A roof fall occurred 40 feet inby spad 7129 in #1 entry and it measured 6 feet in height, 16 feet wide and 20 feet long.

While trying to free up the brake on a mine buggy, he had to jerk his hand back suddenly and came in contact with a nail which caused a laceration to his left middle finger.

EE was using a slate bar to free a canopy out of the mud, when he felt a burning in his lower back, at the time it happened EE indicated that he felt fine. He started missing work from the incident on 09/07/2010.

Rock fall in intake airway 4800 feet from the face. Approximate size of fall 9 feet height, 18 feet width, 80 feet length.

Employee was dropping high-line from the top. As he dropped the high-line he got his ring finger on his left hand caught in a hook of high-line hanger causing a laceration in his finger. Received 5 stitches.

While making weekly examination of right return found foof fall in #8 entry. Approximate size of fall 7 feet hight, 18 feet width and 30 feet in lenght.

During examination of underground belts found rock fall on #4 belt. Approximate size of fall 7 feet height 20 feet width and 60 feet in length.

Getting a piece of rock off the continuous miner with a slate bar and felt pain in lower back, worked the rest of the shift.

EE was changing bits in the Joy miner, he was using a hammer to knock out a stuck bit, He missed the bit and struck his left thumb.

Adjusting pressure on relief valve on the pressure water pump underground. He turned the pressure up quarter of a turn, as he turned to leave the cap of the valve blew off striking him just above the right eye cutting and caused a laceration.

Loading roof straps on to the roof bolter, when a piece of draw rock fell striking injury on his right hand.

EE was traveling to section on mantrip when a piece of draw rock fell and hit him over the right eye brow. Accident happened half way up #4 roadway. The piece of rock was 12 X 32 X 1 thick.

Foreman making weekly examination found rock fall at break 130 in #7 entry on intake. Approximate size of fall: 5' height, 19' width, 40' length. Fall was found at 2:00 pm and was called in to MSHA as soon as foreman got outside.

Employee was walking over to machine buggy to get parts, and said he twisted his right knee causing pain and swelling.

He was moving shuttle car for the continuous miner to travel by, when his head struck a low corner. He was taken to the ER and checked out and released that night with strained muscles in the neck and head area.

He was putting a 4 foot bolt up. He put the bolt in the hole and had to bend the bolt back straight, once he did he bumped his rotation and the bolt caught his hand and spun it around the bolt. He thinks he just strained some ligaments to his thumb. He returned to work the next shift, but is doing restricted work.

EE was loading a drive chain for feeder onto buggy and felt pain in lower back.

He was trying to push a planetary in place with a slate bar, the jack slipped causing the slate bar to go in a upward motion and striking him in the right eye brow.

Roof bolter operator was injured while drilling bolt hole, a piece of draw rock fell, hitting him on right hand. Had to have stitches in hand.

Surface employee was loading buckets of mine sealant into the supply car, when he dropped a bucket. The lid came off of the bucket, allowing some of the sealant to splash in his eye. This resulted in a scratch to his cornea.

Employee was operating a continuous miner when he slipped on the wet mine floor twisting his left knee.

Employee had stepped up on to a scoop in the shop to hook the battery charger up when he got his right foot caught between the steering jack and the frame fracturing a bone in his foot. He is working restrcited duty.

Employee was picking up a conveyor belt roller when he pulled a muscle in his back.

Tried to use foot to hold back curtain.

he was working on seals and dropped a concrete block on his finger(left hand small finger) he did not stop working and he went to the doctor sunday. it is not a loss time accident.

Employee was operating a roof bolter when he got his glove on his left hand caught in the drill pot as he activated the rotation. This resulted in a fracture to his small finger on his left hand.

Employee was using a hammer to insert cutter head bit inserts in the continuous miner when he felt a sharp pain in his lower back.

Employee was assisting his fellow roof bolter opertaor in hanging the roof bolter cable from the mine roof using insulated tie wire. When he turned his head to move, the excess end of the wire struck his right ear and actually went down into his ear. This resulted in a possible puncture to his ear drum.

