Employee was cutting Curtain off of the head of the miner with a belt knife when the knife slipped and he cut his leg. He finished the shift then went to the doctor and received 3 stitches.

Employee was squaring belt to make a splice when the belt knife slipped and cut his right leg causing him to receive 3 stitches.

Employee was loading glue on bolter and pulled back.

Employee had filled up the pod duster at the dust drop on bottom and when he walked back to the bulk dust hose a slug of dust broke loose in the pipe causing the hose to whip up and strike him in the arm and face causing a fractured arm and facial bones and cuts.

Employee was walking to the bolter on # 2 unit in # 6 right entry when he fell and hurt his right thigh. He went to the doctor and was given a full release. He continued to work until he went back to the doctor on 11/17/2014 and was put off of work with restrictions.

Employee was walking across the # 5 entry on # 3 unit when he stepped in a hole and hurt his right knee. He continued to work till he saw a doctor on 11/19/14 and was put off work with restrictions.

Employee was attempting to clean out a clog in a concrete mixer when the machine came on. His left hand was caught by the auger causing a deep laceration to his hand.

Employee was striking a fan drive bolt on #1 car when he missed and hit his thumb causing him to have stitches in his thumb.

Employee was putting the bottom section into the top section of his pinner steels when the insert caught the steels popped together hitting him on his hand causing a fracture.

Employee was walking from #6 entry to #7 entry in the LOCC when he hit his head on a pin board that was in a brow causing him to jam his neck. He continued to work till he saw a doctor on 11/4/2014 and could not get a full duty release.

Employee was starting a brattice on #1 unit and when he bumped the rib with a block, the top of the rib sluffed out and hit him on his shoulder.

Employee was roof bolting on #3 unit in 5 right when he was putting his steel up to the roof and a small rock flaked out and hit his little finger on the bolter chuck causing a small fracture of his right little finger. The doctor would not give him a full duty release.

Employee was walking across the unit and jammed his head into a roof bolt.

Employee was pulling supplies down the slope when he got his right little finger in the pivot point of the supply tractor and received a hair line fracture of his finger.

A roof fall occurred on #2 unit between #7 and #8 entries in the # 86+70 line of cross cuts of the 4th Main East, MMU 006-0. The fall measured 55'x15'x9' and was caused by a abnormal geological zone. The area was supported by 8' cable bolts. The area was mined in September of 2014. There were no injuries, no equipment involved and ventilation was not affected.

Employee was lifting a tongue on the #3 unit sub to hook it up and felt pain in his lower back.

A roof fall occurred in the 3rd main east at X-cut 30-31 in the pillar line of the 2nd 54 belt line. The fall measured 25'x19.5'x8' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension rebar roof bolts in the pillar line. The area was mined in March of 2008. There were no injuries, no equipment involved and ventilation was not affected.

Employee was driving a pin in the 1st 48 belt with a hammer when a piece of metal went into his left eye brow causing him to have 3 stitches.

Employee was trying to get the reel of the roof bolter on # 2 unit in # 8 entry unstuck. He had his hand on the reel when two employees pulled the bolter cable and the reel became unstuck which pulled his left hand into the reel causing him to have stitches.

Employee was at back of bolter when bolter backed up and bumper hit his right foot.

A roof fall occurred in the 3rd main east at X-cut # 35 in the 2nd 54 belt line. The fall measured 60'x20'x9' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension rebar bolts and 8' cable bolts in the intersection. The area was mined in March of 2008. There were no injuries, no equipment involved and ventilation was not affected.

Riding in on Mantrip rock fell out hit him in the face

Employee was loading roof bolts in #5 entry on #3 unit when he mashed his left middle finger between the top of the bolter and a hand full of roof bolts, resulting in a small fracture of his finger.

Employee was driving a permissible ride on #3 unit in #9 entry when he ran into a coal rib which caused the steering wheel knob to hit him on his wrist resulting in a fracture of the large bone.

