Employee was struck on the chin by a rock that came loose from the cone crusher while checking tolerances. Employee was taken to urgent care and received a couple of stitches and then went back to work.

Employee climbed up approximately 4' up side of conveyor to get a wrench and slipped, falling backward. Clothing got caught on the conveyor as they fell. A ladder was nearby, but not used.

THE CRUSHER OPERATOR TURNED THE BELT ON, KNOCKED EE TO GR

EE WAS BENT OVER CUTTING SCREENS WHEN A ROCK FLEW OFF THE JAW CRUSHER AND HIT HIM IN THE BACK OF THE NECK.
