While pulling a 20' long 2"x2"x1/4" stick of angle iron off a rack, the employee felt pain in their left elbow. The employee worked the rest of their shift, but then went to a local clinic on their own accord the following day for an evaluation without informing the company. This evaluation resulted in lost workdays.

Hernia possibly a result of previous fall at work.

While an employee was working to unload a rod car, the employee had a mis-step while transferring from one side of the car to the other and fell against the rods. The employee felt discomfort in their left side (rib area) and in their left shoulder. The employee later sought medical attention and was put on light restrictions.

Worker was assisting plant with pallet change out. Their hands were on the operating handle of the lifting device and the hoist was not centered over the load resulting in a swing and the lifting device came off the pallet. Adjacent pallets were in the work area and with the swing the worker's hand was pinched between one and the lifting device. One finger needed stitches

While an employee was working on a haul truck, dirt and debris fell from an above platform on the truck, some of which, landed in the employee's left eye. The employee sought medical attention. The eye was numbed, dirt or debris was extracted from the eye, and prescription eye drops were administered.

While lowering a removable mantle hand railing down on a mantle cart, the employee's left hand was in the line of fire, injuring two fingers. One finger suffered a laceration requiring sutures and another finger suffered a hairline fracture requiring a brace.

While completing several tasks throughout the morning, the employee began to feel pain in their right shoulder and neck.

Employee states that while taking a slurry sample out of the thickener, they stepped back and their left leg went through some deteriorated grating on the walkway above the thickener. The employee scraped their left shin and twisted their right ankle. The employee was taken to a medical facility for evaluation and was diagnosed with mild sprain of ankle.

Employee states that while changing idlers on the 5/10 shuttle in the filter building, EE pinched their left pinky finger between the idler rack and the support beam.

Employee states was trying to line up a bar for the rail change. A co-worker was swinging a sledgehammer and missed the bar, hitting this employee in the nose. This was a tight area working next to grapple head, which was providing down pressure on rail.

Contractors were cutting holes in a section of chute in order to rig and remove it from the Pellet Plant Screen House. While cutting workers discovered a fire had started in the chute. Contractors notified their supervisor who then called 911. Fire department responded and extinguished the fire.

Employee slipped on a patch of ice while approaching a piece of equipment to talk with the operator. Employee landed on elbow which jarred the shoulder. Employee felt soreness in shoulder. Through an MRI a torn rotator cuff was diagnosed. Surgery to repair the shoulder took place on January 17th.

Employee states that they were attempting to travel down the ramp between 61/161 conveyor shuttles when employee slipped on pellets and fell backwards. As they tried to catch themselves with left hand, they fractured a wrist.

EE was helping install a Steel I-Beam, body was in an awkward position resulting in strain to EE's lower back. EE was placed on light duty

Uneven ground caused by ice, snow and frozen dirt. EE was walking towards EE's service truck when EE twisted ankle. It was verified that EE did not break or fracture the ankle.

Employee was assisting loading bundle rods onto a flat bed when EE pinched fingers on EE's left hand between loose rods.

Employee was reaching under a newly installed pinch valve to align bolt holes. When employee pulled on the plate employee heard a pop and felt pain in left elbow.

Employee was banding a rod bundle when the wire band slipped out of the banding clip and hit EE on the wrist causing a laceration. The laceration required 7 sutures to close.

The employee was working with a hand tool when it slipped

Standing on a 2' pedestal was going to grease a fitting on the motor, went to turn body and foot got stuck on grating and went over backwards landing on cement. Broke left hand.

Employee noticed a gradual onset of shoulder pain that they relates to demolition of overhead electrical conduit using a Sawzall. Ladders were utilized instead of aerial lifts due to tight spaces. First medical evaluation 5/9/19. Surgical repair was done 8/1/19. First time loss was 8/1/19.

Employee was setting a steel column and pulling up on a sleever bar when suddenly EE had pain in lower back. The employee did not report the incident till 1/14/19.

EMPLOYEE WAS STEPPING BACK AND ALL EMPLOYEE'S WEIGHT WAS ON LEFT LEG AND EMPLOYEE'S ANKLE GAVE WAY.