Employee was struck on the top of his head with a piece of draw slate that fell from between the roof bolts. The piece of rock measured 2 feet by 3 feet by 2 inches. This resulted in a strain to his neck.

Employee was operating a dual boom bolter when he got his index finger on his right hand caught between a roof bolt plate and the mine roof as he was installing the bolt. This resulted in an amputation of the tip end of his finger.

Employee was operating a dual-boom roof bolter when he caught his left arm on the corner of a roof bolt plate that was bent down. This resulted in a laceration that required stitches to close.

EE was removing a piece of fallen draw slate from off of the belt conveyor when he got his small finger on his right hand pinched, resulting in a laceration that required stitches to close.

EE INJURED HIS BACK WHILE HE WAS ASSISTING COWORKER IN HANGING THE CONTINUOUS MINER CABLE FROM THE MINE ROOF. EE CONTINUED TO WORK UNTIL 11/8/04 AT WHICH TIME HE WAS TOLD THAT SURGERY WOULD BE REQUIRED.

EE WAS INSTALLING CONVEYOR BELT STRUCTURE WHEN HE PULLED A MUSCLE IN HIS LOWER BACK.

EE WAS UNLOADING PIECES OF BELT STUCTURE WHEN HE SUFFERED A HERNIA IN HIS LOWER STOMACH THAT WILL REQUIRE SURGERY TO REPAIR.

EE WAS PULLING ON A SHUTTLE CAR CABLE WHEN HE INJURED HIS LOWER BACK. EE IS LOSING TIME AWAY FROM WORK.

EE HAD BOARDED THE MANTRIP BUS WHEN SOMEONE ACCIDENTALLY BUMPED HIM CAUSING HIM TO BUMP HIS HEAD ON THE SIDE OF THE MANTRIP. HE RECEIVED A LACERATION TO HIS LEFT EAR THAT REQUIRED 7 STITCHES.

EMPLOYEE WAS UNLOADING A PIECE OF CONVEYER BELT FROM A SUPPLY CAR WHEN HE PULLED A MUSCLE IN HIS LOWER BACK.

A ROOF FALL MEASURING 60' BY 19' BY 4' CAME DOWN IN THE BELT ENTRY AT SPAD #827.

EE WHO IS A ROOF BOLTER OPERATOR, ALONG WITH HIS CO-WORKER HAD MOVED THE BOLTER IN TO THE #5 ENTRY. BEFORE STARTING TO BOLT THE PLACE, A ROCK MEASURING 3'X 6'X 1-3" FELL FROM BETWEEN THE BOLTS STRIKING EE ON HIS SHOULDER WHICH LEFT A CONTUSION ON HIS SHOULDER.

EE WAS PINNING 6X RIGHT. HE WAS ON HIS 6TH ROW, FIST PIN. HE PUT STEEL IN ROOF BUT COULD NOT CLEAR SECOND STEEL. HE MOVED BOOM TO CLEAN BOTTOM UP AND CAUGHT HIS HAND ON BOTTOM OF STEEL HE WAS HOLDING IN ROOF BETWEEN STEEL AND CANOPY.

EE WHO WAS OPERATING A SHUTTLE CAR HAD JUST MOVED THE CABLE ANCHOR. IN THE PROCESS OF MOVING THE CABLE HE CAME IN CONTACT WITH AN EXPOSED POWER CONDUCTOR WHERE THE INSULATION HAD BEEN CUT. THI S RESULTED IN AN ELECTRICAL BURN TO HIS LEFT HAND WHICH REQUIRED MEDICAL ATTENTION.

EE WAS OPERATING THE OFF SIDE OF A DUAL BOOM BOLTER WHEN HE MISTAKENLY LET THE DRILL POT DOWN ON HIS LEFT KNEE. HE CONTINUED TO WORK UNTIL THE 20TH DAY OF MAY 02 BEFORE HE HAD IT CHECKED. HE H AS BEEN OFF WORK SINCE MAY 20TH.