A roof fall occurred in the 1st main west at # 10 neutral entry at spad # 63+22. The fall measured 40'x19'x9' and was caused by a hidden slip in the mine roof. The area was supported by 5' fully grouted bolts and 10' cable bolts in the intersection. The area was mined in April of 2014. There were no injuries, no equipment involved and ventilation was not affected.

Employee was cutting some metal outside at the shop when he tripped and fell on his left elbow. He continued to work until he was off work with surgery on his elbow 08/08/2014.

Employee was loading belt framing into a scoop bucket and framing mashed his left hand.

Employee was changing a tire on a Shuttle car and felt a pain in his lower back.

A roof fall occurred in the 1st main west at # 5 neutral entry at spad # 55+89. The fall measured 55'x19'x10' and was caused by a hidden slip in the mine roof. The area was supported by 5'fully grouted bolts and 10' cable bolts in the intersection. The area was mined in March of 2014. There were no injuries, no equipment involved and ventilation was not affected.

Employee was cutting # 7 left on # 1 unit when a rock fell out grazing down his left shoulder causing him to yank his arm and shoulder away from the rock. he worked until he saw a doctor and had X-rays which showed nothing wrong. The Doctor keep him off work starting on 5/15/14 for therapy.

Employee was throwing the miner cable over on #3 unit in #6 right when he felt a sharp pain between his shoulder blades. He continued to work till he was put off work for therapy on 5-19-14.

Employee was pulling the miner cable on # 2 unit and felt a pain in his lower back.

Employee was throwing over the miner cable slack on #2 unit in #4 entry when his right knee popped out of place. He continued to work till he was kept off work after surgery on 6/11/2014.

Employee was walking in a low area and felt a pull in his lower back.

Employee was putting a set of ram car batteries on charge. When he was raising the battery lids, one lid fell on his right index finger causing him to receive a fracture of his right index finger.

Employee was loading rollers onto the ride and felt something pull in his right elbow.

Employee was throwing a conveyor flight into the tail of the continuous miner when he felt his right shoulder pop, he continued to work until he had surgery on his shoulder on 5/5/14.

Employee was dusting on #1 unit when a rock fell out and hit him on his right hand.

Employee was operating a mini trac on #2 B supply road when he ran over a rock in the road which caused him to jam his head and neck into the roof. He kept working till the doctor put him off work on 3/11/2014.

Employee was standing at the #1 unit feeler in #4 entry with his back to #5 entry. A ram car came to the feeder from #5 entry side to dump. The employee at the feeder did not see the car coming nor did the ram car operator see the employee. The ram car pushed the employee down which resulted in a fracture to both his legs below the knee.

EE was stepping off of the mantrip at #2 unit shack when he tripped over his jacket and hit his head on a roof bolt causing him to have 4 stitches.

Employee was walking across from #7 to #8 entry on #3 unit when he felt his lower back pop. He had been loading pins and boards on the right bolter earlier that morning. This accident happened on 12/3/2013 he continued to work till he had back surgery on 07/15/2014.

Employee was sitting on a pallet of blocks with his back turned to a co-worker who was cutting plastic bands off of the block pallet, he turned his head to see if his co-worker was done cutting the bands and the cut band flew up and hit him in his right eye causing a cut in his cornea.

Employee was walking on the #1 unit and tripped over the miner cable and fell hitting his head.

EE was changing the #1 unit scud pump out and while unthreading the main fitting he caught the middle finger of his left hand between the frame and the fitting causing him to receive 8 stitches in his finger

Employee was taking down the air lock at #2 unit belt tail when the tail moved and a jack pipe fell and hit him on his middle finger of his right hand causing a finger tip fracture.

Employee was bolting on # 2 UNIT WHEN A ROCK HIT HIS DRILL STEEL CAUSING THE STEEL TO HIT HIS RIGHT LEG.

EE was getting out of the road grader and felt a sharp pain in his right knee. He ended up having to have knee surgery.

Employee was cleaning rock off of the unit power box in #4 entry on #1 unit when he felt a sharp pain in his right shoulder. A MRI done at a later date showed a tear in his shoulder.

Employee was loading 54" belt framing into a belt skid and felt a sharp pain in his back, he went to the doctor and had x-rays and a MRI. He is off work with a bulging disc.