Employee states that while using a pry bar to lift idlers from the conveyor belt brackets, EE's finger was pinched between the bar and the conveyor frame. The initial force caused a fracture to the tip of EE's finger.

Employee was cutting steel bands from rod bundles when one of the bands sprung loose and contacted EE's lower leg. The result was a puncture wound that required sutures.

Employee entered elevator and pressed the 1st floor button. Elevator traveled downward and stopped. Control panel in elevator lost power and could not be moved up or down and the doors would not open. Employee had a power plant radio and called for help. Elevator repair company was called and replaced a failed mechanical relay. Once repaired, employee was able to exit elevator

Employee was using a impact tool to remove nuts and bolts from a loader bucket cutting edge when one of the bolt heads spun in their hand causing a laceration.

Employee states was pushing rags into a small opening to stop material from spilling from a rock box when felt a "pop" in finger. When employee removed glove saw that the tip of finger was bent funny and would straighten out.

Employee was pulling pallets with an air winch when EE's finger got pinched between the wire rope and the winch spool.

Employee was using a crescent wrench to loosen a nut when the wrench slipped off the nut causing ee to strike finger against a hand rail support.

Employee was demoing existing duct work. EE torch cut a piece of 3/8" steel plate that weighed approximately 30 pounds. The piece slipped out of left hand and struck steel toe on right foot. Right pinky toe was the only part of the foot not protected by steel toe where the piece fell. The piece of plate fractured the right pinky toe.

Employee was stepping down from a ladder to the main floor when employee twisted ankle.

Contractor employees performing repairs at yard & docks were riding in the yard & dock elevator when it stopped. Elevator repair personnel from Thyssen & Krupp were called. The contractors were removed from the elevator, no one was injured.

Two contractor's working on our furnace down were riding in the pellet plant east elevator when it stopped. Elevator repair personnel from Thyssenn & Krupp were called. The contractors were removed from the elevator, no one was injured.

Employee was injured when a sump that was being lowered into the basement with an overhead crane came into contact with a severely corroded pipe and broke the hanger causing the vertical pipe to fall and come into contact with the employee's ankle, fracturing it. The bolts on the hanger were completely rotten.

Employee was working to unplug a tails pump when the plugged material let loose and hot material sprayed EE's arm resulting in a 2nd degree burn.

Hosing pellets from floor in pelletizer building areas when dirt fell on employee straining back and left knee.

Employee pinched finger while loading a barrel onto a cart resulting in a laceration to the little finger on the employee's left hand. The laceration required 7 stitches.

Employee was using a 50 ton hydraulic ram to unplug a stuck crusher when the piston of the ram slipped off the contact point causing the ram to pivot and strike lower leg.

Employee was climbing from the main deck of our scaffold to an elevated cantilever platform and felt a pain in right knee. Initial medical evaluation suspects a torn meniscus in knee. EE is currently off from work.

Employee was aligning a 24" carbon steel flange on a pipe. Flange slipped off the pipe and struck right heel of employee resulting in a fracture to his right heel.

Employee was riding in the Fine Crusher elevator when it stopped. Elevator repair personnel were on site and were unable to get the elevator to move. The employee was removed from the elevator car after a piece of tin was removed from the elevator shaft. The employee was uninjured.

Inattentive Driving: EE was a passenger in company vehicle driving on plant roadway. Driver reported sun glare caused him to veer and miss a turn in roadway. Continued down 5' steep grade into drainage area. The abrupt stop caused a fractured fibula bone to driver and passenger re-injured a previous non-work related injury to his wrist. Very little damage to company vehicle.

EE closing bin supply electric door. Wrapped the strap around hand and put weight into closing it. Was expecting door to close harder than it did since, door closed rapidly momentum pulled hand between closing doors causing hand to be lacerated by door latch upon doors slamming on hand. Laceration required 18 stitches, and there was no other damage to his hand still off work.

Employee was climbing out of a rod mill when his arm came in contact with a sharp edge of a worn out line. Employee was cut on the right forearm.

Employee was walking and slipped on loose material on the floor.

Employee states that while building pallets he turned around to grab a tool and felt pain in his knee.

