At plant startup, EE walked under the tail pulley frame of the 400 feed conveyor and stood up too soon striking EE's back on the frame of said conveyor.
Employee was struck on the chin by a rock that came loose from the cone crusher while checking tolerances. Employee was taken to urgent care and received a couple of stitches and then went back to work.
Employee climbed up approximately 4' up side of conveyor to get a wrench and slipped, falling backward. Clothing got caught on the conveyor as they fell. A ladder was nearby, but not used.
Employee was changing tire on test trailer. When they jacked up the trailer, a piece of steel that was leaned up against the trailer fell, striking them in the back and right arm.
Miner tripped on a rock while shoveling. Miner landed on rock, striking lower back area. Miner could move on miners own, but was in pain and taken to urgent care. Currently unable to work until follow up appointment.
Employee fell off crusher and sustained injuries to nose and tail bone.
EE was greasing a tail pulley while it was running.The guard was removed by him to clean grease zerk, glove was caught by pulley & pinched his ring finger on the right hand.
RETRIEVING CONE ADJUSTING PUMP, EE SLIPPED ON STEP & FELL ON THE PUMP HE WAS CARRYING.
AT QUITTING TIMIE-THE GENERATOR WAS SHUT DOWN TO MOVE ELECTRIC CABLES IN A BOX. WHEN THE CHANGE WAS COMPLETED, THE GENERATOR WAS STARTED AND WHEN EE TURNED THE BREAKER ON, IT BLEW UP CAUSING B URNS ON BOTH HANDS.
THE CRUSHING CREW WAS GETTING READY TO REPAIR A CONVEYOR BELT THAT WAS DAMAGED. TP DO THE REPAIR THE CRUSHING CREW HAD TO REPOSITION THE BELT. WHILE REPOSITONING THE BELT (EE IS A LOADER OPERA TOR) CAME ON THE SCENE AND ASSUMING THE BELT WAS POSITIONED & LOCKED OUT & READY FOR REPAIRS, GOT ON THE BELT TO ASSIST WITH REPAIRS. THE CRUSHER OPERATOR TURNED THE BELT ON, KNOCKED EE TO GR
EE STATES HE BENT OVER TO PICK UP TWO BLOCKS AND TWISTED HIS BACK WRONG AN IT WENT OUT.
