At plant startup, EE walked under the tail pulley frame of the 400 feed conveyor and stood up too soon striking EE's back on the frame of said conveyor.

Employee was struck on the chin by a rock that came loose from the cone crusher while checking tolerances. Employee was taken to urgent care and received a couple of stitches and then went back to work.

Employee climbed up approximately 4' up side of conveyor to get a wrench and slipped, falling backward. Clothing got caught on the conveyor as they fell. A ladder was nearby, but not used.

AT QUITTING TIMIE-THE GENERATOR WAS SHUT DOWN TO MOVE ELECTRIC CABLES IN A BOX. WHEN THE CHANGE WAS COMPLETED, THE GENERATOR WAS STARTED AND WHEN EE TURNED THE BREAKER ON, IT BLEW UP CAUSING B URNS ON BOTH HANDS.

WHILE REPOSITONING THE BELT (EE IS A LOADER OPERA TOR) CAME ON THE SCENE AND ASSUMING THE BELT WAS POSITIONED & LOCKED OUT & READY FOR REPAIRS, GOT ON THE BELT TO ASSIST WITH REPAIRS. THE CRUSHER OPERATOR TURNED THE BELT ON, KNOCKED EE TO GR
