder. The fall was approx. 20 by 20 by 5 thick. Additional support was added to the area.

A roof fall was found at break 104 in the south mains bleeder. It was located in the right gob eye off of # 5 entry. The fall is approx. 20 wide by 50 ft. long by 8 ft. thick. The fall was timbered and dangered off to prevent travel in the area.

Employee was moving a welder; when he fell, his hand struck a sharp piece of rock. Sutures were required. He returned to his job on the following day.

While mining in number five entry, the mine roof fell, pulling out one row of bolts. The fall was approximately 25ft long by 20 ft. wide by 5 ft. thick.

Mantrip came to a stop and EE struck his head. Sutures were required. He returned to his regular job on his next scheduled shift.

While plastering a brattice, he cut his hand on an old knife blade that had been thrown away. Sutures were required. He returned to work on his next scheduled workday.

A piece of drawrock fell and struck him on his left little finger. Sutures were required.

The employee was bolting top when a piece of rock fell and struck his right foot. He received fractures to his toes on his right foot.

Coy was making a weekly examination and found a roof fall in the Big Block submains. the fall is in the #2 entry Between break 15 and 16. It is approx. 20ft.X 50ftX 8ft thick.

While hanging a curtain the front boom struck his right leg. His right ankle was sprained. He was on restricted duty from 2/12/2007 until 2/19/2007, at that time he returned to full duty.

A roof roof fall was found in the bleeder while conducting a weekly examination. The fall did not affect ventilation. The fall was approximately 110 ft. by 20 ft. by 7 ft. thick. The area was dangered off and roof support was added to the area to prevent another fall in the area.

An unintentional fall of roof was discovered during a weekly check of the Right Fork Submains. The fall was located in the 2nd left panel, break #32-33. The fall was app. 20' x 90' x 7' in the #3 entry.

Loading belt structure in scoop bucket, caught fingers between structure.

An unintentional fall of roof was discovered during a weekly exam on Big Block Submains break #5 in #2 entry. The fall was app. 7' x 20' x 20'.

An unintentional fall of roof was discovered during a week y exam on South Mains, break 95 #3 entry. The fall was app. 7' x 20' x 20'.

employee was cleaning belt and caught his left index finger on the bottom roller stand which resulted in a cut requiring app. 5 stitches. A metal burr on the stand caused the cut.

An unintentional fall of roof was discovered during a weekly check on the 3rd panel of Right Fork submains, break #35 in the #4 entry. The fall was app. 35' x 7' x 20'.

Employee was struck on the left arm by a piece of draw rock while walking between the bolt machine and the rib. Sutures were required to close the wound.

Employee was roof bolting and was struck on the left leg by a piece of draw rock.

Employee was removing a rock from the belt at a head drive when his right arm struck the metal guarding material which resulted in a cut requiring sutures.

An unintentional roof fall occurred at break #17, #3 entry of east West Mains, 2 breaks outby spad #4052. The fall was approximately 35' long, 20' wide, 7 to 8 feet thick.

Employee was struck by draw rock app. 2' x 2' x 1"-0", on the right arm while installing roof bolts. Sutures were required to close the incision. (6 stitches)

Employee caught his left hand in the drill steel while installing roof bolts using gloves. He injured three fingers on his left hand. The glove was caught on a burr on the roof bolt itself and twisted his fingers.

An unintentional fall of roof occurred on the belt of 002 section break #4.

An unintentional fall of roof occurred while retreat mining was being conducted on 001 section. The fall occurred in the left wall at break #7.

An unintentional fall of roof was discovered on a weekly exam in east-west mains, 1st panel, right break #47. The fall was approx 20 x 20 x 8.

Employee was struck on the left leg by a draw rock while installing roof bolts in #5 entry.

An unintentional roof fall was discovered on east-west mains #1 entry break #2 that was approx 120'x20'x4-7' . The area was cribbed off and will not be moved.

EMPLOYEE WAS USING A PRY BAR TO MOVE THE LOW STRUCTURE WHEN THE BAR SLIPPED AND STRUCK HIS LEFT SHIN CAUSING A PUNCTURE WOUND.