Employee was taking the slack out of a drive chain when the chain slipped out of his hand and pinched the tip of his small finger between the chain and sprocket.

Employee alleges that he was scraping flakes off a weld and something got behind his safety glasses and irritated his right eye.

Employee states that he was descending a staircase and when he stepped off the last step he felt pain in the backside of his left knee.

Employee was jumping on a pry bar to break loose a coupling controlling a unloading gate on a rail car. When the coupling came loose, the bar fell and he landed on his left leg jarring his knee.

Employee states that while shoveling material from under a belt, she felt pain in her back.

Employee states that they were on the bull gear reducer platform looking at the bull gear and pallets. As they turned on the platform, their foot slipped off causing them to fall to the floor.

Employee was walking from job trailer to company truck without safety glasses when dust/dirt blew into his right eye. Employee did not report eye irritation until the start of his shift the next morning 09/06/13. Employee was taken to Eye Dr. where foreign body (dirt) was flushed out and given an antibiotic prescription as a precaution for his eye.

Employee was grinding and his glove got caught in the grinder which resulted in a laceration to the employee's left middle finger.

Employee states that when he bent over to pick up his gloves his knee twisted and locked up. He was unable to straighten it out.

Employee's left thumb was caught in a 1 ton air chain hoist - lifting side. Hoist was not de-energized when the employee attempted to move the hoist. Hoist was accidentally activated, which caused employee's thumb to get caught in the hoist mechanism.

No Conditions identified that contributed to the incident.

While replacing a compressor on a chiller, a beam being used for hoisting slid off of its crossmembers. The beam slide into the leg of one of the workers causing a laceration to the lower leg.

The citation is being contested and this report is submitted under protest.**Employee felt a twinge in her back when she got out of her desk chair. Later felt pain and sought medical attention. No conditions identified that contributed to the event.

EE was walking out the Fab Shop door on the west side of the building. There is a 2 x 4 threshold that he tripped on with his right foot. He fell to the outside of the building and broke his fall with his left hand. He was taken to the Clinic and diagnosed with a broken arm.

Little finger got into moving parts of a saw-zall and pinched it.

Employee was carrying a 6' I-beam with a co-worker and felt a "pop" in his right wrist.

A electrician was troubleshooting a faulty air dryer and inadvertently touched a live electrical connection and received a shock/burn.

Individual was using a 4.5 inch grinder when he sustained a minor laceration to #2 digit on Left hand.

On Thursday March 15, 2012 at 2:45 pm, the employee was prying a head pulley over with a pipe. The work was being done on 163 Head Pulley. The pipe slipped and the employee slipped backwards. He fell on his back and his head.

Employee was removing a guard on some equipment with a 15 inch crescent wrench. The wrench slipped off of the nut and his right middle and ring fingers were pinched.

Employee was loosening nuts on the trunion of a ball mill. He was using a 16 lb. sledgehammer and 2.5 in. slug wrench. The hardware was very tight and he had hit one of the nuts several times with the sledgehammer. The last time he hit that nut, it loosened and he lost his footing. His body flew into the mill and he felt shoulder pain. That evening the pain worsened.

Employee pinched their finger while trying to catch themselves after tripping on an obstruction in the floor.

The electrical power source for the Yards & Docks elevator failed while an employee was riding in the elevator car. He was stuck in the elevator while the electricians worked to restore power. There were no injuries involved.

Emp sustained a laceration to the upper right forearm while performing overhead grinding with a 9" angle grinder. Emp's arms were tired so he changed positions (had left hand on switch and right hand supporting head of grinder). The grinder kicked causing employee to lose control of grinder. Grinder fell and wheel contacted emp's right upper forearm causing laceration.

Employee states that he was lifting a lube pump out of a bulk lube tank when he felt pain in his neck and shoulder.

Employee was welding and grinding in fan. Debris was flying around. When employee was done grinding, he removed his eye protection and a piece of debris entered his eye. He visited the clinic on 9/12/11 to have the debris removed.

Employee was using a portable grinder to clean rust off a steel chute prior to welding when the grinder kicked back and hit him under the chin.

