Employee was assigned to "Quarry Yard Clean-up" they twisted their R/ankle while walking.

Employee is an Equip Opr./Quarry Truck Driver. EE was operating a Quarry Truck, EE was waiting for EE's truck to be loaded when a rock "spilled" or "fell" from the Bucket of the WA-900 Komatsu Mach which was loading EE's truck, broke the driver's side window, hit EE on the L/thigh and fell to the floor of the truck EE was operating.

Employee complained of "lower back pain" after exiting the loader for a mid morning break.

Employee was helping change screen panels when they lifted one of the panels they began to feel pain in their groin area.

Employee was working on the "Bucket Wheel Assembly" which is part of the Wash Plant located in the BBQ Quarry Yard. EE stuck their finger in the hole where a "bolt" is supposed to go rather than using the proper tool. EE's finger got caught and cut in the process.

Employee was walking in the primary plant area when they tripped, they did not fall but the awkward step they made to avoid falling caused pain in their right leg groin area,

Employee was working with a "PRY BAR" while replacing Screen Cloth at the Secondary Plant. The Pry Bar slipped and employee's L/Pinky finger in the nail area got caught.

Employee was cleaning the inside of a "CHUTE" when EE began to feel pain in the left side of EE's back and continuing down EE's L/hip & L/leg.

The bucket on the skid steer loader was stuck in the up position. EE was trying to get the bucket to release and rest on the ground. EE was hitting the machine with a hammer. EE got the bucket to release but was standing a little too close to the machine bucket. When it released, the teeth on the bucket scraped EE's left leg in the shin area.

Employee was exiting the Electrical Room located at BBQ Yard, there is one step to ground level, employee missed the step and fell.

EE was using a "pry bar" to loosen screen cloth EE was changing, when EE began to feel pain in lower back.

Employee was cleaning #10 chute. EE noticed a "clump of dirt" falling down toward EE's foot. EE made a "move" to keep it from hitting EE's foot and in doing so, EE twisted ankle.

Employee was using a torch to cut a section from a steel plate when something blew in right eye. EE reported the irritation to supervisor, ee did not feel it needed medical attention just first aid. Overnight the condition got worse, next day ee R/eye was observed to be red and painful. EE was sent to Concentra for treatment.

EE was holding onto an area where a "Nut" was attached with a wrench while another employee was using another tool a "Power Wrench" to loosen the nut. The vibration from the "Power Wrench" caused ee to loose grip, the wrench ee was holding onto "slipped" and the result was a "Fractured finger".

Employee was shoveling stone around #7 screen when ee felt pain in back.

Technician was in the process of repairing a broken lift cylinder mounting bracket on a Caterpillar model 777D Truck. As EE was attempting to align or pull together the mount and the frame with the portable hydraulic press in order to weld the broken bracket, the tools on/off button failed causing the c-clamp to pop off striking/lacerating the area above right eye.

Employee was stepping off a portable ladder EE was using to power wash equipment, when EE somehow fell and hurt wrist.

Employee was using a utility knife and did not position self correctly when using knife. Employee did not use proper procedure when cutting with keeping EE's wrist and arms tight to reduce being struck by.

Employee was helping to set a crusher bowl in place, one of the cables which they were using to support the crusher bowl broke and hit him in the back of the head and neck area.

Employee was using a "portable stair rack" so he could work on a crusher bowl. When he finished climbing the stair rack he said he felt pain in his right knee.

Employee was working on a piece of steel plate. He and a co-worker were aligning the plate. Employee stuck his finger inside on of the holes, the plate shifted and caught his index finger, left hand.

EE was watering the quarry roads. As he was driving down on of the roads he tried to use the brakes to slow down he says the brakes "failed" and he was unable to stop. He managed to turn the wheels towards the embankment was of the quarry and was able to stop the truck that way.

Employee was using a "hand held" welding torch to cut "U bolt" from the equipment. He failed to turn the torch "OFF" while he moved himself to a better position to complete the work. When he reached for the torch, he burned his hand.

EE was working with another employee to remove a "steel cover plate" as they were lifting the plate it slipped and hit the EE on the left thigh.

EE and co worker were replacing a "Jaw Crusher Maganese Bolt". EE was using a "Sledge Hammer" and bar to loosen the bolt, he missed hitting the bar and the hammer motion took him, and the hammer into a steel plate which is part of the jaw. He was still holding on to the hammer handle and his fingers got caught between the plate and handle.

