Stepping down off bulldozer employee's right knee buckled causing employee to fall to the ground

Foot slipped in between hydraulic pipe that runs on the walkway of ladder causing worker to fall twisting right knee

Employee twisted knee climbing into excavator when foot became entangled in the foot pedals.

Helical anchor wire got caught up in spud wire and became wedged. Worker entered water to free stuck cable. EE struck anchor wire with hammer freeing spud causing EE's leg to become pinned between the cable and the spud.

Employee took off hard hat to wipe sweat from head and struck head on metal bar

Employee was removing a hoist cylinder from an articulated haul truck. When re-positioning the sling, the pin was removed too soon causing the cylinder to shift pinning employee's hand between the cylinder and a tire.

Employee stepped in an ice covered hole.

Employee was tightening hydraulic line on excavator when the ratchet wrench slipped out of ee's hand and struck ee on the forehead above the eyebrow.

Slip and Fall

Cleaning bathroom bumped finger on counter and tore nail off right ring finger. Needed tetanus shot.

While working on the center pin on the left side "Z" bar of the Caterpillar front end loader the "Z" bar dropped and amputated the employee's left index finger at the first knuckle. Employee failed to properly secure the "Z" bar prior to working on it.

Shoveling he twisted his back and felt pain.

Getting out of the pit truck, he turned and twisted his back.

Pulling on wrench slipped off the bolt and caused hand to get pinched between the frame of the haul truck and wrench.

Getting in truck he slipped and hit the top step injuring the right side of his chest.

Getting off loader, he felt a sharp pain in his lower knee and thigh.

Fueling his truck, he was stepping down off of truck, he slipped and fell between truck and wall.

He and another employee were picking up a large pipe with a hook on it. The hook slipped off the pipe and hit him in the head requiring three (3) staples.

STEPPING OUT OF CAB OF LOADER TURNED TO GO DOWN STEPS, SLIPPED ON ICE ON DECK BANGING RIGHT KNEEAND FELL TO GROUND.

DRIVER'S HEAD WAS TURNED WHILE HE WAS BACKING UP LOADER, HE HIT A BUMP AND HURT HIS NECK.

PUMPHOUSE ELECTRICITY WAS OUT DUE TO STORM. HE ENTERED PUMPHOUSE AND TRIPPED OVER FLOOR GRATE CAUSING RIGHT KNEE SPRAIN & LEFT RIB CONTUSION.

RE-SPLICING A BELT, THE RAZOR KNIFE WENT THROUGH THE BELT INTO HIS RIGHT LEG.

REPAIRED A WATER PIPE AND WAS CLIMBING DOWN OFF OF CATWALK TO GRAB HYDRAULIC HOSE LINE. HE SLIPPED AND GRABBED THE CATWALK RAIL WITH HIS LEFT ARM, DISLOCATING HIS LEFT SHOULDER.

DRIVING DOWN ROADWAY, DRIVER'S FRONT PASSENGER TIRE STRUCK THE BERN EDGE OF THE ROAD AND HE WAS JOLTED IN THE CAB.

EMPTYING A HEAVY GARBAGE CAN, HE LIFTED THE CAN AND HEARD A POP IN HIS STOMACH. (SOUGHT MEDICAL TREATMENT ON 2-28-01).

PUMPING OUT WORK BOAT, LIFTING GRATING THAT WAS WEDGED HE PULLED ON THE GRATING AND SPRAINED HIS LOWER BACK.

IN WORKBOAT PICKING UP BILGE PUMP TO PUT ON DREDGE, HE TURNED AND SLIPPED ON ICY DECK, FALLING TO THE FLOOR.

UNHOOKING BLOCK FROM CHAIN THERE WAS SLACK IN THE CABLE AND BLOCK FELL AND SMASHED HIS LEFT PINKY FINGER.

EE WAS FUELING UP THE D6 DOZER. WHEN HE STEPPED DOWN ON THE DOZER TRACK HE TWISTED HIS LEFT KNEE.

EE CUT HIS RIGHT SHIN AGAINST THE END OF A STEELPIPE

OPENING AND CLOSING SLIDING WINDOW NUMEROUS TIMES CAUSED LEFT TRAPEZIUS STRAIN.

EMPLOYEE RIGGING SLINGS TO REMOVE A TRUCK LOAD OF PIPE, STANDING ON TOP OF PIPES THAT WERE COVERED IN SNOW AND ICE, SLIPPED AND SPRAINED RIGHT ANKLE.

EE EXITING WEIGH TRAILER, PLACED RIGHT FOOT ON STEP AND TWISTED KNEE AS LEFT FOOT WAS PLACED ON NEXT STEP.

EE WAS ASSEMBLING AN ENGINE & CAUGHT HIS RIGHT INDEX FINGER BETWEEN A HEAVY PART & ENGINE.

EE LACERATED MIDDLE FINGER ON RIGHT HAND WHILE ATTEMPTING TO REMOVE PIN OFF OF FRAME FROM LOADER DURING REPAIRS.

EMPLOYEE MISJUDGED DISTANCE BETWEEN VEHICLE HE WAS DRIVING AND A PARKED 988 FRONT END LOADER.UPON REALIZING HIS ERROR HIS ATTEMPTS TO COUNTER STEER HIS VEHICLE-A 1954 ARMY TYPE TRUCK-FAILED.HE