Employee was loosening bolts on a reducer with a slug wrench and 16 pound hammer. Employee was holding slug wrench when coworker hammered with hammer and hit the employee's right pinky finger. Finger was lacerated and bruised. This information was originally entered on 9/6/11 - forgot to hit submit.

Employee was attempting to catch a glass juice bottle that he had dropped when it broke in his hands. The broken glass cut 2 of his fingers.

Employee was changing tie plates when his finger got pinched between the railroad tie and rail.

Employee states that after he was done grinding on a piece of rusty metal, he felt irritation in his eye. He eventually had to have a small piece of rust removed from his eye.

Employee was using a power hacksaw to cut a piece of steel pipe. As he reached to remove a hand broom from the cutting table, the saw blade contacted the broom handle and pulled his hand into the pipe. The result was a laceration to his finger that required sutures.

Employee was climbing up stairs when his boot caught on some steel treading causing him to twist his knee.

Employee slipped on ice as he was getting out of vehicle. Left leg twisted and employee fell onto left knee.

Employee was walking down a conveyor ramp and turned his ankle.

Employees was lifting/changing idler racks when he felt a pain in his groin. He sought medical attention and was diagnosed with a hernia.

Employee was changing grizzlies when one slipped from its location and contacted his forehead.

Employee was using a cutting torch to remove old bolts when he passed the torch over his off hand. The heat from the torch melted a nitrile glove he was wearing as a liner under his leather gloves. This resulted in 2nd degree burns on two of his fingers.

Employee was changing roll feeder bearings.

Employee was exiting his vehicle. The employee stepped out of his vehicle onto an icy patch that was snow covered. He subsequently slipped and fell and injured his left side ribs.

Employee states that when he stepped out of the truck he was driving he slipped on ice and landed on his left side.

Employee states that while dismounting the 580 backhoe, his foot missed a step and he slipped. As he was falling, he hit his elbow causing a tear to his tricep muscle.

Employee was dropping conveyor idlers under a plugged chute when one of the idlers slipped and pinched his finger between the idler and conveyor frame.

Employee was moving a steel plate when he pinched the end of his finger.

Employee was changing drive belts on a crusher screen, when the belt drive pulley moved and his finger was pinched between the sheave and the belts.

Employee was removing wheel on Gantry Crane when he tripped and bumped his forehead causing a laceration.

Employee was assisting in the removal of an ice chunk from a belt, when the chunk fell and hit him on the finger.

Employee states that when he stepped off a 1' access platform, he felt pain in his knee.

Employee was moving conveyor backstop parts by hand, because overhead hoist was inoperable.

Employee states that he was cleaning up a spill of coarse rock when he slipped and fell backwards against a concrete pedestal. He initially felt no pain but later experienced tingling in his foot.

At approximately 2:00 PM on February 27th, a fellow employee was walking on the north side of 63/163 conveyor, when he slipped on the ice and hurt his back.

Employee was closing drawers on the Lab drying oven. While pushing the drawer closed, the end of her ring finger of her right hand was pinched in the lifting mechanism of the next higher drawer.

Employee was working with a hydraulic punch in the mining shop. The punch broke and a chip flew and cut the employees nose.

At approximately 11:30 AM on January 29th (Injured Employee) along with another employee were changing out the liner on a re-grind mill in the concentrator. Employee was positioning the liner into place, and went to remove the liner clip, when a bolt that was holding the liner in place suddenly adjusted allowing the liner to fall slightly pinching finger.

Unloading structural steel with forklift assinstant. Adjusted load and pinched finger between steel frame and lug nut.

While installing liners in mill cobb, brake liner struck EE on right arm. EE was taken to clinic with bruised arm. Dr. took EE off work.

Employee was removing bolts from mill with air hammer when he pulled a muscle in his right thigh.

Moving a mobile monorail. Employees were pushing the monorail when the pin connecting the beam to the frame broke. The beam fell, struck employee on shoulder, hip and landed on left leg.

Injured employee was struck on left arm by falling tool. Miner working above was using 2" open end wrench, 24" long, to tighten flange nuts on steam line. Lost grip on tool and it dropped 50' to floor below striking contractor employee sweeping floor. Resulted in 2 stitch cut to left upper arm.