Employee was removing a plate from the Magnet on C-10 Conveyor when the plate "bounced upwards" and hit his Left Thumb. Note: RX Prescribed & therapy.

EE was cleaning around #7 screen area. He was shoveling and hand picking stone when somehow a stone "bounced upward" and hit him in the face. He sustained small cuts and bruises on his nose and forehead. Note: X-rays taken and RX prescribed.

Employee was climbing down the steps of the wheel loader he had been operating when he began to feel pain in his lower back area.

EE was installing a "rear end" on a pickup truck, when foreign matter got in his right eye.

Employee was removing "Metal Floors" from #7 Feeder when he felt pain in his back. Note: Rx prescribed for pain

Employee was cleaning top deck of #5 screen, when foreign matter blew in R/eye.

EE had been doing some welding in the secondary maint. Shop. While cleaning his work area, he raised his helmet and foreign matter blew in his left eye.

Employee was lifting a pump when he began to feel pain in his back.

Employee was cleaning the shop area with a broom when he began to feel pain in the muscles of his upper back.

Employee was operating a "articulate truck", he was "dumping a load" when it. "jolted", he said he began to feel pain in his lower back. He stated he had previously injured his back, and was being treated by a chiropractor.

Knocking pins out of a bucket which was attached to a front end loader. A piece of metal broke off and cut his left arm

The support cables on c-19 conveyor became twisted. They were working on re-spooling them when a section in the middle of the conveyor frame "dropped down". When the section dropped, they who were standing on the belt were jolted upward off the belt due to the "wave-like motion" on the belt. When they came back down on the belt, they sustained injuries.

When the section dropped, they were jolted up off the belt due to the "wave like movement" on the belt. They they came back on the belt they sustained the injuries

Employee was on the catwalk located at the c-19 conveyor. He was assisting 2 other employee with the re-spooling of the cable. A section of the conveyor "dropped down" the belt went into a "wave like motion" causing them to be tossed upward off the belt, and the cables somehow whipped in the air and wrapped around his legs, pulled him upward, then dropped him.

EE was cleaning the tip of his welding torch, by wiping it on the palm of his left hand welding glove; after several tries, the glove caught fire and burned his left hand.

EE was helping to change screen decks at the secondary plant. The handling of the screen deck became ackward and began to tilt, he tried to staighten it up, when it cut his finger.

Employee bent over to disconnect a part from a piece of equipment when he felt pain in his lower back and was unable to straighten himself up. He had this condition and problem approx 2 years ago while he was moving an appliance in his home.

Employee was using a "Manlift" to get to the c-9 conveyor. His hand was outside of the cage area. His pinky finger got caught between the manlift and the conveyor crushing it.

Employee was using a hammer to remove "build-up" from #7 screen. A chip from a rock flew upwards and hit him in the left eye. Employee was wearing his safety glasses.

Employee was using a utility knife to cut a piece of conveyor belting. He was in a kneeling position and he was cutting toward his knee. The knife slipped out of position and he cut his thigh.

Employee was using a toll called a breaker bar to loose a filter he was removing from piece of equip. He pulled on bar very hard, and strained or began to feel pain in groin area.

Employee was using th extension part of a ladder to complete a project. The ladder slipped because it has no footings to hold it in place. When it slipped, the employee fell to the floor.

Employee was climbing down from his fueling truck when he missed the last step and fell to the ground injuring his left leg. The conditions were wet and muddy and slippery do to the recent rain storms we had in the area.

ee was working with another employee changing a screen. The screen got stuck, ee applied force to get it to move, when the screen moved, his finger was in the way & it got caught between the screen &: the frame.

Employee was trying to remove a nut from C-9 conveyor using a spanner wrench. He realized he was tightening the nut rather than loosening it, so he stopped applying pressure on the handle. When he stopped, the handle flung back and hit him on the right thumb knucle and broke it.

Employee was walking to his work station when he slipped, fell, and twisted his L/ankle.

Ee was stepping down backwards off the komatsu 650 machine. he had been operating, when he misjudged the last step and fell to the ground hard hitting his right shoulder on the ground.

Wintery mix of snow and ice on ground; employee slipped and fell while walking to his quarry truck.

