Employee fell off the back of #62A loader landing on upper back/neck/shoulder.
Employee was using a pipe wrench and body weight to try and unjam a feeder. The pipe wrench slipped causing the employee to smash their left hand and splitting the tip of the ring finger open and needing to have stitches.
Employee was working on switching out bags on a baghouse and was carrying a baghouse bag cage, took a step down onto uneven surface and rolled EE's ankle. There was immediate swelling and was taken to the ER. There was no broken bones.
Employee was pulling a sulfur boat out of the leako with a pair of tongs and dropped it. Natural instinct was to try and pick it up, when EE reached to grab it EE barely touched the sulfur boat and burned right index finger and thumb.

10/12/24 the employee was cleaning out the building 3/4 debris and product on the ground using a rake hand tool. When the baghouse overflowed down on the employee getting product in their eyes and continued to work their shift. then on Sunday their eyes started to hurt so the employee went to the ER to get them check. On 10/21/2024 employee went back and was given a Prescription
Employee was fueling drills and walking between drills looking up stepped on a rock and twisted knee. Felt something pop in EE's knee, tried to work for a little while before reporting it but the pain got worse.
Employee was cleaning out the 100 crusher and the employee looked into then 100 crusher turned EE's head upward when EE felt a pop in EE's neck right side and employee felt arms go numb and tingly. Employee told supervisor and the supervisor call safety.
While cutting a piece of metal, EE rubbed left eye and might have gotten something in it off of the gloves. EE didn't notice until later in the evening when EE's eye started to get irritated. *EE did not report the injury until the start of the next shift and then was taken to the clinic to be checked out. After being brought back from the clinic, we sent EE home for the day.
Employee was walking on rocky surface and sprained left ankle. Lost time injury.
Was driving a water truck and hit a bump which jolted EE and caused a severe pain in the abdomen. Took to ER and was diagnosed with umbilical hernia and given one day off duty to rest.
Employee was training on water truck 75A. Employee states it is really hard to shift. The truck stalled and they could not get it into gear so they shifted it into gear as hard as they could with all the strength in their arm and felt something kinda pull in their arm. Employee reported the incident the next day to their manager and leadman.
Employee was bent over under a pipe shoveling product into a wheelbarrow. Went to stand up and move wheelbarrow and felt pain in lower back on spine and could not pick up wheelbarrow to move it.
Employee swung a hammer to close a feed gate at an odd angle to prevent a screw to overload. Employee missed the feed gate and smacked left index finger.
Employee was installing liners in a chute. When employee took a step, felt a pop in right ankle.
Employee was breaking down a grader tire. Lifted the tire by self and felt a pop in lower back.
One of our employees was injured when trying to dislodge product stuck in a discharge pipe at one of our furnaces onsite.
Employee was pulling a hose up to the 6th floor of 3&4 furnace, when the employee felt a pop in left knee.
It was a windy day we had two maintenance employees working on primary crusher. One employee was welding while the other was moving parts. The employee moving parts was walking by where the grinding work was being completed when a piece of metal went into EE's eye.
Walking down stairs stepped on a rock and felt a pop in EE's knee.
Guard were not properly attached. When the machine malfunctioned, and broke it caused the guard to be thrown off contacting employee's hard hat.
Maintenance mechanic opened the lid on a job box tool box and when EE was closing the lid it slipped out of EE's hand and slammed down onto the left hand and mostly on the index finder.
An operator had fought a discharge plug and spilled hot product on to the ground and the oiler walked through that pile burning the top and sides of feet.
Employee caught left thumb between the tailgate and body of a side by side, injuring the thumb.
The Individual was using a torch to heat a pipe, to be able to bend it, when a tiny piece of metal popped off and went behind the Individual's safety glasses, getting into eye.
A laborer was shoveling spilled material off of a metal floor in a building when the shovel head struck a bolt EE could not see under the material, causing EE's wrist to be jammed backwards.
EE was using a stationary crane to lift an air compressor. The crane lifted the compressor but only has manual swing capability. When EE pushed the compressor EE felt a slight pain in the back but did not report it as EE did not feel it was significant. The next day the pain was worse, EE reported it and sought treatment.
Tripped on stairs in SRP building. Injured arm
Employee was in Magox building attempting to couple two sections of hose. One section of hose slipped off the coupling and material was blown into face, behind safety glasses and into eyes. Eyes were flushed at the site and at the hospital.
