While driving a bushing onto the boom cylinder of a 988B loader, a metal chip came off and lodged into the right arm of the employee.
Employee was holding an open end wrench on the head of a bolt holding the motor on the roll mill while another employee was tightening the nut with socket. The wrench slipped off the bolt and mashed employee's middle finger on his right hand between the wrench and frame of motor. Required 6 stitches.
Loader operator was working a stockpile when a customer drove back into the quarry in his pickup. Customer pulled in behind the loader and the operator did not see him. The loader operator backed away from the pile and into the pickup pushing it 1 foot. Customer was geting out of pickup and fell to the ground on his elbow.

EMPLOYEE WAS DRILLING HOLES IN ROCK WALL. AS HE WAS SHAKING THE STEEL TO LOOSEN THE LOWER COUPLING THE UPPER COUPLING SHOOK LOOSE AND FELL ABOUT 8 FT ONTO HIS HAR HAT. WE HAD HIM TAKEN TO THE HOSPITAL WHERE HE WAS EXAMINED AND RELEASED SEVERAL HOURS LATER. HE DOES STILL HAVE A HEADACHE SO TOTAL INJURY IS NOT YET KNOWN.
Loader man arrived early and started his loader so it would be warm by the time his shift started. As he was backing out of the loader cab to dismount he missed the ladder and fell off the loader. He fell about 10 feet to the ground and landed on his back. He sprained his upper back and bumped his head on impact. Doctor has advised him to stay home for a few days.