Ee was using a Komatsu WA 30 (small bucket loader) to stack & move "Screen Cloths". The Screen Cloths are metal & stack began to tilt & fall. Ee was backing up on foot & lost his footing & stumbled backwards. He fell & in doing so injured his lower back & left knee. He also sustained some minor scrapes & cuts.

EMPLOYEE WAS WORKING, HE SAYS HE LIFTED A "HEAVY BELT" & HE BEGAN TO FELL PAIN IN HIS BACK.

Ee was holding a "striker wrench" while another employee was hitting the wrench with a hammer. Ee moved his hand and the other employee missed the target and hit ee's third finger left/hand.

EMPLOYEE WAS POUNDING A DENT OUT OF ONE OF THE COMPANY PICKUPS WHEN SOMEHOW HE CUT HIS LEFT THUMB. HE ALSO STRAINED HIS NECK.

EE WAS SHOVELING AT THE SECONDARY PLANT OPERATION WHEN HE MADE AN AWKWARD MOVE AND TWISTED HIS BACK. HE BEGAN TO FEEL PAIN IN HIS LOWER BACK. HE CALLED ON A 2-WAY RADIO AND REPORTED THE PROB LEM TO HIS IMMEDIATE SUPERVISOR.

EE WAS REMOVING MATERIAL WHICH WAS CLOGGING THE SCREEN, HE WAS WORKING BENEATH THE DECK WHEN FOREIGN MATTER BLEW IN HIS EYES.

CONTRACTOR WAS LOADING HIS TRUCK WITH PLASTIC PIPE WHEN HE SLIPPED AND FELL OFF THE TRUCK AND INJURED HIS HIP.

CREW OF 4 MEN PULLING (6)3/0 & (1)1/0 COPPER CABLES UPWARDS THROUGH 2 1/2"CONDUIT WITH CABLE WINCH; PULLING BASKET CAUGHT AT VERY END OF PIPE RUN NYLON SLING SUPPORTING SHEAVE SNAPPED & SHEAVE DROPPED ONTO EE'S LEFT HAND. (20 LB. SHEAVE).

EMPLOYEE WAS WORKING ON A 650 LOADER. HE WALKED UNDER THE COUNTERWEIGHT AND STRUCK HIS HEAD ON THE UNIT. HE JOLTED HIS NECK WHEN HE BUMPED HIS HEAD.

EE STATED HE WAS LIFTING A DRUM OF OIL IN ORDER TO POUR OIL IN THE CRUSHER WHEN HE FELT PAIN IN HIS BACK.

EE WAS WELDING AT THE PRIMARY PLANT WHEN A PIECEOF SLAG BLEW INTO HIS RIGHT EAR. HE LATER FELT PAIN IN THE EAR AND REPORTED SAME TO THE FOREMAN

EE WAS WORKING IN THE SECONDARY PLANT WELDING ONSCREENS WITH ANOTHER EE. HE WAS WELDING ONE SIDEAND SECOND EE WAS WELDING THE OTHER. EE STATES HE WAS WAKEN IN THE MIDDLE OF THE NIGHT WITH THIS PAINFUL BURNING IN BOTH EYES AND WAS HAVING TROUBLE SEEING. HE WAS RUSHED TO EMERGENCY WHERE HE WAS DIAGNOSED WITH ACUTE KERATOCONJUNCTIVITIS INBOTH EYES.

EE WAS WORKING AT THE PRIMARY PLANTL. HE AND HIS FELLOW WORKERS HAD TO MOVE THE CRANE OUT OF THE WAY. IN DOING SO THE HOOK EE WAS MOVING GOT CAUGHT ON A GRIZZLY BAR CAUSING IT TO GO UP AND DOW N IN THE AIR. WHEN IT CAME DOWN IT HIT HIM IN THE RIGHT SHOULDER AREA.

BACK STRAIN--EMPLOYEE WAS HELPING THE MAINTENANCE CREW REMOVE SCREEN PRIOR TO COMPLAINING OF BACK STRAIN.

EMPLOYEE WAS HELPING THE MAINTENANCE CREW IN THE QUARRY YARD WHEN FOREIGN MATTER BLEW IN HIS EYE.

