IE was replacing a g-bolt on a screw conveyor gear box, after locking out the screw the IE attempted to rotate the screw to line up the bolt holes. IE did this by grabbing the drive belts to rotate the shive. This pulled the IEs right ring finger in to a pinch point causing a laceration that required 3 stitches.

Employee was lowering door to bottom of lime pit (about 15') using an attached rope. The employees thumb got caught in the rope under the weight of the door cutting off the tip of the thumb below the nail.

IE stood on a belt conveyor to inspect a stone feeder that may have been leaking. IE did not lock out the belt prior to accessing it. Conveyor belt started up and pulled the IE through a 14' height clearance before IE was able to pull the E-stop cable and climb off the belt. Incident caused multiple fractures in employee's back requiring EE to have surgery.

Employee was removing the guard on a conveyor belt while standing on a ladder. Fell off of ladder about 4 feet and landed on right side. MRI review on 12/2 revealed torn rotator cuff and employee was placed on restricted duty.

IE was unplugging a material transfer chute by banging on it with a pry bar. Felt a pinch in right hand. Went to ER on 8/27 and received a negative x-ray. Followed up at ER on 9/2 when pain and swelling persisted and received restrictions and a referral to Ortho.

Employee felt a pop in right shoulder while raking kiln. X-ray showed no breaks. Follow up with ortho on 5/4/21 resulted in pain medicine and a sling. Awaiting further diagnostic procedures.

Employee tripped while climbing down from the back of a flatbed truck. Fell on upper back/shoulder. Fractured one rib.

At approximately 5:50 AM, EE entered level 5 of the quarry with the M503 water truck for dust suppression activities. In traversing towards the working face, EE hit a pile of chat used for leveling the quarry floor. The impact launched EE in the cab. Despite being restrained by seat-belt, EE received facial wounds and a fracture of the L3 vertebrae.

EE was raking kiln 2 cooler grates when in pulling the rake back, the handle struck a hand rail behind EE causing a left shoulder rotator cuff strain. EE reported the discomfort and was sent to company doctor. EE was diagnosed on 02/06/20 and physical therapy recommended.

EE and another employee had removed a blockage at the 217 vibratory shaker screen. They completed the task and released the equipment. While walking at the base of the structure, a rock fell from the upper level, ricocheted from a rail and struck the EE on left hand. On 01/06, EE was informed ny the doctor that they had a fracture.

Employee injured right knee while walking on the floor.

Emptying Dribble Chutes. Airborne object got into left eye (EE was wearing safety glasses). After attempting to wash out object, EE was taken to the ER and was treated with antibiotics and prescription eyewash.

Worker was operating dust truck that hit the road edge and tipped over into the berm. Had mild abrasion to shoulder and a small contusion to the back of the thigh. Complained about elbow pains a few weeks after. On 08/13/19, recommended for physiotherapy as an MRI showed minor tears in tendon around the left elbow, making this a reportable incident.

Employee was hammering a G-bolt out of a screw conveyor when employee felt a pop in right elbow. Employee was examined by an orthopedic specialist who diagnosed the injury as a strain. Worker returned to work with no restrictions. Follow-up visit to orthopedic specialist on 11/29/2018 resulted in doctor writing script for physical therapy making injury reportable.

Employee was shoveling material buildup on belt conveyor when EE felt a pop in EE's left shoulder. Employee was examined at an orthopedic clinic and diagnosed with a possible rotator cuff tear/strain. Employee scheduled to have physical therapy 2-3 times per week until follow-up visit April 5, 2018. Worker returned to work on regular shift.

Employee was changing bags inside a nuisance dust collector and while exiting through the access door, accumulated lime dust on top of collector sluffed off and fell onto the employee's face and entered EE's right eye causing eye irritation. Employee received medical first aid and was given a prescription ointment for use in treating the irritation.

Employee was climbing down Ducon tank ladder, a short 3 rung stationary ladder. Took left leg off to place it on next rung and right foot slipped off. Left leg went down and landed akwardly on catwalk and right ankle got caught inbetween ladder and frame work. Resulted in strained left knee and sprained right ankle.