Employee was sitting on his golf cart at #3c belt sub when a piece of rock fell out striking him on his right knee. He is off waiting to get results of CT scan.

Employee was turning 8 right X-cut on #1 unit when the miner cable pulled tight pulling him down and before he could get up a rock fell out striking him on his back and leg.

Employee was squaring the 4th 54 belt to make a splice in it when his belt knife slipped and cut his right leg requiring 6 stitches. He returned to work the next day.

A roof fall occurred in the 1st Main South belt line between x-cut 41-42 in the pillar line #5 neutral entry. The fall meas. 35' x 6' x19.25' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension glue bolts and was mined in 8/2006. There were no injuries and no equipment involved and ventilation was not affected.

On July 24, mine experienced an adverse geological event, including fall of roof in alive workings, that impaired ventilation or impeded passage within the 6th Sub Panel East off 1st South Panel. No injuries were involved and MAHA was contacted within 15 minutes.

Employee was in the face of #8 entry on #1 unit running the opposite operator side of the roof bolter. He had stepped back to the middle of the bolter to get a bundle of plates when the operator yelled to him that he was moving up and the bolter tire ran over his right foot causing him to break his toe.

Employee was scaling rock at the 1st 54 tall area when a piece of rock the size of a softball fell out and bounced up and hit him in the lip resulting in three stitches.

Employee was building a brattice on #1 unit when he bent over a trailer to pick up a block and felt something pull in his back.

Employee was at 1c tail kneeling down watching the belt on his left knee and when he went to stand up he pushed up with his right leg and felt his right knee pop. He was taken to the hospital where the X-rays did not show anything but was not released till he could get an MRI done on 6-14-2013.

Employee was dragging a bottom roller on the 1st 54 belt line when he mashed his right ring finger, resulting in him receiving 12 stitches.

Employee was driving a spad in the rib to hang wire for cables on #2 unit in #5 entry when he hit the top causing a piece of top to fall out and hit him in the side of his left foot. This resulted in a hire line fracture of his foot.

Employee was helping pull a header cable for new 3D header when his feet got tangled up in some other cables and he fell and landed on his right wrist causing it to fracture.

Employee was putting the cross over pad on the roof bolter in #4 entry on #1 unit when his right ring finger was between the bolter and the pad causing him to fracture the tip of his right ring finger.

EE was changing batteries on his ram car when he walked around to get the jumper and stepped on a rock twisting his left knee.

Employee was helping put pins and bushings in the M-7 miner on #1 unit in #9 entry when a co-worker was driving the right bushing in a piece of the bushing broke off and struck the employee in the neck. He was taken to the doctor and the metal was taken out, with one stitch put in the incision.

Employee was plastering a brattice between #4-#5 entry across from the feeder. He had his stinger ride parked between him and the feeder with the lights on. A ram car backed into the stinger and pushed it up on top of his legs. Resulting in a fracture to his pelvis and his lower back.

Employee was pulling cable off of the reel of the roof bolter when he twisted and felt a sharp pain in his lower back. He was between #7 and #8 entry in the last open cross cut on #1 unit. He came back to work the next day.

A roof fall occurred in the 3rd Main East intake between x-cut 30-31 in the pillar line #7 intake entry. The fall meas. 25'x6'x19' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension glue bolts and was mined in 3/08. There were no injuries and no EQ. involved and ventilation was not affected.

Employee was lowering the lid on a set of ram car batteries when the lid fell on his left index finger which caused a fracture in the tip of the finger and 6 stitches.

Employee was getting down from a end loader when he stepped on the ground his foot came down on a tire rut, causing him to turn his ankle.

Employee was putting up 8' bolts when the 1' wrench he was using came off and hit him over the right eye giving him 9 stitches.

Employee was sitting on his stinger ride on #1 unit 2 x-cuts out by the face in #5 entry when a sharp piece of draw rock fell out between the pins and struck him on his left knee causing a contusion.

EE was walking down the #8 Entry on #2 unit when he caught his left ear on a nail sticking out of a curtain board causing him to have 7 stitches in his ear.