EE WAS EMPTYING THE GARBAGE CANS WHEN HE STRAINED HIS BACK

EE WAS REMOVING A TIRE FROM PU TRUCK WHEN HE CAUGHT HIS HAND BETWEEN THE HUB & THE RIM. HE FRACTURED HIS R MIDDLE FINGER DAMANGED OTHER PARTS OFTHE R HAND

EMPLOYEE WAS USING A PICK AX WHILE DOING HIS JOB HE SWUNG THE AX BACK AND IN DOING SO, HE HIT HIS HAND ON A PIECE OF METAL AND CUT HIS RING FINGER IN THE JOINT AREA. THE CUT REQUIRED (5) STI TCHES.

EE WAS WORKING INSIDE THE DUST HOPPE AT #9 SCREEN. HE WAS USING AN ELECTRIC HAMMER TO BREAK MATERIAL LOOSE. IN THE PROCESS, HE LOST HIS FOOTING AND FELL. DURING THE FALL HE CUT HIS RT. LEG WHI CH REQUIRED STITCHES.

THE PRIMARY CRUSHER FEED BOX LOCATED AT #3 SCREEN WAS PLUGGED UP. EE & ANOTHER EE WERE INVESTIGATING THE PROBLEM. EE WAS HOLDING ONTO HANDRAIL LOCATED IN THE PROBLEM AREA. A LARGE ROCK FELL FR OM THE AREA OF THE PLUGGED UP FEED BOX & SMASHEDEE'S LEFT HAND WHICH WAS ON THE RAIL. THE ROCK FELL APPROX 7' ON HIS HAND

EE WAS WORKING ON THE INSIDE OF A CATERPILLAR 777 QUARRY HAUL TRUCK. HE WAS REMOVING AND REPLACING THE WEAR PLATES. HE WAS STANDING ON A LADDER IN ORDER TO DO THIS JOB. A PIECE OF STEEL HE HAD REMOVED FELL ON THE LADDER CAUSING THE LADDER TO MOVE, AND HE FELL LANDING ON THE INSIDE OF THE HAUL TRUCK BODY.

EE WAS STANDING ON THE CATWALK LOCATED AT CONVEYOR #15 WHEN A STONE BOUNCED OFF THE CONVEYOR AND HIT HIM IN THE FOREHEAD BETWEEN THE EYEBROWS.

EMPLOYEE STATES HE REMOVED THE LID WHICH OCVERES THE OIL ON CRUSHER #5 WHEN HIS HAND SLIPPED, THE LID FELL AND HIT HIM IN THE LEFT CALF LEG AREA. HE CLAIMS HE CONTINUED TO WORK FOR A COUPLE O F HOURS. THEN HIS LEG BEGAN TO GIVE HIM PAIN. HE WAS SENT TO THE DR FORM WORK AND THE DR FOUND THE AREA IN QUESITON HAD A BRUISE ON IT.

EMPLOYEE WAS WORKING ON THE KAMATSU WA 30 MACHINE #1 ADJUSTING THE THROTTLE LLINKAGE SET SCREW, WHILE THE MOTOR WAS RUNNING AND THE RADIATOR FAM WAS TURNING. HIS LEFT THUMB TIP AND NAIL GOT C AUGHT IN THE RADIATOR FAM.

EMPLOYEE WAS USING A STEEL WEDGE TO REMOVE A TAPER LOCK BUSHING, AFTER GETTING THE DESIRED LOOSENESS WITH THE WEDGE, HE BEGAN TO KNOCK IT OUT OF POSITION, AS HE DID THIS, THE WEDGE SHOT OUT OF POSITION, AND HIT HIM IN THE LEFT LEG CAUSING A DEEP LACERATION.

THE LADDER LEADING UP TO C-36 CONVEYOR IS WHAT HE WAS CLIMBING WHEN HE SLIPPED OFF THE BOTTOM STEP AND TWISTED HIS LEFT ANKLE.

TRACTOR TRAILER DUMP TRUCK PULLED FORWARD AFTER DUMPING A LOAD OF CRUSHED STONE. ITS CONTAINER WAS IN THE UP POSITION AND IT STRUCK HIGH TENSION WIRES. CONTACT WITH WIRES SET THE ENGINE ON FIR E. DRIVER AND MALE PASSENGER EXITED THE TRUCK BYJUMPING FROM TRUCK. FEMALE PASSENGER STEPPED DOWN ONTO METAL STEP ABOVE GAS TANK.EE STATED THAT HE FELT ELECTRICITY AS HE EXITED AND WAS KNOCKE

EMP REPORTED TO WORK ON 4-9-97 AND SAID THE WORKHE DID THE DAY BEFORE CAUSED HIS BACK TO HURT.