Employee was inside rod mill when a liner fell stirking another liner. A small chip came off liner and struck him in the left calf. The metal chip went through the calf. He was treated at the scene and then taken to the clinic. He was checked out by the dr and returned to work.

Employee was operating forklift bring parts into building. A rubber hose was in the way. He lifted the rubber hose by hand and felt a pop in his right shoulder. He reported the injury and went to the Dr. He was checked out and returned to work.

Two employee were visually inspecting the fly ash system for possible material blockage. One of the ee lost his focus of the hazard that exited and reached thru an open valve to clean out 60 me accumulated flynsh. He received a fracture to the right thumb when the air operated value closed unexpected.

EE WAS REMOVING IDLER WHICH WAS STUCK. HE WAS HOLDING HIS HAND UNDER THE RETURN IDLER TO BALANCE IT WHEN IT FELL OFF FROM THE BRACKET BECAUSE OF THE MOTION FROM THE OTHER END. HIS LEFT MIDDLE FINGER WAS MASHED BETWEEN THE IDLER AND FRAME. LACERATION REQUIRED 3 STITCHES.

WHILE INSTALLING A NEW BEARING IN THE SCREEN HOUSE, EE BENT OVER TO PULL A SOCKET FREE FROM A BOLT. SOMETHING HIT A CAN OF ZEP SPRAY AND PUNCTURED THE CAN. SOME OF THE SPRAY HIT EE IN THE FACE AND EYES. HE WAS WEARING SAFETY GLASSES AT THE TIME.

While doing a liner job the person stepped on an air mover and fell backwards. In the process of falling the employee struck his hand on a sharp edge of the air mover.

GETTING INTO CHUTE FOR NORTH SCREEN IN SCREEN HOUSE, EMPLOYEE SLIPPED ON PELLETS AND FINES IN CHUTE, CAUSING LEFT KNEE TO BUCKLE AND TWIST. (EMPLOYEE DID NOT RECEIVE TREATMENT PRIOR TO SURGERY ON 5/14/05. HE WAITED FOR DR OPINION.)

TENSIONING 20 CONVEYOR WIPER BY PULLING RUBBER STRAP. STRAP SLIPPED AND JAMMED WRIST BACK

SMASHED FINGER BETWEEN FULL BARREL OF BANTONITE AND BEAM AS EE TRIED TO MOVE BARREL. EE WAS WEARING GLOVES AT THE TIME

Installing 36 ft. track panels on railroad. A panel had been lowered to the ground to be connected. The employee used a lining pry bar to make the connection. The bar was left for a moment and the panel moved causing the bar to strike the employee's hard hat and the hard hat cut the employee's head.

EE WAS CROSSING A ROAD WHICH WAS COVERED WITH ICE FROM WEATHER CONDITIONS. EE HAD CROSSED ROAD SEVERAL TIMES THIS SHIFT AND WAS AWARE OF CONDITIONS. EE SLIPPED AND FELL, STRIKING HIS HEAD - RECEIVED STITCHES.

WHILE CHANGING BETWEEN IDLES ON THE 60 A CONVEYOR UNDER FURNANCE 12, A HOT CHIP FELL FROM ABOVE AND LANDED IN THE EE S LEFT EYE. THE EE WAS WEARING SAFETY GLASSES AT THE TIME. THE CHIP WENT BE TWEENT HE TOP EDGE OF THE ENTER GLASSES ONT HE EEYE THE DR GAVE THE EE DROPS FOR THE EYE AND OFF WORK FOR 1 DAY.

WHILE WALKING UNDER #1 BELT ON FURNACE 6, SLIPPED ON PALLETS AND FELL AGAINST SECOND STAGE COOLING DUCT. A NAIL WHICH HOLDS INSULATION ON THE COOLING DUCT STUCK IN THE LEFT ARM.

THE EMPLOYEE WAS ATTEMPTING TO SPREAD THE FORKS ON THE EXTEND-A-FORK. IN DOING SO HE MASHED HIS FINGERS BETWEEN THE FORKS AND FRAME.