Metal tread in stairway was dented. Employee stepped in dent twisting foot.
EE was platform when EE struck the top of EE's hard hat on a electric raceway. The impact caused neck pain.
Employee was descending the steps of a loader holding on to the handrail and door handrail. The door was tied in the open position. The tie broke and the door hit the employee in the shoulder.
Employee was working on a flange to install a pipe. The employee used chain falls to lift pipe but pipe got caught on pillar and stairs in area. Employee pulled on the flange to get it unstuck.
Employee was handling the ring and puck process and suddenly lost grip in left hand and felt numbness from shoulder down.
Employee was clearing a plugged discharge chute. When the plug released, the employee attempted to get out of the way of the material and in doing so slammed a hand against the area guarding.
Employee states "wind blew the J-204 belt off track. I was placing rock on belt and was trying to replace back to original position when the wind blew the belt with rock back up in my face and knocked me back a few feet." This became reportable when employee sought medical treatment 4/11/19.
Employee was pressure testing a cooler when a piece of metal came off and hit EE's elbow.
Employee was climbing access ladder to 61A loader, missed grabbing the left handle, swung backwards striking left calf on the tire fender.
Employee was walking down stairs, tripped and fell down the last three landing on posterior and dislocating left shoulder.
Employee was using a rag to plug a hole in a screw conveyor that was leaking product. The material got caught in the screw flight and pulled the employee's right index finger in to the hole. The screw flight severed the right index finger to the first joint.
Employee describes that three months ago they got this checked out and was told to get a CT scan which they did not get done. A week ago, it started getting bigger and started hurting, that is why they reported it now. There is no direct cause that they can report.
Working in a manlift in tight quarters while changing a roller bearing, employee turned and struck temple on a piece of angle iron.
Employee was walking up stairs, left foot slipped off and rolled ankle, going up steps, foot slipped off the steps and rolled ankle.
Product spilled from hose when truck driver was descending the access ladder causing driver to hurry down ladder then step awkwardly off bottom rung resulting in back pain.
Employee was cutting a rubber hose, holding the hose with EE's left hand, pushing the knife upward and cut left hand.
Employee was lifting lid of the BICO grinder, it struck items behind the lid, and bounced back unexpectedly by the employee. The employee's hand was still in the way when the lid came back down and landed on the employee left pinky finger tip.
Employee was bagging product at Magox. When employee removed the bag, the clean air burst blew product in to employee's right eye under safety glasses.
Employee was driving EE's truck with a supervisor. The employee said "something is weird with my eye." The supervisor saw the injured employees left eye start to bleed. Emergency room indicated this was not due to work and cannot be connected to the job.
Employee opened the top drawer of EE's file cabinet and it started to fall towards EE. EE stopped it with EE's hand in the drawer bruising EE's right finger.
Employee was walking from the upper shop to another location but traversed through ice that had not yet melted from the most recent storm. EE slipped and fell.
Employee was driving up the employee parking lot ramp when the van skid and stopped on snow. The employee got out of the van to see what they could do and slipped on snow/ice. Employee sprained the right knee and ankle. The employee had not actually begun work yet when the accident took place.
Employee was cleaning under a screw that was being removed. The screw shifted 2 inches, striking the employee on the head (under hard hat), neck and shoulder. There was no contact marks and the employee did not necessarily want treatment however the company felt with the area affected, employee should be checked out.
Employee was fitting a piece of plate, it came loose and hit the employee on upper right lip, cutting it and chipped front right tooth.
Employee was using an air hose to blow out the truck and lost control of the hose which struck their right eye blowing material in to their eye. The employee did not report this accident until the next shift on 9/7/17.
Employee states that ee was walking to oil the baker cooler in the basement of 3 & 4 herreshoff through 6 to 8 inches of spilled material and stepped in a hole under the material.
Employee states that ee was hammering on a tail bearing to remove and replace. As ee was hammering, a piece of metal broke off and flew under glasses and in to eye.
Product bag burst with product going in to employee's eye, behind safety glasses.
Employee was kneeling on concrete and heard employee's knee pop.
Employee was using a grinder when it slipped and hit left index finger. Employee received sutures and a splint.
Employee was walking from the employee parking lot, using the stairs in the area that had ice built up on the them. The employee slipped and sprained left ankle.