Employee strained lower back while performing a sample size distribution analysis with the sample shaker.

The employee was using a jackhammer with bit to remove buildup that had accumulated between the preheater bull nose and feed bed. The jackhammer bit was wedged between the bull nose and rock bed. When proceeding to begin jackhammering the employee used only one hand to grab the jackhammer, it fell pinching the employees thumb between the jackhammer and preheater ram guard.

Employee was coming down a ladder from platform when stepped to first rung in ladder and his foot slipped causing him to fall. He grasped the railing with his left hand and all of his weight was on his lift arm causing a left shoulder strain.

Employee slipped on ice in upper parking lot after getting out of car. Right ankle sprain.

EE was cutting rubber seal for one touch door when the utility knife contacted his right thigh causing a laceration.

While in process of installing a new motor on air separator, the EE pinched thumb between the motor and base. Left thumb fracture.

Employee was assigned to rake scale from #2 kiln. While raking a piece of scale came over the edge of kiln and hit rake he was using causing him to lose grip on rake and rake hit him on the left jaw.

Employee was walking between a parked maintenance truck and welder when he stepped on a piece of rock approximately 1" x 1", turning his ankle.

Employee was changing screens on 2nd deck of vibrating feeder, as he was exiting top feed end of deck his footing slipped and twisted his upper body causing his shoulder to twist in awkward position causing dislocation of right shoulder.

Employee turned,twisted right knee while stemming blast holes. Dr.visit 9/13-Rt knee strain-Ortho visit 10/13- suggest surgery- WCofArizona requested 2nd opinion-12/13-Ortho visit-suggested surgery- Surgery on 1/16/14:incident became a reportable incident on 1/17/14.

The worker was doing contracted conveyor belt splicing & was kneeling by the belt when he turned to get a hand tool, twisted his knee, heard a pop. He worked for the rest of the day without reporting the incident to his supervisor. He also worked every work day through 7/15/13 & reported the incident on 7/16/13. He's been out of work since 7/17/13 & has filed a workers comp claim.

Employee was climbing down ladder; foot slipped off rung and caught himself with his right arm causing jolting action and causing neck strain.

Employee performing work on NO 1 preheater lost balance and contacted hot surface causing small 2nd degree burn to left arm and forearm

employee got into an altercation with another employee and resulted in employee getting hit in the face with hardhat causing laceration on cheek and a fracture of nose.

Employee was attempting to lift a oxygen bottle by the lid, the lid came off and hit him in the upper lip.

General daily labor lifting cut up screw flights and servicing mobile equipment causing employee to experience pain/soreness in back.(Lumbar strain)

Employee was assigned to cleaning in the 302 tunnel, while shoveling underneath the conveyor belt his shovel got caught on return roller causing his left hand to be jammed against the framework of conveyor belt.

Employee was assisting in removing transfer chute door, when removing last fastener holding the door, the door swung open making contact with his left ear, causing laceration.

Employee was walking down stairs when he lost his footing and fell forward, reaching out to catch himself, jammed his left pinkie finger into handrail causing dislocation of left pinkie finger.

Employee was walking down snow covered slope when he slipped and twisted his left knee.

Employee was attempting to take off bolt from allen wrench attached to a 1/2" IR air impact wrench. He was holding the impact wrench with right hand and his left hand was pulling on the bolt. His right hand slipped and made contact with trigger on impact wrench, causing the bolt to rotate in his left hand, causing lacerations to his left hand palm area.

Employee sustained a fracture of left wrist when employee slipped on ice and fell while walking on the concrete walkway.

Employee was attempting to load a drill steel into a loader bucket. Employee pulled groin muscle.

Employee was walking down stairway, when he twisted his left foot on third step from bottom.

Employee was cleaning out #2 transfer chute with short rake when a piece of scale fell and hit rake causing rake to push up left shoulder.

