Emp was stuck by a angle iron brace that was being used to hold up a roll of belting. The angle moved and the piece struck them in the left shoulder causing them to fall to the ground. They worked the rest of the shift and went to the ER after the shift. They treated them for left shoulder and elbow pain. Employee was placed off work until next Tuesday, 11/18/25.

Miner picked up can of break cleaner out of service truck walking to back of truck and slipped on some rock causing miner to start falling. Miner reached out with right hand to stop fall causing break cleaner can to hit rear bumper of truck. The can exploded in miners hand causing lacerations to EE's index and middle fingers.

Employee was sticking barge for draft and stern wire broke hitting employee in the lower right leg. Employee's lower right leg and ankle were broken as a result. Employee was given first aid and transported by ambulance to Hospital.

Employee was ascending the ladder of their haul truck when a pain was felt in the left leg.

Employee was working on conveyor pully when it slipped striking employee in Right neck and shoulder. Employee reported to supervisor but did not want to go to doctor. Pain never went away and told supervisor EE wanted to go to doctor to be checked out. Employee was taken to doctor on 08/02/2022. Employee was given prescriptions and put on light duty.

Employee was pulling a chain out from under a parts box and tore left bicep.

Employee was helping in hooking up a plastic 8" pipe to pump. Pipe was being pulled with strap and equipment to pump. The pipe broke friction from windrow and struck employee pushing employee into stationary pump. This caused a contusion on employee's back.

Employee was pulling barge rope and strained a shoulder. Employee didn't feel like they needed to go to doctor. On Sunday June 7th employee called supervisor and said they needed to go to doctor because shoulder was hurting worse. Supervisor sent employee to ER because they were the only place open on Sunday, ER released employee to light duty.

Miner was lifting 2 sample buckets into the back of pickup truck and felt a pull in EE's back. Employee strained EE's back. Was taken to clinic and Doctor put employee on light duty for strained back.

Employee was throwing a 1" steel cable and felt a pain in right elbow.

Employee was using an air impact wrench tightening a nut when EE got left ring finger caught behind the washer.

Employee was walking from the parts trailer to the shop when knee popped and gave out.

On 7/15/2015 EE's right index became caught between the timber head and head cable of the ramp barge while taking the cable over the timber head. Tip of finger was surgically amputated the same day. EE was released to restricted duty following finger surgery. 7000-1 will be updated when EE is removed from restricted duty.

On February 27, 2015 welder received welding flash burn to eyes. Receive prescription strength ibuprofen, Tylenol and antibiotic eye drops. Received two days restricted duty and released to full duty on 3/2/2015.

Clevis holding cable broke and struck EE on left leg. At the time of accident it was a first aid DR visit only all x-rays were negative. Two follow up DR visits because of light pain, Dr ordered MRI and referred to Orthopedic. Results torn ACL and surgery was scheduled to repair on 11/20/14. Employee had outpatient surgery and was released to LD the same day.

Employee was descending a ladder into a hopper barge when the ladder slipped on the barge floor. He lost balance and fell approximately 12' to the bottom of the barge fracturing his left ankle, fracturing his left elbow and compressing the L2 disk in his back.

Employee tripped and fell, did not want to go to dr. Feb decided to seek medical attention, medical provider x-ray negative, set up MRI, results read March 25, out patient surgery on March 28 released to restricted duty March 29.

Employee was going to truck and started jogging and fell on rock.

Employee was assisting removing bearing from shaft when sliver went into right leg and groin area.

Employee was cleaning out inside of portable screen deck and material fell and got into left eye.

Employee was working on Snubber Pully on conveyor. When last bolt was removed the pulley rolled foward and pinched right index finger between impact wrench and angle iron resulting in a cut and broken right index finger.

Employee was working on a conveyor belt; wrench slipped causing him to strike his hand against the conveyor resulting in a broken little right finger.

Employee was tightening nut on bolt at plant using cheater pipe in late January 07. Went to dr, neg xray, regular duty. Continued pain, back to dr on 2/20/07. Findings were lumbar strain, bruised rib, restricted duty.

Employees lifting I-Beam to place on lowboy. Beam bumped track of trackhoe and pushed end around knocking employee on ground against berm and beam caught foot. Steel toed shoes, hard hat in place. No damage to mine equipment.

Employee pulled his back while attempting to unclog the feed chute in Unit One.

Employee was checking tension rods on crusher when he turned to walk away and twisted his left leg causing a strained left knee. Employee refused treatment at this time, but requested treatment on 11/17/04 and was sent to doctor.

WHILE SCRAPING LOOSE ROCK FROM WALL, VIBRATION LOOSENED ROCK IN ABOVE AREA, ROCK FELL FROM ABOVE, HITTING ROCKS IN FRONT OF TRACK HOE, AND BOUNCING INTO AND THRU LOWER WINDSHIELD PINNING OPERATOR'S LOWER LEG TO SEAT FRAME, AND BREAKING LOWER RIGHT LEG.

EMPLOYEE WAS OPERATING A CAT 773 50 TON TRUCK WHEN HE LOST CONTROL OF THE VEHICLE CAUSING IT TO FLIP OVER.

EMPLOYEE WAS OPERATING A FORD F8000 WHEN IT STALLED CAUSING HIM TO ROLL DOWN THE BANK AND STRIKEA BARGE.