Employee was cleaning the inside of the pulvasizer. The door slammed closed on right hand.
Employee was re-lighting the burners on the furnace and was shocked when EE contacted a spark plug that was being ignited by another employee. The employee felt a shock.
During a heavy snow fall, employee slipped walking to the maintenance shop.
Employee loading a truck on an extremely windy day and got product in his eyes due to a gust of wind.
Employee states that she was shoved very hard by a fellow employee with the fellow employee's shoulder.
This is not an employee but a contractor who cleans our offices two times per week. I have no official report from the injured party however have reports from employees who came to her aid. The injured party called for help over the radio system. The employees who came to her aid reported that the injured party stated that she broke her leg.
Employee was using a pick ax to loosen hardened material. He struck a piece of concrete that was not visible to him. The energy from that contact traveled down his back causing an acute lower back strain.
Employee was striking a planning stake while staking out the ore and struck his left thumb with the hammer.
Employee states he was walking up the stairs from the Slurry basement when he turned around to go back down the stairs. He caught his foot on the stair and fell backward on to his tail bone hitting the edge of the step.
Employee was using a sledge hammer. He swung and missed hitting his right foot instead. Employee claims this occurred on 1/17/15 however worked through that shift and the next day, 1/18/15, then sought medical treatment on 1/19/15 on his personal insurance however alerted the medical provider that it happened at work however had not notified employer.
Employee grabbed this employee's hand, squeezed it and aggravated an old injury to the hand. He felt something pop, then sharp pain and swelling. Did not occur during work however on break.
EE and co-worker were on a screw about 25' off ground. Co-worker felt the screw shake and looked up to see EE wedged between conduit and railing. EE was reaching out to him with his right arm, co-worker took his arm and tried pulling him up. EE began shaking and passed out. Co-worker lost his grip on EE. EE fell about 5 feet. Co-worker got help and got EE to the ground.
This is a truck driver that is not our employee. He was guiding our truck to attach to his pup when the cover got stuck. He attempted to move the cover over the tongue, it moved smashing his third finger on his right hand. Eight sutures to the finger. No lost time or restricted duty.
Employee was using a pry bar to loosen a part. He put the pry bar on the ground and began to step away and felt a sharp pain in his back.
Employee said he had chest pains while driving to work but decided to come to work anyway. He said he was sweeping and climbing stairs in the cooler building when he felt chest pains again and started to sweat, one hour after his shift started. He decided to leave work. He sought medical treatment at 5:00 p.m. the day after his shift.
employee was stepping to the side to access a computer mouse and rolled her ankle on an uneven surface in the lab. She caught herself before falling.
Employee disconnected airline after turning valve off. The pressure in the hose came out and hit inner left arm at joint.

Employee was pulling chain out of D-119 elevator and felt a pull in groins area.
Employee was scraping a gasket off of a timing belt cover with a razor knife. The knife slipped and cut his right hand.
Stepped forward to grab a bucket in the splitting room and banged his left knee on the corner of the table. The employee reported this to his supervisor on the date of occurrence but refused medical treatment. The employee said he did not need medical attention. On 12/13/13 the employee sought medical treatment on his own then notified the company that he sought medical attention.
Employee was pulling on a stuck valve and over pulled causing a hernia strain.
Employee was greasing a 6" water valve when he pulled on a 4' wrench to close the valve.

Injured employee works for a contractor that has not yet received his Mine ID number. We are reporting it as it happened on our premise. Employee was jackhammering & lost his balance, causing him to slip to the next level down approx. 3 1/2 ft below. Employee landed on his knee. Update: Contractor now has own MSHA ID.
Employee originally reported a back strain on May 11, 2013, from pulling a tarp. He did not seek medical attention at that time. On May 31, 2013, he said that he needed medical attention.
Employee standing behind another employee who was swinging a hammer. The injured employee stepped out of the way of the swinging hammer. He stepped off of the grated flooring.
Through the normal course of work, pain in his right side and noticed an unnatural swelling. He had a gradual cramping, pain and swelling that progressed and would not subside.
Using super sucker truck, employee was increasing the RPM's to move the pto, the truck engaged and the employee attempted to stop it. The truck hit a beam with the impact thrusting employee into the dashboard upon impact.
Employee was using a forklift to traverse a holding area for filled finished product bags. The employee opted to get off the forklift and walk around the holding area. The area is traversed by rolling stock only and this compacted the icy conditions where the employee walked. He slipped on the ice and landed on his left elbow.