Employee was raking out the Ducon.

Employee was using a pipe to unplug a plugged feeder.

employee was unplugging a crusher when a piece of rock came out of the crusher and hit her right little finger.

Employee was stepping off pressure washer trailer when his foot got caught in the trailer and employee fell. NOTE: Employee did not see doctor until 9/20/09.

Employee was climbing up stationary ladder when his foot slipped off second rung on ladder causing him to fall approximately 24" on his feet.

Rubber tire dozer went over a high wall on the 5th level of west quarry. Employee was cleaning off area for drilling.

Employee walking to ready line to start haul truck, on the way he was walking through deep snow. Stepped into deep snow and twisted his right knee.

Employee was walking in crushing plant when he slipped on snow/mud. He fell on his right shoulder, received strain to right shoulder.

Employee was walking down walkway when he tripped on welding cable/pipe in walkway. Sprained left ankle and had lacerations on right knee where he landed.

Employee was rodding out O2 probe, when a piece of scale fell; hit the bar he was using and strained his left elbow.

Employee was getting off dust and twisted his left knee.

A bucket hit the injured worker on the hard hat, causing a laceration on his head.

While getting loaded with rock, the loader operator dropped bucket on side of haul truck causing it to bounce. Bounced driver in seat.

Splicing 104 belt conveyor; lifted plywood and sliding between conveyor belt and idler.

Employee was shoveling mud on 219BC (cleanup around crushing area), sharp pain under left ribcage.

Driving 773 haul truck with vent window open, foreign particle blew into her left eye.

Employee stated that he pulled the disconnect on 219 dust collector and proceeded to remove the guard. The pulley had not completely stopped when the guard contacted the pulley and pulled his right hand into the moving pulley.

Employee was opening rail car hatch, hatch got caught on other door, causing the hatch to snap back, causing pain in lower back.

Employee exiting kiln area when a wooden 4"x4" snapped allowing bricks to roll off of conveyor and struck employee on arm & back causing bruising.

Employee was walking on level surface, dry conditions, when he strained his right knee.

Employee was using sledge hammer, hitting cone on silo. He made contact with edge of inspection door and handle of sledge hammer, smashing right index finger.

Employee was walking by #2 kiln. Kiln went positive blowing doors open, blowing lime dust into eyes.

Employee was climbing out from underneath roof on #2 preheater jack hammer in right hand and rolled left.

EMPLOYEE WAS CLEANING PUG MILL WITH AIR LANCE. AIR LANCE GOT CAUGHT IN PUG MILL, FORCING LANCE AGAINST BERM. MIDDLE LEFT FINGER WAS CRUSHED BETWEEN LANCE AND BERM.

EMPLOYEE WAS CLEANING CHUTE - SWINGING HAMMER. LEFT SHOULDER STRAIN. NOTE: REPORTED INCIDENT ON 11/18/04 - WENT ON RESTRICTED DUTY 1-10-05.

Cleaning off the buildup on 601 WF belt. Left forearm snagged on sharp edge.

WHILE HOISTING DOWN THE HOLE HAMMER UP TO DRILL DECK, THE DECK CENTRALIZING THE BUSHING GOT CAUGHT ON THE GUARD RAIL OF THE DECK. EMPLOYEE PULLED THE HAMMER AWAY FROM THE DECK WITH HIS HAND. W HEN HE DID THIS, THE BUSHING SLID DOWN THE DRILL PIPE. SMASHING HIS FINGER AGAINST THE DRILL BIT.

THE EE HAD SHORT HANDLED SHOVEL & WAS CLEANING THE KILN ROCK OFF THE CENTER FRAME WORK BETWEEN THE TOP & BOTTOM BELTS WHILE BELT WAS RUNNING. THE EES SHOVEL GOT CAUGHT & HE JERKED HIS LEFT HAND BACK & HIS ARM STRUCK AGAINST A SUPPORT BEAM. THE EES ARM WAS FRACTURED JUST BELOW THE ELBOW.