EE WAS LOADING A SHOT AND A DUST PARTICLE OR FOREIGN MATTER BLEW INTO HIS RIGHT EYE

UNLOADING TRUCK, ONE BOX OF HYDROMITE SLIPPED & HE TWISTED HIS RIGHT WRIST TRYING TO CATCH IT.

EMPLOYEE STATES: TWISTING TO RIGHT TO PICK UP 50# BAG AND FELT A PAIN IN LOWER LEFT BACK AND HIP.

EMPLOYEE WAS PLACING A ROCK DEFLECTOR ON PHASE 1 SCREEN DECK. HE WAS STANDING ON A PILE OF ROCK FINES WHICH BROKE LOOSE CAUSING HIM TO FALL. HE TWISTED HIS RIGHT KNEE CAUSING A STRAIN. EE WAS INSTRUCTED ON THE IMPORTANCE OF MAKING SURE THAT THE AREA HE IS WORKING IN IS CLEAN AND FREEOF TRIP SLIP HAZARDS.

EE WAS SITTING IN A HAUL TRUCK BEING LOADED, ALLEGES THAT HIS BACK WAS "JARRED" WHILE THE TRUCK WAS BEING LOADED.

EE WAS HELPING MOVE A CONVEYOR ROLLER WITH A BACKHOE.

EMPLOYEE WAS CLIMBING OUT OF HIS HAUL TRUCK AND SLIPPED ON THE BOTTOM STEP. HE HELD ON WITH HIS LEFT ARM CAUSING HIM TO STRAIN HIS SHOULDER.

EMPLOYEE WAS WELDING ON THE UNDER SIDE OF THE SCREEN AT THE RINSE PLANT. HE WAS SQUATTED DOWN TO REACH THE AREA THAT NEEDED WELDING, LOST HIS FOOTING AND SLIPPED TWISTING HIS RIGHT KNEE.

EE WAS WELDING ON A BARGE MOST OF THE DAY. WHEN HE ARRIVED HOME, HE FELT HIS EYES BURNING. LATER THAT EVENING HE WENT TO THE EMERGENCY ROOM AND WAS TREATED FOR A WELDING FLASH.

SLIPPED & FELL ON THE CAT WALK NEAR PHASE II.

EE WAS DRILLING HOLES TO DRAIN WATER BUILD-UP NEAR THE 68 CONE CRUSHER. HE WAS UNAWARE OF THE POWER LINE LOCATIONS AND SLIGHTLY GRAZED A LINE WITH THE DRILL WHICH CAUSED A SHOCK.

EE WAS WALKING DOWN A SET OF STEPS WHEN HE SLIPPED CAUSING HIM TO TWIST HIS RIGHT KNEE.

EE WAS GRINDING ON D8 TRACK PADO WHEN A FOREIGN OBJECT STRUCK HIM IN THE RIGHT EYE.

EE WAS REMOVING ROCKS FROM A CONVEYOR ON PHASE I. HE DID NOT FEEL ANY TYPE OF PAIN UNTIL HE GOT HOME.

EMPLOYEE WAS LIFTING AN OXYGEN BOTTLE ONTO A 3/4 TRUCK WHEN HE FELT A PULL IN HIS LOWER BACK. DIAGNOSED WITH A LUMBAR STRAIN.

WHILE I WAS PREPARING TO LOAD A SHOT I BACKED INTO PAUL APC EE WHO HAD AN OPEN KNIFE OUT FRO CUTTING THE HYDROMITE. I CUT MY RIGHT BUTTOCK CHEECK WHICH REQUIED STITCHES AND A TETNUS SHOT.

EE STATES: HE WAS LOADING A STEEP SLOPE INJ RIGHT HEEL & TWISTED LEFT FOOT. ALSO REINJURED LOWRBACK AREA. SLIPPED SEVERAL TIMES DUE TO GRADE. HAVE HAD PROBLEMS WITH INJURY AT THIS LOCATION B EFORE. WENT TO CHIRPORACTOR THAT NIGHT FOR TREATMENT AS FOLLOWUP TREATMENT FROM PREVIOUS INJURY WANTS TO SEE A PODIATRIST FOR FOOT PAIN

USING UTILITY KNIFE TO CUT WIRE TIES THAT WERE SECURING A DUST SUPPRESSING WATER HOSE TO A BROKEN HAND RAIL.

EE WAS REPLACING TOOTH ON 992D FEL. WHILE DRIVING TOOTH ONTO SHANK A METAL CHIP WAS PROPELED FROM TOOTH AND STRUCK EE IN CHIN. METAL TO METAL STRIKING WILL BE MINIMIZE AND BRASS HAMMERS WILL B E USED AS APPLICABLE.

EMPLOYEE EXITED TRUCK AND WALKED ACROSS TRUCK HOOD TO VIEW DUMPSITE. EMPLOYEE SLIPPED ON HOOD SURFACE AND FELL ON RAILING (TRUCK). EMPLOYEE GRABBED RAILING TO AVOID FLALING TO GROUND. HE COULDN'T GET BACK ON THE DECKING AND HAD TO FALL TO THE GROUND.

DRILLER WAS ATTACHING DRILL STEEL TO DRILL ON A HILLSIDE AND STEPPED BACKWARDS AND LOST HIS FOOTING AND FELL ON HIS BACK.