Employee was pressure washing a drill using a ladder to reach the top. While climbing down the ladder, the rungs were wet/slippery. Employee lost her footing on the third to last rung and jumped to save from falling. She broke a bone in her right foot.
Employee jack hammering built up material from a tank. Material sloughed off scraping employees left forearm.
Employee was switching bins for bagging purposes, material came out from air spout and blew in face and eyes.
Employee sitting on a chair, reading a manual to a maintenance mechanic. Employee coughed. Mechanic saw that employee looked as if he were not breathing. Went to employee's aid. Employee came to but was unsure of where he was. Employee sent with his wife to hospital, refused ambulance. No C4 has been filled out and no diagnosis. Hospital had him off the following day.
Employee putting pump in place then felt shoulder start to hurt. Employee sought medical treatment on 4/11/11 notifying company of medical care on 4/12/11. No diagnosis was returned, xray report states "pending". Modified duty of no lifting over 50# which is sufficient for the job. Employee on vacation on 4/15/11 has not yet returned with full release or report from physician.

Employee was moving a jack hammer at 3:00 a.m. then a bottle of water at 4:00 a.m. After 4:00 a.m., he felt a sore area and sensitivity around his belly button. Employee finished shift and was off for the next two days (incident happened on Monday in the AM, employee returned to work on Thursday at 8:00 a.m. EE's wife brought in doctor's note on Thursday at 11:30 a.m.

Employee was cutting wedges on table saw when wedge got lodged. Employee tried to dislodge the wedge and the wedge took his hand into the blade. His left middle finger and his left ring finger were cut.
Employee trying to close a bin gate, the gate continued past where the employee thought the gate should stop. The employee's left pinky fingernail passed through a tight spot, the fingernail was pulled off.
Employee was sweeping had chest pain and numbness in arm. Subsequently found out that he had a gall bladder stone being passed.
Employee was training a fellow employee on a bagging system. The trainee removed the bag early and product blew into trainer's right eye causing a corneal abrasion.
LOADING A TRUCK AT THE LOADOUT BINS EMPLOYEE STEPPED ON A HARD DIRT CLOD AND TWISTED HIS LEFT KNEE.
THE EE RAN AN INSTRUMENT IN THE LAB WITHOUT AN EXHAUST FAN. SHE FELT ILL AFTER RUNNING THE INSTRUMENT SO THOUGHT IT WAS THE CAUSE. MSHA STATE & FEDERAL BOTH INVESTIGATED THIS INCIDENT & HAD A NEGATIVE FINDING. AN MSA ORION DELUXE GAS ANALYZER WAS USED TO TEST THE ENVIROMENT. IT SHOWED NO GAS PRESENT
EE WAS WALKING DOWN A SPIRAL STAIRCASE AND MISSTEPPED CAUSING HIM TO TRIP AND TWIST HIS ANKLE.
MINER WAS RELIGHTING A BURNER THAT WENT OUT ON THE FURNACE. THE MINER DID NOT SHUT OFF GAS BUILDUP BEFORE RELIGHTING THE BURNER. WHEN THE BURNER WAS LIT THE BUILDUP OF GAS IGNITED, STARTLING H IM. HE JUMPED DOWN OFF OF THE 3RD RUNG OF A LADDER AND INJURED HIS RIGHT KNEE.
WHILE HAMMERING IN A WEDGE ON A BALL MILL, HE TWISTED HIS LEFT KNEE.
EE WORKS IN LABORATORY.SHE WAS BURNING SAMPLES UNDER A HOOD. THE HOOD FAILED & SHE WAS NOT AWARETHAT IT FAILED. SHE OPENED THE HOOD & WAS EXPOSED TO ACID FUMES.SHE INGESTED THE FUMES. SHE DID NOT GET OUT OF THE ENVIROMENT & LEFT THE HOOD OPEN. SHE DID NOT REPORT THE ACCIDENT UNTIL THE END OF HER SHIFT & DID NOT GO TO DR FOR 24 HRS
EMPLOYEE WAS USING A GRINDER ON A CLUTCH WHEN A HARD WENT INTO HIS EYE.
EMPLOYEE WAS STEPPING OVER A WATER PIPE WITH HISLEFT LEG WHEN HIS RIGHT LEG CAUGHT ON THE PIPE CAUSING HIM TO STUMBLE AND TWIST HIS LEFT ANKLE.THE EMPLOYEE CONTINUED TO WALK ON THE ANKLE NOT KNOWING IT WAS BROKEN.
WITH THE DOOR OPEN ON A PICKUP TRUCK, EMPLOYEE REMOVED THE WHEEL CHOCK CAUSING THE TRUCK TO ROLL FORWARD, PINNING HIM BETWEEN THE PICKUP DOOR AND THE DOOR OF HMS BUILDING.
EMPLOYEE WAS CLEANING UNDER THE DUST PODS ON THE DUST COLLECTOR. IN DOING THIS, HOT MATERIAL FLUSHED OUT FROM THE SYSTEM INTO HIS GLOVE, CAUSING BURNS TO HIS RIGHT HAND. TO DOCTOR 4-9-01.
THE DUST CHAMBER HOPPER WAS PLUGGED WITH PRODUCT. EMPLOYEE TAPPED ON THE BIN WITH HIS LEFT FOOT TO LOOSEN PRODUCT. IN DOING THIS, HOT MATERIAL FLUSHED OUT AND INTO HIS LEFT BOOT, CAUSING BURN S TO THE TOP AND BACK OF HIS LEFT FOOT. TO DOCTOR THE SAME DAY- OFF WORK SINCE.
WHILE LIFTING BRICK AT THE KILN, EE FELT SOME DISCOMFORT IN HIS LOWER BACK. HE CONTINUED WORKING, SWEEPING AND FELT PAIN IN LOWER BACK. TO DR. THE NEXT DAY, OFF WORK SINCE.
EE TRIED OPENING A STRUCK DOOR, IT WOULDN'T OPEN. HE JERKED IT, STRAINING HIS LEFT ARM, ELBOW. THOUGHT PAIN WOULD GO AWAY, IT DID NOT TO DR ON 3-8-01.
WHILE USING A BAR TO CLEAN OUT THE DROP CHUTE IN THE PUG MILL, THE BAR BOUNCED BACK, STRIKING EE IN THE FOREHEAD. TO DOCTOR THE SAME DAY.
WHILE EMPLOYEE WAS WALKING BY THE SCREW IN HIS WORK AREA. IT OVER LOADED WITH PRODUCT AND SHOT OUT LIFTING THE SCREW LID GETING PRODUCT INTO HIS EYES. TO DOCTOR THE SAME DAY. OFF WORK SINCE .
WHILE INSTALLING COUNTER WEIGH TY ROCK SCREEN EEWAS USING A 3/4" RATCHET. IT SLIPPED, HITTING EE IN HIS LOWER JAW & TEETH C-4 WAS JUST RECD

