A contract employee was positioned near the rib, when the corner broke off and fell and struck a timber. The timber then fell and hit the employee in the left shoulder, nose, and mouth. This resulted in a bruised left shoulder and a cut to the lip requiring 4 sutures.

Employee tripped hitting EE's knee on mine floor. Employee was transported to get x-rays and they was negative. EE received a full release. Two shifts later EE went back and was referred to a specialist, on March 23rd EE was diagnosed with a torn meniscus. Due to covid-19 and furloughed we are just returning since March 20th.

condition was damp. area was 19 feet wide and 6 feet 10 inches high. EE, and 2 others, was stacking a brattice when another man threw a block into work area while EE was picking another block striking EE in the right index finger resulting in an impact laceration. EE finished the shift. Went later to the Emergency Department and received 5 sutures.

Employee was installing a conveyor motor on a shuttle car. While aligning the motor, the chain ratchet slipped causing motor to hit frame of shuttle car and roll out on employees right foot. Employee was diagnosed with a fracture to the right third toe.

Roof Bolter was getting ready to bolt #4 left this was the 1st cut out of the break. Co-worker was using the inch tram to set up for a corner bolt when he activated the tram he ran over employee's left foot. He was transported to Mcdowell ARH hospital where he received x-rays which were negative for broken bones, but the toe nail of the 2nd toe was torn off.

Joseph Martin Hospital he was given a cat scan which was negative. He was transferred to Pikeville medical Center and had an MRI and diagnosed with a hernia to the left testicle.

Employee was moving the continuous miner cable out of the haul road with the help of shuttle car operator when he said he felt pain in the back of his left leg. Employee continued to work until 9-10-2014 when he said his leg was still bothering him and he sought medical attention at Williamson ARH Hospital where he was diagnosed with a pinched nerve.

Employee stated that as he got on the A.L. Lee mini trac he felt a twinge in his back but proceeded to load water line onto a flat car with the mini trac and as he was exiting the operators compartment he felt more pain in his lower back that was really bad. He received a CT scan at ST. Joseph Martin Hospital and was diagnosed with bulging disc's in his lower back.

Employee stated that he was backing the continuous miner out of the # 9 heading with his back to the miner when the cats either slipped or he split the cats, it was at this time the tail boom of the miner struck him in the middle of the back knocking the wind out of him. He was complaining of soreness in his back and a tingling sensation in his right leg and foot.

Employee was helping set the continuous miner up for the next cut, he bent down to pull the miner cable out of the roadway he then felt a sharp pain in his genital area. He finished his shift and sought medical treatment the next day and was treated for a strain.

Employee stated that he turned the #4 belt conveyor off by the remote switch and climbed onto the #4 belt to remove a rock from the #5 head drive and the # 4 belt started up and he was propelled into the belt crossover which is located outby the rock box of #5 head drive, which resulted in bruising to the left shoulder and abrasions to the back.

Employee claims to have hurt his back moving a miner cable from one rib to another. He finished that shift without reporting (the 8th). He worked his next shift without incident (9th). He missed work on the 10th. He returned on the 11th to report his injury to the Safety Department. He has not returned to work since.

An unplanned roof fall occurred 40ft. inby spad 1393 in the #3 entry, along the 001 Section North East Mains. Measured approximately 20ft. wide by 16ft. long by 6ft. high in an area that had already been "dangered off", pulling 2 rows of fully grouted roof support. Found by examiner around 10:10pm and reported to MSHA's hotline immediately thereafter.

Employee attempted to lift a piece of metal that was obstructing his path to his tool box. In doing so, he strained his back and finished his shift. He continued to work for the next couple of days before deciding to seek medical treatment and is under care of his family physician and anticipates returning to work within the next few days.

Employee was reaching for a roof bolt plate when a piece of draw rock fell, striking his left fore finger. He was taken to ST. Joseph Martin Hospital and received X-rays. It was determined that a small piece of metal from the plate had penetrated his finger and that he would receive surgery later to have it removed.

A piece of metal slid against victims right ankle, fracturing his lower leg, just above his ankle. He was treated and released at a local hospital and is scheduled for surgery in the near future and is expected to make a full recovery.

While continuous miner operator was loading S/C, a loose portion of mine rib fell, striking him in the back, knocking him into the S/C. He was transported to a local helipad via ambulance and flown to St. Marys hospital in Huntington WV where he was treated for various injuries. Victim is in stable condition and doctors expect him to make a full recovery.

Victim attempted to remove a nylon string from a turning stuck roller while belt was in operation. Roller began to turn, string caught victims right index finger resulting in a cut which required stitches. Victim finished shift and missed no work.

Victim attempted to hold on to bolter wrench while reversing rotation in order to remove a bolt he bent during its installation. The wrench/bolt snagged his glove and pulled him into the steel fracturing his left arm. He was released from hospital same day and is scheduled for surgery later.

Victim (Examiner) claims to have been struck between the shoulders with a piece of rock. He was taken to a local hospital where he received x-rays and was released to return to work the next d ay. He hasn't returned to work as of 6-29-12 and cannot be reached for an explanation.