Employee was walking beside the roof bolter when he slipped and fell into the roof bolter, striking his left side of his face on the roof bolts on the bolter this caused him to fracture his orbital bone.

Employee was on #2 unit in #11 entry repairing the boom of a roof bolter when the boom fell and struck him on the hand causing him to have 12 stitches in his hand and fracture his hand.

EE stated he was grading the hall road and was stopped waiting for the artic. trucks to move. He finished his drink and opened the door and threw the can out, as he was shutting the door he shut it on his left hand. The door striker that latches caught his left middle finger on the inside had a laceration. EE had 15 stitches in E.R. Minor injury.

EE was bolting #3 right on #2 unit when a rock came off of the canopy of the bolter and struck him.

Employee was knocking a hole in a brattice with a sledge hammer when he hit his right index finger with the handle and had to have 7 stitches.

Employee was walking in #1 entry on #3 unit when he twisted his right knee.

Employee had moved the miner into #5 entry on #3 unit and while pulling on the miner cable he strained his back.

Employee was removing a discharge line on a chemical pump when fluid came out and sprayed him in his left eye behind his safety glasses. His eyes were flushed out and he went to the doctor and was told to be off for 2 days.

Employee was cutting a shock off a mantrip in the shop when the shock ruptured and burned him on his right arm and face. He was treated for second degree burns on his arm.

Employee was holding a wing up that had been torn off of a feeder when he lost his grip on the heavy metal wing causing it to fall and smash his tight little finger, he had 8 stitches in his finger.

Employee was putting a 8' roof bolt up when he fell forward and struck his left leg on the roof bolter pot. He had 24 stitches in his leg.

Employee was rock dusting walking backwards with the hose when he tripped and fell on his behind. This cut his behind and he had 3 stitches put in.

Employee hit the back of his head on the corner post of the man trip when a water line he was carrying came loose and struck him on the right side of the head. He received 7 staples in the back of his head.

Employee was bolting in the face of #1 entry on #1 unit when he was putting up his rib bolt and rotated the bolt against the roof and the rotation hose blew and struck him in his left eye.

Employee was putting up his first pin in the face of #2 entry on #1 unit when he dropped his boom causing a pinner steel to flip up and hit him in his head.

A roof fall has occurred in the 2nd subpanel southeast between x-cut 36-37 in the pillar line return neutral. The fall measured 50'x6'x18' and was caused by a kidney bed in the mine roof. The area was supported by 5' fully grouted roof bolts and was mined in March 2012. There were no injuries and ventilation and the passage of men were not affected.

Employee was bending roof bolts when he turned and twisted his right knee.

Employee was lifting a 8' timber into a scoop bucket when he strained his abdomen, this resulted in the employee having to have hernia surgery.

A roof fall has occurred in the 3rd main east x-cut 133(103+20) in the neutral pillar line between entries #4-#5. The fall measured approximately 35'x19.5'x8' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension rebar roof bolts and was mined in February 2009. There were no injuries and ventilation and the passage of men were not affected.

A roof fall has occurred in the 1st sub-panel south east at 26+60 in #7 entry. The fall measured approximately 45'x 20'x 8.5' and was caused by an undetected slip in the mine roof. The area was supported by 5' fully grouted glue bolts and was mined in December 2011. There were no injuries and ventilation and the passage of men were not affected.

A roof fall has occurred in the 1st sub-panel south east at 28+00-28+70 in the #7 entry. The fall measured approximately 65'x 19'x 9' and was caused by an hidden slip in the mine roof. The area was supported by 5' fully grouted glue bolts and was mined in December 2011. There were no injuries and ventilation and the passage of men were not affected.

Employee was swinging boom out with canopy touching roof causing a piece of rock 2'x1'6"x1.5" to fall out striking his left foot.

Employee was in the process of splicing a belt together when a piece of metal broke loose as he hit the splice pin with a hammer lodging in his eye.

Unloading a tire in the battery shack and twisted his knee. This did not become reportable until 7/15/11.