WHILE EMPLOYEE WAS CLEANING UP OUTSIDE TERTIARY CRUSHER BUILDING, HE STARTED HAVING CHEST PAINS AND WAS NOT FEELING WELL. TO HOSPITAL THE SAME NIGHT.
WHILE EE WAS CLIMBING UP OUT OF HAUL TRK HE SLIPPED GRABBED AHOLD OF LADDER, PULLING HIS LEFT ARM & SHOULDER. TO DR 8/17/00 PUT ON RESTRICTED DUTY 8/17/00 TIL 1/5/01. TOOK 1 WK VACATION OFF WORK SINCE

EE STATES WHILE HE WAS BENT DOWN TAKING A SAMPLE, HE TURNED HIS HEAD FAST, HEARD SOMETHING POP, HEADACHE STARTED. OFF WORK SINCE 5-10-00, TO DR 5-15-00, STILL OFF WORK AS OF 5-24-00. THIS WAS NOT REPORTED UNTIL 5-15-00.
IN THE PROCESS OF STEPPING OVER THE D-678 SCREW, EE LOST HER FOOTING IN LOOSE MATERIAL AND FELL, LANDING ON HER RIGHT ARM. TO DR. THE SAME DAY. TO DR. AGAIN 7-7-00. HAD SURGERY 7-10-00. OFF WO RK SINCE.
EE WAS PUTTING A GLASS TUBE INTO A RUBBER STOPPER. THE TUBE BROKE, CUTTING EE'S RIGHT MIDDLE FINGER, REQUIRING STITCHES.