EMP STATES AFTER WELDING AND CUTTING ON 4-2 HE EXPERIENCED PAIN IN BACK AND LEFT HAND ON HIS WAYHOME.

EMPLOYEE WAS WORKING ON THE #1 SCREEN IN THE BOUND BROOK QUARRY YARD, HE STUMBLEED BACKWARDS AND TWISTED HIS LEFT/ANKLE.

EE WAS USING A HAMMER TO NAIL THE PINS THAT HOLD THE BUCKET IN PLACE ON THE SHANKS, HE MISSED HITTING THE PIN AND HIT HIS L THUMB INSTEAD.

EE WAS USING A SLEDGE HAMMER TO GET A BUCKET LOOSE FROM THE MACHINE AND HE TWISTED HIS BACK IN THE PROCESS.

EE WAS WORKING WITH FELLOW EE CHANGING SCREENS AT CONVEYOR #6. HE STATES THAT WHEN HE LEFT THE PLANT HE BAGAN TO FEEL PAIN IN THE LEFT CHEST, MUSCLE AREA.

EMP WAS SHOVELING THE DIRT OFF ONE OF THE SCREENDECKS IN THE PLANT. HIS SHOVEL JAMMED INTO SOLIDSTRUCTURE (ROCK OR SOMETHING) THAT CAUSED THE SHOVEL TO STOP GOING FORWARD AND EMP WAS APPLYING FORCE WHICH CAUSED HIM TO JAM HIS BACK.

EE WAS CLIMBING UP THE STATIONARY LADDER AT C-38 CONVEYOR, WHEN HE LOST HIS FOOTING AND HAND GRIP AND FELL DOWN THE LADDER TO THE GROUND.

AS HE STEPPED DOWN FROM THE CAB AREA HE SOMEHOW TWISTED HIS ANKLE [RIGHT] FOOT.

EMPLOYEE WAS TRYING TO REMOVE TOP SCREEN DECK BROM #3 SCREENING TOWER. HE PULLED ON IT & HE STATES HE FELT PAIN IN HIS BACK ON THE LEFT SIDE.

EE COMPLAINED OF LOW BACK PAIN L/SIDE. AFTER DRIVING THE QUARRY TRUCK FOR THE BETTER PART OF THE DAY.

EMPLOYEE WAS SHOVELING UNDER THE CONVEYOR, HE JAMMED HIS THUMB BETWEEN THE HANDLE OF THE SHOVEL AND THE FRAME HE WAS WORKING UNDER.

EMPLOYEE WAS PULLING ANGLES OUT OF THE CHUT E WHILE DOING REPAIR WORK USING A HOOK BY HIMSELF THE PLATE SLIPPED & SENT INTO THE CHUT E HE TRIED TO PULL IT OUT & HE STRAINED HIS BACK IN THE PRO CESS

EMPLOYEE FOUND HE HAD TO SHUT DOWN POWER IN THE SECONDARY PLANT AFTER STARTING THE WORK DAY HE GRABBED THE POWER BOX HANDLE PUSHED DOWN WITH FORCE THE HANDLE HAD AN UNEVEN EDGE & IT CUT INTO T HE PALM OF HIS HAND IT REQUIRED (3) SUTURES TO CLOSE THE CUT IT WAS DEEP IN THE FLESH

EE WAS WORKING IN THE SHOP AREA CUTTING OR SAWING A PART IN HALF, THE SAW SLIPPED FROM THE AREA HE WAS SAWING ON AND CUT HIS LEFT THUMB. THE CUT WAS DEEP AND IT REQUIRED 8 STITCHES TO CLOSE TH E WOUND.

EMPLOYEE WAS "TORCH CUTTING" A CRUSHER BOWL IN ORDER TO REMOVE IT FROM THE CRUSHER. PRESSURE SEEMS TO HAVE BUILT UP BETWEEN THE BOWL AND THE LINER AND WHEN THE CUT WAS WIDE ENOUGH, THE PRESSUR E RUSHED OUT CAUSING A "FLARE-UP" OF THE FLAME FROM THE TORCH. EMPLOYEE WAS WEARING WELDING GOGGLES INSTEAD OF THE HELMET AND HE RECEIVED BURNS TO THE FACE AND NECK AREA.