THE EMPLOYEE WAS CLIMBING DOWN OFF OF SOME SCAFFOLDING USING PROPER THREE POINT CONTACT AND WHEN HE PUT HIS FOOT ON THE GROUND HE FELT A SLIGHT TWINGE IN THE RIGHT KNEE. HE WENT TO HIS FAMILY DOCTOR AND X-RAYS DID NOT SHOW ANY INJURY. A LATER MRI REVEALED THE INJURY.

AFTER CLEANING OUT RECLAIM HOPPER EE WAS GETTING ON LADDER AND HAD HAND ON RAIL USED FOR GRIZZLIES AND CAUGHT A BURR WHICH LODGED INTO LEFT INDEX FINGER. EE WAS WEARING WORK GLOVES AT THE TIME .

EMPLOYEE WAS COMING DOWN FROM PLATFORM, TURNED TO GRAB HANDRAIL (PLATFORM 50" TALL) GRABBED HANDRAIL AND PAIN IN SHOULDER RESULTED IN HIS HAND RELEASING AND EMPLOYEE FELL TO GROUND, CONDITIONS GOOD, DRY, WELL LIT. LOCATION #2 UPPER CRUSHER.

A LARGE REEL OF ELECRICAL CABLE WAS BEING MOVED MANUALLY & NEEDED TO BE TURNED 90DEG. 2 WORKERS PUSHED IN ONE DIRECTION WHILE THE 3RD WORKER PUSHED THE OPPOSITE DIRECTION THE REEL SPUN MUCH EASIER THAN EXPECTED. ONE WORKERS HAND WAS CAUGHT BETWEEN REEL & STRUCTURAL STEEL R HAND 4 DIGITLACERATION & TUFT FRACTURE

PICKING UP SCRAP STEEL FROM 108 ROD MILL & DROPPED IN SCRAP TUB. A SMALL PIECE OF STEEL CAME BACK & STRUCK EE OVER RIGHT EYE CREATING A LACERATION REQUIRING 6 STITCHES TO CLOSE. THE EE WAS WEA RING SAFETY GLASSES AT THE TIME OF INCIDENT.

USING A GENIE (EXTENSIBLE AERIAL MANLIFT) TWO MEN WERE ATTEMPTING TO CLEAN OUT A PIPE. THEY HAD CLEANED ABOUT 1 1/2' TO 2' WHEN ABOUT 13" BY 20'OF FROZEN FINES AND ICE CAME OUT, STRIKING INJUR ED IN THE RIGHT ANKLE AND KNOCKING OTHER EE OUT OF THE BASKET, WHERE HE HUNG FROM HIS LANYARD.

USING A GENIE (EXTENSIBLE AERIAL MANLIFT) TWO MEN WERE ATTEMPTING TO CLEAN OUT PIPE. THEY HAD CLEANED ABOUT 2' WHEN 13" BY 20' OF FROZEN FINES AND ICE CAME OUT, STRIKING INJURED ON HIS RIGHT A NKLE AND KNOCKING OTHER EE OUT OF THE BASKET, WHERE HE HUNG FROM HIS LANYARD.

THE EE WAS PULLING AN EDUCTER PUMP FROM THE #12 CLASSIFIED PIT. PUMP WEIGHT APPROX. 40 POUNDS. THE EE FELT A STRAIN IN THE RIGHT SHOULDER.

WHILE UNLOADING TRUCK THE EE SLID A BEARING OFF THE TRUCK AND FELT PAIN IN LOWER BACK WHILE TRYING TO PUT BEARING ON CART

WHILE USING A SLEDGEHAMMER THE EE PULLED A MUSCLE IN HIS BACK. EE CONTINUED TO WORK HIS SCHEDULED SHIFTS UNTIL 8/14/02. EE DID NOT REPORT ACCIDENT UNTIL 8/14/02, WHEN HE WAS SCHEDULED BACK.

UPON FURTHER EV ALUATION FROM A LARGER MEDICAL FACILITY AND TWO MORE X-RAYS, THRE ARE NO FRACTURES, DISLOCATIONS OR RADIOPAQUE FOREIGN BODIES.