Employee was receiving instruction on maintenance of MX-4 instruments and when asked to sign a form, he lost control of his emotions and struck a metal file cabinet with his fist, breaking his little finger on his right hand.

Employee was in #5 entry helping to set bits on miner when a rock fell from between bolts driving her to the ground causing her to twist her right knee. Employee returned to work 02/09/2011 on restricted duty.

Employee was installing a timber in a arch when it slipped and fell smashing his middle and ring fingers on his right hand.

A roof fall occurred on the 1st. 54 S/R at 24+00. The area had been supported with 6' tension bolts, 8' cable bolts and truss bolts. The apparent cause was an undetected slip in the mine roof. An action plan for rehab. of this area has been submitted and approved by the district office on 1/31/2011.

A roof fall occured on the 1st.54 supply road at 19+20-20+00 #24-#25 x-cut measuring approx. 18'x14.5'x6' caused by an undetected slip in the mine roof. The area had been mined in Aug. 2005 and supported by 6' tension re-bar roof bolts and cable trusses.

Employee was kneeling on the ground to nail up a curtain when a rock fell from between the rib and the first row of bolts striking him on the knee, contusion of the right knee.

Trimming power leads back so a light on M-1 miner could be repaired when his knife slipped causing a cut on his finger requiring sutures.

Worker had been painting for approximately 1 1/2 weeks at this mine. He accidentally came into contact with some of the paint, causing 1st degree burn/conjunctivitis.

Employee was using the feeder to pull a tailpiece back when one of the hooks broke causing the tailpiece to turn sideways and hit the employee knocking him against the coal rib causing a cut on his elbow.

Employee had set his ATRS and started to raise his canopy when a rock fell from the last bolt to the ATRS striking him on the upper left leg.

An outby roof fall was found by the Mine Supt. at approximatley 9:15 am in the East Main return, #2 entry, #30 X-cut, spad 20+80. It measured approximatley 65'x20'x5.5' The fall was caused by an undetected slip in the mine roof. The area was mined in May 2007 and bolted with 6' tension rebar roof bolts.

Employee was cutting a metal band off of a bundle of roof bolts. When the band cut thru it flew backwards and cut his left hand requiring 6 sutures.

Employee alleges that he injured his back at work on 3/28/2010. He did not follow up with an accident report as required by employer. He is now off work with alleged back injury. No detailed information is available. This is being investigated to verify if this is a work related injury.

While starting a drill hole for roof bolting purposes a small rock fell striking him on the little finger of the right hand resulting in a cut that required 4 sutures.

Employee was cutting right side of #6 entry on 002-0 section when a piece fell between the rib and a roof bolt striking him on the head and face resulting in 5 sutures.

Employee was installing a roof bolt when a rock fell from between roof bolts striking him on the foot.

While attaching a jackpipe to a belt tail the pipe slipped off of the anchor and fell across his lower back.

Employee was cutting belt with a knife, when the knife slipped, cutting his hand.

Employee was riding in a diesel mantrip and as it made a turn his foot was caught between the mantrip and the coal rib, fracturing a bone on top of his right foot.

EE alleges on 01/06/2010 he was walking thru a curtain and hit his head. He did not report this to anyone. On 02/01/2010 he was sent for x-rays, still not telling anyone it was work related. On 02/02/2010 he requested an accident report be filled out. He did not miss work until 06/07/2010. His claim is being investigated to verify if this is a work related injury.

A roof fall was found on the 1st 54 travelway at appox. 10:50 pm. The fall was 60'x18'x8'. Vent. and passage of men were not affected and no equip. was involved. The area had been mined in Aug. 2005 and supported with 9' cable bolts and 6' tension rebar roof bolts. 8' double-lock bolts were also installed in this area.The fall was caused by an undetected slip in the mine roof.

Employee was cutting a plastic curtain with a box knife. The knife slipped causing him to cut his arm. (3 sutures)

Employee was hanging a piece of wing curtain that had fallen down and the scoop operator was cleaning the entry. He was traveling batteries first and could not see the employee that was hanging the curtain. The rear tire of the scoop ran over the right foot of the curtain hanger causing a fracture of the small bone in the ankle.