EMPLOYEE WAS CHANGING FILTERS ON A PIECE OF EQUIP. HE WAS USING A LADDER TO REACH THE AREA WHERE THE FILTERS HAD TO PLACED. HE WAS PUSHING UP ON THE FILTERS WHEN THE LADDER BEGAN TO MOVE, HE F ELL TO THE GROUND INJURING HIS L/SHOULDER, HEAD & ELBOW. EMPLOYEE HAD NO LOST TIME UNTIL OCT.8, 1990 WHEN HE WAS ADMITTED TO THE HOSPITAL FOR "ROTOTOR CUFF SURGERY TO THE L/SHOULDER" HE IS EX

EMPLOYEE WAS LIGHTING A TORCH WHEN THE FALME BLEW OUT OF CONTROL BURNED HIM ON THE 1/THIGH AND GROIN AREA,CAUSING 2ND AND 3RD DEGREE BURNS.

EMPLOYEE WAS WELDING A LEG BRACE ON A PIECE OF EQUIPMENT. A PIECE OF THE STEEL HE WAS USING FELL AND HIT HIM ON THE RIGHT LEG IN THE SHINBONE AREA AND CUT HIS LEG AND CAUSED A BRUISE IN THE SA ME AREA.

EE WAS HELPING TO REMOVE AN OLD ELECTRICAL BOX, WHEN THE BOX SLIPPED AND HIT HIM ON THE LEFT WRIST.

HELPING TO CUT A STEEL BEAM IT BROKE LOOSE IT BUMPED HIS HEAD HE CUT H IS THUMB ON A PIECE OF TIN

EMPLOYEE WAS PULLING HIMSELF UP THE SIDE OF A PIECE OF EQUIPMENT WHEN HE BEGAN TO FEEL PAIN IN HIS SHOULDER.

EMPL WELDING AND CUTTING STRIP OF HOT STEEL FELL AGAINST LEFT ANKLE AND BURNED IT.

EMPLOYEE WAS WE5DING IN A TIGHT AREA WHEN A PIECE OF SLAG RICOCHETED OFF THE EQUIPMENT AND LANDED IN HIS EAR CANAL.

EMPLOYEE WAS CL9ANING UNDER AND AROUND THE CONVEYORS WHEN THE STEEL BAR HE WAS USING GOT CAUGHT. HE PULLED ON IT, IT FLEXED AND HIT HIS LITTLE FINGER AND BROKE IT.

EMPLOYEE WAS CH5NGING A TOOTHE ON A LOADER HE WAS USING A HAMMER TO LOOSEN IT WHEN A PIECE OF METAL SHOT OFFTHETOOTHE & LOGGED IN HIS LEFT FOREARM

EMPLOYEE WAS WO9KING WITH FELLOW EMPLOYEES CHANGING SCREENS WHEN THE SCREEN SLIPPED FROM HIS HANDS AND THEIRS AND HIT I NAME IN THE KNEE AND LEG OF THE RIGHT SIDE AS WELL AS CUTTING HIS RIGHT HAND. 3

EMP WAS WORKING3WITH A STEEL BAR TO RELASE A BLOCKAGE IN ONE OF THE CHUTES, WHEN BLOCKAGE WAS CLEARED THE MATERIAL BROKE LOOSE AND HIT THE STEEL BAR HE WAS WORKING WITH AND IT SWUNG AND HIT E EMP ON LEFT RI6 AREA

EMPLOYEE WAS GE7TING A PIECE OF EQUIPMENT FROM HIS TRUCK TO WORK WITH, WHEN HE SLIPPED, FELL, AND HIT HIS HEAD ON A PIECE OF ANGLE IRON. THE CUT WAS OVER THE LEFT EYE REQUIRING STITCHES.

EMPLOYEE REPORTED SEVERE IRRITATION OF HIS EYES DUE TO WELDING FLASH BURN.

EMPL REP ON 11-2-84 THAT HE HAD STRAINED HIS BACK ON 09-19-84 WHILE TRYING TO LIFT OBJECT IN GAGRAGE.

EMPLOYEE WAS CHECKING SCREEN IN THE WEST MILL AREA. HE PUT HIS HAND BETWEEN THE COUNTER WEIGHT AND THE GUARD, SOME STONE ON THE SCREEN CAUSED THE COUNTER WEIGHT TO MOVE HIS HAND WAS CAUGHT BE TWEEN THE COUNTER WEIGHT AND THE GUARD.
