THE SECOND TEAM MEMBER WAS TRANSPORTED TO THE HOSPITAL WHERE HE DIED ON 10/23/02.

THE EMPLOYEE WAS WORKING FROM THE GALLOWAY WORK PLATFORM IN THE BOTTOM OF THE RODEO VENTILATION SHAFT. THEY HAD JUST FINISHED LOADING AND WIRING IN THE BLAST FOR THE END OF SHIFT. THE ORE BU CKET HAD BEEN CALLED FOR SO THAT THE CREW COULD RIDE BACK OUT TO THE COLLAR FOR THE BLAST TIME AND SHIFT END. EE WAS REMOVING SOME WIRE THAT WAS STUCK ON THE FORKS OF THE FORKLIFT. WHEN THE FORK CAME FREE, IT PINCHED EE LEFT THUMB BETWEEN THE FORK AND THE BACK BOARD OF THE FORK LIFT.

EE WAS DRILLING FOR ROCKBOLTS AND HANGER BRACKETS. THEY WERE IN FULL BODT HARNESS, TIED OFF INSIDE A MAN BASKET, LIFTED BY A FORKLIFT. WHILE COLLARING HOLE, THE JACKLEG BUCKLED AND KICKED BACK PUSHING HIM TO THE SIDE HITTING HIM IN THE FACE. HE HAD NO BROKEN BONES BUT DID HAVE 2 STITCHESAND A SWOLLEN FACE. HE WAS KEPT IN HOSPITAL FOR OBSERVATION.

THE EE WAS ROCK BOLTING STANDING ON MUCK PILE OFUNEVEN MATERIAL. HE STEPPED BACK TO PULL THE BOLTER OUT OF THE HOLE ONTO A ROCK & TWISTED HIS RIGHT KNEE. HE REPORTED THE INJURY TO HIS SUPERVIS OR BUT DID NOT GO TO DR UNTIL 1/17/01. HE RECD A FULL DUTY RELEASE TO RETURN TO WORK WE WERE UNAWARE HE WAS CONTINUING TO HAVE PROBLEMS WITH HIS KNEE UNTIL WE WERE NOTIFIED ON 5/4/01 BY WORKE
EE WAS PULLING ELECTRICAL CABLE AROUND A STEEL COLUMN. HIS THUMB WAS CUT BY A SHARP EDGE ON THE COLUMN.

EMPLOYEE WAS BOLTING AND THE 6' STEEL BROKE(SHANKED) AND CAME OUT OF THE HOLE AND HIT HIM ON THERIGHT SIDE OF THE FACE, TEMPLE AREA AND FOREHEADHE WAS TRANSPORTED TO E.R. AND RECEIVED TWENTY-T WO SUTURES.

THE EE WAS LEAVING THE 1290 STATION AND AS HE WALKED THROUGHT THE STATION GATE A LOAD OF HARD ROCK WAS BEING POURED ONTO THE GRIZZLY APPROX. 15 FEET IN FRONT OF THE STATION GATE. HE HAD REMOVE D HIS SAFETY GLASSES TO CLEAN THEM AND A PIECE OF ROCK FLEW OUT OF THE GRIZZLY STRIKING HIM IN THE RIGHT EYE AND PENETRATING THE ANTERIOR CHAMBER.
EMPLOYEE WAS SETTING TIMBERS AND BUMPED HIS HIP AND NECK ON UNDERSIDE OF GALLOWAY SPEARS. HE DID NOT REPORT THIS ACCIDENT TO EMPLOYER UNTIL 6/17/98 AND IT BECAME REPORTABLE.
THE EE WAS INSTALLING A WING DECK ON THE SCISSOR LIFT, THE DECK WAS PROPERLY SECURED, THE WING EXTENSION SWUNG OUT AND STRUCK THE EE ON THE TOP OF THE HEAD.
EE WAS MOVING A NEW BRAKE SYSTEM FOR THE HOIST ON A CART WHEN THE HANDLES ON THE CART BROKE AND THE BRAKE BLOCK FELL AND HIT HIS ANKLE CAUSING IT TO BREAK.
EE WAS ROCKBOLTING WHEN AIR GOOSENECK BROKE AND AIRHOSE CAME AROUND AND HIT HIM IN THE MOUTH, CHIPPING HIS TOOTH.

EE HAD HIS HAND ON DRILL STEEL WHEN THE MIDDLE SECTION OF STEEL CAME LOOSE AND DROPPED DOWN ON HIS HAND,SMASHING HIS HAND BETWEEN THE TWO STEEL RETAINERS.
EMPLOYEE WAS CUTTING A NOTCH IN AN ANGLE BRACKETWHEN A PIECE OF SLAG BOUNCED OFF SIDE OF BUCK AND INTO HIS EYE.
EE WAS MOVING STEEL. AS HE WAS ROLLING ONE PIECE OF STEEL OFF OF ANOTHER, HE CAUGHT HIS FINGER BETWEEN TWO PIECES, CUTTING FINGER.
INJURED WAS WORKING IN TIGHT QUARTERS WHILE WELDING A BRACKET INTO POSITION. AS HE WAS MANUEVERING HE INADVERTENTLY STRUCK AN ARC WHEN HIS HELMET WAS IN THE UP POSITION. TEH ARC OCCURED NEAR H IS LEFT EYE.
